Refeeding Syndrome Complicated by Wernicke's Encephalopathy Following Severe Hyperemesis Gravidarum and Medical Management of Miscarriage: A Case Report.

Hyperemesis gravidarum (HG) is a severe form of nausea and vomiting in pregnancy that may result in dehydration, malnutrition, and electrolyte imbalance. Wernicke's encephalopathy (WE), caused by thiamine deficiency, and refeeding syndrome (RFS), a metabolic complication of nutritional rehabilitation, are rare but serious sequelae of prolonged starvation in HG. We present the case of a multiparous woman in her 30s with severe HG and missed miscarriage at approximately 16 weeks, who developed refractory hypokalaemia, hypomagnesaemia, and evolving neurological symptoms following medical manageme

Gallbladder neuroendocrine carcinoma: a rare cause of ectopic ACTH secretion.

This report presents a case of a middle-aged man who was incidentally diagnosed with a gallbladder (GB) mass. Though he had some features of Cushing syndrome (CS), including those due to mineralocorticoid effects (persistent hypokalaemia), the classical features were absent. Abdominal CT scan showed a large mass arising from the GB with liver and lymph node metastasis. Biochemical evaluation confirmed adrenocorticotropic hormone (ACTH)-dependent CS. A biopsy from the mass revealed a poorly differentiated neuroendocrine carcinoma. The patient was started on ketoconazole and appropriate manageme

Pulmonary neuroendocrine tumour-associated ectopic Cushing's syndrome: diagnostic challenges and multidisciplinary management.

SUMMARY: Ectopic adrenocorticotropic hormone (ACTH) secretion is a rare cause of Cushing's syndrome (CS), often associated with neuroendocrine tumours (NETs). Early diagnosis can be difficult due to variable clinical manifestations and psychiatric symptoms that may obscure the underlying endocrine disorder. We report a case of severe ectopic ACTH-dependent Cushing's syndrome secondary to a pulmonary NET, highlighting the importance of multidisciplinary management, advanced imaging with positron emission tomography/computed tomography with gallium-68-labelled DOTA-D-Phe-Tyr-octreotide (68Ga-DOT

Efficacy and safety of relacorilant for the treatment of patients with Cushing's syndrome (GRACE): a multicentre, phase 3, double-blind, placebo-controlled, randomised-withdrawal study.

BACKGROUND: Relacorilant is a selective glucocorticoid receptor modulator designed to reduce excess cortisol activity by competing with cortisol for glucocorticoid receptor binding, mitigating the clinical manifestations of endogenous hypercortisolism (Cushing's syndrome). The aim of this study was to assess the efficacy and safety of relacorilant in adults with endogenous hypercortisolism. METHODS: This multicentre, phase 3, double-blind, placebo-controlled, randomised-withdrawal study enrolled adults with endogenous hypercortisolism and hypertension, hyperglycaemia, or both and was conducte

Predicting primary aldosteronism: risk stratification-based and machine learning-based models.

AIMS: Primary aldosteronism (PA) screening is difficult in an unselected population of hypertensive patients, as it can be challenging. We report new algorithms based either on risk stratification or machine learning models to improve the screening of PA. METHODS AND RESULTS: 15,507 patients with confirmed hypertension aged 18-65 years with full workup for PA screening were included in this study. We developed PAstrat, a risk stratification-based algorithm by generating patient groups of interest and assess probability of PA in each group, and two different machine learning models, logistic r

[Primary aldosteronism in resistant hypertension unmasked by a thiazide-induced severe hypokalaemia].

Primary aldosteronism is the most common cause of secondary hypertension but remains frequently underdiagnosed in primary care. We report a 71-year-old man with resistant hypertension and low-normal serum potassium in whom a minimal increase in hydrochlorothiazide dosage (12.5 to 25 mg) precipitated severe hypokalaemia (1.6 mmol/L), metabolic alkalosis, and rhabdomyolysis. An office electrocardiogram showed hypokalaemia-related changes, prompting urgent referral to the emergency department. Further evaluation revealed an elevated aldosterone-to-renin ratio and a left adrenal nodule, supporting

Bartter Syndrome With Normal Aldosterone Level and Acute Pancreatitis: An Unusual Presentation.

Bartter syndrome is a rare autosomal recessive renal tubulopathy, which classically presents with hypokalemic metabolic alkalosis, hyperreninemia, and hyperaldosteronism. We report an atypical presentation in a 16-year-old male who was diagnosed following an initial manifestation of acute pancreatitis. Key biochemical findings included profound hypokalemia (1.4 mEq/L), metabolic alkalosis, hyperreninemia (218.5 uIU/mL), and hypercalciuria, yet with a normal serum aldosterone level, which is a notable deviation from the classic biochemical profile. The acute pancreatitis was attributed to sever

Transplant Renal Artery Stenosis Presenting as Posterior Reversible Encephalopathy Syndrome in a Post-Kidney Transplant Patient: A Case Report.

INTRODUCTION: Transplant renal artery stenosis (TRAS) is a reversible cause of severe hypertension and graft dysfunction. Posterior reversible encephalopathy syndrome (PRES) in kidney transplant recipients is commonly attributed to calcineurin inhibitor (CNI) toxicity; however, the biochemical signatures of CNI toxicity and TRAS differ markedly. CASE PRESENTATION: We report a 32-year-old woman, 5 months post-transplant, presenting with seizures, severe hypertension, and MRI-confirmed PRES. She exhibited hypokalaemia (2.6 mmol/L) and metabolic alkalosis (pH 7.50, bicarbonate 34 mmol/L) - findi

Hypertension action bundle including training of healthcare professionals to improve the detection of primary aldosteronism: a cohort study.

