Chloride at the spotlight-but not alone: towards a multi-ionic diuretic strategy.

Diuretic resistance in heart failure and the cardiorenal syndrome is traditionally managed by escalating diuretic doses, yet this approach often fails to address the underlying pathophysiology. Emerging evidence challenges the sodium-centred paradigm and identifies chloride as a key determinant of diuretic responsiveness, tightly interconnected with potassium, magnesium, and acid-base balance. Disturbances in this multi-ionic network promote maladaptive tubular responses, neurohormonal activation, and persistent sodium retention, ultimately leading to diuretic resistance. Clinical and mechanis

KCNJ16 tubulopathy presenting with recurrent acute flaccid paralysis and sensorineural hearing loss.

A 13-year-old girl, born to third-degree consanguineous parents, presented with acute flaccid paralysis. There was a past history of a similar episode at 5 years of age. Her current laboratory investigations revealed hypokalemia and normal anion gap-metabolic acidosis. Pure tone audiometry showed bilateral sensorineural hearing loss (SNHL). There was no nephrocalcinosis, ataxia, or epilepsy. Whole exome sequencing detected a homozygous deletion (c.269_270del) in exon 4 of KCNJ16 gene that led to frameshift and premature truncation of the protein (p. Ser90CysfsTer15; ENST00000392671.6). Successful Management of Refractory Cannabinoid Hyperemesis Syndrome With Topical Capsaicin: A Case Report With Proposed Mechanism of Action and Literature Review.

INTRODUCTION: Cannabinoid hyperemesis syndrome (CHS) is a debilitating disorder of chronic, heavy cannabis users characterized by cyclic severe nausea, vomiting, and abdominal pain that is classically relieved by hot baths. Standard antiemetics often fail, and dysregulation of endocannabinoid signaling with involvement of heat-sensitive TRPV1 channels has been proposed. Topical capsaicin - a TRPV1 agonist - has been reported in small series and case reports to reproduce the hot-water effect and rapidly relieve symptoms. CASE PRESENTATION: A 28-year-old man with daily high-potency cannabis use

Butterfly Glioma of the Corpus Callosum Mimicking Rapidly Progressive Dementia.

A butterfly-pattern, high-grade glioma involving the corpus callosum is an uncommon but highly aggressive brain tumour that may present with neuropsychiatric manifestations and mimic rapidly progressive dementia, leading to diagnostic delay. Disruption of frontal-callosal connectivity may produce behavioural and cognitive abnormalities resembling behavioural and psychological symptoms of dementia (BPSD), resulting in initial misdiagnosis. A 65-year-old postmenopausal woman presented with a two-week history of vomiting, progressive disorientation, generalized weakness, irrelevant speech, cognit

Implementation of rules for the estimation of compensatory responses in acid-base analysis: A review and proposal for laboratory practice.

Acid-base homeostasis is essential for cellular and systemic function. Its maintenance requires tightly regulated compensatory mechanisms capable of correcting disturbances that threaten physiological balance, as dysregulation can be life-threatening. When a primary metabolic disturbance occurs and alters bicarbonate concentration (HCO₃), the main compensatory mechanism is a respiratory response that adjusts the partial pressure of carbon dioxide (PCO₂). Conversely, a primary respiratory disturbance, whether acute or chronic, leads to changes in PCO₂, and compensatory response occurs through a

Ectopic Adrenocorticotropic Hormone Syndrome in a Patient Diagnosed With Lung Adenocarcinoma and Hospitalized Due to Diabetic Ketonuria: A Case Report.

Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) is most often a paraneoplastic manifestation of small-cell lung cancer and is rare in lung adenocarcinoma. Its features include a cushingoid appearance, skin hyperpigmentation, muscle weakness, hypertension, glucose intolerance, hypokalemia, and metabolic alkalosis resulting from nonsuppressible, ACTH-driven hypercortisolism. We report a 70-year-old man admitted with pneumonia and diabetic ketonuria without acidosis, in whom EAS secondary to a poorly differentiated lung adenocarcinoma was diagnosed. Key biochemical findings were a marke

Admission serum bicarbonate is associated with early diuretic resistance in acute decompensated heart failure.

BACKGROUND: Acute decompensated heart failure (ADHF) exhibits a heterogeneous diuretic response. Chloride-bicarbonate imbalances may reduce loop diuretic efficiency. The effect of admission bicarbonate levels on diuretic resistance in unselected ADHF patients remains unclear. We aimed to evaluate whether higher admission bicarbonate identifies patients at risk of early diuretic dose escalation and less effective early decongestion. METHODS: Our study was planned as a retrospective, single-centre, consecutive ADHF cohort. This study included 1000 hospitalised patients with ADHF (mean age 69 ye

Home Parenteral Support in Severe Gitelman Syndrome: A Case Report.

