BACKGROUND: Acute respiratory acid-base changes induce pH-dependent electrolyte redistribution. Although this response has been characterised in vitro, the in vivo changes depend on multicompartment buffering and remain poorly defined.
METHODS: We conducted a multicentre retrospective study of patients undergoing apnoea testing for organ donation after determination of brain death, a clinical model allowing in vivo study of acute ventilatory changes. The primary outcome was the pH-dependent change in strong ion difference (SID). Secondary analyses examined individual electrolytes, pH-SID effe

BACKGROUND Hyponatremic-hypertensive syndrome is a rare but distinctive manifestation of renal ischemia caused by unilateral renal artery stenosis. It is characterized by severe arterial hypertension accompanied by hyponatremia, hypokalemia, metabolic alkalosis, and polyuria, resulting from complex interactions between the ischemic kidney and the contralateral functioning kidney. Owing to its rarity and potentially life-threatening course, early recognition and appropriate management are crucial. The purpose of this report is to highlight the clinical presentation, diagnostic challenges, and t

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterized by fluctuating skeletal muscle weakness. Bulbar-predominant exacerbations may be difficult to recognize in older adults with multiple comorbidities, particularly when respiratory deterioration is initially attributed to cardiopulmonary disease. We report the case of an 83-year-old woman with generalized acetylcholine receptor antibody-positive MG, dementia with behavioral disturbance, moderate chronic obstructive pulmonary disease (COPD), chronic kidney disease, and previous deep vein thrombosis who wa

Secretin is a broad activator of epithelial bicarbonate secretion and regulates local and systemic pH balance along with metabolic functions. Its receptor, widely expressed across epithelial and neuroendocrine tissues, enables coordinated responses to digestive phases, systemic alkalosis, and volume regulation. This positions secretin as a corrective hormone of the postprandial alkaline tide and a critical signaling pathway in the prevention of excessive systemic alkalosis. Its signaling intersects with the renin-angiotensin-aldosterone system, where secretin, angiotensin II, and aldosterone j

Calcium-alkali syndrome is an increasingly recognised cause of parathyroid hormone (PTH)-independent hypercalcaemia resulting from excessive calcium and absorbable alkali ingestion, commonly through calcium-containing antacids or supplements. Its clinical presentation may mimic neurological or malignant disease, potentially leading to extensive investigation and delayed diagnosis. A 64-year-old woman presented following a fall with paraparesis and lower back pain. Investigations demonstrated severe hypercalcaemia (adjusted calcium 4.83 mmol/L), suppressed PTH of 1.4 pmol/L, metabolic alkalosis

BACKGROUND: Normothermic machine perfusion (NMP) is an emerging strategy for pediatric liver transplantation, but pediatric-specific viability criteria remain undefined. Lactate clearance, extrapolated from adult protocols, is a central marker of allograft function. In our pediatric NMP (pNMP) experience, we observed a novel pattern: initial clearance followed by secondary rise, termed rebound lactic acidosis.
METHODS: We retrospectively reviewed pediatric liver allografts preserved on the TransMedics Organ Care System at two centers (January 2025 through June 2026). After excluding donors >

A 13-year-old girl presented with 4 months of recurrent projectile vomiting and weight loss without diarrhea. Investigations confirmed hypochloraemic metabolic alkalosis, strongly positive coeliac serology, nutritional deficiency, and biopsy-proven Marsh-Oberhuber Grade 3C total villous atrophy. A strict gluten-free diet resulted in clinical resolution and histological recovery to Marsh Grade 1.

Accumulation of ∼200 h of hypoxia exposure (pre-acclimatization) has been suggested to prevent the development of acute mountain sickness when a person is subsequently exposed to high altitude. These pre-exposures are typically undertaken intermittently at or near the place of residence, primarily during sleep in hypoxic tents, and are likely to induce effective ventilatory acclimatization. However, the physiological consequences of different modes of intermittent hypoxia (IH) exposure on ventilatory acclimatization remain largely obscure and are the subject of much controversy. The aim of thi

We present the perioperative management of a 22-year-old female patient with congenital heart disease and Fontan circulation presenting for laparoscopic cholecystectomy. Careful preoperative review of recent cardiac testing and assessment of the patient's physical status allowed for an individualized anesthetic approach. Perioperative management began in the preoperative holding area with preload optimization. Intraoperative management focused on supporting flow through the Fontan circulation through positioning adjustments, ventilatory management, and vasopressor support. Collaboration with t

