Ptch2 Deficiency Triggers Lipoma Formation and Adipogenic Transcriptome Reprogramming in Nile tilapia (Oreochromis niloticus).

The Hedgehog (Hh) signaling pathway is a key regulator of adipogenesis and lipid metabolism. However, the specific role of its receptor, Patched2 (Ptch2), in these processes remains unclear. Here, using a CRISPR/Cas9-mediated ptch2 homozygous mutation model in Nile tilapia (Oreochromis niloticus), we found that Ptch2 deficiency induced visceral and perirenal lipomatosis characterized by small, multinucleated adipocytes. Comparative adipose transcriptomics revealed pronounced adipogenic reprogramming, with marked upregulation of genes governing de novo lipogenesis (e.g., acaca, fasn), fatty aci

Madelung Deformity: A Current Concepts Review.

Madelung deformity is a rare congenital wrist deformity characterized by growth disturbance of the volar-ulnar aspect of the distal radial physis, leading to progressive wrist deformity, pain, and stiffness. Madelung deformity is frequently associated with genetic conditions such as Leri-Weill dyschondrosteosis and Turner syndrome, and more recently, mutations in the GNAS gene have been implicated. Diagnosis is based on clinical findings and radiographic criteria, with plain radiographs demonstrating characteristic volar-ulnar radial curvature, positive ulnar variance, and carpal subluxation.

Complete radiologic and clinical reversal of lumbar spinal epidural lipomatosis via GLP-1 agonist.

Spinal epidural lipomatosis (SEL) is a rare cause of lumbar canal stenosis, typically linked to obesity or corticosteroid use. Severe cases often require decompression. We report a 48-year-old man with chronic low back pain and morbid obesity (153 kg) who presented with extensive SEL and critical stenosis. Previous conservative therapies failed. A non-surgical strategy using GLP-1-induced weight loss (semaglutide) and medical cannabis for pain control was initiated. Over a follow-up of one year, his weight dropped to 93.8 kg, with major improvement in pain and mobility. MRI demonstrated near-c

Cowden Syndrome: Imaging Review and Cancer Surveillance.

Cowden syndrome (CS) is a rare autosomal-dominant inherited variant of PTEN (phosphatase and tensin homolog gene) hamartoma tumor syndrome caused by mutations to the PTEN tumor suppressor gene, leading to activation of the PI3K/AKT/mTOR intracellular signaling pathway. Patients with this syndrome present with multisystem hamartomatous lesions and are particularly predisposed to developing malignancies, including breast, thyroid, renal, endometrial, and colorectal malignancies. Mucocutaneous lesions, especially trichilemmomas, and macrocephaly are commonly seen in the 2nd decade of life and usu

Dercum's Disease: Bridging Present Understanding and Emerging Directions.

Dercum's disease (DD) is a rare condition characterized by intense, asymmetrical, chronic burning pain localized in adipose tissue, often accompanied by subcutaneous fat nodules, leading to a significant reduction in quality of life. It typically affects overweight or obese adults between 35 and 50 years of age, with a marked female predominance. Despite numerous hypotheses proposed over time, the pathophysiology of DD remains poorly understood. Diagnosis is particularly challenging, as it relies solely on clinical evaluation. Given the overlapping features with other conditions, including sym

Syndromes Aassociated with Lipomatous Tumors (SALT): A comprehensive review.

Lipomas are among the most common specimens encountered in surgical pathology. Beyond distinguishing a lipoma from an atypical lipomatous tumor or well-differentiated liposarcoma (ALT/WDLS), there are important clinical considerations to keep in mind, as lipomas and lipomatosis may occur in association with various syndromes. These syndromes can be inherited or sporadic, and some are linked to an increased risk of other malignancies. Moreover, several are associated with systemic findings that carry significant morbidity and mortality. Careful attention to specific clinicopathologic features-t

Hydroxyl radical footprinting modification reveals an intradomain communication pathway in EFL1 disrupted by a Shwachman-Diamond syndrome-associated mutation.

Shwachman-Diamond syndrome (SDS) is a rare genetic disorder characterized by pancreatic insufficiency and neutropenia. While most cases are linked to mutations in the SBDS gene, some involve mutations in the GTPase EFL1. This protein works with SBDS to release the anti-association factor eIF6 from the 60S ribosomal subunit during ribosome biogenesis. The pathogenic EFL1 R1095Q mutant (R1086Q in yeast) exhibits altered guanine nucleotide recognition and impaired eIF6 release, prompting an investigation into its structural consequences. Using the yeast Efl1 orthologue in X-ray hydroxyl radical f

A Case of Encephalocraniocutaneous Lipomatosis (ECCL) With Atypical Clinical Presentation Diagnosed on Molecular Testing: FGFR1 ECCL Tumor Risk.

Encephalocraniocutaneous lipomatosis (ECCL) is a neurocutaneous condition caused by postzygotic mosaic activating variants in genes including FGFR1, NRAS, or KRAS. It primarily affects the skin, eyes, and central nervous system. Diagnosis is typically based on characteristic clinical features and/or molecular confirmation. Here we report a unique case of ECCL in a 12-year-old female with abdominal wall lipoma, ipsilateral lower limb overgrowth, and brachydactyly, in whom somatic mosaicism for FGFR1 was identified using resected lipomatous tissue. Imaging studies confirmed additional spinal lip

Madelung's disease involving the scrotum: a case report and diagnostic pitfalls with liposarcoma.

BACKGROUND: Madelung's disease is a rare metabolic disorder characterized by diffuse, symmetrical adipose tissue proliferation, strongly associated with chronic alcohol consumption. Scrotal involvement is exceedingly rare and can mimic malignancy. CASE PRESENTATION: We report a 59-year-old man with a history of chronic alcohol intake who presented with progressive bilateral scrotal enlargement. Unlike previously reported cases of scrotal Madelung's disease, which typically demonstrate homogeneous and unencapsulated fat proliferation, MRI in this patient revealed atypical imaging features, inc

Case Report: Spinal epidural lipomatosis with incomplete cauda equina syndrome treated with unilateral biportal endoscopic technique.

Spinal epidural lipomatosis (SEL) involves the aberrant growth of normal adipose tissue in the spinal canal. It lacks of distinctive clinical manifestations. However, it can infrequently present as incomplete cauda equina syndrome (CES), a condition necessitating urgent surgical intervention. We present a case of SEL primarily manifesting as acute incomplete CES. The patient underwent unilateral biportal endoscopic (UBE) spinal canal decompression and adipose tissue excision. This procedure led to satisfactory postoperative neurological improvement. This case underscores the efficacy of unilat

Dual Diagnosis of Thyroid Amyloidosis and Lipomatosis: An Uncommon Concurrence.

Lipoid lesions of the thyroid are extremely rare. Adipose changes can be seen in both malignant (carcinomas) and benign conditions. The two most common benign entities are thyrolipoma and thyrolipomatosis, which have been reportedly associated with systemic amyloidosis. Radiologically, these lesions can mimic malignancy, especially when presenting as a biochemically active condition. We present a rare case of concurrent thyroid lipomatosis and serum amyloid A amyloidosis (AA) manifesting as goiter and subclinical hyperthyroidism. A 72-year-old male presented to Danbury Hospital with complaints
