Respiratory Acid-Base Disorders.

Respiratory acid-base disorders are often not thought of as frequently as their metabolic cousins, which occur more frequently in the emergency department. Although most respiratory and acid-base disturbances are driven by lung pathology, central nervous system and other organ systems can and do play a role as well. Although managing the airway and appropriate mechanical ventilation may be necessary, it is akin to placing a band-aid on a large wound. It is crucial for the emergency clinician to discover the etiology of the disturbance as management depends on treating the underlying etiology t

Illuminating the renal response to pH stress with single-nucleus RNA sequencing.

Maintenance of whole-body pH is essential for human health. The kidneys play a crucial role in defending pH homeostasis by excreting excess acid in the urine and returning alkali buffers to the blood. Consequently, renal insufficiency causes serious and harmful effects on pH balance. While a serious and common complication of chronic kidney disease (CKD), pH imbalances themselves appear to be catalysts of kidney injury. Renal adaptations to pH imbalances contribute to compensated acid-base disorders and are vital to correcting whole-body pH. However, overstimulation of these adaptive processes

Intra-individual reliability of blood bicarbonate responses and gastrointestinal symptoms following sodium citrate supplementation.

BACKGROUND: Sodium citrate (SC) can elevate extracellular buffering capacity, yet the intra-individual reliability of its blood bicarbonate ([HCO₃-]) kinetics and gastrointestinal (GI) responses is unclear, limiting individualized dosing strategies. METHODS: Twelve healthy males (21 ± 1 yr) ingested a solution containing 0.5 g·kg-1 SC on two visits 3-7 days apart. Capillary [HCO₃-] was sampled at baseline and every 30 min to 240 min to derive baseline and peak [HCO₃-], time to peak (TTP), time to exceed +5 and +6 mmol·L-1 above baseline, and area under the curve (AUC). Reliability was quantif

A Case Report of Milk-Alkali Syndrome Secondary to Excessive Antacid Use.

INTRODUCTION: Milk-alkali syndrome is characterized by the triad of hypercalcemia, metabolic alkalosis, and acute kidney injury resulting from excessive intake of calcium and absorbable alkali. Despite falling out of prominence with the advent of modern ulcer treatments, milk-alkali syndrome has experienced a resurgence with the widespread availability of over-the-counter calcium preparations, which now account for up to 10% of hypercalcemia cases. CASE REPORT: A 60-year-old man with multiple comorbidities presented to the emergency department with altered mental status after his scheduled ky

Systematic Review of Pharmacologic Treatments for Hypochloremic Metabolic Alkalosis in Critically Ill Children.

Metabolic alkalosis, characterized by an increase in serum bicarbonate (>28 mEq/L) and serum pH (>7.45), is associated with clinical complications including arrhythmias, mental confusion, and seizures. Non-pharmacologic measures are recommended first-line for treatment of hypochloremic metabolic alkalosis, but pharmacologic treatment may be needed. This review of the literature provides detailed descriptions of dosage regimens, efficacy, safety, and other considerations for use of pharmacologic agents. The literature search included published human studies in the English language from EMBASE a

Extreme Metabolic Alkalosis Caused by Temporary Jejunostomy-A Case Report and Physiopathological Insights.

Background and Clinical Significance: Metabolic alkalosis is the most common acid-base disturbance in hospitalized and critically ill patients, with extreme alkalemia (pH > 7.65) linked to mortality rates exceeding 80%. Jejunostomy-related intestinal losses can lead to severe hypochloremic metabolic alkalosis, a rare but life-threatening condition. This case report highlights the clinical presentation, diagnostic approach, physiopathology, management, and outcome of a patient with extreme metabolic alkalosis induced by a temporary jejunostomy. Case Presentation: We report the case of a 72-year

Gitelman Syndrome With Two Variants of Uncertain Significance: A Case Report.

Gitelman syndrome (GS) is an autosomal recessive salt-wasting tubulopathy caused by pathogenic variants in SLC12A3, characterized by renal potassium wasting, hypokalemic metabolic alkalosis, hypomagnesemia, and typically low urinary calcium excretion. We report the case of a 47-year-old woman with recurrent hypokalemia and episodes of asthenia, myalgias, and generalized muscle weakness. She denied gastrointestinal losses and diuretic or laxative use, and examination showed normal blood pressure. Electrocardiography revealed sinus rhythm without QT prolongation, and renal ultrasonography showed

Transient antenatal Bartter syndrome type 5 presenting as shock and metabolic acidosis in a preterm neonate.

A preterm male neonate born at 28 + 2 weeks gestation, with a birth weight of 1520 g (large for gestational age) and antenatal polyhydramnios, presented with prematurity, early-onset sepsis, and septic shock. Between days 4 and 9 of life, the neonate developed marked polyuria, hyponatremia, and compensated metabolic acidosis with normal to transiently elevated serum potassium levels, initially obscuring the diagnosis of a tubular disorder. Subsequently, the biochemical profile evolved to hypokalemia, metabolic alkalosis, and hypercalciuria, consistent with Bartter syndrome. The clinical course

A rare case of apparent mineralocorticoid excess induced by chronic liquorice ingestion.

