SGLT-2 inhibitor induced Euglycemic diabetes ketoacidosis in post laparoscopic distal pancreatectomy: A case report.

INTRODUCTION AND IMPORTANCE: Euglycemic diabetic ketoacidosis is a rare but serious adverse effect of sodium-glucose cotransporter 2 (SGLT2) inhibitors. They are a newer class of oral hypoglycemic drugs commonly prescribed for managing patients with diabetes mellitus. CASE PRESENTATION: We present a case of a woman in her 60s with type 2 diabetes mellitus hospitalized for operation of pancreatic body tumor who developed dapagliflozin-associated euglycemic diabetic ketoacidosis despite having stopped the medication before operation. She underwent laparoscopic distal pancreatectomy and occurred

SGLT2 inhibitors and new frontiers in heart failure treatment regardless of ejection fraction and setting.

Sodium-glucose cotransporter 2 inhibitors (SGLT2i) have been shown to reduce cardiovascular (CV) mortality and heart failure (HF) hospitalizations, independently from left ventricular ejection fraction (EF). Their efficacy has been assessed both in patients with reduced and preserved EF, with notable benefits in renal outcomes as well. The initiation of SGLT2i in the early phase of hospitalization for acute HF has proven to be safe and beneficial. The EMPULSE and DICTATE-AHF trials support early empagliflozin and dapagliflozin use, respectively, reducing worsening HF events, improving quality

Safety and Efficacy of Sodium-Glucose Cotransporter 2 Inhibitors in Older Adults with Variable Disease States: A Meta-analysis of Large Placebo-Controlled Trials.

BACKGROUND: Recent guidelines recommend the use of sodium-glucose cotransporter 2 (SGLT-2) inhibitors (SGLT2i) in patients suffering from cardiorenal diseases. However, the safety and efficacy of SGLT2i in older adults with atherosclerotic cardiovascular disease (ASCVD), heart failure (HF), and chronic kidney disease (CKD) remain unclear. METHODS: Online databases were queried from inception to 11 July 2023 to identify primary or secondary analyses for inclusion. Efficacy outcomes included all-cause mortality, cardiovascular (CV) death, hospitalization for heart failure (HHF), major adverse c

Fluid management in hemorrhagic shock.

PURPOSE OF REVIEW: Fluid management in hemorrhagic shock is a controversial topic, and there are evolving clinical guidelines and evidence-based practice. This review aims to highlight the physiological aspects in the light of current evidence on which volume replacement solution to use. RECENT FINDINGS: Current evidence and international guidelines are shifting from a liberal to a restrictive volume resuscitation strategy, emphasizing the potential risks associated with aggressive fluid therapy. The 'lethal triad of trauma' (hypoperfusion, acidosis, and coagulopathy) plays a crucial role in

[Diagnostics and management of perioperative bleeding disorders].

Perioperative bleeding is and remains a major complication during and after surgical interventions, resulting in increased morbidity and mortality. The main causes of a non-primary vascular-related bleeding are congenital or multifactorial pre-existing hemostatic disorders that have not yet been diagnosed, the operating procedure itself and acquired hemostatic abnormalities as a secondary phenomenon, e.g., after excessive volume depletion in hypothermia, trauma or extracorporeal circulation, concomitant hypothermia and loss or consumption of coagulation factors and thrombocytes. Efficacy and Safety of Sodium-Glucose Cotransporter-2 Inhibitors in Patients with and without Advanced CKD: Systematic Review and Meta-Analysis.

KEY POINTS: Treatment with sodium-glucose cotransporter-2 inhibitor (SGLT-2i) provides kidney and cardiovascular protection in patients with advanced CKD. The glycosuric action of SGLT-2i is attenuated in advanced CKD resulting in no change in glycated hemoglobin and fewer volume depletion events. The use of SGLT-2i did not increase the incidence of AKI, urine infections, fractures, or treatment discontinuation due to adverse events. BACKGROUND: The efficacy and safety of sodium-glucose cotransporter-2 inhibitor (SGLT-2i) in patients with advanced CKD, defined as an eGFR <30 ml/min per 1.73 m

Fluid responsiveness and hypotension in patients undergoing propofol-based sedation for colonoscopy following bowel preparation: a prospective cohort study.

PURPOSE: Fasting and bowel preparation may deplete intravascular volume in patients undergoing colonoscopy. Nevertheless, rigorous demonstration of volume depletion and assessment of clinical consequences is lacking. We designed this study to explore the relationship between intravascular volume status and intraprocedural hypotension and to compare transthoracic echocardiography (TTE) and the ClearSight™ (Edwards Lifesciences, Irvine, CA, USA) noninvasive cardiac output monitor to measure intravascular volume status. METHODS: We recruited adult patients undergoing elective colonoscopy followi

Associations Between Temperature and Location of Injury or Condition Among Heat-Related Emergency Department Visits.

