Finerenone Use in Coenzyme Q8B-Related Focal Segmental Glomerulosclerosis.

Genetic focal segmental glomerulosclerosis (FSGS) caused by mutations affecting coenzyme Q (CoQ) biosynthesis is a rare podocytopathy that is associated with poor response to immunosuppression. Finerenone is a nonsteroidal mineralocorticoid antagonist that reduces oxidative stress, glomerular inflammation, and fibrosis, features seen in CoQ deficiency. We report the first case of its use in COQ8B-related FSGS. A 35-year-old woman was evaluated for persistent postpartum proteinuria and underwent a kidney biopsy that showed features of FSGS. She had a strong family history of early-onset kidney

Overcoming Hyperkalaemia to Optimise Heart Failure Therapy: The Emerging Role of Novel Potassium Binders.

Hyperkalaemia is common in patients with heart failure. It limits the initiation, optimisation and continuation of guideline-directed medical therapy, particularly mineralocorticoid receptor antagonists. Two potassium binders, patiromer and sodium zirconium cyclosilicate (SZC), have emerged as strategies to address this barrier. SZC is an inorganic crystalline compound that binds potassium in exchange for sodium and hydrogen throughout the gastrointestinal tract, resulting in rapid potassium lowering, whereas patiromer is a non-absorbed organic polymer that exchanges potassium for calcium in t

Spontaneous Tumour Lysis Syndrome in Metastatic Endometrial Carcinoma.

Tumour lysis syndrome (TLS) is a potentially fatal oncological emergency characterised by rapid cellular breakdown resulting in hyperkalaemia, hyperphosphataemia, hyperuricaemia, and hypocalcaemia, with subsequent acute kidney injury, cardiac dysrhythmias, and multi-organ dysfunction. It most commonly occurs following initiation of anti-cancer therapy in highly proliferative haematological malignancies. Spontaneous tumour lysis syndrome (STLS), occurring in the absence of a therapeutic trigger, is considerably rarer, particularly in solid tumours. We report a 55-year-old female presenting with

Finerenone versus spironolactone in heart failure with mildly reduced or preserved ejection fraction: a real-world study.

AIMS: Finerenone has shown benefit in chronic kidney disease, type 2 diabetes, and heart failure with mildly reduced or preserved ejection fraction (HFmrEF/HFpEF), but head-to-head data against spironolactone are limited. We compared 12-month effectiveness and safety after finerenone versus spironolactone initiation in heart failure with left ventricular ejection fraction ≥40%. METHODS AND RESULTS: We conducted a retrospective active-comparator, new-user cohort study using the TriNetX Network from 9 July 2021 to 31 March 2025. Adults of both sexes with HFmrEF/HFpEF initiating finerenone or sp

The Ratio of Silicon to Chitin in Diatom Cell Walls Influences Their Resilience to Environmental Stress.

Polymer based nanoparticles for drug delivery is an alternative approach to overcome drug resistance and drug toxicity especially for cutaneous leishmaniasis treatment. The present study shows synthesis and characterization of Miltefosine loaded chitosan nanoparticles (MFS-CNPs). The synthesized MFS-CNPs were experimented to evaluate the in vitro cytotoxicity and efficacy of the synthesized drug loaded nanoparticles by hemolysis assay and 3-(4, 5- dimethylthiazol-2-yl)-2,5-diphenyletetrazolium bromide (MTT) assay. MFS-CNPs were synthesized by ionic gelation method with sodium tripolyphosphate.

Does higher kidney risk justify more finerenone? Reappraising absolute benefit against the safety window of FINEARTS-HF.

Chronic kidney disease has long been viewed as a reason for caution when prescribing mineralocorticoid receptor antagonists (MRAs) in heart failure (HF), a doctrine rooted in the safety record of the steroidal agents: in TOPCAT, drug discontinuation for adverse events rose steeply as kidney function declined. The prespecified kidney-risk analysis of FINEARTS-HF, which reported consistent relative benefit of the non-steroidal MRA finerenone across KDIGO risk bands with a non-significant interaction on the primary endpoint, is widely read as a mandate to treat higher-risk patients more readily.

Subacute oral glyphosate induces dose-dependent renal injury in male mice (Mus musculus).

