Tonsillar Actinomycosis Associated With Cervical Lymphadenopathy: A Rare Benign Condition Mimicking Malignancy.

Tonsillar actinomycosis is an uncommon chronic infection that can clinically and radiologically mimic malignancy. Because of its nonspecific presentation and tumor-like appearance, particularly when associated with cervical lymphadenopathy, it often poses a diagnostic challenge. A 20-year-old patient presented with a four-month history of progressive left tonsillar hypertrophy. Nasofibroscopy showed an irregular tonsillar surface with nasopharyngeal bulging, raising suspicion of a neoplastic lesion. Contrast-enhanced CT revealed asymmetric left tonsillar enlargement with homogeneous thickening

Survival and Closure by Secondary Intention of a Pharyngocutaneous Fistula After Fulminant Descending Necrotizing Mediastinitis.

Descending necrotizing mediastinitis is a rare, life-threatening extension of deep neck infection. We report the case of a 51-year-old woman who initially presented with a sore throat and was treated as uncomplicated pharyngitis before rapidly developing neck swelling, chest pain, dyspnea, septic shock, pneumomediastinum, and bilateral pleural empyemas. Combined cervicothoracic exploration showed dishwater fluid in the neck and mediastinum without esophageal perforation, and cultures grew Streptococcus anginosus and mixed oral flora with Actinomyces odontolyticus bacteremia. Her course was com

Misnomers in Dermatology and Other Medical Specialties-How to Deal with Science Fiction?

Background/Objectives: A "misnomer" is a term that is erroneous or inaccurate. Dermatology is replete with well-established misnomers such as "Mycosis fungoides", "keratoacanthoma", "actinomycosis", "granuloma pyogenicum", or "Kaposi sarcoma". Misnomers have originated from misconceptions or mistranslations that were passed down historically, and reflect the comparative-descriptive origin of terminology. We aimed to collect and categorize (non-)dermatological misnomers and to assess strategies for abandoning them. Methods: A questionnaire-based survey with 411 senior academic physicians of fiv

Actinomycosis Associated With a Nasal Rhinolith.

Rhinolithiasis is a rare entity characterised by the formation of calcified masses within the nasal cavity, resulting in chronic unilateral symptoms. We report a case of a patient presenting with a two-year history of unilateral epistaxis and foul nasal discharge. Clinical examination and computed tomography revealed a calcified mass in the right nasal cavity, which was endoscopically removed. Histopathological examination of the mass revealed Actinomyces infection. The patient was successfully managed with a three-month course of oral amoxicillin. This case highlights the importance of histop

Actinomycosis after lingual mucosa graft corporoplasty for Peyronie's disease: first reported case.

OBJECTIVE: To report the first documented case of actinomycosis following lingual mucosa graft (LMG) corporoplasty for Peyronie's disease (PD) and to highlight its clinical course and successful conservative management. METHODS: A 55-year-old male underwent plaque incision and LMG corporoplasty for severe PD. Eight months postoperatively, he presented with a fluid collection at the graft site. Imaging revealed a well-circumscribed lesion, and aspiration identified Actinomyces spp. without systemic symptoms. RESULTS: The infection was managed successfully with aspiration, compressive therapy,

Inflammatory Pseudotumor Due to Actinomycosis Simulating Post-Transplant Lymphoproliferative Disorder.

INTRODUCTION: Hepatic actinomycosis is a rare infection caused by anaerobic, gram-positive Actinomyces bacteria. Liver involvement represents 5% of all actinomycosis cases and usually occurs secondary to intra-abdominal infections, spreading via the porto-mesenteric venous system. Clinical symptoms and imaging findings are typically heterogeneous and nonspecific, often mimicking malignant tumors, which makes diagnosis difficult and frequently one of exclusion. Immunosuppressed patients, including liver transplant recipients, have increased risk and the disease should be considered when a solit

High-dose stereotactic radiotherapy boost in the radical treatment of head and neck tumors.

BACKGROUND: The current results of head and neck cancer treatment remain suboptimal therefore, novel approaches are needed to improve its effectiveness. This prospective, single-center study aims to assess the results and safety of combining conventionally fractionated radiotherapy with a high-dose stereotactic radiotherapy boost in radical treatment. METHODS: Twenty-eight patients with head and neck tumors of diverse localizations and histopathologies were recruited. The stereotactic boost was delivered to primary tumors or metastatic nodes up to 6 days before or after conventional radiother

[Actinomycosis in cattle - Five clinical cases].

