Navigating Heart Failure and Atrial Fibrillation Therapy in MELAS.

BACKGROUND: Mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) is a mitochondrial DNA disorder in which medications that impair mitochondrial function may worsen disease manifestations. Management of heart failure and arrhythmias in this population is therefore challenging. CASE SUMMARY: A 54-year-old woman with MELAS presented with acute hypoxemic respiratory failure, atrial fibrillation with rapid ventricular response, and newly reduced left ventricular ejection fraction. Transesophageal echocardiography demonstrated a left atrial appendage thrombus, precl

Changes in ruminal lipopolysaccharide, fermentation characteristics, nutrient digestibility, blood variables, and microbiota in sheep with subacute ruminal acidosis and its regulation by polymyxin CP7.

The objectives of this study were to (1) determine the effects of induced subacute ruminal acidosis (SARA) on ruminal lipopolysaccharide (LPS), nutrient apparent digestibility, ruminal fermentation parameters, hematological indices, and ruminal microbiota; and (2) evaluate the responses associated with polymyxin CP7 supplementation during SARA in Hu sheep. Eight rumen-fistulated Hu sheep sequentially underwent three 29-day stages: a control stage (CON, basal diet), a SARA induction stage (SAR, high-barley diet), and a bacteriocin-supplemented stage (BAC, high-barley diet with 500 mg/kg polymyx

Ethylene Glycol Poisoning Without Fomepizole in a Regional Australian Intensive Care Unit: The Lactate Gap as the Diagnostic Key and the Consequences of Nasogastric Ethanol Therapy.

A 62-year-old man presented comatose with pH 6.88 and no ingestion history. A point-of-care lactate above 31 mmol/L with a laboratory lactate of 0.8 mmol/L revealed a lactate gap, diagnosing ethylene glycol poisoning despite a near-normal osmolar gap. High-dose continuous renal replacement therapy corrected the acidosis; he survived.

Robot-assisted ileal ureter replacement for complex ureteral strictures: a systematic review and single-arm meta-analysis of surgical and functional outcomes.

Complex, long-segment ureteral strictures present significant reconstructive challenges. While open ileal ureter replacement is the historical gold standard, it carries substantial perioperative morbidity. This systematic review and single-arm meta-analysis evaluate the surgical and functional outcomes of robot-assisted ileal ureter replacement (RIUR). Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search of four databases was conducted through September 2026 to identify studies reporting adult RIUR outcomes for complex strictu

Validation of the discriminatory performance of the iPREFACE score for clinically significant fetal acidemia: an outcome-enriched nested case-control study.

BACKGROUND: Cardiotocography is widely used for monitoring fetal well-being during labor; however, its clinical utility remains limited due to a high false-positive rate, particularly in predicting adverse neonatal outcomes such as cerebral palsy. Despite its implementation, the incidence of cerebral palsy has not decreased, whereas cesarean section rates have increased. Severe fetal acidemia has been shown to be associated with neonatal brain injury. The iPREFACE score was developed to address this limitation by evaluating the accumulation of fetal stress due to uterine contractions based on

Influence of lactate on the active force produced in skinned psoas and soleus muscle fibers from female rabbits.

Lactate has been recognized as an energy source, suggesting its potential use as an ergogenic supplement. However, its effect on muscle contraction remains unclear. This study examined the effect of lactate on muscle active force using skinned fibers. Rabbit psoas (N = 8) and soleus (N = 5) fibers were activated using three activating solutions as follows: (1) activating solution, pH 7.0 (Normal); (2) activating solution with lactate, pH 6.0 (Lactate pH 6.0); (3) activating solution with lactate, pH 7.0 (Lactate pH 7.0). Under these conditions, the fibers were isometrically activated at an ave

Early-onset autosomal dominant distal renal tubular acidosis caused by a de novo SLC4A1 R589C variant with growth failure and rickets.

Autosomal dominant (AD) distal renal tubular acidosis (dRTA) generally has a later onset and milder clinical course than autosomal recessive forms. However, delayed diagnosis may result in prolonged metabolic acidosis and severe growth impairment. Here we report the case of a Japanese boy with a de novo SLC4A1 (NM_000342.4):c.1765C > T (p.Arg589Cys) variant who presented with marked growth failure from 6 months of age and was diagnosed with AD-dRTA at 27 months. Because severe short stature was the predominant clinical feature, venous blood gas analysis was performed and revealed metabolic aci

Urgent Start of Peritoneal Dialysis Versus Hemodialysis in Kidney Failure in Severe Metabolic Acidosis: A Prospective Study.

BACKGROUND: Severe metabolic acidosis is a life-threatening complication of kidney failure and a common indication for urgent KRT. Although hemodialysis (HD) is traditionally considered the preferred modality for rapid acid-base correction, comparative evidence with urgent-start peritoneal dialysis (PD) remains limited. METHODS: We conducted a prospective cohort study including 68 consecutive dialysis-naïve patients with kidney failure and severe metabolic acidosis requiring urgent KRT at a tertiary referral center (HD n=31; PD n=37). Modality selection was based on nephrologist clinical judg

Targeting the Tumor Microenvironment Network to Overcome Immunotherapy Resistance in Non-Small Cell Lung Cancer: Mechanisms to Clinical Translation.

BACKGROUND: Immunotherapy has transformed the treatment landscape for advanced non-small cell lung cancer (NSCLC), yet resistance mediated by the tumor microenvironment (TME) limits its efficacy in most patients. This review aims to synthesize the current understanding of the TME as an integrated network driving immunotherapy resistance and to evaluate emerging therapeutic strategies aimed at overcoming it. METHODS: We conducted a comprehensive narrative review of key literature in the field, sourced from PubMed and major conference proceedings. The focus was on mechanistic studies elucidatin

Effects of correcting metabolic acidosis in chronic kidney disease patients: An umbrella review.

