Aldosterone and Cardiovascular Risk Across the Lifespan.

Aldosterone excess, particularly in the context of primary aldosteronism, is associated with adverse cardiovascular outcomes. Historically considered a condition of resistant hypertension with hypokalaemia, patients with primary aldosteronism often experienced prolonged diagnostic delay with significant end-organ damage involving the renal, cardiovascular, and central nervous systems at diagnosis. Emerging research has revealed a wide spectrum of renin-independent aldosteronism, ranging from subclinical disease with normal or mildly elevated BP to overt disease marked by resistant hypertension

Salidroside ameliorates abnormalities in electrophysiological indices induced by perfusion of the heart with low-potassium solutions.

Malignant arrhythmias related to hypokalemia are a key risk factor for cardiac arrest, but the specific mechanism remains unclear. In this study, using electrophysiological mapping and transcriptomics techniques, the effects of hypokalemia and paclitaxel (SAL) on isolated rat hearts were investigated. Hypokalemia (3.5-2.0 mmol/L) dose-dependently triggered abnormal arrhythmias and increased the incidence of arrhythmias, while SAL (5 ug/ml) improved this situation. Transcriptomics revealed that hypokalemia upregulated Mt-nd6 and disrupted the inflammatory/immune pathways, while SAL reversed the

Atrial arrhythmogenesis and electrophysiological remodeling in hypokalaemic metabolic syndrome: the role of apamin-sensitive SK channels.

AIMS: Metabolic syndrome (MetS) increases the risk of atrial arrhythmias (AA). Hypokalaemia, often secondary to diuretics or diet, frequently coexists with MetS and influences calcium handling. This study investigates the role of apamin-sensitive small-conductance calcium-activated potassium (SK) channels in atrial arrhythmogenesis under hypokalaemic MetS conditions. METHODS AND RESULTS: High-fat diets (HFD) were fed to mice at 8 weeks old for 16 weeks to mimic MetS. SK current was measured via voltage clamp, and Kcnn1-Kcnn3/CACNA1S, CACNA1C, and CACNA1D expression was assessed by qRT-PCR. We

Hypokalaemia and atrial fibrillation detected by implanted loop recorders.

BACKGROUND AND AIMS: Potassium levels influence cardiac electrophysiology, yet their day-to-day association with atrial fibrillation (AF) remains unclear. This study investigated the association between plasma potassium (p-potassium) and daily AF in at-risk individuals undergoing continuous electrocardiographic monitoring. METHODS: This is a post hoc analysis of the LOOP study randomizing participants with stroke risk factors to implantable loop recorder (ILR) screening for AF (n = 1501) or usual care. The ILR raw data were linked to p-potassium measurements collected in routine care allowing

Diuretics in patients with chronic kidney disease.

Diuretic drugs act on electrolyte transporters in the kidney to induce diuresis and are often used in chronic kidney disease (CKD), given that nephron loss creates a deficit in the ability to excrete dietary sodium, which promotes an increase in plasma volume. This rise in plasma volume is exacerbated by CKD-induced systemic and intra-renal activation of the renin-angiotensin-aldosterone-system, which further limits urinary sodium excretion. In the absence of a compensatory decrease in systemic vascular resistance, increases in plasma volume induced by sodium retention can manifest as a rise i

Early insulin degludec with continuous intravenous insulin infusion in the management of diabetic ketoacidosis: A randomized controlled trial.

AIMS: To determine the effectiveness and safety of early combination therapy with insulin degludec and intravenous insulin infusion (IVII) compared with IVII alone in diabetic ketoacidosis (DKA) management. MATERIALS AND METHODS: This prospective, open-label, randomised controlled trial included 80 adults (≥18 years) with DKA. Participants were randomised to either the intervention group, which received early subcutaneous (SC) insulin degludec (0.3 units/kg SC within 3 h of diagnosis) plus standard IVII, or the control group, which received standard IVII alone. The primary outcome was time to

Implied ADR-Admissions: A Cohort Study Introducing a Novel Administrative Data Approach for Identifying Drug-Related Hospitalisations.

BACKGROUND: Adverse drug reactions (ADRs) are a key contributor to unplanned hospitalisations, particularly in patients with polypharmacy. Traditional detection methods, such as expert reviews or diagnostic coding, are limited in scalability and sensitivity. OBJECTIVE: This study introduces and evaluates a novel scalable method, implied ADR-admissions, that links drug exposures to adverse events using administrative data to improve the detection of plausible drug-related hospitalisations. METHODS: A retrospective cohort study was conducted using linked health data from 123,662 individuals ag

Guidelines for the management of diabetes-related ketoacidosis (DKA) have been poorly adopted and implemented, resulting in a lack of improvement in outcomes.