SUMMARY: Apparent mineralocorticoid excess (AME) syndrome is a rare condition caused by the inhibition of the renal enzyme 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2). The most common acquired cause is chronic liquorice ingestion. We report the case of a 61-year-old woman who developed profound hypokalaemia, metabolic alkalosis, and QT prolongation due to excessive liquorice tea consumption. This case underscores the importance of detailed dietary history in patients with unexplained electrolyte disturbances and demonstrates the potentially fatal cardiovascular consequences of a widely

Potential Factors of Diabetes in Gitelman Syndrome and the Choices of the Appropriate Hypoglycemic Drugs: A Literature Narrative Review.

Gitelman syndrome (GS) is a rare, autosomal recessive salt-losing tubulopathy caused by mutations in the SLC12A3 gene. It involves dysfunction of the sodium-chloride cotransporter positioned on the apical membranes of the distal convoluted tubule cells, causing sodium shortage and mimicking the use of thiazide diuretics. Hyperaldosteronism secondary to sodium depletion and hypovolemia causes hypokalaemia and metabolic alkalosis. This is associated with inhibition of the Transient Receptor Potential Cation Channel, Subfamily M, Member 6 -TRPM6 channel, which leads to urinary magnesium leakage a

Unmasking a rare and dangerous trio in early pregnancy: hyperemesis gravidarum complicated by transient thyrotoxicosis and starvation ketoacidosis.

BACKGROUND: Hyperemesis gravidarum is a pregnancy-related condition marked by intractable nausea and vomiting, often resulting in volume depletion, electrolyte abnormalities, and weight loss. While the typical biochemical profile includes hypochloremic, hypokalemic metabolic alkalosis, ketoacidosis is uncommon and typically associated with prolonged starvation. Although thyrotoxicosis has been reported in association with HG, the concurrent occurrence of ketoacidosis is exceedingly rare, particularly in the absence of an identifiable precipitating factor. CASE PRESENTATION: We describe the cas

Ectopic ACTH Syndrome Caused by Pheochromocytoma: A Case Report and Systematic Review.

INTRODUCTION: Ectopic adrenocorticotropic hormone (ACTH) syndrome caused by pheochromocytoma is extremely rare, and its highly variable clinical manifestations make diagnosis challenging. CASE PRESENTATION: This report describes a case of a 48-year-old female with typical Cushingoid features who complained of edema in both lower limbs for 20 days. Laboratory testing showed hypokalemia with metabolic alkalosis, high levels of cortisol and ACTH, and the absence of circadian rhythm. Neither low-dose or high-dose dexamethasone suppression testing could inhibit cortisol secretion. Abdominal comput

Influence of prior alkalosis or acidosis on physiological responses during passive hyperthermia.

This study manipulated the blood pH of participants to be mildly acidic or basic prior to passive hyperthermia to examine its effects on thermoregulatory and chemo-regulatory responses, including ventilatory parameters, rectal temperature (Tre) and thermal perception. Twelve healthy males attended three experimental sessions in a double-blind, randomised, counterbalanced design. Passive hyperthermia was induced by hot water immersion at 40°C following the consumption of corn starch (placebo; PLA), ammonium chloride (AC) or sodium bicarbonate (SB). Compared with PLA, SB consumption increased bl

Unmasking of a Heterozygous SLC12A3 Variant.

Three months after starting 25 mg chlorthalidone, a patient was admitted to the medical intensive care unit to manage hypokalemia, metabolic alkalosis, hypo-osmolar hyponatremia, hyperglycemia, and 30 lb weight loss. The patient received ∼936 mEq potassium over 8 days, of which 456 mEq was administered during the first 2 days of admission. In the first 24 hours, the individual received intravenous fluids that delivered 406 mEq sodium chloride. By day 2, serum potassium level increased from 2.5 to 3.0 mEq/L, bicarbonate fell from 40 to 35 mEq/L, serum sodium improved from 121 to 134 mEq/L, and

Physical Activity in Patients With Epilepsy: A Risk Factor or a Healthy Habit?

The approach to physical activity in patients with epilepsy has substantially changed over the last decade. Despite multiple positive effects of physical activity on general health and well-being, patients with epilepsy have long been advised not to engage in sports activities. Recent studies have led physicians to formulate updated recommendations and to encourage patients to remain active. It has been demonstrated that sport does not increase seizure prevalence, and the rate of sport-induced injuries in people with epilepsy is comparable to that of the general population. Additionally, physi

Hyperventilation syndrome following epinephrine nasal packing for intubation-related epistaxis: a case report.

BACKGROUND: Epistaxis is the most common complication of nasotracheal intubation. Epinephrine-impregnated nasal gauze packing is usually employed as a hemostatic method for intubation-induced epistaxis. Systemic absorption of the topically administered epinephrine may lead to adverse effects. Currently reported adverse events associated with topical epinephrine administration are predominantly cardiovascular reactions. Here we describe a rare case of hyperventilation syndrome potentially induced by epinephrine-impregnated nasal packing for treatment of epistaxis in a patient who underwent naso

CFTR knock-out mice exhibit impaired renal compensation to dietary NaCl depletion.

BACKGROUND: People with cystic fibrosis (CF) are at increased risk for fluid and electrolyte imbalances due to continuous salt loss. Here we study if renal CFTR dysfunction can contribute to salt-wasting in CF. Mice do not sweat and hence a CFTR KO mouse model was applied. METHODS: To explore sweat-independent mechanisms contributing to salt-wasting in CF, we NaCl-depleted CFTR KO and WT mice for 7 days. Thereafter, the renal adaptation, systemic electrolyte and acid-base homeostasis, plasma aldosterone and urine sodium excretion were assessed. Eventually, we also studied the pendrin KO mouse
