Identifying Risk Factors for PICU Mortality in Pediatric Cancer Patients: A 10-Year Retrospective Study.

This 10-year retrospective study of pediatric cancer patients requiring intensive care evaluates mortality risk factors. The data included patients' demographics, cancer types, admission reasons, treatment modalities, and laboratory results. Standardized mortality ratio (SMR), mortality at different intervals, length of stay, and risk factors for death were analyzed. The most common malignancy was leukemia (n=116, 37.0%), followed by noncentral nervous system solid tumors (n=71, 22.6%). The acute kidney injury (AKI) rate was 23.7% (n=74), with 55.4% requiring renal replacement therapy (RRT). R

Selecting appropriate weight parameters for nutritional prescription during nutrition support: a scoping review.

Nutritional Support (NS) is essential for preserving or restoring nutritional status in clinical settings. Accurate nutritional prescription depends on selecting an appropriate Body Weight (BW), yet clinical factors such as edema, ascites, inflammation, immobility, and obesity can compromise the reliability of Current Body Weight (CBW). Although multiple guidelines exist for NS implementation, there is no clear consensus on which BW should be used across different clinical conditions. To map and summarize evidence regarding the types of BW recommended for nutritional prescription in adults rec

Bilateral postpartum renal cortical necrosis complicated by peripartum cardiomyopathy after postpartum hemorrhage: a case report.

A 36-year-old woman developed postpartum haemorrhage after an uncomplicated vaginal delivery, followed by disseminated intravascular coagulation, thrombotic microangiopathy, and persistent anuric kidney failure. Contrast-enhanced computed tomography showed bilateral reversal of corticomedullary enhancement, consistent with renal cortical necrosis. Despite kidney replacement therapy and 13 litres of ultrafiltration, she remained oxygen-dependent, prompting cardiac evaluation on postpartum day 11, which revealed severe left ventricular systolic dysfunction (ejection fraction 25%) consistent with

Time from acute kidney injury to continuous renal replacement therapy and outcomes in critically ill children.

OBJECTIVES: Literature in adults have demonstrated no differences in mortality based on the timing of renal replacement therapy (RRT) initiation. Pediatric studies have suggested that a longer duration between intensive care unit (ICU) admission and continuous RRT (CRRT) initiation is associated with worse outcomes. We aimed to investigate how the time between acute kidney injury (AKI) and CRRT initiation impacts outcomes in children, as this relationship remains uncertain. METHODS: Retrospective cohort study of children (<18 years) admitted to the ICU who received CRRT between 2/2014 and 2/2

Timing of Loop Diuretic Initiation for De-Resuscitation in Sepsis-Associated Acute Kidney Injury: A Landmark Analysis with External Validation.

BackgroundPositive fluid balance is associated with worse outcomes in sepsis-associated acute kidney injury (SA-AKI), but the optimal timing for active fluid removal (de-resuscitation) is unknown. We examined whether earlier loop-diuretic therapy is associated with lower mortality than later initiation.MethodsUsing MIMIC-IV v3.1, we performed a landmark-anchored cohort analysis informed by the target trial framework, with the 48-h landmark as time zero. Adults with Sepsis-3, Kidney Disease: Improving Global Outcomes (KDIGO) acute kidney injury within 48 h, a positive 48-h fluid balance and no

Thoracic fluid content as a predictor of ventilatory weaning and postoperative pulmonary complications in infants undergoing open heart surgery: A prospective observational study.

BACKGROUND AND AIMS: Infants undergoing open heart surgery (OHS) are at potential risk of postoperative pulmonary complications due to fluid overload following cardiopulmonary bypass (CPB), which may delay weaning from mechanical ventilation. Therefore, this study aimed to evaluate the postoperative thoracic fluid content (TFC) by electrical cardiometry in predicting successful weaning from mechanical ventilation in infants undergoing OHS. METHODS: A prospective observational study was conducted in a tertiary referral cardiac centre over a 1-year period, from July 2022 to June 2023, and 100 i

Prevalence of Fluid Overload in a Cohort of Patients With Acute Pancreatitis: Results From a Retrospective Tertiary Single-Center Study.

OBJECTIVES: Aggressive fluid expansion has been an undisputed aspect of navigating the early stages of acute pancreatitis. Evidence produced by the WATERFALL trial supports a more conservative approach to fluid resuscitation as patients face higher odds of fluid overload while experiencing no benefit of their underlying condition. This retrospective monocentric observational study seeks to identify fluid-related complications in patients with acute pancreatitis of any grade of severity. METHODS: One hundred twenty-nine patients with acute pancreatitis including moderately severe and severe co

Diversifying aquapheresis in critical care: An institutional experience.

Volume overload is a frequent complication in critically ill patients. Ultrafiltration (UF) uses a semipermeable membrane for removal of plasma water driven by a transmembrane pressure gradient. Employed outside dialysis, it is referred to as aquapheresis (AQ). This study is a retrospective review of our experience with AQ beyond management for congestive heart failure (CHF). The use of AQ was at the discretion of the nephrologist overseeing the case. The study population was categorized according to hospital unit and specific indications for AQ therapy. A total of 69 patients underwent AQ in

Role of point-of-care ultrasound in septic shock.

Sepsis and septic shock are critical medical emergencies with high rates of morbidity and mortality. Point-of-care ultrasound (POCUS) has been demonstrated to be a vital instrument in the management of these patients, facilitating dynamic haemodynamic assessment and prompting immediate clinical decision-making. POCUS has been demonstrated to facilitate early identification of shock type, assessment of fluid response, and detection of infectious foci in unstable patients. It is evident that ultrasound protocols, including RUSH and VExUS, offer a systematic approach to the assessment of perfusio

Time-Dependent Fluid Accumulation Index for Phase-Specific Risk Stratification in Septic Shock with Heart Failure.

OBJECTIVE: Fluid management in patients with septic shock and coexisting heart failure is a critical challenge, as it requires balancing resuscitation and the risk of fluid overload. This study investigated the potential of the fluid accumulation index (FAI), which is measured serially during the initial 72 h of intensive care unit (ICU) care, to provide dynamic prognostic information to guide fluid management in this high-risk population. METHODS: Restricted cubic spline (RCS) analysis was used to explore the relationships between FAI levels at different time points within 72 h of ICU admiss

Renal cell carcinoma masquerading as acute exacerbation of right-sided heart failure: a case report.

BACKGROUND: Renal cell carcinoma is the most common malignancy involving the kidney. This cancer has a propensity for extensive invasion and metastatic spread. Tumor thrombus invasion into the inferior vena cava or renal vein occurs in less than 15% of patients with late stage renal cell carcinoma. Further extension up the inferior vena cava into the chambers of the heart is exceedingly rare, documented in less than 1% of all cases.
