Fluid Overload-Associated Large B-Cell Lymphoma Presenting as Isolated Pleural Effusion.

Primary effusion-based lymphomas are uncommon and may pose significant diagnostic challenges. Fluid overload-associated large B-cell lymphoma is a recently recognized entity in the 5th edition of the World Health Organization Classification of Hematolymphoid Tumors and should be included in the differential diagnosis of effusion-based lymphomas, particularly in elderly immunocompetent patients with conditions that predispose to fluid overload. Background and Clinical Significance: We report a case of fluid overload-associated large B-cell lymphoma to add to the limited literature and highlight

Trendelenburg maneuver to predict fluid responsiveness in patients under mechanical ventilation with spontaneous breathing: a prospective study.

BACKGROUND: Both hypovolemia and fluid overload are associated with adverse outcomes in critically ill patients, yet many methods for assessing fluid responsiveness have limitations. The Trendelenburg maneuver, which transiently increases cardiac preload through a head-down tilt and operates on the same principle as passive leg raising but is easier to perform and more comfortable, may offer a practical adjunct in light-sedated patients under mechanical ventilation with spontaneous breathing activity, a common clinical scenario. METHODS: In this single-center, prospective study, mechanically v

Anti-phospholipase A2 Receptor Membranous Nephropathy Presenting as Common Geriatric Complaints: A Case Report.

Anti-phospholipase A2 receptor (anti-PLA2R) antibodies are an established cause of primary membranous nephropathy (MN). However, typical symptoms of proteinuria, such as peripheral oedema and frothy urine, are not easily identified in the elderly, and other non-specific presentations, such as hypoglycaemia and fluid overload, may lead to delays in diagnosis. We report a case of anti-PLA2R-associated MN in an elderly woman who presented with fluid overload and recurrent hypoglycaemia, both of which are common in this age group. Renal function and anti-PLA2R antibody titres improved substantiall

Restrictive versus standard hyperhydration during high-dose cyclophosphamide for hematopoietic stem cell transplantation: a retrospective cohort study.

PURPOSE: Cyclophosphamide is a commonly used chemotherapeutic agent in hematopoietic stem cell transplantation (HSCT), but its use can lead to adverse effects such as hemorrhagic cystitis (HC) and electrolyte disturbances, including hyponatremia. While standard hydration protocols are used to mitigate these risks, the optimal regimen remains unclear. This study explores the impact of a restrictive hydration regimen on HC incidence and electrolyte imbalances in patients undergoing high-dose cyclophosphamide treatment as part of HSCT conditioning. METHODS: A retrospective cohort study was condu

Dialysis-associated muscle cramps related to high-flow arteriovenous fistula and high cardiac output failure reversed with fistula ligation.

Muscle cramps are common in patients undergoing haemodialysis, but their pathophysiology is not well understood. The effect of regional blood flow reduction on the neuromuscular compartment as a result of a high-flow arteriovenous fistula (HFAVF) was not previously implicated in dialysis-associated muscle cramps. We report a patient with HFAVF (blood flow rate 2450 ml/min) causing high cardiac output failure who developed severe muscle cramps during dialysis, significantly limiting ultrafiltration and causing fluid overload. Ligation of the fistula resulted in complete reversal of muscle cramp

An interpretable machine-learning model for predicting in-hospital mortality in patients with sepsis-associated acute kidney injury.

INTRODUCTION: Sepsis-associated acute kidney injury (S-AKI) is a severe complication in critically ill patients, linked to increased short-term mortality and chronic kidney disease. Existing prognostic tools like SOFA and SAPS II lack full representation of intricate clinical variable interactions. Machine learning (ML) models have potential in intensive care, but few validated and interpretable models focus on the in-hospital mortality rate of patients with S-AKI. This study aims to create and validate ML models for forecasting in-hospital mortality in S-AKI patients, identifying the most eff

The impact of volume and sodium chloride supplementation on diuretic response during decongestion of decompensated heart failure patients.

AIMS: For many years, fluid and sodium restriction have been considered an essential strategy for achieving effective decongestion in acute heart failure (AHF), but this paradigm has recently been questioned. This analysis aims to evaluate and compare the effectiveness of three different fluid strategies for decongestion: no fluid, fluid with sodium/chloride, and fluid without sodium/chloride in AHF. METHODS: This post-hoc analysis of two prospective, single-centre, mechanistic studies included 55 patients with AHF and fluid overload. All patients received standardized furosemide dosing. A to

Contrast-Related Acute Pulmonary Edema Following Diagnostic Coronary Angiography: A Case Report.

