The effect of pre-exercise hyperventilation on repeated high-intensity inclined sprint performance.

High-intensity exercise induces systemic acidosis, which contributes to muscular performance decline. Controlled pre-exercise hyperventilation has been proposed as a non-invasive intervention to induce respiratory alkalosis and counteract acidosis-related acute fatigue. In this study, we hypothesized that a real-world-applicable pre-exercise hyperventilation protocol can improve incline sprint performance. This randomized, single-centre, controlled crossover study included 36 recreational athletes aged 18-40 years. Each participant completed a series of three inclined treadmill interval sprint

Compound Heterozygous SLC12A3 Variants in Gitelman Syndrome Presenting With Ventricular Fibrillation and Cardiac Arrest.

BACKGROUND: Gitelman syndrome (GS) is an autosomal recessive salt-losing tubulopathy characterized by hypokalemia, hypomagnesemia, and metabolic alkalosis. Although often considered benign, GS may predispose to malignant ventricular arrhythmias. CASE PRESENTATION: A 41-year-old male presented with cardiac arrest due to ventricular fibrillation (VF). Severe hypokalemia (1.6 mmol/L) and hypomagnesemia were identified, with no structural heart disease on imaging. Recurrent VF persisted despite antiarrhythmic therapy and resolved only after electrolyte correction. Genetic testing revealed pathoge

Comparative analysis of acidosis defense mechanisms in preimplantation embryos in BALB/c strain mice: in vivo vs in vitro development.

CONTEXT: Intracellular pH (pHi) regulation is critical for all cell types, including preimplantation embryos. Embryos maintain pHi homeostasis through ion exchangers that respond to both acidic and alkaline challenges. AIMS: While alkalosis defense mechanisms decline in later embryonic stages, this study investigated whether acidosis defense mechanisms - specifically Na+/H+ exchanger (NHE) activity alone and in conjunction with sodium-dependent chloride/bicarbonate exchanger (NDBCE) - remain active across developmental stages in in vivo- and in vitro-derived embryos. METHODS: In vivo-derived

Clinical, hematological and biochemical findings in bovines with primary abomasal impaction.

The reported biochemical changes in primary abomasal impaction (PAI) have mainly focused on electrolytes; the changes in C-reactive protein (CRP) and liver enzymes have not been evaluated particularly for adult buffaloes (Bubalus bubalis). The aim of this study was to describe the blood gas parameters, CRP, clinical, hematological, and biochemical characteristics of PAI in a prospective study. The study was conducted on 15 animals (nine buffaloes and six cows) diagnosed with PAI. The diagnosis was based on consistency of abomasum on laparo-rumenotomy and/or necropsy. The age of the animals was

A Rare Case of Bartter Syndrome Type 3 Diagnosed in Elderly Age.

Bartter syndrome (BS) type 3 typically presents in childhood and is caused by defects in NaCl transporters of the thick ascending limb of the loop of Henle. We report a 66-year-old woman with asymptomatic but severe hypokalemia (2.0 mmol/L), metabolic alkalosis, and hyperreninemic hyperaldosteronism. Initial clinic blood pressure was 157/88 mmHg, but repeated office and home measurements were ~120/70 mmHg, consistent with white-coat hypertension. Imaging excluded renovascular disease. Sequential diuretic testing supported BS physiology: a thiazide loading test increased fractional excretion of

Finerenone in Treating a 12-Year-Old Boy Suffering Gitelman Syndrome Without Causing Gynecomastia.

Gitelman syndrome (GS) is an autosomal recessive disorder caused by mutations in the SLC12A3 gene, characterized by hypomagnesemia, hypokalemia, alkalosis, and hypocalciuria. Finerenone, a non-steroidal mineralocorticoid receptor antagonist (MRA), has shown cardiorenal protective effects and improved prognoses in type 2 diabetes (T2DM) and chronic kidney disease (CKD). However, its use in non-diabetic nephrology has been infrequently reported. This report presents a case of a male child with GS who was treated with finerenone in combination with potassium and magnesium supplements to elevate p

An Amino Acid with Killer Instincts: Pulmonary Embolism Secondary to Hyperhomocysteinemia.

