Feasibility and acceptability of weight-based net fluid removal during continuous kidney replacement therapy: A nursing survey.

PURPOSE: Among critically ill patients with acute kidney injury, observational studies show that weight-based net fluid removal (i.e., net ultrafiltration, UFNET) during continuous kidney replacement therapy (CKRT) is associated with mortality. However, the feasibility and clinician acceptability of a weight-based UFNET strategy are unclear. METHODS: In a survey study, we evaluated nurses' perceptions and attitudes toward the weight-based UFNET strategy supported by an automated fluid removal calculator during CKRT. We surveyed ICU nurses caring for patients enrolled in the Restrictive versus

Early Extubation After Thoracic Esophagectomy Restricts Fluid Overload and Prevents Pulmonary Complications and Surgical Site Infections: A Retrospective Cohort Study.

Background: Esophagectomy is an invasive treatment for esophageal cancer associated with postoperative complications and mortality. Herein, to prevent postoperative complications, early extubation (EE) in the operating room without overnight mechanical ventilation (MV) was introduced. Methods: We compared overnight MV and EE to evaluate the impact on short-term outcomes post-esophagectomy. In total, 91 patients with thoracic esophageal cancer who underwent subtotal esophagectomy were included. In total, 26 patients were extubated in the operating room postoperatively (EE group), and 65 were ex

VExUS-ED: Early venous congestion is associated with clinical fluid overload in patients with sepsis.

BACKGROUND: Initial fluid resuscitation is central to sepsis management, yet excessive fluids may exceed a patient's fluid tolerance, increasing the risk of fluid overload and organ dysfunction. The Venous Excess Ultrasound (VExUS) score allows noninvasive bedside assessment of venous congestion, but its prevalence and predictive value in the emergency department (ED) are unclear. METHODS: We analyzed prospectively collected data from the Acutelines data-biobank (University Medical Center Groningen, Netherlands). Adults with suspected sepsis requiring initial fluid resuscitation underwent VEx

[Research advances in fluid therapy for severe acute pancreatitis].

Severe acute pancreatitis (SAP) is a condition with high mortality, characterized by persistent (>48 hours) organ dysfunction. Fluid therapy serves as the cornerstone of acute-phase management in SAP, crucial for reducing morbidity and mortality. In recent years, significant research progress has been made in the treatment concepts, methods, and strategies of fluid management for SAP patients, contributing to improved patient outcomes. Based on an understanding of the pathophysiological characteristics of SAP, it is important to recognize the significance of individualized fluid management str

Severe, late-onset neutropenia associated with prolonged furosemide administration in an extremely preterm infant: a case report and literature review.

BACKGROUND: Furosemide is commonly used off-label in the neonatal intensive care unit to promote diuresis and improve pulmonary mechanics in preterm infants, but prolonged exposure carries known renal and ototoxic risks and—rarely—hematologic toxicity. Drug-induced neutropenia is uncommon in neonates and sparsely reported with loop diuretics. We describe an extremely preterm infant who developed profound, late-onset neutropenia temporally associated with extended furosemide therapy. CASE PRESENTATION: A male infant born at 25+3 weeks’ gestation (birth weight 720 g) received intravenous then en

Peripheral perfusion-guided versus routine fluid therapy in sepsis: A randomized controlled pilot trial.

BACKGROUND: Sepsis remains a leading cause of morbidity and mortality among critically ill patients. Fluid resuscitation is essential, but conventional protocols often lack individualized assessment of tissue perfusion, risking underresuscitation or fluid overload. The peripheral perfusion index (PPI), derived from pulse oximetry, offers a practical, noninvasive way to dynamically guide fluid therapy and may improve outcomes. The objective of the study was to evaluate whether PPI-guided targeted fluid therapy improves clinical and microvascular outcomes in septic intensive care unit patients c

Extracapillary Proliferation Superimposed on Primary Membranous Glomerulonephritis: A Case Report.

We report the case of a 34-year-old patient with no significant medical history who was followed for primary membranous glomerulonephritis confirmed by renal biopsy, initially revealed by nephrotic syndrome and preserved renal function. After nine months of conservative treatment, the patient presented with progressive deterioration of renal function and increased proteinuria, leading to the initiation of immunosuppressive therapy. Persistent renal deterioration prompted a second biopsy, which revealed extracapillary proliferation in addition to membranous nephropathy. The clinical course was

Early Positive Fluid Balance as a Clinical Marker of Systemic Capillary Leak and Clazosentan Intolerance After Aneurysmal Subarachnoid Hemorrhage.

BACKGROUND: Clazosentan, a selective endothelin A receptor antagonist, mitigates cerebral vasospasm following aneurysmal subarachnoid hemorrhage (aSAH). Although it reduces vasospasm-related events, it is also associated with adverse events (AEs) related to fluid retention, which are hypothesized to stem from systemic capillary leaks. These AEs often necessitate early discontinuation of therapy. This study aimed to identify early clinical markers of clazosentan intolerance, specifically focusing on fluid balance. METHODS: This sub-analysis utilized data from the RECOVER registry, focusing on

Skin grafting while preconditioning with microaxial flow pump: case report of a successful bridge to candidacy approach.

