Three-Dimensional Concept in Simultaneous Periareolar Augmentation-Mastopexy.

BACKGROUND: Breast ptosis commonly arises from tissue laxity associated with aging, weight fluctuations, pregnancy, or lactation, often accompanied by upper-pole volume depletion. Post-lactational atrophy further exacerbates ptosis, causing physical and psychological distress. Traditional mastopexy techniques typically focus on vertical tightening and may insufficiently address horizontal glandular laxity. This study evaluates a three-dimensional (3D) conceptual approach for simultaneous augmentation-mastopexy via a circumareolar incision, aiming to correct mild-to-moderate breast ptosis and r

A Diagnostic Blind Spot in Pseudo-Bartter Syndrome: Aberrant Renin-angiotensin-aldosterone System Activation from Focal Renal Scarring.

A 40-year-old woman presented with intermittent limb weakness, edema, and persistent hypokalemia. She had a history of recurrent urinary tract infections, but no diuretic or laxative use. Laboratory findings revealed metabolic alkalosis with elevated plasma renin activity and aldosterone levels. The urinary electrolyte response to fluid administration suggested pseudo-Bartter syndrome due to the volume depletion. Abdominal imaging revealed renal scarring, suggesting focal renin release from the ischemic regions. This case highlights the importance of recognizing secondary systemic renin-angiot

TRPV1-expressing C-fiber afferents preferentially drive renal sympathetic nerve activity in response to acute administration of low-dose furosemide in rats.

This study investigates the role of renal nerve afferents in sympathetic vasomotor responses during acute low-dose furosemide administration intravenously or directly in the renal pelvis. We hypothesized that furosemide activates renal nerve afferents, modulating sympathetic vasomotor activity. To test this hypothesis, we conducted simultaneous recordings of renal sympathetic nerve activity (rSNA) and splanchnic sympathetic nerve activity (sSNA) in Wistar rats. The effects of intravenous furosemide infusion (1 mg/kg/h) on mean arterial pressure, heart rate, rSNA, and sSNA in control (CTRL, n =

Inconsistent Pulse Oximetry Signal in Multiple Sites as an Early Indicator of Severe Hypovolemia.

A low-quality pulse oximetry signal should not be overlooked, as this may indicate cases of severe hypovolemia. Although this sign is not specific for volume depletion, it can provide a quick, non-invasive indication about possible hypovolemia if a proper signal cannot be obtained, especially when difficulties are encountered in obtaining SpO2 readings from multiple sites. Severe hypovolemia is a preventable risk factor for shock and other electrolyte abnormalities, which, if untreated, may lead to cardiovascular collapse or even necessitate cardiopulmonary resuscitation (CPR). This case repor

[Myth "Volume Deficiency in Sepsis"].

Septic shock is a life-threatening condition caused by a dysregulated immune response to an infection and can lead to tissue and organ damage and ultimately to death. Early recognition and elimination of tissue perfusion loss caused by sepsis are key elements in the treatment of patients with septic shock. Hemodynamic management is divided into four phases. 1) initial phase, 2) optimization, 3) stabilization, 4) de-escalation. Fluid therapy is widely accepted in the initial phase, but recent evidence challenges its universality, highlighting fluid overload risks and increased mortality, necess

Analyzing and predicting patient admissions related to acute heat at the Chemnitz Hospital (Germany).

BACKGROUND: Climate change is leading to an increase in acute heatwaves, thereby raising risks to human health and healthcare systems. The aim of this study was to quantify the relationship between heat and hospital admissions, and to develop a predictive model for patient admissions. METHODS: A retrospective secondary data analysis was conducted for the period 2018-2023, using data from Chemnitz Hospital and the Regional Climate Information Service of TU Dresden. Hospital admissions with the following diagnoses were analyzed: E86 (Volume depletion), I20 (Angina pectoris), I21 (Acute myocardi

Dynamic left ventricular outflow tract obstruction in critical illness: etiology-driven management and the role of bedside transthoracic echocardiography.

BACKGROUND: Dynamic left ventricular outflow tract obstruction (LVOTO) is an increasingly recognized life-threatening condition in critically ill patients, with ICU mortality rates reaching 53% in septic shock populations. Bedside transthoracic echocardiography (TTE) has become indispensable for early detection due to its diagnostic sensitivity and real-time monitoring capabilities in ICU settings. CASES: We present three prototypical cases managed at our tertiary center: (1) A 66-year-old male with volume depletion-induced LVOTO (peak gradient 29 mmHg) reversed by fluid resuscitation; (2) A

Volume Loading May Compromise Left Ventricular Filling in Patients with a Borderline Hypoplastic Left Ventricle.

Given the high prevalence of end-organ damage in the long term after the Fontan procedure, patients presenting with borderline hypoplastic left ventricle (LV) are increasingly directed towards biventricular circulation. We present the case of a patient with borderline LV who developed severe pulmonary hypertension shortly after biventricular repair. We initially thought it was purely caused by a hypoplastic LV; however, it was partly induced by a delayed/prolonged right ventricular (RV) contraction that compressed the interventricular septum and impaired left ventricular filling, which was exa

Severe Rhabdomyolysis in an Elderly Patient With Diabetes and Vascular Disease: Interplay of Statin Therapy, Sodium-Glucose Cotransporter-2 (SGLT2) Inhibition, and Thiazide-Induced Hypokalemia.

