Pre-hospital fluid resuscitation and return of spontaneous circulation in earthquake victims with cardiac arrest during the first 24 hours after the 2023 Kahramanmaraş earthquakes: Implications for disaster risk management.

BACKGROUND: Earthquakes can cause crush injuries and post-extrication deaths. Hyperkalemia, metabolic acidosis, hypovolemia, and circulatory failure are key mechanisms of cardiac arrest in victims at risk of crush syndrome. Although intravenous fluid resuscitation is recommended in disaster care, evidence on resuscitation outcomes remains limited. This study evaluated earthquake victims with cardiac arrest during the first 24 h after the 2023 Kahramanmaraş earthquakes and examined the association between pre-hospital fluid resuscitation, admission metabolic parameters, and return of spontaneou

Kaolin integrated gellan gum composite sponge for rapid hemostasis.

Uncontrolled bleeding can result in severe complications, including hemorrhagic shock, hyperthermia, acidosis, coagulopathy, multiple organ failure, and infection. The development of hemostatic agents or devices capable of achieving rapid hemostasis is therefore critical to minimize excessive blood loss during surgical procedures. In this study, a novel kaolin-integrated, gellan gum based composite hemostat sponges were developed via ionic crosslinking mechanism. The established safety and efficacy of kaolin as a hemostatic agent were leveraged in the design, while a composite strategy was emp

Pathophysiology of aldosterone dysregulation in difficult-to-control hypertension.

Aldosterone, a key component of the renin-angiotensin-aldosterone system (RAAS), plays a central role in sodium balance, vascular tone, blood pressure control, and cardiovascular remodeling. Dysregulation of aldosterone mechanism, including excessive production, deficiency, and inappropriate receptor activation, represents a significant contributor to cardiovascular and renal disease which is often underrecognized. Aldosterone dysregulation may manifest as primary aldosteronism that is increasingly identified as a common cause of secondary hypertension, or by secondary hyperaldosteronism that

Dynamic effects of dietary shifts and temporal variation on the rumen microbiota of dairy goats differing in SARA tolerance.

Subacute ruminal acidosis (SARA) is a common metabolic disorder observed in intensive dairy production systems and is frequently associated with disruptions in rumen microbial homeostasis. In this study, a SARA model was successfully constructed by increasing the proportion of dietary concentrate and rumen-degradable starch (RDS) to examine the dynamic succession of the rumen microbiota across individuals. Forty-seven dairy goats were selected and classified into a control group (CON), low-RDS susceptible/tolerant groups (LRDSS/LRDST), and high-RDS susceptible/tolerant groups (HRDSS/HRDST) acc

The acidic tumor microenvironment in hepatocellular carcinoma: a barrier to adoptive cellular immunotherapy.

The metabolic interface between tumor and immune cells is an emerging determinant of immunotherapy resistance, yet its therapeutic exploitation in hepatocellular carcinoma (HCC) remains limited. Adoptive cellular immunotherapies, including NK cell infusion, tumor-infiltrating lymphocytes (TILs), and chimeric antigen receptor T (CAR-T) cells, have transformed hematologic oncology but repeatedly fail to achieve durable responses in HCC. We argue that an underappreciated driver of this resistance is the acidic tumor microenvironment (TME): hypoxia-driven glycolytic reprogramming acidifies the HCC

A newborn with glycogen storage disease type Ib presenting with hypoglycemia and evidence of mitochondrial dysfunction.

Glycogen storage disease type Ib (GSDIb) is an inherited disorder of carbohydrate metabolism that commonly presents in infancy. Here, we describe a case of GSDIb who presented on the first day of life with severe hypoglycemia and lactic acidosis, prompting a broad differential diagnosis. Additional testing revealed hyperlipidemia, relatively low ketones, and elevations of TCA-cycle intermediates in urine suggestive of primary or secondary mitochondrial dysfunction. The diagnosis of GSDIb was established by genome sequencing showing homozygous likely pathogenic variants in SLC37A4. This case hi

H+ exchange and pH profiling of select non-glycolytic cytosolic phosphate transfer and oxidation-reduction reactions.

