Neurological Manifestations in Whipple's Disease: A Scoping Review of Clinical Features, Laboratory Findings, and Neuroimaging Characteristics of 111 Cases.

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes. METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

Secondary actinomycosis in a recurrent odontogenic keratocyst: A clinicopathological diagnostic challenge with 5-year follow-up.

OKC is an odontogenic developmental cyst with a significant risk of recurrence. Secondary infections of the lesion with Actinomyces organisms are rare and may pose a dilemma in managing the recurrent lesion. A 20-year-old male patient reported to the outpatient department with a radiolucent multilocular lesion of the posterior right side of the mandible. A diagnosis of OKC was made based on histopathology, and a conservative approach involving decompression and marsupialisation with subsequent enucleation of the cyst and peripheral ostectomy was planned. The lesion healed without any complicat

Recent Progress in the Microbial Production and Applications of Indole Alkaloids.

AIMS: Morphine-6-glucuronide (M6G), one of the active metabolites of morphine, has attracted considerable interest as a potent opioid analgesic with an apparently superior therapeutic index. To date studies have used the intravenous route, which is generally unacceptable in the treatment of cancer related pain. The aim of this study was to define the pharmacokinetics, toxicity and cardio-respiratory effects of three alternative routes of administration of M6G. METHODS: Ten healthy volunteers participated in an open randomized study. Subjects received M6G 2 mg as an intravenous bolus, 20 mg or

Multiple strategies to improve the therapeutic efficacy of mesenchymal stem cell‑derived exosome transplantation in ischemic stroke (Review).

UNLABELLED: Methotrexate (MTX) is a class of antifolate chemotherapeutics and immunosuppressants that are effectively used in the treatment regimen of cancer and autoimmune diseases. In the current MASLD era, methotrexate-induced hepatotoxicity must be understood in the context of host metabolic dysfunction, obesity, diabetes, and alcohol use that amplify oxidative and inflammatory liver injury. But its administration is being restricted due to hepatic toxicity, which increases asymptomatic in transaminases and progresses to fibrosis. This review examines the complex mechanisms underlying MTX-

Probing the Higgs portal to a strongly-interacting dark sector at the FCC-ee.

This work explores exotic signatures from confining dark sectors that may arise in the e + e - collision mode at the Future Circular Collider. Assuming the Higgs boson mediates the interaction between the Standard Model and the dark sector, dark quarks can be produced in e + e - collisions. The ensuing strong dynamics may lead to semi-visible jet final states, containing both visible and invisible particles. We investigate semi-visible jets with different fractions of invisible states, and enriched in leptons and photons. When the invisible component is large, selections based on kinemat

Comments on: "Imaging features of Whipple's disease: fat-containing mesenteric lymphadenopathy and jejunal mucosal fold thickening".

I read with interest the article by Dariva Machado et al., describing the case of Whipple's disease (WD), a systemic infection caused by Tropheryma whipplei that was initially misdiagnosed as sarcoidosis. I congratulate the authors on providing essential diagnostic clues and fully agree with their emphasis on the importance of radiologic findings in distinguishing WD from other granulomatous conditions. I would like to comment on another important differential diagnosis.

When it's not RA: unmasking Whipple's disease in seronegative, difficult-to-treat rheumatoid arthritis.

BACKGROUND: Whipple's disease (WD) is a rare systemic infection caused by Tropheryma whipplei, often misdiagnosed as seronegative rheumatoid arthritis (RA) due to overlapping clinical features. Delayed diagnosis may lead to multisystem complications, particularly in patients treated with immunosuppressive or biologic therapies. CASE PRESENTATION: We describe two cases of WD initially misdiagnosed as refractory seronegative rheumatoid arthritis. Both patients were middle-aged men with persistently elevated inflammatory markers and poor responses to multiple biologic treatments. One developed c

Tropheryma whipplei Infection Presenting Initially with Chest Pain.

BACKGROUND: Tropheryma whipplei is a rare pathogenic bacterium, a Gram-positive actinomycete widely distributed in the natural environment. It mainly infects individuals with immunodeficiency, but is rarely observed in individuals with normal immune function. Most patients present with non-specific symptoms, and since it is not feasible to culture the bacteria using conventional methods, diagnosing this infection is very difficult. In this case, next-generation sequencing (NGS) detection via bronchoscopy confirmed the final diagnosis of Tropheryma whipplei infection. METHODS: Bronchoscopy and

Ring Finger Mass: A Rare Case of Digital Actinomycosis Surgically Removed From a Young Patient.

