Molecular mechanisms of exercise-induced improvements in Alzheimer's disease: a focus on lipid homeostasis.

Alzheimer's disease (AD) is the most prevalent type of dementia, and its pathophysiological mechanisms involve multiple factors, including genomic factors, metabolomic factors, and environmental factors. Lipid dysregulation occurs both centrally and peripherally in patients with AD, and the severity is closely associated with disease progression. Applied studies based on genome-wide association studies, genomic analyses, lipidomic analyses, mass spectrometry, and machine learning, have identified lipids as a key potential target for early diagnosis and intervention in AD. However, due to the c

Streamlining Eligibility Assessment for Alzheimer's Disease-Modifying Therapies: Prediction of MMSE Scores Using the Digital Clock and Recall.

INTRODUCTION: The eligibility of anti-amyloid disease-modifying therapies (DMTs) and their integration into clinical practice in some institutions requires a specific range of Mini-Mental State Examination (MMSE) scores. Reliance on this pencil-and-paper psychometric instrument imposes operational burdens and risks perpetuating health disparities due to the test's known educational and cultural biases. This study evaluates the efficacy of the Digital Clock and Recall (DCR™) - a rapid, FDA-listed digital cognitive assessment - to crosswalk to MMSE scores using machine learning, thereby offering

Elucidating the phase 1 trial experience among study participants following completion of the INTERCEPT-AD study of sabirnetug (ACU193) for early Alzheimer's disease: a qualitative interview study.

BACKGROUND: Recruitment and retention remain persistent challenges in Alzheimer's disease (AD) clinical trials, particularly as studies increasingly focus on earlier disease stages and require longer participation and more invasive procedures. Understanding how trial enrollment and participation are experienced is critical to improving acceptability and sustaining engagement. Qualitative interviews conducted alongside clinical trials offer an opportunity to capture participant and study partner perspectives on the trial experience. In INTERCEPT-AD, a phase 1 trial evaluating safety and tolerab

Alpha-synuclein co-pathology in a real-world early Alzheimer's disease cohort.

BACKGROUND: Most Alzheimer's disease (AD) cases show mixed pathology, with α-synuclein (αSyn) aggregates present in a substantial proportion. The cerebrospinal fluid (CSF) α-synuclein seed amplification assay (αS-SAA) enables in vivo detection of pathogenic αSyn aggregates, but its clinical significance remains unclear. METHODS: We prospectively evaluated 108 individuals with mild cognitive impairment or mild dementia due to suspected AD undergoing lumbar puncture for anti-amyloid therapy (ATT) eligibility. CSF AD biomarkers and αS-SAA were analyzed alongside cognitive, olfactory, and rapid e

Educational attainment mitigates hippocampal-related episodic memory decline in individuals at risk of Alzheimer's disease.

Ageing and Alzheimer's disease lead to declines in the brain region underlying memory for past events (hippocampus), and subsequently in memory for past events (episodic memory). However, some people show considerable adaptability in maintaining cognitive processes despite brain ageing or disease. The cognitive reserve model proposes that individual factors (e.g., higher education) may provide cognitive resilience. We measured whether the expected hippocampal-memory association differs according to educational attainment (a proxy of cognitive reserve). At baseline, 62 older adults (65-88 years

A high-throughput conditioned-media-based screening system identifies inhibitors of aggregation induced by iPSC-secreted amyloid β.

In early drug discovery, in vitro screening is frequently used, but selected candidates often fail in vivo. Induced pluripotent stem cell (iPSC)-based disease models offer improved physiological relevance; however, the high costs of media and differentiation procedures limit large-scale testing. Here, we develop a high-throughput conditioned-media-based screening system-the High-throughput screening technology for Aggregation Inhibitors of Diseased cell-derived Aggregative Proteins (HaiDap) system-to identify inhibitors of aggregation induced by iPSC-secreted amyloid β (Aβ). Using conditioned

Associations between Handwriting Decline and Cognitive-Motor Impairment in Dementia.

INTRODUCTION: Handwriting reflects the integration of cognitive and motor functions, making it a potential indicator of neurodegenerative changes. Handwriting deterioration often mirrors cognitive and motor decline in dementia, particularly Alzheimer's disease (AD). However, objective assessment methods remain limited. This study is based on the AD-HS instrument, which includes 42 handwriting features and three linguistic features. Their assessment is non-trivial. To ensure an objective and reliable evaluation, the features are given exact and unambiguous definitions. The formal, quantitative

Cofilin-1 Exacerbates Tau-Induced Mitochondrial Damage, Oxidative Stress, and Apoptosis in Alzheimer's Disease.

Alzheimer’s disease (AD) is characterized by the accumulation of neurofibrillary tangles (NFTs) composed of misfolded tau and by early mitochondrial dysfunction. Although tau aggregation has been closely linked to mitochondrial impairment and neuronal toxicity, the molecular factors that amplify tau-induced mitochondrial damage remain incompletely understood. Previous studies have shown that cofilin-1, an actin-binding protein, facilitates tau aggregation and propagation under pathological conditions. In the present study, using tauopathy-based cellular models, we investigated whether cofilin-

The dynamic impairment of synaptic transmission in the PCx-IL engram circuit contributes to early olfactory memory decline in Alzheimer's disease.