BACKGROUND: Despite animal research of cellular and systemic acidosis for more than 100 years, there is no dominant evidence-based understanding of the cause of metabolic acidosis. Embedded in this crisis state is the understanding that the hydrogen ion (H+) involved in some chemical reactions, either as a substrate or product (H+ exchange (H+ e)), does not change the pH of biological solutions. This research documents the direct roles of the H+ e of a selection of non-glycolytic cytosolic chemical reactions in changing the pH of an unbuffered aqueous solution. METHODS: Reagents were prepared

Waterlogging and hypoxia in plant abiotic stress.

Waterlogging is a major abiotic stress affecting 12-27% of global cultivated land and causing average yield losses of 20-33% in cereals and legumes. This poses a serious threat to food security in flood-prone regions. Soil saturation reduces gas diffusion nearly 10,000-fold compared to air. This rapidly induces hypoxia or anoxia in the root zone within hours to days. This oxygen limitation forces a metabolic shift from efficient aerobic respiration (approximately 36 ATP per glucose) to anaerobic fermentation (approximately 2 ATP per glucose). This shift results in severe energy deficits, cytop

The Effects of Extracellular Vesicles From Cancer Cells in Acidosis or ER Stress to Endothelial Cells: Angiogenesis and Endothelial Activation.

Tumor-derived extracellular vesicles (EVs) facilitate metastatic progression by transferring bioactive cargo to recipient cells, thereby increasing endothelial permeability and promoting angiogenesis. The harsh conditions of the tumor microenvironment, such as extracellular acidosis, can alter the content of EVs, leading to changes in recipient cells, including endoplasmic reticulum (ER) stress. This study aimed to investigate whether EVs released by tumor cells adapted to acidosis or ER stress can alter endothelial functional properties. ER stress was induced in MDA-MB-231 breast cancer cells

Case Report: When metabolism meets malabsorption: Imerslund-Gräsbeck syndrome with coexisting propionic acidemia presenting as recurrent metabolic crises.

BACKGROUND: Propionic acidemia and Imerslund-Gräsbeck syndrome are rare inherited metabolic disorders with distinct pathophysiological mechanisms. Their occurrence in the same child is extremely unusual and can create diagnostic and therapeutic challenges because of overlapping neurological and hematological manifestations. CASE: A 1.5-year-old boy presented with recurrent episodes of encephalopathy precipitated by febrile illnesses, associated with vomiting, severe metabolic acidosis, ketosis, and refractory focal status epilepticus. Following repeated metabolic crises, developmental stagnat

When Stroke Is Not a Stroke: A Case Report of Crimean-Congo Hemorrhagic Fever Presenting With Focal Neurological Deficits.

Crimean-Congo hemorrhagic fever (CCHF) is a potentially fatal tick-borne viral zoonosis with a wide geographic distribution, a significant case-fatality rate, and a risk of healthcare-associated transmission. Although CCHF commonly presents with nonspecific febrile illness followed by hemorrhagic manifestations, neurological symptoms may occur and can mimic acute stroke, creating diagnostic challenges in the emergency department. We report the case of a 72-year-old male farmer in the United Arab Emirates who presented with acute slurred speech, confusion, and right-sided facial droop, promptin

Learning from the kidney: improving heart recovery from hypothermia.

Extending donor organ preservation times would substantially improve patient outcomes by widening safety margins and enabling better donor-to-patient matching. While clinical 4°C static storage typically preserves donor kidneys for 18-24 h, donor hearts are limited to ∼4 h. This clinical disparity is traditionally attributed to the higher baseline metabolic demand of cardiac tissue. This narrative review challenges this conventional assumption by comparing the distinct metabolic profiles of the heart and kidney at both 37°C and 4°C. We propose two mechanisms to explain how kidneys maintain rob