High ambient temperature poses significant health risk globally. However, the relative importance of different exposure pathways leading to health risks remains unclear. For 9 US states during 2016-2018, ED visit records for heat exhaustion and heat stroke (HEAT), fluid and electrolyte imbalance (FEI), volume depletion (VD), and acute kidney injury (AKI) were identified via diagnosis codes. Co-diagnosed Y92 subcodes (Y codes) were used to categorize the patient's location at the time of injury or condition. Logistic regressions were used to estimate nonlinear associations between same-day temp

Canagliflozin: A Comprehensive Review of Advances in Preclinical Research.

Canagliflozin is a synthetic sodium glucose transporter inhibitor (SGLT2i). It is the latest approved class of medicine used in the treatment of type 2 diabetes mellitus. As diabetes is emerging as the most common metabolic disorder, SGLT2i has lightened up the path of treatment with additional benefits. SGLT2 is located in the proximal convoluted tubules of the nephron and is responsible for glucose reabsorption. Thus, inhibiting the SGLT2 leads to a decline in glucose concentration in the plasma. The secondary effects of canagliflozin, such as cardiac protection, renoprotective, and decrease

Atrial Flutter With Intraventricular Conduction Delay, Hypotension, and Bradycardia in the Setting of High-Output Ileostomy With Renal Failure, Hyperkalemia, and Metabolic Acidosis: A Case Report With Brief Literature Review.

After ostomy creation, some patients tend to develop high outputs from their ostomy site, which puts them at risk for volume depletion, renal failure, and electrolyte and acid-base abnormalities. This case emphasizes the need for a multidisciplinary team-based approach for the care of patients after ostomy creation, especially those with persistent high output and significant cardiac comorbidities, including cardiac arrhythmias. Patient education and subsequent follow-ups and assessments are important to prevent life-threatening complications in these patients.

Case-crossover assessment of the modifying effects of home medication use on acute kidney-related morbidity due to elevated ambient heat exposure in Atlanta, GA, from 2013 to 2019.

BACKGROUND: Elevated ambient temperatures increase the risk for acute kidney-related morbidity, and medication use may increase vulnerability to heat exposure. This study examines whether medication use, and combination medication use (ie, polypharmacy), modifies the risk for temperature-related volume depletion (VD) and acute kidney injury (AKI). METHODS: This study uses patient-level emergency department (ED) visit data from four Atlanta-area hospitals during the warm season from 2013 to 2019. ED patient-visits who reported a medication were matched with up to five ED patient-visits without

Severe hyperchloremic metabolic acidosis with SGLT2 inhibitors in patients with urinary diversion.

Urinary diversion after cystectomy using autologous intestinal segments has been the gold standard treatment in several urinary tract diseases. The most frequent metabolic consequence is hyperchloremic metabolic acidosis, due to ammonium hydrogen and chloride ions absorption in exchange for the excretion of bicarbonate and sodium ions in the bowel conduit. Sodium-glucose co-transporter-2 (SGLT2) inhibitors are a class of antihyperglycemic agents that block the reabsorption of filtered glucose in the renal proximal convoluted tubules, promoting greater urinary glucose and sodium excretion. Intradialytic Changes and Prognostic Value of Ventriculo-Arterial Coupling in Patients With End-Stage Renal Disease: Protocol for an Observational Prospective Trial.

BACKGROUND: The acute effect of hemodialysis (HD) on left ventricular mechanics has been evaluated in several studies; however, their results are not consistent. Eventually, the heart and the arterial system behave as an interconnected system and not as isolated structures; thus, the evaluation of the interaction of cardiac contractility with the arterial system would provide a more comprehensive understanding of cardiovascular function and cardiac energetics. However, there have not been any studies demonstrating changes in terms of volumes, contractility, intraventricular pressure gradient d

Management of Arginine Vasopressin Deficiency (Central Diabetes Insipidus) in Neonates and Infants.

BACKGROUND: Arginine vasopressin deficiency (AVP-D), previously called central diabetes insipidus (central DI), is the inability to concentrate urine despite elevated serum osmolality (i.e., volume depletion) related to inadequate production of the posterior pituitary hormone vasopressin. Without treatment, which typically consists of fluids and pharmacologic vasopressin analogs, AVP-D can quickly lead to hypernatremia and dehydration. Management of AVP-D in neonates and infants is particularly challenging for many reasons: their inability to communicate thirst, their limited renal concentrati

A case of perioperative euglycemic ketoacidosis in a patient without diabetes: are current guidelines enough?

BACKGROUND: Euglycemic ketoacidosis (eKA) is a serious and potential life-threatening complication of SGLT2-inhibitor (SGLT2i) use. eKA associated with SGLT2i has been increasingly reported in patients without diabetes likely due to more widespread use. The perioperative setting is a particularly vulnerable time for patients due to decreased carbohydrate intake, volume depletion, medication changes, and increased counterregulatory hormone activity due to surgical stress. Current guidelines recommend that patients with type 2 diabetes hold SGLT2i for at least 24-72 h prior to elective surgery.
