Lipodystrophy: an uncommon cause of insulin resistance and young-onset diabetes.

Insulin resistance due to lipodystrophy syndromes is an uncommon but important cause of diabetes. It is often overlooked due to limited awareness. Standard glucose-lowering therapies often fail to achieve durable metabolic control in patients with lipodystrophy. Long-term complications include cardiovascular events, pancreatitis, kidney failure, metabolic dysfunction associated steatotic liver disease, and sepsis. We report a case of a 15-year-old boy presenting with darkening of skin and inability to gain weight. There was no family history of diabetes or similar disorders. Laboratory evaluat

Pathway-Specific Polygenic Risk Scores and Cardiorenal Complications in Asians With Type 2 Diabetes.

AIM: Risk of cardiorenal complications varies among patients with type 2 diabetes (T2D). T2D pathway-specific polygenic risk scores (psPRS) capture genetic susceptibility related to distinct intermediate biological processes associated with T2D and cardiorenal outcomes, predominantly in European populations. Their relevance in multi-ethnic Asian populations remains unclear. We explored associations between T2D-derived psPRS and cardiorenal outcomes and circulating proteins linked to cardiorenal complications. METHODS: We included 2057 multi-ethnic individuals with T2D from the prospective SMA

Lipolysis in Health and Disease: Pathways, Regulation, and Metabolic Consequences.

PURPOSE OF REVIEW: Lipolysis regulates lipid distribution, energy availability, and metabolic homeostasis in organisms that store triacylglycerols. In multicellular organisms, adipose tissue evolved as a specialized organ that centralizes lipid storage and release, buffers nutrient fluctuations, and protects non-adipose tissues from lipid overload. Dysregulated adipocyte lipolysis occurs across diverse metabolic conditions, including obesity, diabetes, lipodystrophy, and inflammatory states. This review synthesizes evidence that defective suppression of basal lipolysis and impaired responsiven

Dilute Hyaluronic Acid for Lipoatrophy: Three Cases and Literature Review.

Localized lipoatrophy, characterized by subcutaneous fat loss, can result from trauma, injectable drugs, panniculitis, or idiopathic causes, leading to outcomes ranging from self-resolution to permanent disfigurement. This case series presents a novel approach using dilute hyaluronic acid (HA) for three cases of lipoatrophy (trauma-induced, radiation-induced, and steroid-induced), with all cases showing notable improvement after one session. This approach represents a promising, low-risk alternative for managing lipoatrophy of various etiologies, with potential benefits for patient quality of

Genotype-first approach reveals monogenic lipodystrophy is underdiagnosed, with health and mortality risks.

BACKGROUND: Monogenic lipodystrophy is a metabolic disorder that predisposes to diabetes and cardiovascular disease, yet its true prevalence and clinical spectrum remain uncertain. We used a genotype-first approach with an aim to estimate the prevalence, phenotypic spectrum, risk of cardiometabolic disorders and all-cause mortality associated with monogenic lipodystrophy in the population. We also assessed how these clinically unselected cases differ from clinically identified cases. METHODS: We analysed whole-genome sequencing data from 490,414 UK Biobank participants to identify pathogenic

Lipodystrophy look-alikes: navigating diagnostic overlap with endocrine diseases.

Lipodystrophy syndromes are rare and heterogeneous disorders characterized by partial or generalized absence of subcutaneous adipose tissue. These conditions are associated with severe metabolic complications, including insulin resistance, diabetes, dyslipidemia, and hepatic steatosis. Because of their rarity and clinical variability, lipodystrophies are often under-recognized and may be mistaken for more common endocrine disorders, leading to misdiagnosis and delayed treatment. Several endocrine diseases share overlapping clinical manifestations with lipodystrophy, such as abnormal fat distri

Familial partial lipodystrophy type 2 associated with a novel LMNA variant (c.604G>C; p.Glu202Gln): a Colombian family case series.

INTRODUCTION: Familial partial lipodystrophy type 2 (FPLD2) is a rare autosomal dominant laminopathy caused by LMNA gene variants. It is characterized by progressive gluteofemoral lipoatrophy and severe metabolic derangements, including insulin resistance and metabolic dysfunction-associated steatotic liver disease. METHODS: Three Colombian women (two sisters and a daughter) underwent standardized clinical phenotyping, dual-energy X-ray absorptiometry (DXA), cardiometabolic laboratory testing, and next-generation sequencing-based testing for lipodystrophy-related genes with copy-number varian

Bone phenotype in familial partial lipodystrophy type 2: Insulin resistance and sclerostin.

