Prospective Study of a Point-of-Care Diagnostic Test for Acute Kidney Injury in a South Asian Hospital.

INTRODUCTION: Serum creatinine (SCr) is used to diagnose acute kidney injury (AKI) but has limited utility to distinguish volume depletion from tubular injury. Urine neutrophil gelatinase-associated lipocalin (uNGAL) distinguishes these causes of elevated SCr. METHODS: We tested a novel point-of-care (POC) dipstick to measure uNGAL at presentation at the emergency medical services department (EMSD) in a tertiary care hospital in Puducherry, India. The primary outcome was a sustained elevation of SCr; the secondary outcome was in-hospital mortality. The study enrolled 396 patients with a mean

Managing the side effects of sodium-glucose cotransporter-2 inhibitors.

Although sodium-glucose cotransporter-2 (SGLT2) inhibitors are widely used because of their exceptional cardiovascular and renal benefits, their side effects and corresponding management strategies have not been well characterized to date. The latest evidence suggests that class effects of SGLT2 inhibitors include volume depletion, genital fungal infections, and euglycemic diabetic ketoacidosis, while medication-specific side effects include urinary tract infections with dapagliflozin and potential increased risk of amputations with canagliflozin. Management strategies include close monitoring

Severe Metabolic Acidosis Related to Simultaneous Use of Metformin and Dapagliflozin: A Case Report.

We report the case of a 73-year-old woman with type 2 diabetes mellitus (T2D) who developed severe high anion gap metabolic acidosis while on chronic metformin and dapagliflozin therapy. She presented with dehydration following gastrointestinal symptoms, and venous blood gas revealed profound acidosis. Laboratory evaluation showed acute kidney injury and elevated metformin, lactic acid, and ketone blood levels. Despite initial supportive care, she required ICU admission, renal replacement therapy, and vasopressor support. Acidosis resolved within 36 hours. This case highlights the potentially

Differential glycemic effects of DWP16001 in diabetic dogs according to baseline glycemic status: a multicenter randomized controlled trial.

BACKGROUND: Sodium-glucose cotransporter 2 inhibitors are widely used in human medicine for their insulin-independent glucose-lowering effects. However, their clinical efficacy and safety for managing diabetes mellitus in dogs have not been established. This study aimed to evaluate the efficacy and safety of DWP16001, a selective sodium-glucose cotransporter 2 inhibitor, as an adjunct to insulin therapy for client-owned dogs with diabetes mellitus. METHODS: This multicenter, randomized, double-blind, placebo-controlled clinical trial involved 61 dogs receiving stable insulin therapy. They wer

Characteristics, Management, and Outcomes of Hospitalized Patients with Orthostatic Hypotension.

Orthostatic hypotension (OH) is a common inpatient condition associated with falls, syncope, and mortality. However, standardized approaches for inpatient management of OH are lacking and may vary across clinical specialties. In this retrospective observational cohort study, we reviewed the electronic medical records of patients admitted to Beth Israel Deaconess Medical Center between April 1, 2015 and June 1, 2021 with a diagnosis of OH or medication-related hypotension. Variables of interest included admitting service, presenting symptoms, suspected etiology, and management. Among the 400 in

Thiazide-induced hyponatraemia: distinguishing hypovolaemic and euvolaemic subtypes to guide management strategies.

Hyponatraemia is the most prevalent electrolyte imbalance observed in acute clinical settings, occurring in up to 30% of inpatients. Thiazide-induced hyponatraemia (TIH) is common, accounting for approximately one quarter of hyponatraemia cases among hospitalised patients. TIH presentations may occur with either a diuretic-induced volume depletion and subsequent hypovolaemic hyponatraemia or with a syndrome of inappropriate antidiuresis (SIAD)-like presentation with euvolaemic hyponatraemia (which is more common). Hyponatraemia is associated with prolonged hospital length of stay and greater r

Orthostatic hypotension after kidney transplantation in a patient with diabetic kidney disease: an underrecognized and therapeutic challenge.

Orthostatic hypotension (OH) is characterized by an excessive drop in blood pressure upon standing, leading to impaired quality of life, increased fall risk, and potential cardiovascular complications. It is frequently associated with autonomic dysfunction in patients with neurodegenerative diseases, diabetes mellitus, and aging. Despite its potential impact, OH may be underrecognized in kidney transplant (KT) recipients, particularly in the early post-transplant period, when diuresis-induced hypovolemia may serve as a precipitating factor. We present a case of severe OH in a woman in her 50s

Copeptin in the diagnosis and management of renal tubular disorders.

Copeptin, a stable glycopeptide derived from the precursor of arginine vasopressin (AVP), has emerged as a valuable surrogate biomarker for AVP due to its stability and ease of measurement. This narrative review explores the physiological role of copeptin, its utility as a diagnostic and prognostic biomarker in different kidney diseases, and its clinical relevance in renal tubular disorders. The clinical application of copeptin as a diagnostic biomarker is best established in the differential diagnosis of polyuria-polydipsia syndrome (PPS), distinguishing nephrogenic diabetes insipidus (NDI) f

Benefit-Risk Assessment of GLP-1 Receptor Agonists: Implications for Dermatologists and Plastic Surgeons.

