Actinomyces species constitute part of the normal microbial flora of the oral cavity. When mucosal integrity is disrupted, these organisms can produce invasive cervicofacial infections. Tonsillar actinomycosis can closely mimic an aggressive oropharyngeal malignancy in elderly patients with significant comorbidities. A 67-year-old man with type 2 diabetes mellitus, hypertension, and ischemic heart disease presented with a chronic progressively enlarging right tonsillar mass accompanied by throat discomfort. Clinical assessment and computerized tomography imaging scan revealed a 42 mm × 24 mm ×

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

Abdominal actinomycosis is a chronic infection caused by anaerobic Gram-positive Actinomyces species. Because of its infiltrative growth, abscess formation, and fibrotic reaction, it may closely mimic malignant disease. We report the case of a 53-year-old woman presenting with abdominal pain and a palpable epigastric mass. Imaging showed a pelvic mass with abdominal wall involvement and a hepatic lesion, initially suggestive of advanced pelvic malignancy. Tumor markers were within normal limits. Because of persistent diagnostic uncertainty, the patient underwent elective exploratory laparoscop

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Actinomycosis is a chronic, granulomatous infection caused by Actinomyces species, a group of anaerobic, gram-positive bacteria commonly found in the human oral cavity, gastrointestinal, and female genital tracts. Although it predominantly affects the cervicofacial region, rare manifestations such as gallbladder actinomycosis can occur. This report presents a case of gallbladder actinomycosis in a 61-year-old man who presented with a two-week history of right upper quadrant pain, jaundice, nausea, and vomiting. Imaging revealed biliary obstruction with common bile duct stones, leading to endos

Actinomycosis is a rare chronic granulomatous infection and can be caused by Gram-positive anaerobic bacteria which are normal commensals of the oral cavity and pharynx. These organisms can involve different parts of the maxillofacial region, rarely affecting the jaws. Actinomycotic osteomyelitis is an infection of the jaw bones, typically associated with trauma or an underlying nonspecific infection or disease. Herein, we describe a rare case of actinomycotic osteomyelitis of the mandible in a healthy male patient. Comprehensive surgical treatment with curettage and peripheral ostectomy was p

We present the case of a 60-year-old man admitted to the hospital presenting with high FUO (fever of unknown origin), strong headache, face erythematous-desquamative cutaneous lesions, long history of abdominal pain, and diffuse myalgia. He was also previously treated with immunosuppressants and currently managed with corticosteroids for a seronegative rheumatic disease. Given the immunocompromised state, an infective etiology was suspected. The patient was referred, as part of standard clinical practice, to perform an 18 F-FDG PET/CT to identify potential causes of FUO.

INTRODUCTION: Actinomyces species, Gram-positive filamentous anaerobic microaerophilic organisms, are commensals of the human oropharynx, gastrointestinal, and urogenital tracts. Actinomycosis is rare and occurs when tissue integrity is compromised, typically in a polymicrobial fashion. There is an emerging rise in Actinomyces species-associated infections, with attendant therapeutic challenges.
AIM: We evaluated the pattern, presentation, and risk factors for Actinomyces species-associated infections.
MATERIAL AND METHODS: Blood culture, tissue, fluids, bone, and swab samples with isolated

OBJECTIVES: To investigate the role of both diagnostic and interventional paediatric bronchoscopy in the management of respiratory diseases in children in low- and middle-income countries (LMICs).
DESIGN: A review of published English literature from January 2014 to February 2024.
RESULTS: Indications for bronchoscopy in LMICs will vary depending on the burden of infectious diseases like tuberculosis (TB) and HIV, and the expertise and equipment available. TB diagnosis in children remains challenging due to the paucibacillary nature of the disease and its overlap with other infectious diseas

BACKGROUND: Previous studies have shown that intravenous ceftriaxone or meropenem for 14 days, followed by oral trimethoprim-sulfamethoxazole for 1 year, cures 98% of people with Whipple's disease. However, intravenous therapy requires hospitalisation and carries risks for treatment-associated complications. The aim of this study was to investigate whether oral-only treatment for Whipple's disease is non-inferior to intravenous therapy.
METHODS: This phase 2/3, prospective, open-label, randomised, controlled, non-inferiority trial enrolled individuals aged 18 years or older with confirmed Whi

Whipple's disease is a rare condition that can present with nonspecific constitutional symptoms that can mimic those of cancer diagnosis. A high index of suspicion as well as adequate testing are necessary in order to avoid delay in diagnosis and treatment of Whipple's disease. Herein, we describe a patient who presented with an initial diagnosis of follicular lymphoma which did not respond to chemotherapy, and who, after further workup, was diagnosed with Whipple's disease including gastrointestinal, central nervous system, and cardiac compromise.

Actinomycosis is a rare, chronic, and slow-progressing bacterial infection caused by Actinomyces species. The condition's rarity, nonspecific clinical symptoms, and occasional occurrence in atypical locations often lead to delays in diagnosis, which are critical for initiating timely treatment and preventing further complications. We report the case of an 82-year-old man who initially developed pustules on the dorsum of his left hand, which gradually progressed over 15 years into an exophytic mass. Histopathological examination and culture confirmed Actinomycosis, and imaging studies revealed

BACKGROUND: Whipple's disease, caused by Tropheryma whipplei, is a rare chronic infection predominantly affecting the gastrointestinal tract. Ocular involvement is uncommon, and periorbital manifestations are exceedingly rare. This case report highlights a unique presentation of Whipple's disease as an eyelid abscess in a patient with type 1 diabetes mellitus.
CASE PRESENTATION: A 38-year-old male with poorly controlled type 1 diabetes mellitus presented with a two-weeks history of progressive swelling in the left upper eyelid. The swelling initially appeared three months prior and was associ

PURPOSE OF REVIEW: Whipple's disease (WD), triggered by Tropheryma whipplei ( T. whipplei ), is a rare, chronic, inflammatory, systemic infectious disease that typically manifests in adults. The most frequent initial manifestations include arthritis, followed by diarrhea, abdominal pain, and weight loss. Half the world's population is exposed to T. whipplei , but only one in a million develop WD. This suggests that acquired or inborn errors of immunity (IEI) may underlie WD. Anti-TNF treatment is a well established risk factor for flare-ups of WD.
RECENT FINDINGS: We have also reported two ra

Introduction: Actinomyces are commensal bacteria that colonize various sites in the human body. Under certain predisposing conditions, they can proliferate and cause granulomatous inflammation in different tissues. Pelvic actinomycosis is an extremely rare condition; its significance lies in the fact that it can be easily misdiagnosed as gynecological malignancy based on clinical manifestations and imaging findings. According to the literature, this type of inflammatory disease is a common mimic of ovarian cancer. Objective and methods: This paper aims to provide a comprehensive overview of th
