Prevalence of anemia and associated factors in patients with heart failure admitted to Jimma university medical center.

BACKGROUND: Anemia is common among adults with heart failure and is linked to increased risks of illness, decreased physical abilities, lower quality of life, more hospital visits, and higher mortality rates. In Ethiopia, about 23% of reproductive-age women and 18% of adult men suffer from anemia. However, data is lacking about its prevalence and associated factors in admitted heart failure patients in our setup. OBJECTIVES: To determine the prevalence of anemia and associated factors in heart failure patients who were admitted to the emergency and medical ward at Jimma University Medical Cen

Diuretics in critically ill patients: a narrative review of their mechanisms and applications.

Diuretics remain the cornerstone therapy of critically ill patients with volume overload as a result of cardiac failure, acute kidney injury or aggressive fluid resuscitation. This review summarises the principles of applied renal physiology, describing the mechanisms of action, the clinical applications, and the adverse effects of commonly used diuretics during critical illness. Loop diuretics, and in particular furosemide, remain the most popular, despite evidence of any effect on mortality or, indeed, on the need for renal replacement therapy. The efficacy of loop diuretics after administra

Electrolyte abnormalities and clinical outcomes in children aged one month to 13 years hospitalized with acute gastroenteritis in two large referral hospitals in Botswana.

Acute gastroenteritis (AGE) is a common childhood disease, with a median of 2.5 diarrhoea episodes per child per year in both low- and middle-income countries. Morbidity and mortality from AGE result from a number of causes, including electrolyte abnormalities. This study characterized children hospitalized for AGE in Botswana with and without electrolyte abnormalities. This was a prospective observational study of children under the age of 13 years who were admitted with AGE in Princess Marina Hospital (PMH) and Nyangabgwe Referral Hospital (NRH) between May 2011 and April 2013. All children

Acquired 11β-Hydroxylase deficiency in etomidate and (Iso)propoxate abusers: A nascent endocrine condition.

BACKGROUND: Etomidate, a general anaesthetic, is known to possess inhibitory activity on steroid 11β-hydroxylase at subanaesthetic concentrations. An emerging trend of abuse of etomidate as well as its analogues propoxate/isopropoxate has recently been observed. Their effects on adrenal steroidogenesis as drugs of abuse remain to be elucidated. Steroid excretion patterns of etomidate and propoxate/isopropoxate users were analysed for evidence of disrupted steroidogenesis. METHOD: This is a retrospective, cross-sectional study. Urine steroid profiling by gas chromatography-mass spectrometry-ba

Gitelman Syndrome in a Toddler With Normal Blood Test Findings Except on Sick Days.

A 3-year-old, male, Japanese patient with a history of seizure clusters was urgently admitted for febrile status epilepticus. Blood tests found metabolic and respiratory acidosis linked to the seizures and poor oral intake along with unexplained hypokalaemia, hypomagnesaemia and elevated corrected bicarbonate. These abnormalities resolved with fluid therapy, and follow-up testing after discharge found no electrolyte or acid-base disturbances. A review of the previous episodes of seizure clusters revealed similar findings, including hypokalaemia and metabolic alkalosis, raising suspicion of Git

Hypokalaemia and peritoneal dialysis-related peritonitis: Association, risk factors and outcomes.

BackgroundPeritonitis is a serious complication associated with risks of death and transfer to haemodialysis for patients receiving peritoneal dialysis (PD). To mitigate such risks, it is important to identify potentially reversible risk factors, such as hypokalaemia.MethodPatients who started PD at the Princess Alexandra Hospital, Australia from 1st January 2013 to 31st December 2022 were included. Hypokalaemia, defined as serum potassium <3.5 mmol/L, was assessed at the time of PD initiation and evaluated as categories (<3.5 mmol/L, 3.5-4.5 mmol/L and >4.5 mmol/L) based on 6-month average af

Paraneoplastic Cushing Syndrome Unmasking Small Cell Lung Cancer: A Rare Presentation.

We present a case of a middle-aged woman who presented with chest pain and shortness of breath. Laboratory tests revealed persistent hypokalaemia, hyperglycaemia, and metabolic alkalosis despite treatment. Imaging identified a mass near the right hilum suggestive of lung malignancy. Endocrine evaluation showed markedly elevated cortisol and adrenocorticotropic hormone levels, consistent with paraneoplastic Cushing syndrome caused by ectopic hormone production. The analysis of the lung biopsy obtained through bronchoscopy confirmed the diagnosis of small cell lung cancer (SCLC). The patient was

Heart Failure, Kidney Function, and Elderly Age, Rather than Levofloxacin Therapy, Are Associated with QTc Prolongation in COVID-19 Patients.

Background: Prolongation of the QT interval is directly related to the risk of ventricular arrhythmias and sudden cardiac death. Age, comorbidities, and treatment schemes have been shown to influence its prolongation and may also significantly affect the course of SARS-CoV-2 infection. Fluoroquinolones, widely used during the COVID-19 pandemic, are known for their ability to prolong the QT interval. Risk of ventricular arrhythmias has also been reported in patients with infectious diseases, and this risk may have been associated with high levels of interleukin-6 (IL-6). Purpose: The aim of thi