Actinomycosis in cattle is often detected late and is usually too advanced by that point to be successfully treated. Five cases referred to the Ruminant Clinic were reviewed as part of this case series. The aim of this review was to highlight the differences and similarities among the cases, as well as their clinical courses. The recommendation to the livestock owners in cases 1 and 2 was to slaughter the animals due to the advanced stage of the disease. Cases 3 and 4 involved animals in which the changes were detected early and could therefore be treated. The diagnosis in case 5 was confirmed

Whipple's disease: the elusive cardiac invader in the shadows of systemic illness case report and literature review of the cardiac implications of Whipple's disease.

Tropheryma whipplei, the causative agent of Whipple's disease, is a rarely diagnosed pathogen that typically presents as a chronic, multisystemic illness. While its role in blood culture-negative endocarditis (BCNE) is increasingly recognised-accounting for approximately 4% of such cases-its other cardiac manifestations remain underappreciated. We report a case that underscores the broader cardiac implications of Whipple's disease, emphasising the diagnostic challenges faced when typical findings are absent. This report aims to raise awareness among cardiologists about the spectrum of cardiac

Pelvic actinomycosis: diagnostic challenges and management strategies based on a retrospective case series and literature review.

BACKGROUND: Pelvic actinomycosis is a rare, chronic infection caused by Actinomyces species, most associated with prolonged intrauterine device (IUD) use. Due to its nonspecific clinical presentation, it is frequently misdiagnosed as malignancy or other inflammatory conditions. This study aims to analyze the clinical features, diagnostic approaches, and outcomes of pelvic actinomycosis through a retrospective review of four cases and a supporting literature review. CASE DESCRIPTION: We reviewed the medical records of four female patients diagnosed with pelvic actinomycosis between 2021 and 20

An interesting case series of Actinomycosis: An imposter in many chronic infectious diseases from a tertiary care hospital, New Delhi, India.

Actinomycosis is a rare chronic infection caused by Actinomyces spp. affecting skin and mucosal sites, including oral, cervicofacial, respiratory, gastrointestinal, and urogenital regions. Clinical samples were received in Robertson's Cooked Meat media and cultured aerobically, anaerobically, and in CO2 incubation. Anaerobic isolates were identified using conventional and VITEK 2, and susceptibility was determined by Epsilometer strips. Actinomyces meyeri was detected in one case and Actinomyces odontolyticus in three cases; three isolates were metronidazole-resistant. Effective management req

Unveiling Atypical and Uncommon Appendiceal Pathologies in Pediatric Patients With Appendicitis.

Acute appendicitis typically presents with classic symptoms; atypical manifestations lacking distinct features are common. Failure to recognize vague presentations delays diagnosis and increases complications. We present a pediatric case series of three male patients aged seven, 12, and 15 years with confirmed appendicitis but unusual additional pathological findings only discovered after appendectomy. The seven-year-old patient exhibited severe diffuse tenderness with evidence of perforation, abscess, and marked inflammation; resection revealed multiple obstructing appendiceal lipomas underly

When Common Presentations Conceal Rare Diagnoses: A Case Report of Splenic Actinomycosis.

Splenic abscess secondary to Actinomyces species is an exceedingly rare condition that presents with a constellation of nonspecific symptoms often leading to delayed presentation, diagnostic uncertainty, and misidentification. While Actinomyces is typically an indolent infection, splenic abscess has a high mortality when untreated due to the risk of splenic rupture. This case report highlights a rare instance of isolated Actinomyces splenic abscess in a 52-year-old man with a history of intravenous drug use, hepatitis C, and recent incarceration who presented with several weeks of hemoptysis,

Abdominopelvic Actinomycosis Mimicking Malignant Tumors: Two Case Reports.

BACKGROUND: Abdominopelvic actinomycosis is a rare chronic suppurative granulomatous infection that often mimics malignant tumors on imaging, leading to preoperative misdiagnosis and unnecessary radical surgery. This report aims to highlight key clinical and imaging features that can aid in differentiating this entity from malignancy. METHODS: We retrospectively reviewed two cases of abdominopelvic actinomycosis confirmed by surgical pathology. Clinical presentations, laboratory findings, imaging characteristics (ultrasound, MRI, and CT), and pathological results were analyzed. RESULTS: Both

[Case 6: Pulmonary histoplasmosis presenting as a pseudo-tumoral lesion].

Infectious pulmonary pseudo-tumoral lesions represent a significant diagnostic pitfall, as they can mimic primary lung carcinoma and may result in delayed diagnosis and treatment. They include a broad spectrum of etiologies, ranging from common infections such as tuberculosis, aspergillosis, actinomycosis, and histoplasmosis to rarer bacterial and fungal causes. Among these etiologies, pulmonary histoplasmosis may present as a nodular pseudo-tumoral lesion. Although rare in Europe, this diagnosis should be considered even in immunocompetent patients, particularly if they traveled to endemic zo
