Whipple's disease (WD) is a rare chronic infectious illness caused by Tropheryma whipplei (T.whipplei), primarily affecting the gastrointestinal tract and joints, with rare lung involvement. Diagnosis can be challenging due to nonspecific symptoms and the difficulty in isolating the bacterium. This report describes a patient with pneumonia resulting from T.whipplei and Streptococcus pneumoniae infections during lymphoma treatment with chemotherapy and glucocorticoids. Imaging revealed pulmonary infections characterized by diffuse ground glass opacities and small nodules. Metagenomic sequencing

For individuals with cell-mediated immunodeficiency (CMI), infections with opportunistic organisms such as mycobacteria, filamentous bacteria, fungi, and protozoa can lead to poor outcomes. The first part of this continuing medical education series reviews the diagnosis and management of cutaneous infections in hosts with impaired CMI. The second part addresses infections in patients with neutropenia and humoral immunodeficiency.

BACKGROUND: Tonsillectomy is a common surgical procedure primarily performed for obstructive sleep apnea (OSA) and recurrent acute tonsillitis. Actinomyces are gram-positive, anaerobic bacteria commonly found in the oral cavity and known to form part of the human microbiome, occasionally associated with localized infections. While Actinomyces bacterial colonization in tonsils has been previously studied, its correlation with surgical indications and outcomes remains unclear.
OBJECTIVES: The study aimed to investigate the relationship between Actinomyces colonization in tonsillar specimens and

RATIONALE: Legionella pneumophila (LP) and Tropheryma whipplei (TW) have been identified as pathogens that can coexist in sewage environments, which can both cause pneumophila and can impact various organs, leading to extrapulmonary manifestations. It is rare to find literature on pneumonia caused by atypical pathogens, such as the combination of LP and TW. There are no reports on specific drug treatments for such combined infections.
PATIENT CONCERNS: A 55-year-old male with no underlying diseases developed community-acquired pneumonia. His condition worsened after empirical treatment with p

A 77-year-old man presented with right-sided chest pain and dyspnea. Computed tomography revealed a loculated pleural effusion, and thoracentesis yielded purulent fluid. Actinomyces georgiae was identified using pleural fluid culture. Despite treatment with ampicillin-sulbactam and thoracic drainage, the patient's condition worsened, requiring video-assisted thoracoscopic surgery. The patient recovered completely after completing an 11-month course of antibiotics. This is the first reported case of empyema associated with A. georgiae. Actinomyces should be considered as a potential cause of em

PURPOSE: Actinomyces europaeus and Clostridium innocuum are rare causes of necrotizing fasciitis. For both organisms, it is necessary to know the specific species because variations in antibiotic sensitivity exist, making treatment challenging.
SUMMARY: An 86-year-old white female presented to the emergency department with severe pain in her right thigh. A computed tomography scan showed pockets of gas and edema in the subcutaneous tissue extending from the right lateral hemipelvis to the right inguinal area and proximal thigh. OBJECTIVES: Microbial infections can influence destructive jawbone lesions, yet the role of Entamoeba gingivalis (E. gingivalis) in these lesions is poorly understood, particularly when co-infected with Actinomyces. This study introduces a novel clinicopathological perspective on jawbone destruction linked to E. gingivalis and Actinomyces co-infection, aiming to improve diagnosis and treatment. In this study, we tried to evaluate minimally invasive methods for improving diagnosis and treatment for jawbone destruction, possibly linked to E. gingivalis and Actinomyces co-infection. We analyzed 1

Actinomycotic osteomyelitis in the maxilla is a rare complication, particularly in diabetic patients following dengue infection. Actinomycosis, caused by Actinomyces israelii, typically arises in immunocompromised hosts, with dengue-induced immune dysregulation and diabetes contributing to its pathogenesis. This case report describes a 42-year-old diabetic male presenting with oroantral communication, loose posterior maxillary teeth, and persistent pus discharge 20 days postdengue. Imaging revealed bone erosion, and histopathology confirmed actinomycosis. The patient underwent partial maxillec

Actinomycosis rarely occurs in the sinonasal region. Here, we report a case of sinonasal actinomycosis associated with an odontogenic cyst of ectopic dental origin. A 28-year-old female patient was referred to our department with a complaint of left buccal pain. Nasal endoscopy revealed a polypoid mass with purulent discharge in the left nasal cavity. Contrast-enhanced computed tomography revealed a mass lesion occupying the left maxillary sinus and nasal cavity, along with an ectopic tooth adjacent to the lesion. T2-weighted magnetic resonance imaging showed a mass lesion with a low-intensity

Whipple's endocarditis is a rare infectious condition caused by the bacterium Tropheryma whipplei. Although its systemic form is the most widely recognized, the infection exhibits a broad spectrum of extraintestinal manifestations, with blood culture-negative infective endocarditis being the most common form of cardiac involvement. The case presented in this study underscores the importance of clinical investigation for accurate diagnosis and the application of appropriate treatment.

Schaalia (Formerly Actinomyces) turicensis is a single-cell prokaryotic microorganism and a common component of the normal human oral flora. If it enters other parts and causes infection, it may lead to actinomycosis. We present the case of a 74-year-old female patient from Fuyang who developed bacterial endometritis due to Schaalia turicensis. The bacterial culture of painless cervical dilation with drainage and curettage showed slow-growing, Gram-positive rods, which were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) as Schaalia tur

Whipple's disease is a rare systemic disease caused by a chronic infection with Tropheryma whipplei, which can often only be diagnosed years after the first symptoms appear. The symptoms are varied and sometimes unspecific. In addition to cachexia, recurrent joint swelling and abdominal complaints, neurological symptoms with dementia, myoclonus and motor neuron disease also occur. In the reported case, symptoms such as joint swelling and spasms of the legs occurred during neurological rehabilitation following a paramedian pons infarction. The close collaboration between the neurological rehabi

Disseminated actinomycosis is a rare, slowly progressing infection caused by Actinomyces species and can manifest as the formation of multiple abscesses and sulfur granules in infected tissues. In this report of this rare disease, the pathogens were not traced at the initial stage, and extremely rare but dangerous complications occurred, resulting in a new research perspective for the early identification of this rare disease. This study reports a case of disseminated actinomycosis in a 59-year-old Asian female with a history of lung infection. She experienced splenic rupture after mild percus

RATIONALE: Whipple disease is exceptionally rare in patients with lymphoma in China, particularly when presenting with pulmonary symptoms as the initial manifestation. Respiratory involvement is atypical for Whipple disease and may easily be overlooked, especially in patients receiving targeted drug therapy for lymphoma. In such cases, it can be challenging to distinguish between disease progression, drug-induced pneumonia, and infectious etiologies, as the clinical and radiological findings often overlap.
PATIENT CONCERNS: A 39-year-old patient with lymphoma and persistent fever did not resp