Gitelman syndrome (GS), also known as familial hypokalaemia-hypomagnesemia, is a rare autosomal recessive renal tubular disorder characterized by hypokalaemia, hypomagnesemia, metabolic alkalosis, and hypocalciuria. Electrolyte replacement of potassium and magnesium remains the mainstay of management, with oral supplementation achieving this in the majority of patients. We report the case of a 65-year-old woman with a severe manifestation of GS, managed with high-dose intravenous electrolyte replacement as part of a home parenteral support (HPS) regimen in addition to oral supplementation. Comparative Study of Plasma Electrolyte Status in Pre-eclamptic and Normotensive Pregnant Women in Zaria.

BACKGROUND: Pre-eclampsia is a major cause of maternal and perinatal morbidity, characterized by endothelial dysfunction and renal impairment that may alter electrolyte balance. However, findings on electrolyte disturbances remain inconsistent, and the absence of population-specific reference intervals limits clinical interpretation in low-resource settings. OBJECTIVE: To evaluate plasma electrolyte patterns among pre-eclamptic and normotensive pregnant women and establish locally relevant reference intervals for electrolyte parameters in Zaria, Nigeria. METHODS: A comparative cross-sectiona

Acid-Base and Electrolyte Disorders: Acid-Base Disorders.

Maintaining a normal physiologic acid-base balance is essential for the functioning of every organ system in the body. The renal and pulmonary systems are the primary regulators of acid-base balance. There are four principal acid-base disorders: metabolic acidosis, metabolic alkalosis, respiratory acidosis, and respiratory alkalosis. A systematic approach to evaluation is critical to identify potential etiologies. A history, physical examination, metabolic profile, and arterial blood gas measurement provide a full assessment of acid-base status. However, availability of arterial blood gas meas

Reappraisal of topiramate-associated metabolic acidosis: a systematic review using physicochemical acid-base analysis.

BACKGROUND: Topiramate is traditionally described as causing a hyperchloremic, non-anion gap metabolic acidosis due to carbonic anhydrase inhibition. However, the underlying physicochemical mechanisms have never been systematically reassessed using the Stewart approach. METHODS: We conducted a systematic review of all published cases and re-evaluated the available biochemical data using the Stewart physicochemical model. RESULTS: Among 38 identified patients, only seven had sufficient information for full or near-complete strong ion analysis. In every patient with genuine metabolic acidosis,

Case Report: Congenital chloride diarrhea in a prepubertal girl with perianal fistula and colonic ulcers.

Congenital chloride diarrhea (CCD) is a rare genetic disorder characterized by persistent watery diarrhea and electrolyte imbalances. Herein, we report the case of a 6-year-old girl with a significantly delayed diagnosis of CCD, who had been misdiagnosed with Bartter syndrome since infancy due to atypical symptoms. Notably, the patient presented with rare gastrointestinal complications, including colonic ulcerations and an active perianal fistula. While the colonic ulcerations resolved under standard conservative medical treatment, the concurrent perianal fistula completely healed using an inn

Common electrolyte disorders for the internist.

Electrolyte, acid-base, and mineral metabolism disorders are among the most common and clinically significant problems experienced in internal medicine patients. Their development reflects disturbances in regulated homeostatic mechanisms involving the kidneys, bones, endocrine systems, and cellular ion transport. Disorders such as hyponatremia, hypernatremia, hypokalemia, hyperkalemia, metabolic acidosis, metabolic alkalosis, and abnormalities of calcium and phosphorus metabolism are associated with substantial morbidity and mortality when not promptly recognized and appropriately treated. Investigating the role of urinary electrolyte parameters in the differential diagnosis of hypokalemia.

BACKGROUND: Hypokalemia is a common clinical electrolyte disorder. In normotensive patients with renal potassium (K+) wasting, distinguishing vomiting-related upper gastrointestinal (UGI) disorders from renal tubular diseases (RTDs), including Gitelman syndrome (GS) and distal renal tubular acidosis (dRTA), is challenging due to overlapping manifestations. Assessment based on isolated urinary potassium excretion has limited diagnostic value. MATERIALS AND METHODS: We retrospectively analyzed 86 normotensive hypokalemic patients with inappropriate renal K+ loss admitted to our department (2016