BACKGROUND: Microaxial flow pumps (mAFP) are a well-established treatment modality for patients in cardiogenic shock, significantly improving haemodynamic and end-organ-function. In complex heart failure patients currently ineligible for advanced heart failure therapies, such as durable left ventricular assist device (dLVAD) implantation or heart transplantation (HTx), mAFP can serve as a preconditioning tool in a bridge-to-candidacy strategy. CASE SUMMARY: A 61-year-old male patient with known dilatative cardiomyopathy and multiple comorbidities presented in cardiogenic shock, accompanied by

COPD-Asthma-Heart Failure Overlap: Optimizing Bronchodilators and ICS in a High-Risk Triad.

Chronic obstructive pulmonary disease (COPD), asthma, and heart failure (HF) frequently coexist, forming a high-risk clinical triad characterized by overlapping symptoms and pathophysiological mechanisms. This COPD-asthma-HF overlap complicates diagnosis and treatment due to shared clinical features such as dyspnea, wheezing, and exercise intolerance. Epidemiologically, up to 30% of COPD patients have concomitant HF, while 15-20% of obstructive airway disease patients exhibit asthma-COPD overlap, many of whom also develop HF due to chronic hypoxia, systemic inflammation, or shared risk factors

The fluid paradox: dissociation between clinical and ultrasound parameters of fluid overload - results of the INCIVEX exploratory study.

INTRODUCTION: Fluid management in critically ill patients relies on clinical assessment, fluid balance calculations and, increasingly, point-of-care ultrasound. However, the rela-tionships between these assessment modalities remain poorly understood, with conflicting evidence regarding their correlation and clinical utility. The study aimed to eva-luate correlations between clinical indicators of fluid overload, cumulative fluid balance, and comprehensive ultrasound parameters including cardiac function, lung ultrasound, and the venous excess ultrasound (VExUS) protocol in critically ill patie

Aggressive fluid resuscitation in predicted severe acute pancreatitis: a secondary analysis of a multicenter randomized controlled trial.

BACKGROUND: A recent trial showed that early aggressive fluid resuscitation in acute pancreatitis(AP) was associated with increased fluid overload characterized by respiratory symptoms/signs. However, the generalizability of these results in more severe AP is unknown. We aimed to evaluate the association between early fluid strategies and the incidence and duration of respiratory failure in patients with predicted severe acute pancreatitis(pSAP). METHODS: This is a secondary analysis using data from a multicenter, cluster-randomized trial of pSAP. The entire cohort was divided into three grou

Early Prediction of Postoperative Peritoneal Dialysis Using Lung Ultrasound Scoring in Neonates After Cardiopulmonary Bypass.

BACKGROUND: Neonates and young infants undergoing cardiac surgery with cardiopulmonary bypass (CPB) are highly vulnerable to pulmonary dysfunction, systemic inflammation, capillary leak, and fluid overload, which may lead to acute kidney injury (AKI) and the need for peritoneal dialysis (PD). Lung ultrasound (LUS) is a bedside, radiation-free tool that allows real-time assessment of lung aeration and pulmonary congestion. However, its role in predicting postoperative renal support remains limited. This study aimed to evaluate whether early postoperative LUS scores could predict the need for PD

Associations of Bioelectrical Impedance-Derived Phase Angle and Hydration Parameters with Clinical Severity in Ambulatory Chronic Heart Failure.

Background/Objectives: Malnutrition and altered body composition are frequent in chronic heart failure (HF) and are associated with worse functional status and prognosis. Bioelectrical impedance analysis (BIA) is increasingly used in nutritional assessment, although its interpretation may be confounded by fluid overload. This study aimed to evaluate the association between BIA-derived parameters and clinical and biochemical markers of disease severity in ambulatory patients with chronic heart failure. Methods: This cross-sectional study included adult outpatients with chronic HF consecutively

Case Report: Bexagliflozin as an adjunct decongestive strategy in a cat with congestive heart failure and advanced chronic kidney disease.

Sodium-glucose cotransporter 2 inhibitors (SGLT2i) have established cardiorenal benefits in human medicine; however, clinical reports describing their use as a decongestive strategy in cats with congestive heart failure (CHF) complicated by advanced chronic kidney disease (CKD) remain limited. This case describes an 11-year-old neutered male British Shorthair cat with advanced CKD (IRIS stage 3-4) that developed CHF manifested by pleural and pericardial effusion. Thoracic radiographs confirmed pleural effusion, and echocardiography identified pericardial effusion with a hypertrophic cardiomyop

Association between VExUS score and worsening renal function during diuretic therapy in the ICU.

BACKGROUND: In intensive care unit (ICU) settings, venous excess ultrasound (VExUS) score has gained attention for predicting Acute Kidney Injury (AKI). This led to the identification that venous congestion via VExUS should prompt diuretic therapy. However, in acute heart failure (AHF), a share of the literature considers creatinine elevation as a sign of efficient decongestion. Thus, the relationship between VExUS, diuretic response, and renal outcomes remains unclear in ICU patients. METHODS: Secondary analysis of a prospective observational study conducted in a cardiovascular ICU (2019-202

From mechanisms to clinical practice: a review of diuretic therapy in pediatric nephrology.

Diuretics are a cornerstone of supportive therapy in pediatric patients, widely used for the management of edema, fluid overload, hypertension, and various other kidney diseases. Their clinical utility, however, is strongly influenced by developmental differences in kidney physiology, age-related pharmacokinetics, underlying kidney disease, and the risk of adverse effects unique to children. This review presents an extensive, evidence-informed synthesis of diuretic therapy in pediatric kidney disorders, encompassing pharmacology, mechanisms of action, indications, drug interactions, and safety