Glyphosate is the most heavily applied herbicide worldwide and human exposure is now near-ubiquitous, yet its renal toxicity at near-threshold doses remains incompletely characterized. We tested whether subacute oral glyphosate produces integrated, dose-dependent renal injury in male mice (Mus musculus). Adult male mice (n = 10/group) received vehicle or technical-grade glyphosate at 10, 50 or 100 mg/kg/day by oral gavage for 28 days, with 50 mg/kg anchored near the chronic NOAEL. Serum renal-function markers, renal oxidative-stress markers (MDA, GSH, SOD, CAT) and hematoxylin-eosin histopatho

Cardiovascular Outcomes With Potassium Binders in Patients with Heart Failure and Hyperkalemia.

BACKGROUND AND AIMS: Hyperkalaemia complicates heart failure (HF) and may limit guideline-directed therapy. Potassium binders differ in pharmacologic properties and sodium load, raising cardiovascular safety concerns. This study compared cardiovascular outcomes associated with patiromer, sodium zirconium cyclosilicate (SZC), and sodium polystyrene sulfonate (SPS). METHODS: A retrospective observational study was conducted within the TriNetX network including adults with HF and hyperkalaemia initiating patiromer, SZC, or SPS (2019-2024). Three pairwise 1:1 propensity-matched comparisons were p

Natural Oil and Polycystic Ovary Syndrome: A Comprehensive Review of Therapeutic Benefits.

INTRODUCTION: Polycystic ovary syndrome (PCOS) is a common endocrine and metabolic disorder affecting 70-75% of women. This condition is frequently linked with large and dysfunctional ovaries, high levels of androgens, and insulin resistance. A variety of conventional treatments, including metformin, oral contraceptives, and anti-androgen agents, have been used to treat PCOS and its complications, but they have been linked to several negative side effects, including hyperkalemia, weight gain, cardiovascular and hepatic toxicity, vitamin B12 and folic acid deficiency. As a result, there is grow

Benefit-risk of colchicine and spironolactone in acute myocardial infarction: a prespecified generalised pairwise comparisons analysis of the CLEAR trial.

BACKGROUND: Composite outcomes in cardiovascular trials often group events of unequal clinical importance, and conventional analyses may obscure treatment trade-offs. Generalised pairwise comparisons (GPC), expressed as a win ratio (WR), allow for hierarchical ranking of events and incorporation of recurrent outcomes, providing a potentially more intuitive assessment of benefit-risk. METHODS: In a prespecified exploratory analysis of the 2×2 factorial, randomised CLEAR (Colchicine and Spironolactone in Patients with Myocardial Infarction) trial (7062 patients within 72 hours of acute myocardi

Natriuretic response to an acute oral potassium load in healthy individuals and patients with chronic kidney disease: a randomized controlled trial.

BACKGROUND: The health benefits of dietary potassium have been linked to potassium-induced natriuresis. However, it is unknown whether potassium loading in patients with chronic kidney disease (CKD) induces natriuresis and to what degree it increases plasma potassium. We hypothesize that potassium-induced natriuresis remains intact and that potassium loading is safe in patients with CKD. METHODS: In this placebo-controlled crossover study, 18 healthy individuals and 9 patients with non-diabetic CKD stage G3b-G4 received an acute oral load of potassium chloride (KCl), potassium citrate (K-cit)

Evaluation of a powerless portable dialysate generator in a porcine model of continuous renal replacement therapy.

INTRODUCTION: Polytrauma is often characterised by rhabdomyolysis, hyperkalaemia and renal failure, which is treated with acute haemodialysis. Transport and storage of dialysate are expensive and cumbersome and require space that may be unavailable in resource-limited scenarios. We examined a portable, powerless dialysate generator in a porcine model of continuous renal replacement therapy (CRRT). METHODS: Anaesthetised swine (n=12) underwent bilateral nephrectomy and placement of a haemodialysis catheter. Intravenous potassium and urea were infused, and animals received CRRT with either a di

Plant-based low-protein diet for preventing malnutrition‒inflammation complex syndrome in adults with CKD: a single-centre preliminary experience.