UNLABELLED: Metabolic acidosis is a frequent complication when chronic kidney disease (CKD) develops, plus it is linked to adverse outcomes such as muscle wasting, inflammation, and progression of kidney disease. This article is intended to evaluate how correcting metabolic acidosis affects adult CKD patients via summarizing evidence from systematic reviews with meta-analyses. We conducted an umbrella review. Systematic reviews with meta-analyses assessing alkali therapy in CKD patients were included. Methodological quality was assessed using a Measurement Tool to Assess Systematic Reviews (AM

The Urine Ammonium-pH Index is a Measure of Kidney Tubular Function.

Dysfunction of the tubulointerstitial compartment is a key driver of chronic kidney disease (CKD) progression. However, tubular function remains largely unaddressed by routine clinical assessment. Here, we show that the urine ammonium-pH index (uAPI), a composite of urinary ammonium and pH, reflects kidney tubular function and predicts kidney function decline. Using acid/base, dietary, and potassium perturbations, segmental disruption of tubular ammonium handling, mathematical modelling, and data from patients with renal tubular acidosis, we identified defective ammoniagenesis as the main uAPI

[Successful treatment of hypokalemic cardiac arrest with massive potassium replacement in a patient with Bricker urinary diversion].

Severe hypokalemia is a rare but potentially reversible cause of malignant arrhythmias and cardiac arrest. Early recognition of significant fluid, electrolyte, and acid-base disturbances in patients with intestinal urinary diversion may be life-saving. We present the clinical course of a 55-year-old woman who developed severe hyperchloremic metabolic acidosis and extreme hypokalemia following prolonged vomiting and high-output fluid loss through a Bricker ureteroileocutaneostomy. The patient was admitted to the intensive care unit in a peri-arrest condition, where cardiac arrest occurred short

The radiologist's dilemma: Differentiating non-ischemic pneumatosis intestinalis from ischemic bowel necrosis.

Pneumatosis intestinalis (PI) is defined by gas accumulation within the gastrointestinal wall and is frequently perceived as an indication of life-threatening bowel ischemia necessitating surgical intervention. However, PI can manifest as a benign entity, generating a significant diagnostic challenge between surgical and conservative management strategies. A 78-year-old male with chronic obstructive pulmonary disease (COPD) presented with mild abdominal pain and extensive small bowel intramural gas identified on computed tomography (CT), devoid of hemodynamic instability or lactic acidosis. [Diabetic ketoacidosis in cats: pathogenesis and diagnosis].

Diabetic ketoacidosis represents a serious complication of diabetes mellitus. In 1/3 to 2/3 of reported cases, diabetes mellitus is known beforehand. At presentation, affected cats are severely ill. Anorexia, vomiting and dehydration are the most common signs and affected cats usually present with volume depletion. Hepatic lipidosis and pancreatitis are frequent concurrent conditions. It is necessary for the disease to be promptly recognized and diagnosed in order to initiate necessary therapeutic measures as rapidly as possible in order to improve the prognosis of affected animals. Antenatal oxytocin and respiratory adaptation after prelabor cesarean delivery in late-preterm lambs: a randomized factorial experiment.

BACKGROUND: Late-preterm infants born at 34-36+6 weeks' gestation and delivered by elective cesarean section before the onset of labor are at high risk of neonatal respiratory distress, in which impaired fetal lung fluid clearance plays a major role. No preventive strategy specifically targeting this mechanism is currently available. OBJECTIVE: To evaluate whether antenatal exposure to oxytocin, alone or combined with dexamethasone, improves early neonatal respiratory adaptation in a late-preterm lamb model delivered by cesarean section. STUDY DESIGN: In this randomized 2 × 2 factorial study

An Infant With Extreme Hypernatremia Treated With Peritoneal Dialysis.

Reports of survival of infants with serum sodium levels exceeding 200 mmol/L are extremely rare. We describe a 34-day-old, 3.6-kg male infant who developed extreme hypernatremia (serum sodium >200 mmol/L) and multiple organ dysfunction syndrome following accidental salt poisoning. When continuous renal replacement therapy (CRRT) was not feasible because of the patient's low body weight and lack of appropriately sized equipment, peritoneal dialysis (PD) was initiated as rescue therapy. Within 24 hours, serum sodium decreased to 186.7 mmol/L, with parallel improvements in metabolic acidosis, coa

Suspected Mesenteric Malperfusion in Acute Type A Aortic Dissection During Pregnancy: A Case Report and Review of Reperfusion-First Strategies.

Aortic dissection during pregnancy is rare but potentially catastrophic. Acute Type A aortic dissection generally requires urgent surgical repair. When visceral malperfusion is present, however, the optimal sequence of interventions may be complex, particularly when pregnancy and recent cesarean delivery increase operative risk. Endovascular reperfusion followed by delayed aortic repair has been proposed in selected patients, although evidence supporting this strategy in pregnancy remains limited. A 37-year-old woman at 31 weeks' gestation presented with chest and back pain. Initial CT pulmona

Hyperglycemic Hyperosmolar State As the Initial Presentation of Type 1 Diabetes Mellitus in an Adolescent: A Case Report.

Hyperglycemic hyperosmolar state (HHS) is a rare but life-threatening hyperglycemic emergency in children and adolescents. It is characterized by severe hyperglycemia, hyperosmolality, and dehydration, with absent or limited ketoacidosis. Although more commonly associated with type 2 diabetes mellitus, HHS may also occur as the initial presentation of type 1 diabetes mellitus in pediatric patients. We report the case of a previously healthy 13-year-old boy presenting with a three-week history of polydipsia and nocturnal polyuria. Initial evaluation revealed profound hyperglycemia (>1,000 mg/dL
