Navigating Hyperkalaemia- The A-to-H Clinical Strategy.

Hyperkalaemia is a life-threatening dyselectrolytaemia which is characterized by elevated serum potassium levels. It needs a systematic and overarching approach to effectively manage a case of hyperkalaemia. We propose a structured A-to-H approach to guide clinicians in managing hyperkalaemia, ensuring that no critical aspect is neglected or inadvertently missed. This approach involves Analytical accuracy, Background, Clinical context, Diagnosis and drugs, Evaluation, Follow-up, Guidelines adherence, and Health education. With this article, we reiterate the need to properly assess and promptly

Immunosuppressive therapy and nutritional diseases of patients after kidney transplantation: a systematic review.

BACKGROUND: Kidney transplantation (kTx) is by far the most effective method of treating end-stage renal disease, with immunosuppressive therapy being obligatory for all, except identical twins. Despite kTx being the most effective treatment for end-stage renal disease, the patients face significant morbidity. They are often burdened with diabetes, anaemia, lipid disorders, all of which pose heightened risks for cardiovascular disease. Knowing that nutritional status plays a significant role in post-transplant results including graft survival, we conducted this systematic review with the aim t

Hyperkalaemia during a general anaesthetic in a 5-year-old domestic short-haired cat.

This case report details an incident of hyperkalaemia that developed under general anaesthesia in a domestic cat with no prior health concerns. A 5-year-old male neutered domestic short-haired cat presented for dental extractions and subsequently developed severe bradycardia and electrocardiographic abnormalities during the procedure. Initial treatment with atropine provided limited improvement, and further investigation revealed a serum potassium concentration of 7.7 mmol/L. Despite interventions, the anaesthetic procedure was aborted due to persistent hyperkalaemia. The cat recovered smoothl

Combination therapy as a new standard of care in diabetic and non-diabetic chronic kidney disease.

UNLABELLED: Combination therapy, involving the use of multiple medications together, is becoming a new standard of care for chronic kidney disease (CKD). For people with CKD, combination therapy offers the promise of preventing kidney failure and reducing the risk of heart problems. This approach is appealing because different drugs target distinct mechanisms involved in CKD progression. For instance, some target immune responses, others reduce kidney inflammation and scarring, while others improve blood pressure within the kidneys. Data from large clinical trials suggest that each treatment w

Guidelines for the management of diabetes-related ketoacidosis (DKA) have been poorly adopted and implemented, resulting in a lack of improvement in outcomes.

AIMS: The Joint British Diabetes Society-Inpatient (JBDS-IP) group recommends reducing fixed rate intravenous insulin infusion (FRIII) from 0.1 to 0.05 units/kg/h when blood glucose falls <14 mmol/L to reduce the risk of complications associated with acute management of diabetes-related ketoacidosis. However, whether this change results in real-world improvements is not known. METHODS: We performed a retrospective review of DKA admissions between October 2021 and March 2023 across five hospitals in the United Kingdom. We collated data on demographics, biochemical profiles, management interven

Split red blood cell units contain defined extracellular K+ levels, which are improved by a washing procedure.

BACKGROUND AND OBJECTIVES: We should control free K+ during massive transfusion (>80 mL/kg) of red blood cells (RBCs) in small children. To manage this, several national and international guidelines recommend using RBCs stored only up to 7 days. We tested a washing step for RBCs in saline-adenine-glucose-mannitol (SAGM) with or without irradiation to reduce supernatant K+ levels, improve quality and potentially extend the shelf life of stored RBCs. MATERIALS AND METHODS: RBCs of 240-330 mL were prepared from whole blood donations, then split into halves and stored in SAGM solution at 4 ± 2°C

Low-dose Spironolactone Combined with ACEIs/ARBs May Reduce Cardiovascular Events in Patients with CKD Stages 3b-5: A Nationwide Population-Based Cohort Study in Taiwan.

Background: ACE inhibitors (ACEIs) and angiotensin receptor blockers (ARBs) are commonly prescribed for hypertension and chronic kidney disease (CKD) management, but they can increase the risk of renal function deterioration and hyperkalaemia. Spironolactone, known for reducing cardiovascular events in CKD patients, faces limited use due to the risk of hyperkalaemia. This study evaluates the potential efficacy and complications of low-dose spironolactone as an adjunct therapy in patients with CKD stages 3b to 5 who are maintained on ACEIs or ARBs. Materials and methods: Hypertensive CKD patien

Risk of Hospital Admissions and Death in Patients with Heart Failure and Chronic Kidney Disease: Findings from a Novel Multidisciplinary Clinic.

INTRODUCTION: Patients with heart failure (HF) and chronic kidney disease (CKD) are often suboptimally treated due to concerns of hyperkalaemia, declining kidney function, and hypotension. They commonly suffer from fluid overload which can lead to frequent hospitalisations and death. This research aims to determine the characteristics associated with hospital admissions and death in patients with CKD and HF. METHODS: Consecutive patients with CKD stage 3-5 and HF (regardless of ejection fraction) attending a large, specialised CKD-HF clinic between 12/Sept/2019 and 11/Nov/2021 were identified

In Silico Investigation of Mineralocorticoid Receptor Antagonists: Insights into Binding Mechanisms and Structural Dynamics.

The mineralocorticoid receptor (MR) is a steroid hormone receptor that plays a key role in regulating sodium and water homeostasis and blood pressure. MR antagonists are a guideline recommended for therapy for the treatment of hypertension and cardiovascular disease but can cause hyperkalaemia. Modelling was performed for binding of the endogenous ligands aldosterone and cortisol and MR antagonist spironolactone to the ligand binding domain (LBD) of the MR. A molecular docking screen of compounds that were structurally similar to known antagonists was performed, leading to the identification o

Initiating renin-angiotensin system inhibitors in chronic kidney disease.

Chronic kidney disease (CKD) is a heterogeneous group of disorders characterised by abnormalities in kidney structure or function, present for at least 3 months. Renin-angiotensin system (RAS) inhibitors are established treatments in the management of CKD, reducing the risk of adverse kidney and cardiovascular events. Despite an extensive evidence base and decades of clinical experience, they remain underused, underdosed and frequently discontinued due to concerns about changes in kidney function and hyperkalaemia. In this article, we provide an overview of initiating RAS inhibitors in adults

Optimizing renin-angiotensin-aldosterone inhibition in advanced chronic kidney disease: balancing benefits and risks.

PURPOSE OF REVIEW: Renin-angiotensin-aldosterone system inhibitors (RAASi), including angiotensin-converting enzyme inhibitors (ACEi) and angiotensin II receptor blockers (ARBs), are fundamental in chronic kidney disease (CKD) management, particularly in proteinuric conditions. However, their use in advanced CKD (eGFR <30 ml/min/1.73 m 2 ) remains debated because of risks of hyperkalaemia, acute kidney injury (AKI), and hypotension. This review evaluates the latest evidence, including the STOP-ACEi trial, to inform the risks and benefits of RAASi in advanced CKD. RECENT FINDINGS: The STOP-ACE

Aldosterone-targeted therapies: early implementation in resistant hypertension and chronic kidney disease.

Treatment-resistant hypertension (TRH) often coexists with chronic kidney disease (CKD), and the presence of both conditions increases the risk of adverse cardiovascular outcomes. Patients with TRH and CKD exhibit enhanced aldosterone and mineralocorticoid receptor expression, which promote inflammation and fibrosis in cardiac and renal tissues, contributing to the development and progression of cardiorenal diseases. Both achieving optimal blood pressure (BP) control and mitigating the risk of aldosterone-related adverse events are cornerstones in the management of patients with TRH and CKD. M
