Metabolic causal factor of immune cell function and fate in the tumor microenvironment.

This study provides an in depth review of understanding the metabolic regulation of immune cells in cancer. Evidence has accumulated in the rapidly developing field of immune metabolism, in which immune cell function and fate are closely linked to their cellular metabolic programs, particularly within the tumor microenvironment. The TME is a metabolically disadvantaged niche characterized by hypoxia, nutrient deprivation, acidosis, and accumulation of immunosuppressive metabolites, all of which result from intense competition for essential resources such as glucose and amino acids. These metab

Visualizing extracellular vesicles in cancer: from biogenesis to theranostic applications.

Within the complex landscape of the tumor microenvironment(TME), extracellular vesicles(EVs) function as sophisticated nanoscale signaling hubs that govern the intercellular crosstalk driving metastatic progression and immune modulation. Despite their burgeoning potential as next-generation biomarkers and biocompatible nanocarriers, the clinical translation of EVs is currently impeded by an incomplete elucidation of their complex spatiotemporal kinetics in vivo. This review provides a critical synthesis of the EV pipeline, from microenvironment-modulated biogenesis to advanced theranostic appl

SLC4A1 mutations that cause distal renal tubular acidosis alter cytoplasmic pH and cellular autophagy.

Distal renal tubular acidosis (dRTA) is a disorder characterized by the inability of the collecting duct system to secrete acids during metabolic acidosis. The pathophysiology of dominant or recessive SLC4A1 variant-related dRTA has been linked with the mis-trafficking defect of mutant kAE1 protein. However, in vivo studies in kAE1 R607H dRTA mice and humans have revealed a complex pathophysiology implicating a loss of kAE1-expressing intercalated cells and intracellular relocation of the H+-ATPase in the remaining type-A intercalated cells. These cells also displayed accumulation of ubiquitin

Colistin-Associated Acute Kidney Injury in Burn Patients: Incidence, Associated Factors and Prognosis.

Colistin is often essential in burn patients presenting infections caused by multidrug-resistant organisms. A retrospective study was conducted in the intensive burn care department in Tunisia over 14 months to assess prevalence and predictive factors of colistin-related acute kidney injury (AKI). Among 70 patients (mean age 34±13 years), 28.6% developed AKI, with onset at 5±2 days. Associated factors included total burned surface area >41% (OR=2.6), UBS score >58 (OR=6.8), metabolic acidosis at admission (OR=28) and contrast agent injection (OR=107). AKI was reversible in 40% of cases. Colist

Acute liver failure and severe lactic acidosis as initial manifestations of primary intestinal diffuse large B-cell lymphoma: Case report.

Diffuse large B-cell lymphoma (DLBCL) typically presents with lymphadenopathy or extranodal masses, but rarely, predominantly as scattered case reports, as acute liver failure (ALF) with severe lactic acidosis. We report a 79-year-old man who presented with fulminant ALF, leukemoid reaction, and extreme lactic acidosis. Despite intensive care, he rapidly progressed to multiorgan failure and died. Autopsy revealed primary intestinal DLBCL with diffuse hepatic infiltration. This case illustrates the diagnostic challenge in emergency settings and highlights the concept of a "metabolic catastrophe

Case of Infant With Metabolic Crisis From Lactational Hypoglycin A and Fatty Acid Defect.

Hypoglycin A is a mitochondrial toxin found in unripe soapberry fruits such as ackee. This toxin disrupts fatty acid β-oxidation, leading to severe metabolic derangements. Although toxic exposure through ingestion has been well documented, transfer through human milk is not well characterized. We report a full-term, exclusively breastfed 5-month-old infant, a carrier of medium-chain acyl-CoA dehydrogenase (MCAD) deficiency, who developed profound hypoketotic hypoglycemia, metabolic and respiratory acidosis, and vasoactive-refractory shock 2  days after maternal ingestion of unripe ackee fruit.

Evolutionary dynamics of acid resistance in tumors: A mathematical model.

Acidosis in tumors arises from reprogrammed metabolism and compromised vasculature, creating a harsh, acidic microenvironment that drives the evolutionary selection of acid-resistant cell phenotypes. A mathematical model was proposed to integrate phenotypic evolution, microenvironmental acidification, and tumor density dynamics. Three key mechanisms were incorporated in it: frequency-dependent selection favoring acid-resistant cells below a critical pH, stress-induced phenotypic switching, and a positive feedback loop where resistant cells produce excess acid that intensifies selection pressur

Review: Tail lesions in dairy cows - a summary of what's new about the old.