AIMS: The Joint British Diabetes Society-Inpatient (JBDS-IP) group recommends reducing fixed rate intravenous insulin infusion (FRIII) from 0.1 to 0.05 units/kg/h when blood glucose falls <14 mmol/L to reduce the risk of complications associated with acute management of diabetes-related ketoacidosis. However, whether this change results in real-world improvements is not known. METHODS: We performed a retrospective review of DKA admissions between October 2021 and March 2023 across five hospitals in the United Kingdom. We collated data on demographics, biochemical profiles, management interven

Tocilizumab and electrolyte imbalance: severe hypokalaemia leading to paralysis.

Tocilizumab, an interleukin-6 (IL-6) receptor antagonist, is widely used for inflammatory diseases such as rheumatoid arthritis and adult-onset Still's disease (AOSD). While its known side effects include hypersensitivity, neutropenia and liver dysfunction, hypokalaemia is a rare adverse effect. We report a male patient in his early 30s with AOSD who developed severe hypokalaemia (2.0 mmol/L) and acute flaccid paralysis after his third intravenous tocilizumab infusion. He had no prior neurological or electrolyte disturbances. Electrocardiography showed U waves and mild QT prolongation, while b

Hyperosmolar therapy for severe subarachnoid haemorrhage: a protocol for a multicentre prospective observational study (OSMO-SAH study).

INTRODUCTION: Hyperosmolar therapy is crucial for the management of cerebral oedema and high intracranial pressure. Mannitol and hypertonic saline (HTS) at different concentrations are commonly used in clinical practice, but evidence of their use is mostly derived from studies about traumatic brain injury. The aim of this study was to evaluate the efficacy and safety of hyperosmolar therapy for severe aneurysmal subarachnoid haemorrhage (SAH). METHODS AND DESIGN: This is a prospective multicentre observational study in which adult patients with severe aneurysmal SAH (Hunt-Hess score ≥3) will

Real-life evaluation of an alert system to detect the risk of unreported dyskaelemia in haemolysed blood samples from a hospital emergency department.

BackgroundIn vitro haemolysis is a recurrent problem in emergency department samples. Potassium is one of the most critical tests and sensitive to increasing levels of in vitro haemolysis. Haemolysis results in a large number of invalidated potassium test results.MethodsWe set up an alert system to detect the risk of dyskalaemia based on potassium value and haemolysis index (HI). We retrospectively evaluated the effectiveness of the alert system for haemolysed blood samples from the emergency department.Results54 605 samples were included. Women more frequently had a sample with a high HI valu

Clinical and molecular mechanistic insights into the WDR72 mutation.

Distal renal tubular acidosis (dRTA) is a rare kidney disorder with an incidence of less than 1 in 100 000 people, characterised by metabolic acidosis and hypokalaemia. While mutations in genes such as ATP6V1B1, ATP6V0A4 and SLC4A1 are commonly associated with dRTA, this study reports a rare variant of the WDR72 gene in siblings from the Punjabi population in India. The individuals exhibited amelogenesis imperfecta (AI), hypokalaemic periodic paralysis (HPP) and acidaemia due to impaired ion transport in the distal convoluted tubule. Genetic testing revealed novel variants in the WDR72 gene: c

Respiratory Failure in Hyperthyroidism: Focus on Thyrotoxic Periodic Paralysis.

Thyrotoxic periodic paralysis (TPP) is a rare but serious and potentially life-threatening complication of hyperthyroidism. It is typically characterized by acute flaccid muscle weakness of the lower limbs and hypokalemia. While most cases resolve with cautious potassium replacement and appropriate management of thyrotoxicosis, some progress to respiratory failure, significantly worsening the prognosis. This review examines 14 reported cases of TPP complicated by respiratory failure (published between 2004 and 2025), highlighting clinical gaps in early recognition and the atypical presentation

Evaluation of natriuresis-guided depletion after cardiac surgery: protocol for a single-centre, open-label, randomised controlled trial-the EASY-CS study.

INTRODUCTION: Fluid overload (FO) is a common complication following cardiac surgery with cardiopulmonary bypass (CPB) and is associated with increased morbidity and mortality. Loop diuretics, particularly furosemide, are widely used to promote sodium and water excretion, but their postoperative use remains largely empirical. International guidelines recommend early assessment of diuretic response using spot urinary sodium concentration, traditionally measured by automated laboratory analysers. Recent advances now enable bedside measurement of natriuresis using point-of-care (POC) urinary sodi

Identifying unilateral aldosterone-producing adenomas using published algorithms and imaging: a systematic review and meta-analysis.

OBJECTIVE: Primary aldosteronism (PA) caused by a unilateral aldosterone-producing adrenal adenoma is potentially curable by surgery. Radiologically visible adrenal adenomas are not necessarily functional, so adrenal vein sampling (AVS) is recommended to lateralise aldosterone excess. However, AVS is technically challenging and limited in availability. We sought to evaluate the diagnostic accuracy of published algorithms for lateralisation without AVS and identify the top-performing algorithms with the highest specificity. DESIGN: Systematic review and meta-analysis. METHODS: Algorithms to p
