Limitations of Oral Mucosal Moisture Assessment as an Indicator of Intravascular Dehydration in Postoperative Cardiovascular Surgical Patients in the ICU: A Prospective Pilot Diagnostic Accuracy Study.

BACKGROUND: Oral mucosal moisture (OMM) measured by using a portable device (Mucus®, LiFE Co., Ltd., Koshigaya, Saitama, Japan; approval No. 22200BZX00640000) quantifies tongue moisture, yet its diagnostic value for intravascular volume depletion (hypovolemia) in critically ill patients remains unclear. OBJECTIVES: To evaluate the diagnostic accuracy of OMM for hypovolemia in postoperative cardiovascular surgical patients in the intensive care unit (ICU). METHODS: The present single-center, prospective diagnostic accuracy study (Japan, Nov 2024-Sep 2025) enrolled consecutive postoperative ca

From Congestion to Cardiorenal Protection: The New Therapeutic Balance Between SGLT2 Inhibitors and Loop Diuretics in Heart Failure.

Sodium-glucose cotransporter-2 inhibitors (SGLT2i) have redefined the therapeutic landscape of heart failure (HF), both with reduced (HFrEF) and preserved (HFpEF) ejection fraction. Historically, treatment has relied on diuretics to relieve congestion, with limited prognostic impact and dose-related adverse effects. SGLT2i, originally developed for type 2 diabetes mellitus, have demonstrated in randomized trials a significant reduction in hospitalizations and cardiovascular mortality, with benefits extending to non-diabetic patients. SGLT2 inhibitorsTheir mechanism combines moderate osmotic na

Acute kidney injury after chimeric antigen receptor T-cell therapy is associated with inferior survival in patients with relapsed/refractory large B-cell lymphoma.

Baseline risk factors for acute kidney injury (AKI) during chimeric antigen receptor T-cell (CAR-T) therapy are not well described. Hence, we evaluated the incidence and risk factors associated with AKI among patients undergoing CAR-T for relapsed/refractory large B-cell lymphoma. Among 155 patients, 28 (18%) developed AKI with a median time-to-peak creatinine from CAR-T of 9.5 days (range = 3-30). Aetiologies included volume depletion (n = 20, 71%), cytokine release syndrome (n = 5, 18%), nephrotoxins (n = 5, 18%), tumour lysis syndrome (n = 1, 4%) and multifactorial (n = 6, 21%). On univaria

Volume Assessment in Patients Undergoing Long-Term Dialysis.

Accurate assessment of fluid status is a priority for patients with kidney failure undergoing long-term dialysis. There is wide variation in current volume-related practices between dialysis units and an urgent need to develop better evidence to guide practice. Clinical decisions relating to volume management are implicitly based on assessment of volume status, and there are numerous different but imperfect methods of assessment. Isotope-based dilutions are impractical for clinical use and may not be a gold standard for patients with kidney failure. Individual trends in body weight and BP have

Safety and efficacy of early initiation of sodium-glucose co-transporter inhibitors 2 in patients hospitalized for acute heart failure: A meta-analysis of randomized controlled trials.

AIMS: Data on the early use of sodium-glucose co-transporter 2 inhibitors (SGLT2i) in patients with acute heart failure (HF) are conflicting, and mostly evaluating soft endpoints (i.e., indices of congestion, renal function, ejection fraction, and diuresis). The aim was to perform a meta-analysis of randomized controlled trials (RCTs) to assess their impact after an HF decompensation event. METHODS AND RESULTS: Two electronic databases were screened for eligible studies. Efficacy endpoints were all-cause death, cardiovascular death, HF hospitalization, length of hospital stay, and N-terminal

Secondary hyperaldosteronism associated with severe infantile atopic dermatitis: a case series.

Recent literature has highlighted an association between severe atopic dermatitis (AD) in infancy and biochemical disturbances consistent with secondary hyperaldosteronism. Transepidermal sodium loss driving volume depletion has been hypothesized to contribute to this process, but direct evidence of the underlying mechanisms remains sparse. We present a case series of 3 children, aged 3 to 7 months, who were admitted with severe exudative eczema associated with hyponatremia and hyperkalemia, despite evidence of effective renin-angiotensin-aldosterone system activation and renal mineralocortico

Adenine at lower doses acts in the kidney as an aquaretic agent and prevents hyponatremia.

