Ectopic ACTH syndrome secondary to small-cell oropharyngeal cancer.

A woman in her 80s with known metastatic small-cell oropharyngeal cancer was admitted after a fall. There was proximal limb weakness with hypertension, metabolic alkalosis, hypokalaemia and hyperglycaemia. CT scans revealed progression of her cancer with enlarging liver and lung metastases. Her admission was complicated by delirium and progressive dysphagia relating to her primary oropharyngeal malignancy. Cortisol and adrenocorticotropic hormone (ACTH) were markedly raised and were not suppressed after overnight and high-dose dexamethasone tests. Typical clinical features of Cushing's syndrom

Paralytic ileus in 57 cows - symptoms, diagnosis and treatment.

BACKGROUND: Paralytic ileus (PI) also known as functional ileus or adynamic ileus occurs when intestinal motor activity is impaired. Cessation of ingesta passage leads to the accumulation of fluid and gas causing intestinal dilatation. With this type of ileus, intestinal transit is functionally impaired in the absence of a physical obstruction. The present retrospective study describes the diagnosis, treatment and outcome of 57 cows with PI. RESULTS: Colic occurred in 43.9% of the cows, and intestinal and rumen motility was reduced or absent in 92.9% (52/56) and 82.5% (47/57) of the cows, res

Hyponatremia and rhabdomyolysis from excessive water intake for ureteral stone expulsion in a patient with mental disorder: A case report.

RATIONALE: Primary polydipsia refers to excessive water intake due to psychogenic or non-psychogenic causes without being secondary to conditions such as hyperglycemia or renal dysfunction. Most cases of primary polydipsia are psychogenic in nature, with few cases of non-psychogenic primary polydipsia reported in the literature. In this case, the patient's excessive water intake appeared to be influenced by both psychogenic and non-psychogenic factors. PATIENT CONCERNS: A 43-year-old male patient with a history of psychiatric illness was diagnosed with ureteral stones and was engaged in exces

Liquorice-induced pseudohyperaldosteronism: a rare cause for severe hypertension.

This case report describes an adult man in his 50s with a history of type 2 diabetes and previously well-controlled hypertension, who presented with uncontrolled hypertension, muscle weakness and fatigue. Biochemical testing revealed hypokalaemia. There was no evidence of renal/renovascular disease. Evaluation for primary hyperaldosteronism revealed suppressed renin concentration with low aldosterone levels. Detailed history taking revealed the daily consumption of three to four cups of liquorice root tea for 3 months. Glycyrrhetinic acid, a metabolite of glycyrrhizic acid found in liquorice,

Establishment and Validation of Sensitive Liquid Chromatography-Tandem Mass Spectrometry Method for Aldosterone Quantification in Feline Serum with Reference Interval Determination.

Aldosterone, a mineralocorticoid hormone synthesised in the adrenal cortex, is essential for maintaining electrolyte balance and fluid homeostasis. Its role in feline physiology remains underexplored, despite its importance in regulating sodium reabsorption and potassium excretion via mineralocorticoid receptors in renal tubules. This study is warranted given aldosterone's importance in cats, particularly in light of their unique physiological traits, including highly concentrated urine and sensitivity to hydration status. Primary hyperaldosteronism, the most common feline adrenocortical disor

Prevalence of Primary Aldosteronism Among Patients With Hypertension.

BACKGROUND: Despite being a leading cause of secondary hypertension and association with a higher incidence of cardiovascular morbidity and mortality the prevalence of primary aldosteronism (PA) is poorly studied. OBJECTIVE: We assessed the prevalence of PA in participants being treated as essential hypertension (EHTN). DESIGN AND PARTICIPANTS: In this prospective, cross-sectional study, 492 participants with diagnosis of EHTN were evaluated. The screening for PA was conducted by measuring plasma aldosterone concentration (PAC) and direct renin concentration (DRC), from which the aldosterone

Thyrotoxic periodic paralysis: diagnostic and management considerations.

Thyrotoxic periodic paralysis is a rare but potentially life-threatening complication of thyrotoxicosis characterised by hypokalaemia-induced flaccid paralysis due to an intracellular potassium shift rather than total body depletion. We report a male in his early 40s of Filipino origin who awoke with acute bilateral lower-limb weakness after he had consumed a large carbohydrate meal and performed strenuous exercise within the preceding 12 hours. Venous blood gas was normal; initial serum potassium was 1.7 mmol/L with phosphate 1.07 mmol/L. The ECG showed sinus tachycardia, first-degree atriove

A case of ascending paralysis in pregnancy: Thiamine deficiency mimicking Guillain-Barré syndrome.

Thiamine (vitamin B1) deficiency may present with diverse symptoms and is often triggered by chronic alcoholism, severe malnutrition or hyperemesis gravidarum. While typically diagnosed clinically, atypical presentations may delay recognition and treatment. We report a 26-year-old pregnant woman in her second trimester with prolonged nausea and vomiting since early pregnancy. She developed ascending lower limb paralysis, dysarthria, horizontal nystagmus, and lagophthalmos. Initially managed as hypokalaemia periodic paralysis with thyrotoxicosis and later Guillain-Barré syndrome (GBS), her symp

Primary Aldosteronism and Cognitive Dysfunction: A Case-Control Study.

Background: Primary aldosteronism is characterized by elevated aldosterone levels, leading to adverse effects such as hypertension, hypokalaemia and increased risk for cardiovascular and cerebrovascular events. Aldosterone impacts the central nervous system by promoting vascular remodelling and oxidative stress, potentially impairing cognitive function. The presence of mineralocorticoid receptors in the hippocampus, a key region for cognition, further suggest a link between primary aldosteronism and cognitive dysfunction. This study aims to further explore the association between hyperaldoster

Resistant hypertension post-transsphenoidal surgery for pituitary Cushing's disease, leading to a diagnosis of primary aldosteronism.

SUMMARY: We describe a case of a 42-year-old gentleman, 5 years post-transsphenoidal surgery (TSS) for pituitary-dependent Cushing's disease, initially presenting with malignant hypertension. Despite an initial improvement in his blood pressure post-TSS, he was found to be persistently hypertensive on follow-up despite no clinical or biochemical evidence of recurrence of hypercortisolism. His blood pressure remained elevated despite five antihypertensive agents. His renin concentration was <5 mIU/L (9-103.5) and aldosterone concentration was 877 pmol/L (0-670). A subsequent CT of the adrenals

Staff Training to Improve the Preschool Educational Environment for Children with Special Needs.

The verification of model transformations is important for realizing robust model-driven engineering technologies and quality-assured automation. Many approaches for checking properties of model transformations have been proposed. Most of them have focused on the effective and efficient detection of property violations by contract checking. However, there remains the fault localization step between identifying a failing contract for a transformation based on verification feedback and precisely identifying the faulty rules. While there exist fault localization approaches in the model transforma