BACKGROUND: Chronic kidney disease (CKD) is a rising public health concern. Low protein intake reduces uremic toxin generation and improves the hemodynamic profile of the kidneys, postponing the need for eventual dialysis. Vegetarian diets have been shown to positively influence cardiovascular health and reduce uremic toxin levels and inflammation in CKD patients. The so-called “Malnutrition-Inflammation Complex Syndrome” (MICS) is a nontraditional cardiovascular risk factor that is associated with poor overall outcomes in CKD patients. The type of protein, not only the amount, may be relevant

Efficacy and Safety of Finerenone in Patients with Idiopathic Membranous Nephropathy: A Multicentre, Prospective-Retrospective Real-World Study.

INTRODUCTION: The efficacy and safety of finerenone in patients with idiopathic membranous nephropathy (IMN) remain unclear, especially given the need for additional supportive therapeutic options. METHODS: This prospective-retrospective, multicentre, observational real-world study evaluated the efficacy and safety of finerenone in IMN patients with proteinuria ≥1 g/24 h. The primary outcome was the percentage change in proteinuria from baseline to 6 months with finerenone treatment. Secondary outcomes included the stratified reduction in proteinuria, and changes in estimated glomerular filtr

[Paradigm shift in the treatment of chronic hyperkalaemia to optimise cardiorenal patient prognosis].

Chronic hyperkalemia is an electrolyte imbalance that occurs mainly in patients with progressive kidney failure. It usually develops over a long period of time and symptoms are often mild. As chronic hyperkalemia is associated with increased morbidity and mortality, it should be treated. Treatment begins with a review of the medications that may be responsible for the hyperkalemia and careful therapy and correction of the metabolism. Dietary assessment now focuses on potassium intake from non-vegetable sources. Reducing the dose and/or discontinuing renin-angiotensin-aldosterone inhibitors sho

Drug stewardship for RAS inhibitors and SGLT2 inhibitors in chronic kidney disease: stay on, restart.

Renin-angiotensin system inhibitors (RASis) and sodium-glucose co-transporter 2 inhibitors (SGLT2is) are the cornerstones of management for patients with chronic kidney disease (CKD). The use of these treatments is limited by frequent discontinuations, increasing the risk of death and compromising cardiovascular and renal health. This narrative review explores scenarios that lead to discontinuation: adverse effects (i.e. RASi-related acute kidney injury and hyperkalaemia), progression of CKD, acute illness, surgery and contrast administration. After AKI, we recommend restarting RASis and SGLT2

Comparative safety profiles of spironolactone, eplerenone, and finerenone: a pharmacovigilance study based on FAERS data from 2004 to 2024.

BACKGROUND: Spironolactone, eplerenone, and finerenone are three commonly used mineralocorticoid receptor antagonists (MRAs) with cardioprotective and renoprotective effects. However, comparative real-world safety evaluations remain limited. This study aimed to assess and compare the adverse event (AE) profiles of the three MRAs using data from the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS). METHOD: We conducted a retrospective descriptive analysis using FAERS data from Q1 2004 to Q4 2024. Four disproportionality methods were employed to detect AE signals, includ

Potassium and the kidney.

In the past decade, understanding of the mechanisms by which the kidneys sense dietary potassium imbalance and affect the cardiorenal system has substantially advanced. These mechanisms explain why low-potassium diets promote salt-sensitive hypertension, whereas potassium-rich diets enhance natriuresis and lower blood pressure. Increasing dietary potassium may therefore be an effective strategy to reduce cardiovascular disease risk. Clinical trials have shown that replacing regular salt with potassium-enriched substitutes lowers blood pressure and reduces cardiovascular events and mortality. E

Case Report: Inhaled salbutamol in the successful treatment of life-threatening acute hyperkalaemia in an anaesthetised horse.

Hyperkalaemia is an uncommon complication of general anaesthesia in healthy horses. This case report describes the occurrence of life-threatening acute hyperkalaemia in a 13-year-old, female French Trotter anaesthetised for experimental right and left atrial 3D electro-anatomical mapping. Intra-operative development of hyperkalaemia (7.55 mmol/L) (Ref. 3.00-4.00 mmol/L) with atrial standstill on ECG necessitated transvenous ventricular pacing while initial treatment with insulin and glucose was initiated. Plasma potassium levels continued to increase (8.00 mmol/L) prompting adjunctive treatmen
