Multicentre aortic thromboembolism thrombolysis evaluation in a retrospective study of 183 cats: the MATTERS study.

INTRODUCTION: Although intravenous tissue plasminogen activator (TPA) is a mainstream treatment for humans with acute ischaemic stroke, it is uncommonly used in feline arterial thromboembolism (FATE). The purpose of this study was to retrospectively compare clinical reperfusion, functional recovery, complications, and survival in a large multicentre FATE population treated with or without systemic thrombolysis. METHODS: A retrospective evaluation of electronic medical records from two veterinary hospitals (2007-2020) was conducted. The inclusion criteria were a clinical diagnosis of FATE or u

Real-world effectiveness and safety of difelikefalin for chronic kidney disease-associated pruritus: a systematic review.

INTRODUCTION: Chronic kidney disease-associated pruritus (CKD-aP) affects 55% of hemodialysis patients and substantially impairs quality of life (QoL) and treatment adherence. Difelikefalin, a selective kappa-opioid receptor agonist, received regulatory approval following the KALM trials. However, the applicability of these findings to routine practice remains unclear. We systematically evaluated real-world evidence (RWE) on the effectiveness and safety of difelikefalin in patients with CKD-aP. METHODS: Comprehensive searches of PubMed, Embase, CENTRAL, Scopus, Web of Science, and CINAHL were

Diagnosis and management of acute hyperkalaemia.

Hyperkalaemia is a common and potentially life threatening electrolyte abnormality because of its association with malignant cardiac arrhythmias and sudden cardiac arrest. This condition is frequently encountered across acute and chronic care settings and disproportionately affects patients with comorbid conditions like chronic kidney disease, heart failure, diabetes mellitus, and those receiving drugs that impair renal potassium excretion. The most frequent causes of hyperkalaemia include impaired renal potassium elimination, from chronic kidney disease or pharmacological inhibition of the re

Early management of crush injury and crush syndrome: a Faculty of Pre-Hospital Care consensus statement.

Guidance is required for UK prehospital care providers managing patients with suspected crush injury to minimise preventable morbidity and mortality. This consensus statement gives relevant and practicable recommendations following a structured evidence review and a consensus process. Differences between this and previous guidance reflect, in part, the context for which these recommendations are intended. The role of aggressive fluid administration and concerns regarding hyperkalaemia are de-emphasised. This consensus statement may also be relevant for providers working in other developed heal

Drug failure in diabetic kidney disease: translational barriers, target validation and trial de-risking strategies.

Diabetic kidney disease (DKD) remains a difficult therapeutic setting in nephrology, metabolism and clinical pharmacology. Standard care has moved from glucose- and blood-pressure-centred treatment toward disease-modifying therapy built around renin-angiotensin-aldosterone system (RAAS) blockade, sodium-glucose cotransporter 2 (SGLT2) inhibitors, glucagon-like peptide-1 receptor agonists and nonsteroidal mineralocorticoid receptor antagonism. Yet DKD drug development is also defined by repeated translational failure. Many agents with strong mechanistic rationale have failed to improve hard kid

Correlated seasonal dyskalaemia in a primary care population: winter hyperkalaemia and summer hypokalaemia.

OBJECTIVES: In vitro serum potassium is affected by ambient temperature with peaks in the winter and troughs in the summer. We investigated whether primary care phlebotomy locations with higher winter hyperkalaemia rates exhibited higher summer hypokalaemia and evaluated a simple screening metric to identify high-risk locations. METHODS: Retrospective audit of Indexor-tracked primary care potassium results analysed at a UK hub laboratory between 1 July 2024 and 30 June 2025 was performed. Bivariate multilevel logistic regression modelling assessed whether locations with higher hyperkalaemia r

Potassium additives in ultra-processed foods identified from food label-analysis: More than meets the eye.

BACKGROUND AND AIM: Nutritional guidelines for managing hyperkalaemia in individuals with chronic kidney disease (CKD) emphasise an individualised approach, focusing on adjusting dietary potassium intake by selecting low-potassium foods and avoiding ultra-processed food (UPF). However, little is known about potassium additives in UPFs. Therefore, this study aimed to assess i) the prevalence of UPFs containing potassium additives across three supermarket chain stores, ii) which food categories most frequently contained potassium additives, iii) which potassium additives were most common and iv)

Eplerenone vs. spironolactone in heart failure: long-term cardiorenal and mortality outcomes in a registry-based cohort.

AIMS: Eplerenone and spironolactone differ in pharmacological profile, but whether this translates into meaningful differences in clinical outcomes in heart failure (HF) remains uncertain. We compared the two agents in a registry-based new-user cohort with serial laboratory data. METHODS AND RESULTS: In this observational cohort study, adults with a first HF diagnosis in Stockholm County (2005-2019) who were prescribed either spironolactone or eplerenone within 6 months were identified from linked Swedish registries with individual-level laboratory results. Follow-up was analysed under an int

Mineralocorticoid receptor antagonists: Efficacy and safety in chronic kidney disease.