Rhabdomyolysis is a potentially life-threatening complication of drug therapy, particularly in elderly patients exposed to polypharmacy. Statins are a well-known cause, while thiazide-induced hypokalemia and volume depletion can further predispose to muscle injury. Sodium-glucose cotransporter-2 (SGLT2) inhibitors such as empagliflozin provide cardio-renal benefits but may potentiate statin toxicity and exacerbate dehydration and electrolyte imbalance, amplifying rhabdomyolysis risk. We report a 78-year-old woman with diabetes, hypertension, hyperlipidemia, and carotid artery disease, hypothyr

"Sweating Kidneys" refers to perirenal effusion as a complication of SGLT2i: A case reports.

A 76-year-old male, diagnosed with type 2 diabetes one year prior, was prescribed dapagliflozin. Imaging revealed perirenal effusion resembling "sweating kidneys".The exact mechanism of SGLT2 inhibitor-induced perirenal exudation remains unclear, but hypotheses include volume depletion that leads to increased renal capsule permeability, inflammation of urinary infections that extends to the kidneys, and hypersensitivity reactions that cause swelling and fluid exudation.

Initial factors associated with in-hospital mortality in both critically ill and non-critically ill hospitalized patients with acute kidney injury in Northern Tanzania: a single center cohort study.

Acute kidney injury (AKI) is prevalent in Intensive Care Unit settings, with rates exceeding 50%. While many studies from sub-Saharan Africa focus on critically ill AKI patients, limited data exist on non-critically ill patients, hindering effective dialysis prioritization. Studies from developed countries suggest AKI is also common in non-critical settings. This study aimed to assess mortality rates among critically ill and non-critically ill hospitalized AKI patients and identify early mortality predictors at the time of AKI diagnosis. A single-center prospective cohort study was conducted a

Acute Kidney Injury After Accelerated Dosing of Tirzepatide in a Patient with Multiple Comorbidities: A Case Report.

BACKGROUND Tirzepatide is effective for glycemic control and weight management in type 2 diabetes and obesity. Clinical trials have demonstrated tirzepatide's lower risk of acute kidney injury (AKI) compared with existing glucagon-like peptide-1 receptor agonists, along with benefits including reduced albuminuria and stable estimated glomerular filtration rate. Rare cases of AKI have been reported, potentially associated with dehydration from gastrointestinal side effects, polypharmacy, or comorbidities. We describe AKI in a non-diabetic, multimorbid patient after rapid tirzepatide dose escala

Acute Kidney Injury from Mononuclear Cell-Predominated Interstitial Nephritis After Introduction of a Glucagon-Like Peptide-1 Receptor Agonist: A Case Report.

BACKGROUND Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have become important in the treatment of diabetic kidney disease (DKD). Despite their reno-protective effect, kidney injury from GLP-1 RAs has been rarely reported. The reported kidney injuries varied from mild symptoms to dialysis at presentation and occurred 2 days to 2 years after drug initiation. We report a case of a patient with DKD who developed an episode of interstitial nephritis after dulaglutide administration. CASE REPORT A 63-year-old woman with stage 3b diabetic kidney disease and depressive disorder was prescribed

Choosing the right antihypertensive drug to avoid intradialytic hypotension.

Intradialytic hypotension (IDH) is a significant complication in haemodialysis (HD) patients, affecting cardiovascular stability and treatment outcomes. Antihypertensive medications, while crucial for blood pressure control, can exacerbate IDH, necessitating careful drug selection and management. This review highlights the differential effects at the HD population level of drug classes such as beta and alpha-beta blockers, angiotensin-converting enzyme inhibitors (ACEIs), angiotensin II antagonists, diuretics and calcium antagonists. Beta and alpha-beta blockers are linked to a higher risk of

Effect of Sodium-Glucose Co-transporter 2 (SGLT2) Inhibitors in the Management of Hyponatremia Associated With Syndrome of Inappropriate Antidiuresis.

BACKGROUND: Hyponatremia due to syndrome of inappropriate antidiuresis (SIAD) is a common and challenging electrolyte disorder, particularly in hospitalized and elderly patients. Conventional treatments, such as fluid restriction and vasopressin receptor antagonists, have limitations related to efficacy, tolerance, availability, and cost. Sodium-glucose co-transporter 2 inhibitors (SGLT2i), primarily used for type 2 diabetes, have shown potential in promoting osmotic diuresis and free water clearance, making them a promising therapeutic alternative for SIAD-induced hyponatremia. METHODOLOGY:

Comprehensive Overview of Hypernatremia: Pathophysiology, Diagnosis, and Management.

Hypernatremia, defined as an elevated serum sodium concentration, is a critical electrolyte imbalance with significant neurological and systemic effects. Effective management hinges on understanding the condition's underlying aetiology, such as dehydration, excessive sodium intake, or impaired renal water handling. The approach to treatment varies by onset and severity: acute hypernatremia (<48 hours) necessitates rapid but controlled correction to prevent complications like vascular rupture or cerebral bleeding, while chronic hypernatremia (≥48 hours) requires gradual intervention to mitigate

The relevance of acute kidney injury in dehydrated children.

INTRODUCTION: Dehydration is the leading cause of acute kidney injury in children, yet its true burden and clinical relevance in the general pediatric population remain poorly characterized. The majority of existing research on pediatric acute kidney injury primarily focuses on cases occurring after cardiac surgery or in critically ill children, leaving dehydration-associated AKI understudied. METHODS: A PRISMA-guided systematic search of PubMed and EBSCO databases was conducted to identify studies published from 2010 to 2025 investigating pediatric acute kidney injury due to dehydration.