Acute pulmonary edema (APE) secondary to diagnostic angiography is an uncommon complication of the procedure. It may result from several factors, including underlying congestive cardiac failure with poor left ventricular (LV) systolic function, baseline fluid overload, excessive use of contrast during diagnostic angiography in patients with severe coronary artery disease and LV impairment, and, very rarely, an anaphylactoid reaction to iodine-containing intravenous contrast agents. We report the case of a 60-year-old man who presented to the ED with an intermittent history of worsening nonexer

Negative impact of very mild volume overload on the heart function in patients on chronic hemodialysis.

OBJECTIVES: Heart failure is associated with higher mortality of hemodialysis patients. Fluid overload is a major risk factor for cardiovascular disease in these patients, but it is frequently clinically silent. The optimal assessment of ideal dry weight is still searched for. We hypothesized that even very mild fluid overload would be associated with the left ventricular ejection fraction and on the size of heart chambers. METHODS: Inclusion visit data of a cohort observation study were analyzed: bioelectrical impedance, echocardiography with hemodynamic estimations and basic laboratory test

Fatal hyponatremia by massive ingestion of water to commit suicide.

Hyponatremia (blood sodium level <136 mmol/L) is common in routine medical practice and is usually quickly resolved. Symptoms include headaches and edema. In the most severe cases, it can lead to neurological disorders and even death. A 33-year-old woman, who had been hospitalized for 15 days after attempting suicide with medication, was found unconscious by the nursing staff. The biological work-up carried out in the intensive care unit revealed severe hyponatremia (113 mmol/L). Toxicology was negative. Cerebral CT scan showed severe diffuse cerebral edema. The autopsy performed the day after

Novel Integration of Extracorporeal Membrane Oxygenation and Continuous Renal Replacement Therapy in Pediatric Patients With Severe Burns: A Case Report.

Acute respiratory distress syndrome (ARDS) and acute kidney injury (AKI) represent critical complications that significantly burden the management of pediatric patients with extensive burns. We present a case of a four-year-old boy who sustained severe burns covering 56% of his total body (Burn Index 29). The patient developed ARDS on Day 3, which led to the initiation of venovenous extracorporeal membrane oxygenation (ECMO) on Day 5. Following the subsequent development of AKI and fluid overload, continuous renal replacement therapy (CRRT) was introduced on Day 6. To overcome the limitations

Clonal analysis of a case with fluid overload-associated large B-cell lymphoma evolving from primary large B-cell lymphoma of the central nervous system.

A 62-year-old female was referred to our hospital because of left pleural effusion. She had a history of primary large B-cell lymphoma (LBCL) of the CNS (PCNSL), which was treated with chemoradiotherapy 14 years ago. Imaging studies revealed no other lesions. Pathologically, large atypical lymphoid cells were present in the pleural fluid and positive for CD20 and CD79a, while EBV and HHV-8 were negative. She was diagnosed with HHV8-negative effusion-based lymphoma (EBL) and achieved complete response after two cycles of rituximab monotherapy. We performed targeted-capture sequencing of lymphom

POCUS-guided fluid removal on haemodialysis using LVOT VTI in a patient with complex hemodynamics, case report.

BACKGROUND: Fluid removal during hemodialysis plays a vital role in achieving optimal patient outcomes. Traditional methods to guide dialysis prescription by estimating dry weight are often sufficient, however, lack precision when assessing the nuanced interplay of venous and arterial physiology in hemodynamically complex patients. Left ventricular outflow tract velocity–time integral (LVOT VTI) provides a dynamic marker of forward stroke volume and can offer additional insight into intravascular volume status when conventional ultrasound markers of congestion are limited. LVOT VTI reflects ef

Bridging evidence and practice: implementation science and bundled care strategies for fluid management in critically ill neonates.

Fluid overload (FO) is a common and modifiable risk factor in critically ill neonates. FO is associated with prolonged mechanical ventilation, multi-organ dysfunction, and increased mortality. Despite substantial observational evidence and consensus-driven guidelines, standardized fluid management strategies are inconsistently applied across neonatal intensive care units (NICUs). A critical knowledge gap exists between evidence and practice. Early single-center studies suggest bundle feasibility and effectiveness but are limited in scope and generalizability. Incorporating implementation scien