INTRODUCTION: Alkalemia is the most common acid-base disorder in hospitalized patients; however, data on patients with severe alkalemia admitted to the emergency department are limited. This study aimed to characterize the acid-base patterns of patients with severe alkalemia presenting to the emergency department and to describe the clinical presentation and short-term clinical outcomes of those requiring hospitalization.
PATIENTS, MATERIAL AND METHODS: We conducted a retrospective analysis of arterial and venous blood gas measurements and medical records of all adult nontrauma patients admit

We report a rare case of a monogenic form of hypertension where a boy in early childhood presented with a 2-day history of right-sided facial palsy. Severe hypertension was evident with signs of end-organ damage. This report aims to highlight the diagnostic process when a common presentation, such as facial palsy, uncovers a serious and rare underlying condition that demands urgent multidisciplinary care. Further workup revealed that the patient had hypokalaemic metabolic alkalosis with low serum renin and aldosterone levels. His urinary steroid profile was suggestive of Apparent Mineralocorti

Endogenous Cushing syndrome is a metabolic syndrome that results from the excessive production of cortisol from the adrenal gland. Hypercortisolism can result in serious complications, such as hypertension, myocardial ischemia, glucose intolerance, immunosuppression, and electrolyte disturbances. The mainstay of treatment is surgical resection for resectable tumors, along with pharmacological therapy in selected cases, because severe hypercortisolism often requires prompt medical stabilization before definitive surgical management can be safely performed. We report a 34-year-old male patient w

BACKGROUND: Despite animal research of cellular and systemic acidosis for more than 100 years, there is no dominant evidence-based understanding of the cause of metabolic acidosis. Embedded in this crisis state is the understanding that the hydrogen ion (H+) involved in some chemical reactions, either as a substrate or product (H+ exchange (H+ e)), does not change the pH of biological solutions. This research documents the direct roles of the H+ e of a selection of non-glycolytic cytosolic chemical reactions in changing the pH of an unbuffered aqueous solution.

Dual inherited defects affecting urea cycle function are exceptionally rare. We report the first case, to our knowledge, of the coexistence of argininosuccinic aciduria (ASA) and citrin deficiency (CD). A girl born to consanguineous Tunisian parents presented at 6 months of age with recurrent seizures, vomiting, developmental delay, and severe hyperammonemic encephalopathy. Laboratory investigations revealed hyperammonemia, respiratory alkalosis, transaminitis, dyslipidemia, hyperglutaminemia, and mild hypercitrullinemia. Brain imaging showed delayed myelination and basal ganglia abnormalities

OBJECTIVES: This study aimed to investigate the impact of blood gas sample waiting time on laboratory parameters in intensive care patients, focusing on pH-related changes.
METHODS: This prospective longitudinal observational study was conducted in the pulmonary diseases intensive care unit of a training and research hospital in Turkey between July 1 and November 30, 2023. A total of 81 patients were included in the study. Blood gas samples were categorized into three groups based on initial pH levels: acidosis, normal, and alkalosis. The samples were stored at 22 °C and analyzed at 0, 30, an

A man in his 40s with morbid obesity and type 2 diabetes mellitus presented with early-morning drowsiness and respiratory distress after several days of nocturnal dyspnoea and unsupervised loop diuretic use. Initial assessment showed hypoglycaemia, acute-on-chronic hypercapnic respiratory acidosis with metabolic alkalosis and obstructive ventilatory mechanics, requiring intubation. Within 24 hours of extubation, he re-presented with recurrent drowsiness and striking bilateral cerebellar ataxia despite normal neuroimaging. Repeat arterial blood gases again demonstrated acute-on-chronic hypercap

Gluconeogenesis, nitrogen metabolism and acid-base regulation are fundamental for the maintenance of metabolic homeostasis. These three processes form an interconnected triangle, with liver-kidney crosstalk and the hormone glucagon at its core. Both kidney and liver perform gluconeogenesis, and they function in co-ordination to regulate the formation and excretion of the organic osmolyte urea and the acid equivalent ammonium, and to determine the usage and de novo generation of bicarbonate. Within the liver, glucagon promotes fasting-induced gluconeogenesis and the bicarbonate-consuming format

BACKGROUND: Valproate poisoning is a potentially life-threatening condition that may result in central nervous system depression, hyperammonemia, metabolic disturbances, and multiorgan dysfunction. Management of massive sustained-release valproate overdose remains challenging because delayed gastrointestinal absorption may lead to prolonged toxicity despite conventional decontamination measures.
CASE PRESENTATION: A 23-year-old woman with a history of previous suicide attempts was admitted approximately 6 hours after ingesting ninety 500-mg sustained-release valproate tablets (total dose 45 g