We have previously reported that adenine at high doses interferes with the vasopressin signaling pathway, causes massive diuresis and volume depletion, and ultimately leads to renal failure. In the present study, we examined the effects of adenine on renal salt and water handling in a time course and dose-response study in rats housed in metabolic cages and fed control or adenine-containing diet at 1500, 2000, 2500 mg/kg and euthanized after 1, 3, and 7 weeks. Adenine at 2000 and 2500 mg/kg caused early and significant polyuria, polydipsia, and decreased urine osmolality in a dose-dependent ma

The influence of fluid resuscitation strategy on outcomes from dengue shock syndrome: a review of the management of 691 children in 7 Southeast Asian hospitals.

INTRODUCTION: The pathognomonic feature of dengue shock syndrome (DSS) is a transient capillary leak syndrome resulting in profound intravascular volume depletion. WHO management guidelines recommend particular parenteral fluid regimens during the critical leakage phase, including synthetic colloid solutions in certain circumstances. We set out to describe the actual fluid management strategies employed in different settings and to investigate relationships with clinical outcomes. METHODS: We performed a retrospective review of paediatric DSS cases managed at seven hospitals across Malaysia,

Synergistic Effects of Thread Lifting, Autologous Fat Grafting, and Multi-Modal Laser Therapy in Facial Rejuvenation.

BACKGROUND: Facial aging is a multifactorial process involving tissue descent, volume depletion, and dermal degeneration. Minimally invasive modalities like thread lifting, autologous fat grafting, and laser treatments address different aging components. However, their synergistic impact remains underexplored. OBJECTIVES: The aim of the author of this study is to assess the clinical efficacy and histologic rationale of combining permanent thread lifting, autologous fat grafting, and nonablative laser therapy for comprehensive facial rejuvenation. METHODS: In this retrospective study, a total

Pembrolizumab-Associated Unilateral Optic Neuritis.

This study aimed to assess the effects of dehydration on echocardiographic indices in healthy cats: specifically, it aimed to assess the effects of volume depletion on diastolic function. Nine experimental cats were subjected to both a dehydration and placebo protocol separated by a 21-day washout period. Echocardiography was performed at baseline and on completion of each protocol. Results were compared between the two protocols. Volume depletion was induced by intravenous administration of furosemide. Volume depletion showed a significant association with increased interventricular septal an

Excess dietary sodium partially restores salt and water homeostasis caused by loss of the endoplasmic reticulum molecular chaperone, GRP170, in the mouse nephron.

The maintenance of fluid and electrolyte homeostasis by the kidney requires proper folding and trafficking of ion channels and transporters in kidney epithelia. Each of these processes requires a specific subset of a diverse class of proteins termed molecular chaperones. One such chaperone is GRP170, which is an Hsp70-like, endoplasmic reticulum (ER)-localized chaperone that plays roles in protein quality control and protein folding in the ER. We previously determined that loss of GRP170 in the mouse nephron leads to hypovolemia, electrolyte imbalance, and rapid weight loss. In addition, GRP17

Abrupt Decline in Estimated Glomerular Filtration Rate after Initiating Sodium-Glucose Cotransporter 2 Inhibitors Predicts Clinical Outcomes: A Systematic Review and Meta-Analysis.

BACKGRUOUND: The initiation of sodium-glucose cotransporter-2 inhibitors (SGLT2i) typically leads to a reversible initial dip in estimated glomerular filtration rate (eGFR). The implications of this phenomenon on clinical outcomes are not well-defined. METHODS: We searched MEDLINE, Embase, and Cochrane Library from inception to March 23, 2023 to identify randomized controlled trials and cohort studies comparing kidney and cardiovascular outcomes in patients with and without initial eGFR dip after initiating SGLT2i. Pooled estimates were calculated using random-effect meta-analysis. RESULTS:

The role of lung and cardiac ultrasound for cardiovascular hemodynamic assessment of women with preeclampsia.