OBJECTIVE: As a blood pressure clinic, we have developed a hypertension action bundle including a training programme to facilitate secondary hypertension screening, delivered during three phases: P1 (2016-2018): for hospital practitioners; P2 (2019-2021): for practitioners working outside of hospitals; P3 (2022-2024): consolidation phase. We hypothesised that a training programme could enhance diagnosis of primary aldosteronism (PA). DESIGN AND METHOD: This study included patients diagnosed with PA between 2016 and 2024. The primary objective was to investigate the incidence of PA diagnosis a

Case report: CMR work-up for a case of long QT syndrome, dilated left ventricle, and hypertension-when incidental findings unmasked the fundamental issue.

BACKGROUND: Cardiac magnetic resonance imaging (CMR) is increasingly used for detailed evaluation of cardiac structures and function. Its wide field of view also allows identification of extracardiac abnormalities, some of which may significantly impact clinical management. CASE SUMMARY: We present a case of a 44-year-old woman referred for evaluation of prolonged QT interval and hypertension. Investigations revealed mild left ventricular dilatation, and suspicion of long QT syndrome (LQTS). CMR clarified the underlying cardiac abnormalities, identifying a patent ductus arteriosus (PDA) as th

Secondary causes and renal outcomes in apparently resistant hypertension: a retrospective cohort study.

OBJECTIVE: In this study, we aimed to determine the prevalence and spectrum of secondary hypertension among patients with apparently resistant arterial hypertension (aRH) and to compare the rate of decline in estimated glomerular filtration rate (eGFR) between those with primary aldosteronism (PA) and true resistant arterial hypertension (RAH). METHODS: This retrospective cohort study involved 790 patients with aRH referred to a hypertension excellence centre. All patients fulfilled pharmacological criteria for aRH and underwent a screening protocol to evaluate secondary hypertension. We comp

Renin-aldosterone testing: The essentials.

Primary aldosteronism (PA) is a common yet significantly underdiagnosed cause of secondary hypertension, estimated to affect around 9.4% of hypertensive adults. This is critical because patients with PA face markedly higher rates of adverse cardiovascular and renal outcomes, risks substantially mitigated by targeted treatment (surgery or medical therapy). Despite this, screening rates remain critically low. This article provides a practical, evidence-based approach to PA screening specifically for generalist settings, focusing on contemporary test interpretation and mitigating common pitfalls.

MRI-based personalisation of neoadjuvant chemotherapy duration in HER2-positive early breast cancer (TRAIN-3): primary results from a multicentre, single-arm, phase 2 study.

BACKGROUND: Neoadjuvant treatment comprising six to nine cycles of anti-HER2 based chemotherapy yields high pathological complete response rates and excellent survival outcomes in patients with stage II-III HER2-positive breast cancer but comes with noteworthy side-effects. The TRAIN-3 study investigated if patients treated with this regimen who have a rapid complete radiological response on MRI are candidates for early surgery. The study showed that one-third of patients with hormone receptor-negative tumours and one-sixth of patients with hormone receptor-positive tumours had a pathological

Bortezomib and vorinostat in combination with mitoxantrone, dexamethasone, and pegasparaginase during induction and reinduction for infants with acute lymphoblastic leukaemia: a multicentre single-arm phase 1/2 study.

BACKGROUND: Acute lymphoblastic leukaemia in infants remains a therapeutic challenge. In preclinical studies we previously identified bortezomib and vorinostat as active agents against KMT2A rearranged (KMT2Ar) leukaemia. The aim of this study was to determine the tolerability of incorporating bortezomib and vorinostat into an acute lymphoblastic leukaemia-chemotherapy backbone for newly diagnosed infants with acute lymphoblastic leukaemia both with and without KMT2Ar. METHODS: In this single-arm phase 1/2 study conducted at 18 hospitals in the USA and Canada, we enrolled patients aged 1 year

Structured missingness in biomedical research.

Antipathogen defense; b-caryophyllene; vector-virus mutualism Plants respond to insect herbivory by producing and releasing distinctive blends of volatile compounds. These herbivore-induced plant volatiles (HIPVs) serve several roles, including direct defense through toxic or repellent effects on herbivores and indirect defense by attracting the herbivores' natural enemies (Bleeker et al. 2009; Veyrat et al. 2016). HIPVs also act as warning signals, informing undamaged tissues within the same plant and nearby plants of a pending insect attack (Heil and Bueno 2007). HIPVs are mostly composed of

Surufatinib plus tislelizumab as later-line therapy for metastatic colorectal cancer: a single-arm, phase II trial.

BACKGROUND: Metastatic colorectal cancer (mCRC) patients who progress following the standard chemotherapy have maintained a favourable performance status. Nevertheless, there are limited effective treatment options, particularly for a considerable proportion of tumours with a microsatellite-stable (MSS) phenotype. Based on strong preclinical evidence demonstrating the synergistic effect between an anti-PD-1 antibody, tislelizumab, and an anti-VEGFR1-3 antibody, surufatinib, we evaluated their efficacy and safety in this population of patients with treatment-refractory disease. METHODS: This si

Immunohistochemistry confirms pulmonary metastases from an adrenal adenocarcinoma causing primary hyperaldosteronism in a cat.

CASE SUMMARY: This report describes a 14-year-and-9-month-old, male neutered cat with a primary adrenal tumour secreting aldosterone with pulmonary metastases, alongside concurrent hyperthyroidism and congestive heart failure. The cat presented with hyporexia and muscular weakness, including marked cervical ventroflexion. Hypokalaemia, hypertension, marked hyperaldosteronaemia and presence of a left-sided adrenal mass were consistent with primary hyperaldosteronism, and pulmonary nodules were detectable radiographically. Congestive heart failure and poor quality of life resulted in euthanasia.