This review summarises the most relevant literature on tail necrosis in cattle, with a particular focus on dairy cows. Tail inflammation and necrosis are widespread phenomena that have been recognised as affecting entire herds since the early 1970s. Initial reports from various European countries described a new health issue affecting beef cattle, with prevalence rates reaching 100% and severely impairing animal welfare. A review of the literature shows that, since these initial reports, the focus has remained relatively consistent on internal causes relating to feed-related rumen acidosis in

An Exploratory Randomized Controlled Trial of 0.9% Saline Versus Lactated Ringer's Solution on Intraoperative Metabolism Among Patients Undergoing Lumbar Spinal Surgery.

INTRODUCTION: Intravenous crystalloid fluid infusion is a mandatory nutritional intervention received by surgical patients. Crystalloids vary in the pH value and electrolyte balance when administered; these effects directly alter plasma and urine compositions and can considerably affect the patient's intraoperative metabolism. METHODS: This randomized controlled study compared 0.9% saline solution and lactated Ringer's solution in terms of intraoperative metabolism among 56 patients undergoing lumbar spinal surgery. Blood and urine samples were obtained before and after surgery for arterial b

Genetic deletion of ASIC3 alters left ventricular remodeling and autonomic function after myocardial infarction in mice.

Cardiac afferent neurons have been shown to trigger overactivation of neurohormonal systems known to drive adverse cardiac remodeling following myocardial infarction (MI). Acid-sensing ion channels (ASICs) that are highly expressed in cardiac sympathetic afferents sense ischemia-induced myocardial acidosis. We hypothesized that genetic deletion of ASICs might abrogate disadvantageous remodeling after MI by disrupting afferent signaling pathways otherwise resulting in overactivation of neurohormonal responses. To test this, we induced MI in wild type (WT) and ASIC3-/- mice and assessed cardiac

Severe Lactic Acidosis With Multi-organ Dysfunction Secondary to Terlipressin-Induced Widespread Vasospasm in a Patient With Liver Cirrhosis and Oesophageal Variceal Bleeding: A Case Report.

Terlipressin is widely used for acute variceal bleeding and hepato-renal syndrome, but rare complications such as severe lactic acidosis with multi-organ dysfunction due to systemic vasospasm can occur. We report a 58-year-old male with type 2 diabetes and alcoholic liver cirrhosis who developed a lactate level of 20 mmol/L and a pH of 6.97. He became systemically unwell hours after terlipressin initiation. Further investigations excluded alternative causes of lactic acidosis. Clinical improvement occurred, followed by discontinuation of terlipressin and initiation of octreotide, highlighting

Forage-free diet in finishing cattle: Effects on performance, ruminal fermentation, and enteric methane mitigation.

Ruminant production is largely pasture-based, and although this system is beneficial during the cow-calf and growing phases, it often becomes nutritionally limiting during finishing, delaying slaughter and increasing greenhouse gas emissions. High-grain diets accelerate weight gain and may reduce enteric methane (CH4) emissions per unit of animal output, but they increase the risk of ruminal acidosis. Ionophores are effective in mitigating this disorder, yet their use is restricted in several countries, stimulating interest in sustainable alternatives such as plant-derived bioactive compounds.

Microenvironment plays a critical role in modulating tumor cell dormancy: Current perspectives and potential treatment options.

Tumor dormancy is a fundamental phenomenon in cancer biology, characterized by malignant cells that remain viable but non-proliferative, thereby frequently evading detection and treatment. This review examines the intricate role of the tumor microenvironment (TME) in regulating tumor cell dormancy. The TME encompasses a diverse array of components, including immune cells, extracellular matrix proteins, and soluble factors, all of which contribute to a dynamic interplay that influences tumor cell behavior. Key mechanisms involved in the maintenance of dormancy include immune surveillance, where

Serial determination of caffeine and active metabolites in a case of severe caffeine poisoning causing recurrent cardiac arrest after hemodialysis: a case report.

Caffeine is highly dialyzable, and cases of severe caffeine poisoning causing recurrent symptoms after hemodialysis (HD) and the serial evaluation of active metabolite and caffeine levels have rarely been reported. A previously healthy 17-year-old female with cardiac arrest due to ventricular fibrillation was transferred by ambulance to our emergency department. Laboratory tests revealed severe lactic acidosis and hypokalemia. As cardiac defibrillation was ineffective, veno-arterial extracorporeal membrane oxygenation was initiated after tracheal intubation. Eleven hours after admission, inges