The mineralocorticoid receptor (MR) has a multifaceted role in normal physiologic functions. However, research has uncovered the deleterious consequences of overactivation of the MR. It has been implicated in chronic kidney disease (CKD) via its strong association with glomerulosclerosis, interstitial fibrosis, proteinuria, and decline in glomerular filtration rate, as well as in cardiovascular diseases through mechanisms such as excessive extracellular matrix accumulation, oxidative stress, haemodynamic changes, and sustained inflammation. As such, efforts have poured into developing therapeu

NR0B1 Gene Variants as Rare Forms of Primary Adrenal Insufficiency in Children: Case Report and Narrative Review.

Primary adrenal insufficiency (PAI) is a severe and potentially life-threatening condition characterised by the inability of the adrenal cortex to produce enough glucocorticoids and/or mineralocorticoids. The clinical signs of PAI are primarily due to deficient steroid hormone synthesis and include weight loss, orthostatic hypotension secondary to dehydration, hyponatremia, hyperkalaemia, and hypoglycaemia. In the paediatric population, PAI is most commonly associated with inherited monogenic disorders, particularly enzyme deficiencies. X-linked adrenal hypoplasia congenita (AHC) is a rare con

Rhabdomyolysis Associated With Heated Massage Therapy in a Patient Without Major Trauma: A Case Report.

A 29-year-old professional dancer developed rhabdomyolysis after distal radial fracture fixation complicated by laryngospasm, negative pressure pulmonary oedema, suxamethonium use, and ICU admission. Creatine phosphokinase (CPK) peaked at 36,429 U/L with dark urine and hyperkalaemia. Recent prolonged heated massage-bed use, with minor trauma and suxamethonium, may have contributed. Biopsy confirmed skeletal muscle necrosis.

Efficacy and safety of finerenone in patients with chronic kidney disease: an individual participant data pooled analysis (INFINITY).

BACKGROUND: Overactivation of the mineralocorticoid receptor is a common pathway for chronic kidney disease (CKD) progression across multiple disease aetiologies. Finerenone, a non-steroidal mineralocorticoid receptor antagonist, has shown kidney and cardiovascular benefits in CKD due to type 2 diabetes, but its efficacy and safety across disease aetiologies, and levels of glycaemia, estimated glomerular filtration rate (eGFR), and albuminuria have not been evaluated. The aim of this study was to evaluate the efficacy and safety of finerenone across the spectrum of CKD. METHODS: We conducted

Antimicrobial toxic epidermal necrolysis in an ICU patient with multisystem organ failure and sepsis.

Toxic epidermal necrolysis (TEN) is a rare, life-threatening dermatological emergency characterised by blistering and desquamation, often triggered by medications. Early identification and discontinuation of offending agents are critical for improving outcomes. Here, we describe a woman in her 30s with a complex medical history, including systemic lupus erythematosus and common variable immunodeficiency, who developed TEN following antifungal and antibacterial therapy. She initially presented with respiratory distress and was treated for pneumonia. Over the ensuing days, her clinical course rap

PRO/CON debate: nonsteroidal versus steroidal mineralocorticoid receptor antagonists in chronic kidney disease: progress, pragmatism, and remaining uncertainties.

Mineralocorticoid receptor antagonists (MRAs) are a central component of contemporary cardiorenal protection in chronic kidney disease (CKD). Classic steroidal MRAs, spironolactone and eplerenone, have long been used to treat hypertension and heart failure and to reduce albuminuria, but their wider use in CKD has been constrained by hyperkalaemia and endocrine side-effects. Nonsteroidal MRAs (nsMRAs), typified by finerenone, were developed to maintain antiproteinuric and antifibrotic efficacy while improving tissue selectivity, pharmacokinetics, and tolerability. Large outcome trials in type 2

Transplant Renal Artery Stenosis Presenting as Posterior Reversible Encephalopathy Syndrome in a Post-Kidney Transplant Patient: A Case Report.

INTRODUCTION: Transplant renal artery stenosis (TRAS) is a reversible cause of severe hypertension and graft dysfunction. Posterior reversible encephalopathy syndrome (PRES) in kidney transplant recipients is commonly attributed to calcineurin inhibitor (CNI) toxicity; however, the biochemical signatures of CNI toxicity and TRAS differ markedly. CASE PRESENTATION: We report a 32-year-old woman, 5 months post-transplant, presenting with seizures, severe hypertension, and MRI-confirmed PRES. She exhibited hypokalaemia (2.6 mmol/L) and metabolic alkalosis (pH 7.50, bicarbonate 34 mmol/L) - findi

The Endocrine Giants of Chronic Kidney Disease.

Chronic Kidney Disease (CKD) is increasingly being recognized as an endo-metabolic disorder in which a cascade of hormonal and electrolyte imbalances impacts the morbidity and mortality. We propose six "endocrine giants" of CKD- anaemia, secondary hyperparathyroidism, hypovitaminosis D, hypogonadism, hyperkalaemia, and hyponatraemia- each exerting a detrimental effect on cardiovascular, skeletal, metabolic, and neurological health. Their early recognition and prompt management can attenuate downstream cardiometabolic sequelae. This "endocrine giants" framework provides an integrative approach