Preeclampsia remains the leading cause of maternal morbidity and mortality and is associated with abnormal body fluid homeostasis and cardiovascular dysfunction. Moreover, 2 distinct hemodynamic phenotypes have been described in preeclampsia, which might require different therapeutic approaches. Fluid restriction is mandatory in women at risk of pulmonary edema, whereas additional fluid administration may be required to correct tissue hypoperfusion in women with intravascular volume depletion. As clinical examination alone cannot discriminate among different hemodynamic patterns, optimal manag

Bilateral Renal Ischemia-Reperfusion Model for Acute Kidney Injury in Mice.

Acute kidney injury (AKI) is defined as a rapid decline in renal function, in which persistent kidney dysfunction gradually progresses to chronic kidney disease (CKD) due to the irreversible loss of nephrons and their maladaptive repair. In recent years, the incidence of AKI has been increasing concerning diverse etiologies, including volume depletion, sepsis, nephrotoxicity, muscle injury, and major trauma, in which ischemia-reperfusion injury (IRI) accounts for most episodes. Development of the IRI model in mice is induced by surgical clamping of the renal pedicles, which provides powerful a

The usefulness of point-of-care ultrasound in dehydrated patients in a pediatric emergency department.

BACKGROUNDS: Dehydration is among the most common causes of Pediatric Emergency Department admission; however, no clinical signs, symptoms, or biomarkers have demonstrated sufficient sensitivity, specificity, or reliability to predict dehydration. METHODS: We conducted a prospective, monocentric, observational study at Giannina Gaslini Hospital, a tertiary care pediatric hospital. Our study aimed to compare inferior vena cava ultrasound measurement with volume depletion biomarkers to understand if point-of-care ultrasound could help grade, evaluate, and better manage dehydration in children p

Dyselectrolytemia in Children With Severe Pneumonia: A Prospective Study.

Background Pneumonia is a condition characterized by inflammation of the lung parenchyma. It is one of the leading causes of mortality in children below five years of age. While predominantly prevalent in developing countries, it is also associated with significant healthcare-associated costs in developed countries. Among the many risk factors for childhood pneumonia, incomplete immunization, nonexclusive breastfeeding for less than six months, delayed weaning, poor household air quality, malnutrition, and low birth weight are the most commonly found. Electrolyte disturbances, also known as dy

Positive correlation between echocardiographic tricuspid E peak velocity and central venous pressure in dogs: A preliminary study.

In the absence of vascular obstruction, central venous pressure (CVP) is a hydrostatic pressure in the cranial and caudal vena cava, providing valuable information about cardiac function and intravascular volume status. It is also a component in evaluating volume resuscitation in patients with septic shock and monitoring patients with right heart disease, pericardial disease, or volume depletion. Central venous pressure is calculated in dogs by invasive central venous catheterization, which is considered high-risk and impractical in critically ill patients. This study aimed to investigate the

[SGLT2 inhibitors, also in frail older adults?].

Sodium-glucose cotransporter 2 inhibitors (SGLT2 inhibitors) have gained prominence in the treatment of diabetes mellitus type 2, heart failure, and chronic kidney disease. However, concerns arise for frail older adults, given their underrepresentation in trials and heightened susceptibility to adverse drug events. This review summarizes the clinical effects of SGLT2 inhibitors in older adults with frailty. SGLT2 inhibitors seem to exhibit consistent cardiovascular benefits irrespective of age. As such, these drugs can be beneficial for older adults with 'cardiovascular frailty': in other word

Renal Replacement Therapy in Cirrhosis: A Contemporary Review.

Acute kidney injury is a common complication of decompensated cirrhosis, frequently requires hospitalization, and carries a high short-term mortality. This population experiences several characteristic types of acute kidney injury: hypovolemic-mediated (prerenal), ischemic/nephrotoxic-mediated (acute-tubular necrosis), and hepatorenal syndrome. Prerenal acute kidney injury is treated with volume resuscitation. Acute-tubular necrosis is treated by optimizing perfusion pressure and discontinuing the offending agent. Hepatorenal syndrome, a unique physiology of decreased effective arterial circul
