Early diastolic murmurs in end-stage renal failure.

An early diastolic murmur thought to indicate functional aortic regurgitation was heard in 7 of 74 consecutive patients with end-stage renal failure assessed for chronic intermittent haemodialysis and transplantation. In all 7 cases the murmur was transient and related to episodes of hypertension and fluid overload and disappeared on correction of these factors. In a further 2 patients aortic regurgitation resulted from a structural abnormality of the aortic valve. Thus, an early diastolic murmur is not uncommon in this situation and does not necessarily indicate organic aortic valve disease w

Left ventricular function in chronic renal failure.

Left ventricular function was studied in 14 patients with end-stage chronic renal failure using non-invasive methods (echocardiography and systolic time intervals). Patients were divided into 3 groups. Group 1 consisted of 5 patients who were normotensive at the time of study and group 2 of 7 patients who were hypertensive when studied. Group 3 consisted of 2 patients: one was receiving propranolol and the other, studied 302 days after renal transplantation, was receiving digitalis for recurrent episodes of cardiac failure. All except the patient receiving propranolol had normal left ventricul

Pulmonary edema and respiratory insufficiency in acute pancreatitis.

Pulmonary edema, cardiac enlargement, and respiratory insufficiency may occur in patients with acute pancreatitis. The mechanisms are complex and incompletely understood, but probable etiologic factors include fluid overload, left ventricular failure, impaired respiratory excursion and microatelectasis, and a nonspecific response of the lung to various types of pulmonary injury including hypotension, intravenous crystalloids, and the effects of circulating pancreatic enzymes. Recognition of the association of pulmonary edema and respiratory insufficiency with pancreatitis is importance because

Significance of the electrocardiogram in heart contusion due to blunt trauma.

In a series of 24 patients with major blunt chest trauma, 11 presented evidence of heart injury upon re-examination of serial ECGs and four were confirmed at autopsy. The clinical diagnosis had been recorded in only one patient. In every case of closed chest trauma, regardless of the degree of visible injury, cardiac injury must be suspected. A presumptive diagnosis is then made, usually on the basis of ECG changes, which cannot be ruled out by any other cause. Even in some cases with normal ECGs the clinician's suspicion is the only guide. With a presumptive diagnosis made, the patient is tre

[Regional distribution of pulmonary perfusion during fluid overload in man].

The effects of a fluid vascular overload (Dextran 40,000, 10 p. 100, 44 +/- ml/kg/45 min) were studied in 9 normal subjects in the upright position. The following were found: an increase in pulmonary blood volume (+30 p. 100), and in extravascular pulmonary water (+ 70 p. 100), a significant fall in pulmonary compliance and an increase in bronchial resistance. Radio-isotopic study of the regional distribution of pulmonary perfusion showed a clear inversion with decrease in perfusion at the bases and increase at the apices. From the theoretical standpoint, this inversion of distribution of perf

[Ascites in patients under chronic hemodialysis].

In this collaborative study 9 cases of intractable ascites in chronic hemodialysis patients are evaluated. Six patients died within 14, two more within 18 months after onset of ascites. Possible causes of the pathogenesis (repeated fluid overload, hydrostatic and oncotic pressure flux forces, alteration of membrane permeability) are discussed. Renal homograft transplantation seems to be the only successful treatment.

Cardiac contusion: a capricious syndrome.

Cardiac contusions are being recognized with frequency. Among 507 patients with non-penetrating chest injuries, 210 had serial electrocardiograms sufficient to evaluate the heart. Forty-five of these 210 patients (21%) had cardiac contusions. These 45 patients and 3 others who were confirmed to have cardiac contusions at necropsy, comprise the 48 patients in this series. Life-indangering cardiac complications occurred in 14 (29%) of the 48 patients, and 4 patients died. The development of cardiac complications following cardiac contusions appears to have a significant relationship to the prese

Clinical determinants of survival from postoperative renal failure.

