A 13-Year Nationwide Analysis of Nocardia and Actinomyces Infection Outcomes in Liver Transplant Recipients.

Liver transplant (LT) recipients have a profound susceptibility to infections. Although Nocardia and Actinomyces (NAs) are well-known bacteria that typically affect immunosuppressed patients, a scarcity of research exists on the effects of LT with NA infections. Our study aims to evaluate the outcomes associated with NA infections in patients with LT. Patients were selected from the National Inpatient Sample (NIS) from 2008 through 2020. International Classification of Disease revision 9 (ICD-9) and ICD revision 10 (ICD-10) codes. Patients admitted with a history of LT were subdivided into tho

Fecal Fistula Following Appendectomy as an Unusual Late Complication: A Rare Case Report.

Post-appendectomy fecal fistula is a rare complication, but it is associated with significant morbidity. Etiological factors of post-appendectomy fecal fistula formation include severe peri-appendicitis involving the base of the appendix as well as adjoining caecal wall; neoplasia of the appendix and caecum; infective bowel conditions especially intestinal tuberculosis, actinomycosis and Crohn's disease; distal obstruction and gossypiboma. Diagnosis of post-appendectomy fecal fistula is usually made on CT-scan but Fistulography is also useful in the late stages of the disease. Treatment of pos

Primary Actinomycosis of the Stomach: A Review of the Literature for A Rare Entity.

Background/Objectives: Primary gastric actinomycosis is extremely rare and only a limited number of cases are published in the literature. Actinomycosis is caused by anaerobic Gram-positive bacteria; these microorganisms are members of the normal human microbiome and occasionally lead to infection, especially in immunocompromised patients or patients subjected to abdominal surgery. Advances in personalized medicine, including tailored antimicrobial therapy based on individual patient profiles, may enhance treatment efficacy and reduce unnecessary interventions. Methods: A review was performed

Primary Axillary Actinomycosis: A Case Report on the Integration of Culture and Molecular Diagnostics for Accurate Diagnosis of Polymicrobial Infections.

Actinomycosis is a chronic suppurative granulomatous disease caused by Actinomyces spp. Although cutaneous actinomycosis is rare, dermatologists must consider it due to its potential coexistence with other pathogens, often as part of polymicrobial infections. We present a rare case of primary axillary cutaneous actinomycosis in a young woman, likely triggered by cosmetic axillary hair removal and home shaving practices. Histological examination revealed characteristics of actinomycosis, including sulfur granules and Gram-positive filamentous structures. Bacterial cultures failed to isolate Act

Endotracheal Actinomycosis Combined With Mucormycosis: A Case Report and Literature Review.

Actinomycosis and mucormycosis are rare infections, and their coexistence in a single host is extremely uncommon. Actinomycosis of the trachea is a chronic septic condition caused by actinomycete infection, often misdiagnosed due to the difficulty of obtaining microbiological evidence. Mucormycosis, an invasive fungal infection, is characterised by rapid progression and high mortality, commonly occurring in immunocompromised patients. A 58-year-old woman with poorly controlled diabetes presented with a whitish mass in the main bronchus, identified via bronchoscopy. Pathological biopsy confirme

Isolated joint involvement in Whipple's disease: a cohort study.

BACKGROUND: To analyse the joint manifestations of Whipple's disease and compare clinical isolated joint involvement (CIJI) with systemic joint involvement (SJI). METHODS: A cohort study included patients diagnosed with Whipple's disease and joint involvement at two expert centres. Patients were divided according to their clinical presentation at diagnosis: those with CIJI and those with SJI. RESULTS: Of the 60 patients, 17 (28 %) exhibited CIJI. Both groups were predominantly middle-aged men (75 %) with a median time to diagnosis of six (2-11) years. Joint involvement was similar, with init

An investigation into breast infections caused by Actinomyces species.

We investigated the management of Actinomyces breast infection. Eight different Actinomyces species were isolated from 26 patients during the study period (December 2018-November 2021). All tested isolates were susceptible (at standard or high dose) to penicillin and 75 % to clindamycin. Twenty-three patients had a history of smoking. Three had coexisting breast cancer. Prescribers need to be educated about the lack of activity of metronidazole on Actinomyces.

Application of metagenomic next-generation sequencing in the diagnosis and treatment of acute pneumonia caused by Tropheryma whipplei.

OBJECTIVE: The treatment plan and process for acute pneumonia caused by Tropheryma whipplei have not been clearly defined. The study aimed to conduct a retrospective analysis of the treatment for patients with acute pneumonia, caused by Tropheryma whipplei, diagnosed through metagenomic next-generation sequencing (mNGS) of bronchoalveolar lavage fluid (BALF). METHODS: All patients underwent routine blood examinations and chest CT scans. Electronic fiberoptic bronchoscopy was performed to collect BALF samples from the lesion subsegments. The BALF samples were subjected to mNGS analysis. During

Urachal actinomycosis mimicking a urachal adenocarcinoma: case report and systematic review.

Actinomycosis is a chronic bacterial infection caused by Actinomyces israelii, a Gram-positive anaerobic bacterium typically found in the oral cavity and gastrointestinal tract. This infection can present in various clinical and radiological forms, affecting cervicofacial, thoracic, abdominal, and pelvic regions, often resembling malignancies. This extremely rare disease should be considered in the differential diagnosis of abdominopelvic masses. Accurate identification and appropriate management are challenging and depend on the clinician's suspicion. Most patients undergo surgery, and are tr

Whipple Disease Initial Presentation as Supraclavicular Lymphadenopathy in a Patient With Rheumatoid Arthritis: A Diagnostic Challenge.

Whipple disease is a rare systemic disease caused by Tropheryma whipplei. It can present with a wide range of nonspecific symptoms that might overlap with underlying medical conditions, posing a diagnostic challenge. We present a 61-year-old man with a past medical history of rheumatoid arthritis and chronic inflammatory demyelinating polyneuropathy, who presented with persistent leukocytosis and supraclavicular lymphadenopathy. A positron emission tomography scan revealed hypermetabolic activity in the supraclavicular, abdominal, and pelvic lymph nodes along with a myocardial hypermetabolic l

Jaw Osteomyelitis.

Mandibular osteomyelitis is caused most commonly by members of the oral flora in the setting of dental infection or dental manipulation, often in patients with underlying mandible pathology. Recognizing and treating mandible infections is especially challenging when they are associated with osteonecrotic conditions that are themselves difficult to manage and that may be associated with symptoms and testing results that mimic infection. Optimized diagnosis and management of mandible infections require collaboration between specialists in surgical and medical aspects of care, with the goal of cl