Olfactory dysfunction has emerged as a promising target for the early diagnosis and treatment of Alzheimer's disease (AD). However, the mechanisms underlying neural circuit disruption associated with olfactory dysfunction in AD remain poorly understood. We conducted single-cell RNA sequencing (RNA-seq) and ex vivo electrophysiological studies to determine the link between olfactory memory in AD and dynamic synaptic transmission disorders in PCx-IL engram cell circuits. Clinical functional magnetic resonance imaging (fMRI) data revealed that connectivity between the piriform cortex (PCx) and th

Estradiol ameliorates AD pathology and cognitive deficits by SORLA-mediated APP endosomal trafficking.

Alzheimer’s disease (AD) shows a notable female bias in both prevalence and severity. This bias is closely linked to declining estradiol levels during menopause. Our study demonstrated that estradiol can inhibit the accelerated progression of AD caused by ovariectomy (OVX) in female APP/PS1 mice. Transcriptome analysis revealed that estradiol prevented the decline in sortilin-related receptor 1 (Sorl1), which encodes the sorting receptor SORLA, in the hippocampus of female APP/PS1-OVX mice. Neuronal Sorl1 knockdown blocked estradiol’s ability to alleviate AD symptoms. Mechanistically, estradio

A large language model-based self-learning and critical agent framework for multimodal Alzheimer's disease diagnosis.

OBJECTIVE: We developed and evaluated a training-free, large language model (LLM) multiagent framework, consisting of role-prompted LLM instances, that simulates hospital-style team diagnosis for Alzheimer's disease, improving interpretability and generalizability for multimodal data. METHOD: We designed a self-learning and critical multiagent workflow comprising nine single-dimension agents and five senior agents that iteratively validate, retrieve experience, and refine decisions via self-evaluation and critique. Agents are augmented with an "intelligence" module and an experience-retrieval

Proteomics signatures associated with cognitive trajectories: evidence from the English Longitudinal Study of Ageing.

Alzheimer's disease and related dementias (ADRD) pose a growing global health challenge, with early detection critical to slowing cognitive decline and prevent ADRD. We analyzed high-throughput plasma proteomics in 2,460 cognitively healthy adults from the English Longitudinal Study of Ageing (ELSA) to identify proteins linked to 15-year cognitive trajectories, including verbal fluency, episodic memory, and orientation. Mixed-effect linear models revealed 34 proteins associated with faster orientation decline and 18 with accelerated episodic memory decline. Enrichment analyses implicate extrac

Predicting cognitive impairment using novel functional features of spatial proximity and circularity in the digital clock drawing test.

The digital clock drawing test (dCDT) is a cognitive screening tool employing a digital pen. While many studies rely on summary statistics of dCDT features to predict cognitive outcomes, these approaches often involve subjective decisions such as feature selection and imputation. In this study, we introduce novel dCDT features, expressed as mathematical functions, and compare them to commonly used summary features. We included dCDTs from 3,415 participants from the Framingham Heart Study. Random forest models with five-fold cross-validation were trained to distinguish participants with mild co

Peripheral Sensory Stimulation for Long-Term Improvement in Mild Cognitive Decline: A Prospective Interventional Study.

BACKGROUND: Despite recent breakthroughs in pharmacological treatment for Alzheimer's disease, high costs and the complex procedure to monitor safety have limited access for many patients. Less invasive and more accessible non-pharmacological therapies that support neuroplasticity and slow cognitive decline are needed. Processing Inner Strength Toward Actualization (PISTA) stimulation applies structured tactile input to promote cortical-subcortical activation. This study evaluated the long-term effects of PISTA on cognition and pain in older adults with mild cognitive impairment or early demen

Evaluation of Plasma-Derived hsa_circ_003077 for Non-Invasive Diagnosis of Alzheimer's Disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder affecting the central nervous system and is the most common form of dementia in the elderly. Current diagnostic methods are limited in the early and definitive diagnosis of the disease, necessitating the need for new and more reliable biomarkers. Circular RNAs (circRNAs) are non-coding, single-stranded, and highly stable RNA molecules commonly found in the eukaryotic transcriptome. Recent studies have shown that changes in the expression levels of circRNAs may play a role in AD pathogenesis. Furthermore, these molecules are c

Astrocyte Ca2+ Dysregulation in Alzheimer's Disease Mouse Models: Revisiting the Dogma of Hyperactivity.

Astrocytes are essential gatekeepers of brain homeostasis, and the disruption of their functions can contribute to the development of several neurological diseases. Among astrocyte signaling pathways, the intracellular second messenger Ca2+ plays a pivotal role in regulating the release of gliotransmitters, which actively modulate fundamental processes in the brain such as synaptic plasticity and memory function. Several studies over the years support the idea that dysregulated astrocytic Ca2+ homeostasis represents a relevant mechanism in Alzheimer's disease pathogenesis. Early works in trans