In a series of 114 consecutive patients with acute renal failure, the over-all mortality rate was 60 per cent; 62 per cent of the patients had a documented episode of hypotension just prior to the development of acute renal failure. In 11 patients, a second episode of renal failure developed following recovery from the initial episode of acute renal failure; all of these patients died. The urine output rate during the course of acute renal failure was inversely related to the mortality rate in the series as a whole. The mean duration of acute renal failure in survivors of the present series wa

Massive Ringer's lactate infusion: comparison with dextrose 5% and whole blood.

The objectives of this study were to compare in dogs the effects of massive infusion of Ringer's lactate, 5% dextrose in water, and whole blood. Special interest centered upon lung function, central venous pressure, and dilutional effects upon the blood components. Three groups of 5 healthy dogs each received respectively Ringer's lactate, 5% dextrose, or whole blood over 60 minutes in the amount of 135 ml/kg. Arterial blood gases and pH, right atrial and systemic arterial pressures, hemoglobin and hematocrit values, and plasma sodium, chloride, potassium and protein levels were determined. Co

Continuous positive airway pressure in the prophylaxis of the adult respiratory distress syndrome (ARDS).

Alternate patients believed to be at risk for developing ARDS were placed on CPAP for the first twenty-four hours post upper abdominal surgery. Ten of the 58 controls developed ARDS, requiring prolonged mechanical ventilation with a 30% mortality rate. Only one of the 59 CPAP patients developed evidence of ARDS. This patient, however, is believed to have had fluid overload, and responded quickly to diuretics and was extubated within two days. Thus, prophylactic CPAP has greatly decreased our postoperative morbidity and mortality related to respiratory causes.

Interstitial pulmonary edema following bromocarbamide intoxication.

Bromocarbamides are sleep-inducing drugs which can lead, in man, to intoxication and death due to respiratory failure. To prove whether hemodynamic factors or the changed endothelial permeability induce pulmonary edema, animal experiments were performed. The fine structural changes in pulmonary edema in rabbits were observed at 60, 90 and 120 minutes after oral administration. The major findings were a) large blebs between capillary endothelium and alveolar epithelium and b) interstitial edema of the vessel wall. The bleb contents were much less electron dense than the blood contents in the ca

Fluid replacement monitoring: effect of dextran overload, norepinephrine drip, and positive pressure ventilation on systemic arterial, right atrial pulmonary wedge, and left atrial pressures in dogs.

The effects of dextran overload, norepinephrine drip and positive pressure ventilation upon right atrial, pulmonary wedge, left atrial and systemic arterial pressures were studied in 15 dogs. Rapid intravenous infusion of Dextran 70 invariably produced a marked and statistically significant (p < .001) rise in right atrial, pulmonary wedge and left atrial pressures. The rise in left atrial pressure invariably exceeded the rise in right atrial pressure, and the difference in maximum pressures averaged 10.8 mm Hg (p < .001). Thus acute fluid overload and pulmonary edema can be produced by rapid i

Plasma renin activity in acute poststreptococcal glomerulonephritis and the haemolytic-uraemic syndrome.

Plasma renin activity (PRA) was determined in 10 children with acute glomerulonephritis and in 10 with the haemolytic-uraemic syndrome (HUS). Low renin levels were found in the hypertensive children with acute nephritis, all of whom had evidence of fluid overload. The amount of this overload correlated directly with the degree of hypertension and inversely with PRA. All the hypertensive children with HUS had high levels of plasma renin, and the highest levels were found in those cases who were subsequently shown to have the more severe degree of renal arteriolar occlusion. The findings emphasi

The management of acute hepatic failure.

Acute fulminant hepatitis leads to complex biochemical and circulatory disturbances including coma. These can be predicted to some extent by knowledge of the functions of the normal liver cells. Ineffective haemostasis, coma, hypoglycaemia, electrolyte abnormalities, hypotension and renal circulatory failure are particularly important. Management should be governed by the abnormalities found in the individual patient. Fluid overload and sedation must be avoided. More complex methods of temporary hepatic support such as exchange transfusion or perfusion of the isolated pig's liver should only b
