INTRODUCTION: Dent disease type 2 (DD-2) is a rare X-linked recessive renal tubulopathy caused by mutations in the OCRL gene. Clinically it is characterised by low molecular weight proteinuria (LMWP), hypercalciuria, and nephrocalcinosis. The phenotypic spectrum of DD-2 is expanding with the identification of new pathogenic variants.
CASE REPORT: A 12-year-old male presenting with marked short stature and poor weight gain was found to have LMWP, haematuria, hypercalciuria, and bilateral nephrocalcinosis on initial clinical evaluation. Renal function was normal. There was no relevant family hi

Diamond-Blackfan anaemia (DBA) is a rare inherited bone marrow failure syndrome causing pure red cell aplasia. Patients frequently require long-term red cell transfusions, predisposing them to iron overload. This case describes a patient with DBA with rapid ejection fraction decline secondary to acute-onset iron overload cardiomyopathy. A 28-year-old male patient with known transfusion-dependent DBA and established iron overload and on deferasirox, presented with a one-week history of orthopnoea, dyspnoea and peripheral oedema. Echocardiography demonstrated severe biventricular impairment with

Severe hypokalemia is a rare but potentially reversible cause of malignant arrhythmias and cardiac arrest. Early recognition of significant fluid, electrolyte, and acid-base disturbances in patients with intestinal urinary diversion may be life-saving. We present the clinical course of a 55-year-old woman who developed severe hyperchloremic metabolic acidosis and extreme hypokalemia following prolonged vomiting and high-output fluid loss through a Bricker ureteroileocutaneostomy. The patient was admitted to the intensive care unit in a peri-arrest condition, where cardiac arrest occurred short

BACKGROUND: Complement-dependent cytotoxicity (CDC) of CD38 monoclonal antibodies has been associated with infusion-related reactions and might contribute to reduced clinical efficacy of CDC-reliant CD38 antibodies, particularly in patients with relapsed or refractory multiple myeloma with 1q21+. We aimed to evaluate felzartamab, a novel anti-CD38 monoclonal antibody that retains potent antibody-dependent cellular cytotoxicity and antibody-dependent cellular phagocytosis while exhibiting markedly attenuated CDC activity in Chinese patients with relapsed or refractory multiple myeloma.

VIPoma is a rare pancreatic neuroendocrine tumor characterized by the overproduction of vasoactive intestinal peptide (VIP), typically manifesting as watery diarrhea, hypokalaemia and metabolic acidosis. Its presentation can be atypical, complicating diagnosis. We report the case of a 30-year-old male who initially presented with obstructive jaundice and fatigue, with imaging revealing a pancreatic head mass and liver metastases. Histopathological analysis confirmed a lowgrade pancreatic neuroendocrine tumor. The clinical course was complicated by portal vein thrombosis and metastatic spread.

Quinine, a Cinchona alkaloid and a cornerstone of malaria therapy in resource-limited settings, produces predominant sodium-channel blockade alongside modest inhibition of the rapid delayed-rectifier potassium current (IKr). Although generally safe at the population level, even modest IKr blockade can precipitate life-threatening Torsade de Pointes (TdP) in patients with severely impaired repolarization reserve. We report an 8-year-old boy with severe Plasmodium falciparum malaria who, on the third day of intravenous quinine, developed profound QTc prolongation (730 ms) followed by recurrent T

AIMS: To evaluate potential pharmacokinetic interactions between phenobarbitone and retigabine, a new antiepileptic drug.
METHODS: Fifteen healthy men received 200 mg of retigabine on day 1. On days 4-32, phenobarbitone 90 mg was administered at 22.00 h. On days 26-32, increasing doses of retigabine were given to achieve a final dose of 200 mg every 8 h on day 32. The pharmacokinetics of retigabine were determined on days 1 and 32, and those for phenobarbitone on days 25 and 31.
RESULTS: After administration of a single 200 mg dose, retigabine was rapidly absorbed and eliminated with a mean

BACKGROUND: The alcohol industry builds engagement with United Nations (UN) organisations to enhance its corporate image and influence policy, supported by the UN's endorsement of public-private partnerships (PPPs). However, the extent of the alcohol industry's relationships with the UN remains unclear due to limited reporting.
METHODS: We searched the websites of 57 UN-affiliated entities and 18 transnational alcohol corporations (TNACs) for evidence of partnerships or relationships between the UN and the alcohol industry. We summarised the UN entities and TNACs involved in formal partnershi