Glucagon-like peptide 1 receptor agonists (GLP-1 RAs) have gained prominence for their efficacy in treating type 2 diabetes and obesity. Recent evidence suggests that their pleiotropic effects-beyond glycemic control and weight loss-include anti-inflammatory, immunomodulatory, and antioxidative effects, which may beneficially support various dermatologic conditions such as psoriasis, hidradenitis suppurativa, acanthosis nigricans, and Hailey-Hailey disease. However, GLP-1 RAs are also associated with emerging cutaneous adverse drug reactions, including bullous, exanthematous and vasculitic man

Relapse of Euglycemic Diabetic Ketoacidosis With Prolonged Glucosuria and Ketonuria Associated With Pre-operative Sodium-Glucose Co-transporters 2 (SGLT-2) Inhibitor Use.

Sodium-glucose co-transporters 2 (SGLT2) inhibitors are an important class of medications that have become increasingly used not only in the management of type 2 diabetes but also for cardioprotection and slowing the progression of renal dysfunction. However, clinicians should be aware of the risk of euglycemic diabetic ketoacidosis associated with their use, particularly in cases of severe illness, volume depletion, decreased carbohydrate intake, or recent surgery. We present a case of a 68-year-old male, whose type 2 diabetes mellitus was controlled with dapagliflozin 10 mg daily, and was el

Rescue percutaneous transluminal septal myocardial ablation for difficult ventilator weaning in drug-resistant hypertrophic obstructive cardiomyopathy: a case report.

BACKGROUND: Hypertrophic obstructive cardiomyopathy is a genetic cardiac disorder characterized by left ventricular outflow tract (LVOT) obstruction that can lead to haemodynamic instability. Managing preload in mechanically ventilated patients with hypertrophic obstructive cardiomyopathy is challenging because volume depletion exacerbates LVOT obstruction, whereas fluid overload induces pulmonary congestion. Weaning from mechanical ventilation (MV) in such cases requires precise volume management; however, conventional pharmacological therapy may be insufficient. Moreover, the role of percuta

Sodium-Glucose Cotransporter 2 (SGLT2) Inhibitors in Non-diabetic Heart Failure: A Systematic Review of Randomized Controlled Trials and Real-World Evidence.

Sodium-glucose cotransporter 2 inhibitors (SGLT2i) were initially developed to improve glycemic control in patients with type 2 diabetes. More recently, growing evidence has highlighted their cardiovascular benefits, particularly in patients with heart failure (HF), irrespective of diabetic status. This systematic review evaluates the efficacy and safety of SGLT2i, including dapagliflozin and empagliflozin, in non-diabetic patients with HF across different phenotypes, namely heart failure with reduced ejection fraction (HFrEF) and preserved ejection fraction (HFpEF). A comprehensive literature

A Case Series on Rhabdomyolysis in a Tertiary Care Centre.

Rhabdomyolysis is a condition characterized by the breakdown and necrosis of muscle tissue, leading to the release of intracellular components into the bloodstream. It results from the destruction of the sarcolemmal membrane, which allows these substances to enter circulation. The clinical presentation can vary widely - from asymptomatic elevations in serum muscle enzymes to severe complications, such as volume depletion, metabolic and electrolyte imbalances, and acute kidney injury (AKI). In this report, we present three cases of rhabdomyolysis treated at Rajiv Gandhi Government General Hospi

Autosomal Dominant Polycystic Kidney Disease and Idiopathic Arginine Vasopressin Deficiency: A Peculiar Case Report of Accelerated Kidney Function Decline.

Autosomal dominant polycystic kidney disease (ADPKD) is a common genetic kidney disorder characterized by progressive cyst growth and kidney impairment. Arginine vasopressin deficiency (AVP-D) is a rare disorder resulting from reduced arginine vasopressin production, causing polyuria and thirst. The coexistence of ADPKD and AVP-D is rarely documented in the literature. We report what may be the first documented case of a patient diagnosed with ADPKD and idiopathic AVP-D. Initially managed with intranasal desmopressin, the patient's kidney function declined earlier than expected based on her AD

Association Between Hyperuricemia and Dehydration in Children With Acute Gastroenteritis.

OBJECTIVES: Acute gastroenteritis (AGE) is one of the most common diseases in children, and detecting dehydration status is important for managing it. Volume depletion and altered urate excretion through the gastrointestinal tract can cause hyperuricemia. This study aimed to evaluate the role of hyperuricemia in assessing the severity of dehydration in children. METHODS: This retrospective study included patients aged 7 to 18 years who were diagnosed with AGE. We used both the clinical dehydration scale and measured amount of weight loss at the time of emergency room visiting to evaluate the