Pulmonary embolism (PE) is a life-threatening manifestation of venous thromboembolism, typically associated with identifiable risk factors such as immobilization, surgery, malignancy, or trauma. However, in young individuals without apparent triggers, underlying thrombophilic states must be investigated. Hyperhomocysteinemia is an often overlooked yet reversible metabolic risk factor contributing to thrombogenesis. We report a 31-year-old male electrician presenting with acute-onset breathlessness, pleuritic chest pain, and cough. Clinical evaluation revealed tachypnea, tachycardia, and hypoxe

Sex differences in renal acid-base regulation.

PURPOSE OF REVIEW: Sex-dependent differences are evident in most organs and contribute to variability in physiological function and disease susceptibility, which can consequently affect therapeutic responses. The kidney is no exception to this finding and recent data suggest that sexual dimorphism exists in the regulation of acid-base homeostasis. RECENT FINDINGS: While proteins involved in the renal regulation of acid-base homeostasis have been characterized over several decades, researchers have only recently begun investigating how biological sex affects these processes. Although males and

Blood acid/base balance in poultry: Dietary, environmental, and physiological determinants and emerging diagnostic opportunities.

Acid/base balance is a central integrative process in poultry physiology, linking respiratory gas exchange, renal ion handling, skeletal mineral buffering, and cellular metabolism, with direct consequences for growth performance, skeletal integrity, eggshell quality, cardiopulmonary function, and survival. Although the mechanistic basis of acid/base regulation in birds has been well described for decades, direct assessment of blood acid/base status remains uncommon in commercial poultry production, where diet formulation targets and performance outcomes are often used as lagging and indirect p

From Ion Channels to Blood Pressure: Genetic Disorders of Renal Tubular Transport.

Blood pressure regulation is fundamentally dependent on renal sodium and electrolyte handling. Genetic disorders of renal tubular transport provide representative evidences that illuminate the molecular mechanisms linking ion channels to systemic hemodynamics. Monogenic conditions such as Bartter syndrome, Gitelman syndrome, Liddle syndrome, and Gordon syndrome demonstrate how specific alterations in tubular sodium, potassium, chloride, and magnesium transport translate into distinct blood pressure phenotypes. Salt-wasting disorders are characterized by hypokalemic metabolic alkalosis and low

Impact of post-filter ionized calcium target range on circuit survival and citrate-related complications in pediatric continuous kidney replacement therapy.

BACKGROUND: Regional citrate anticoagulation (RCA) is the preferred strategy for continuous kidney replacement therapy in children; however, the optimal post-filter ionized calcium target remains uncertain. Lower targets may increase anticoagulation but raise citrate exposure and metabolic complications. We aimed to compare anticoagulation efficacy and metabolic safety between a low-target (0.25-0.35 mmol/L) and a high-target (0.30-0.40 mmol/L) post-filter ionized calcium protocol in critically ill children. METHODS: This retrospective cohort study included critically ill children receiving c

Liddle syndrome caused by SCNN1B mutation presenting with severe early-onset hypertension and hypokalemia in an adolescent: a case report.

BACKGROUND: Liddle syndrome is a rare autosomal dominant form of monogenic hypertension caused by gain-of-function mutations in genes encoding the epithelial sodium channel (ENaC). The disease is characterized by early-onset hypertension, hypokalemia, metabolic alkalosis, and suppression of the renin-angiotensin-aldosterone system (RAAS). Early diagnosis is essential because targeted therapy with ENaC inhibitors can effectively control blood pressure and prevent end-organ damage, whereas mineralocorticoid receptor antagonists are ineffective. CASE PRESENTATION: We report a 15-year-old Chinese