Actinomycosis is an uncommon chronic granulomatous infection that typically resides as commensals in the oral cavity, gastrointestinal tract, and female genital tract. Involvement of the extremities, particularly the digits and underlying bone, is extremely rare, especially in young age, often leading to diagnostic delays and therapeutic uncertainty. A 18-year-old female presented with a progressively enlarging, painless mass on the dorsum of the left ring finger. Clinical examination revealed a firm, non-tender, subcutaneous lesion with no signs of systemic illness. Radiographic imaging demon

Actinomyces sp. detected by next-generation sequencing in paraffin-embedded, formalin-fixed tissues of a dog with severe panophthalmitis and periocular cellulitis.

A 9-mo-old, castrated male Saint Bernard dog was presented for evaluation of periorbital swelling, severe uveitis, and secondary glaucoma. Concurrently, chest radiographs had evidence of pneumonia. Enucleation was performed after failure of aggressive medical management. Histopathology of the globe confirmed severe necrosuppurative panophthalmitis and periocular cellulitis with myriad intra- and extracellular bacteria forming long filamentous chains. The bacteria were gram-positive and GMS-positive but acid-fast-negative. Next-generation sequencing (NGS) was performed on formalin-fixed, paraff

Anoscrotal fistulas: causes and management in a 10-year French case series.

PURPOSE: Anoscrotal fistula is a rare variant of anal fistula, often mimicking primary scrotal disease and causing diagnostic delay. Unlike typical cryptoglandular fistulas, anoscrotal extensions are frequently linked to Crohn's disease, tuberculosis, hidradenitis suppurativa, or actinomycosis. Published data remain scarce, especially in Europe. This study aimed to describe the clinical features, etiologies, imaging findings, management, and outcomes of anoscrotal fistulas treated in a French tertiary center. METHODS: We retrospectively reviewed all men managed for anoscrotal fistula at the I

A Rare Case of Sinonasal Actinomycosis.

Actinomycosis is an infection caused by Actinomyces species--filamentous, gram-positive, anaerobic bacteria commonly found in the oral cavity. Although cervicofacial involvement is more frequently encountered, actinomycosis may involve the sinonasal tract and present with imaging findings that resemble neoplastic lesions. We report a 79-year-old woman who presented with a six-month history of left-sided purulent nasal discharge and nasal obstruction. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed an expansile soft-tissue lesion with bone destruction in the left maxillar

Deciphering whipple's disease complexity.

Whipple's disease is a sporadic infectious condition, with an incidence rate of approximately 1 per million individuals. The causative agent is the gram-positive bacterium Tropheryma whipplei. The disease manifests with a wide range of clinical symptoms, including non-specific presentations such as diarrhea, arthralgia, and fever, as well as the more pathognomonic lipodystrophy. This diversity in presentation poses a significant diagnostic challenge even for experienced clinicians. Our review aims to provide an updated overview encompassing the latest insights into Whipple's disease, focusing

Actinomycotic Cholecystitis and Pancreatitis: Report of an Unusual Case.

BACKGROUND Actinomycosis is an uncommon bacterial infection caused by the commensal organism Actinomyces spp., which can become pathogenic upon tissue injury or disruption of the mucosal barrier. While primarily recognized for its cervicofacial and abdominopelvic presentations, actinomycosis can also involve the gallbladder and potentially the pancreas, posing diagnostic and therapeutic challenges due to its ability to mimic other conditions. CASE REPORT We present the case of a 26-year-old woman with a history of recurrent acute pancreatitis and cholecystitis, followed by the development of t

A Life-Threatening Cause of Stridor, Primary Tracheal Actinomycosis.

Actinomycosis is a chronic, suppurative infection caused by Actinomyces israelii, an anaerobic bacterium that is part of the normal oral flora. A 47-year-old woman presented to the clinic with dyspnea and sore throat. Computed tomography(CT) revealed thickening of the proximal tracheal lumen and airway narrowing. Pathological examination of biopsy specimens obtained by rigid bronchoscopy revealed periodic acid-Schiff (PAS)(+) bacterial colonization with filamentous bacilli, consistent with Actinomyces. This report describes an uncommon presentation of tracheal actinomycosis in which tracheal d

Endometrial actinomycosis: a treatable cause of postmenopausal bleeding.

Postmenopausal bleeding (PMB) raises significant concern in women, as it may signal early uterine malignancy. Consequently, all episodes of PMB require thorough investigation. Approximately 60-80% of cases are linked to atrophic endometrium, with about 10% each attributable to endometrial hyperplasia and carcinoma. However, one case involved a woman with PMB, where no definitive cause was identified beyond a forgotten intrauterine contraceptive device. Further evaluation revealed a rare uterine infection as the underlying cause of the PMB.