BACKGROUND: The impact of insulin-resistance on bone mineral density (BMD) remains unclear. Sclerostin inhibits bone formation, and increases with ageing, metabolic syndrome, and renal failure. Genetically-determined low sclerostin levels are associated with high BMD. Our aim was to determine the bone phenotype and the influence of insulin-resistance without obesity on sclerostin levels in Familial Partial Lipodystrophy type 2 (FPLD2). METHODS: Demographic, metabolic, anthropometric, and bone parameters were compared after adjustment on age and menopausal status across four groups of women (1

Multi-omic signatures of genetic mechanisms inform on type 2 diabetes biology and patient heterogeneity.

Type 2 diabetes (T2D) is a heterogeneous disease shaped by genetic pathways related to insulin resistance and β-cell dysfunction, but how this heterogeneity is reflected molecularly remains unclear. We integrated partitioned polygenic scores (pPS) with proteomic and metabolomic profiling to define molecular signatures of T2D and their clinical relevance. We analyzed UK Biobank participants with genomic, proteomic, and metabolomic data. In a disease-free training subset, we used LASSO regression to identify multi-omic signatures associated with each pPS by jointly modeling proteins and metaboli

A pcyt-1 Allelic Series Reveals In Vivo Consequences of Reduced Phosphatidylcholine Synthesis in C. elegans.

Phosphatidylcholine (PC) is the most abundant phospholipid in eukaryotic membranes and is synthesized in part via the rate-limiting enzyme PCYT1A. In humans, hypomorphic PCYT1A variants cause diverse disorders, including retinal dystrophy, lipodystrophy with fatty liver, and spondylometaphyseal dysplasia. To define how graded reductions in PC synthesis affect organismal physiology, we generated and characterized a series of mutant alleles in the Caenorhabditis elegans homolog pcyt-1, including variants corresponding to disease-causing human mutations, as well as an auxin-inducible degradation

Surgical Management of Dorsocervical Fibro-Lipodystrophy (Buffalo Hump): Systematic Review and Meta-Analysis.

BACKGROUND: Dorsocervical fibro-lipodystrophy, commonly known as "buffalo hump," presents a challenging deformity often associated with HIV-related lipodystrophy or idiopathic etiologies. Although various surgical techniques have been reported, a comprehensive evaluation of their safety, efficacy, and recurrence rates remains lacking. This systematic review and meta-analysis aimed to evaluate outcomes of surgical interventions, particularly suction-assisted lipectomy, in managing dorsocervical fibro-lipodystrophy. METHODS: A systematic review with meta-analysis was conducted using PubMed and

Clinical, genetics and in silico analysis of a novel BSCL2 variant in a patient with CGL2 from Iranian Azeri Turkish ethnic group: expanding the genotypic spectrum through a comparative review.

Congenital generalized lipodystrophy type 2 is a rare autosomal recessive disorder caused by mutation in the BSCL2 gene. Here we report a novel variant (NM_001122955.4:c.828_835dup p.(Arg279ProfsTer21)) in an 18-year-old female with congenital generalized lipodystrophy type 2. The patient presented with severe lipoatrophy, muscular hypertrophy and insulin resistance. This frameshift variant introduces a premature stop codon, probably triggering nonsense-mediated decay. This finding expands the BSCL2 mutational spectrum and highlights the importance of genetic analysis in consanguineous populat

The formation, dynamics, and disassembly of caveolae.

Caveolae are a highly abundant surface feature of many mammalian cells, including skeletal muscle, smooth muscle, adipocytes, and fibroblasts. Dysregulation of caveolae has been implicated in a range of diseases, including muscular dystrophies, lipodystrophies, pulmonary hypertension, and various cancers. Despite their well-established links to human disease, our molecular understanding of caveolae function at the cellular level is only now emerging. Recent advances have begun to clarify how changes in caveolar components contribute to the diverse cellular processes attributed to these special

Massively parallel functional genomic assays in endocrinology: from promise to delivery.

The late 20th century saw major advances in mapping genes and mutations in endocrine disorders, driven by biochemical studies that identified defects endocrinologically and by family-based studies that mapped them genetically. In the 21st century, however, the world of genetics has been transformed by the mapping of the human genome and the development of next-generation sequencing technologies that allow genomes to be sequenced at population scale. This has led us to be swamped by a surfeit of information about genetic variation, with the so-called genetic 'variants of unknown significance' (

Giant nonencapsulated pubic lipoma: a case report and literature review on differential diagnoses of atypical lipomas.

Lipomas have been previously documented in the literature. Here, we present a rare case of a lipoma with exceptionally unusual size, localization, and structure. Due to exceptional presentation, we reviewed the literature for differential diagnoses. A 62-year-old female presented with a history of a slow-growing mass in the mons pubis over the past five years. The patient underwent open surgical excision. Postoperative pathological examination revealed a tumor measuring 210 × 150 × 90 mm and weighing 1,280 g; notably, no capsule formation was observed, and no evidence of malignancy was detecte