AIMS: Loss-of-function (LOF) mutations of the cardiac Na+ channel (SCN5A) are causatively associated with the Brugada Syndrome (BrS). However, the onset of Ventricular Fibrillation (VF) is a rare event, and critical factors favouring the pathological phenotype remain often elusive. This study explores how concomitant triggering conditions may impact on VF onset in a symptomatic proband carrying the S805L/SCN5A BrS mutation.
METHODS AND RESULTS: Clinical, in-vitro, numerical, and structural analyses were performed. A 67-year-old male was resuscitated after cardiac arrest, and clinical analysis

AIMS: Several diuretic strategies, including furosemide i.v. boluses (FB) or continuous infusion (FC), are used in acute heart failure (AHF).
METHODS AND RESULTS: We systematically searched phase 3 randomized clinical trials (RCTs) evaluating diuretic regimens in admitted AHF patients within 48 h and irrespective of clinical stabilization. We calculated the odds ratio (OR) of FC or FB plus another diuretic (sequential nephron blockade, SNB) compared to FB alone on 24 h weight loss (WL) and worsening renal function (WRF), with a random-effects model with inverse variance weighting. BACKGROUND: Tazobactam/piperacillin (TAZ/PIPC), widely used for treating various infections, is often discontinued due to hypokalaemia, although the preventive measures remain unclear. This study aimed to identify the incidence, clinical characteristics and associated risk factors for TAZ/PIPC-induced hypokalaemia.
METHODS: Hospitalised patients receiving TAZ/PIPC therapy for >2 days were included. The primary endpoint was the incidence of hypokalaemia (serum potassium level ≤3 mEq/L) and identification of associated risk factors. Survival classification and regression tree (CART) analyses we

BACKGROUND: Posaconazole immediate release (IR) suspension is the standard product for enteral feeding tube (EFT) administration but has erratic pharmacokinetic behaviour, often resulting in subtherapeutic concentrations. Limited evidence suggests that posaconazole oral delayed-release (DR) tablets may be crushed.
METHODS: This was a retrospective cohort study of hospitalized adults who received EFT administration of posaconazole IR suspension or crushed DR tablets between 2018 and 2024. Patients were eligible if they had at least one serum drug concentration drawn ≥5 days after posaconazole

SUMMARY: Apparent mineralocorticoid excess (AME) syndrome is a rare condition caused by the inhibition of the renal enzyme 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2). The most common acquired cause is chronic liquorice ingestion. We report the case of a 61-year-old woman who developed profound hypokalaemia, metabolic alkalosis, and QT prolongation due to excessive liquorice tea consumption. This case underscores the importance of detailed dietary history in patients with unexplained electrolyte disturbances and demonstrates the potentially fatal cardiovascular consequences of a widely

IntroductionEx vivo perfusion (EVP) in heart transplantation (HTX) enables transportation of explanted organs over longer distances as well as monitoring of cardiac metabolism under unloaded coronary perfusion, thereby expanding the possible pool of suitable donors for HTX. We aimed at identifying possibly modifiable risk factors predicting impaired outcome after HTX through analysis of perfusion parameters, donor and recipient characteristics.MethodsWe included 64 patients after HTX using EVP (TransMedics Organ Care System) between February 2010 and October 2022 in this retrospective, single-

Primary aldosteronism is common in those diagnosed with hypertension and distinguishing primary aldosteronism from its mimics allows tailored management. A woman in her 30s presented to an Australian hospital with hypertensive crisis (209/100 mm Hg) and spontaneous hypokalaemia (2.9 mmol/L). Laboratory tests revealed hyperreninaemic hyperaldosteronism with a normal aldosterone-renin ratio. CT imaging showed focal areas of soft tissue encasement and enhancement surrounding multiple arteries, including the left renal artery, raising suspicion of a medium-vessel vasculitis. Further investigation

BackgroundSpinal muscular atrophy (SMA) is caused by loss-of-function of the survival motor neuron 1 (SMN1) gene and deficiency of the ubiquitously expressed SMN protein. Genetic therapies can partially rescue motor units and improve prognosis of SMA, but effects of SMN shortage in other tissues has not been studied in detail.MethodsWe longitudinally assessed renal function in a cohort of patients with SMA before and after the start of genetic therapies.ResultsWe enrolled 263 patients with SMA types 1c-4. Median age was 33 years (IQR: 22-49). Fifty (19%) patients had serum cystatin C based eGF
