Alzheimer's disease (AD), the leading cause of global dementia, is a multifactorial process that goes beyond the accumulation of β-amyloid (Aβ) plaques and tau protein tangles, including glia cell-mediated neuroinflammation, vascular dysfunction, metabolic alterations, and synaptic loss. Its complex etiology also involves oxidative stress and mitochondrial dysfunction. Multiple neurotransmitter systems involved in the pathogenesis and the various cognitive and non-cognitive symptoms of AD are thus altered. The cholinergic system, historically the first to be associated with AD, suffers early d

Alzheimer's disease (AD), the most common form of dementia, is a progressive and irreversible neurodegenerative disorder. Structural MRI is widely used for diagnosis, revealing brain changes associated with AD. However, these alterations are often subtle and difficult to detect manually, particularly at early stages. Early intervention during prodromal stages, such as mild cognitive impairment (MCI), can help slow disease progression, highlighting the need for reliable automated methods. In this work, we introduce a dual-level evaluation framework comparing fifteen deep learning architectures,

Background/Objectives: Alzheimer's disease (AD) is a neurodegenerative disease for which there are no highly effective treatments, which highlights the need for novel therapeutics. Cannabidiol (CBD) has demonstrated antioxidant, anti-inflammatory and neuroprotective properties. Chronic CBD treatment (20 mg/kg and 50 mg/kg) reverses social recognition memory deficits of APPSwe/PS1∆E9 (APP/PS1) transgenic mice; however, it does not produce effects on AD-relevant brain pathology. Methods: Here, we investigated whether chronic high-dose CBD treatment (i.e., 100 mg/kg intraperitoneally) in early sy

Background and Objectives: Dual-phase amyloid PET imaging has been proposed to provide complementary information regarding amyloid burden and cerebral perfusion. This exploratory pilot study evaluated whether semiquantitative parameters derived from dual-phase PET/CT could differentiate individuals operationally classified as Alzheimer's disease with mild functional impairment (AD-MFI) from those with mild cognitive impairment (MCI). Materials and Methods: Twenty-four participants (AD-MFI, n = 19; MCI, n = 5) underwent dual-phase amyloid PET/CT and structural MRI. Early phase SUV (eSUV), delay

BACKGROUND AND PURPOSE: Electroencephalogram (EEG) signals play a vital role in analyzing neurological activity and diagnosing various neurological disorders. With the rapid growth of the Internet of medical things (IoMT), EEG-based diagnostic systems have become more interconnected and scalable. However, early and accurate diagnosis of neurological diseases remains challenging due to the intrinsic complexity, high dimensionality, and noise-prone nature of EEG signals. This study aims to design an automated, robust, and intelligent diagnostic support framework for reliable early detection of n

Synapse loss is the strongest pathological correlate of cognitive decline in Alzheimer's disease (AD) and is most pronounced around amyloid plaque pathology in the brain. Although mechanisms remain incompletely understood, hyperactivity downstream of soluble amyloid beta (Aβ) is strongly implicated in synapse degeneration. Engulfment of synapses by reactive astrocytes was observed in end-stage disease tissue, particularly around plaques. Due to astrocytes' role in synaptic modulation, we hypothesised that astrocytes could modulate synapse degeneration downstream of soluble Aβ earlier in diseas

'Early hippocampal hyperactivity' is well-documented in Alzheimer's disease (AD), yet its mechanisms in vivo before amyloid-β plaque deposition at the levels of synaptic transmission and neural oscillations remain unclear. Here, we perform in vivo high-resolution patch-clamp and high-throughput probe recordings in the hippocampus of amyloidopathy mice before plaque deposition. At the cellular level, we observe reduced inhibitory synaptic input, hypoactivity of fast-spiking interneurons, and enhanced bursting in pyramidal neurons. At the network level, we reveal accelerated hippocampal oscillat

Alzheimer’s disease (AD), a devastating neurodegenerative disorder characterized by synaptic loss, cognitive decline, oxidative stress, neurotransmitter imbalance, amyloid-β accumulation, tau tangles, chronic neuroinflammation, and dysregulated signaling, is further exacerbated by endothelin (ET) system dysregulation, where elevated ECE-2 and ET-1 correlate with Aβ-driven pathology in vitro and in vivo. Against this backdrop, Sovateltide (IRL-1620), a highly selective endothelin-B receptor agonist, has emerged as a next-generation, multi-target therapeutic with remarkable neuroprotective promi

Alzheimer's disease (AD) is a neurodegenerative disorder and the leading cause of dementia. Early diagnosis and monitoring of disease progression are crucial for effective intervention. This study presents a novel disease progression model based on Variational Probabilistic Principal Component Analysis (VPPCA), which uses a Bayesian framework for dimensionality reduction and uncertainty quantification. By analyzing 1,021 amyloid-positive patients from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database, we extracted 25 features, including CSF (ABETA, TAU, PTAU), PET (FDG, AV45), an

O-linked N-acetylglucosamine (O-GlcNAc) is a post-translational modification of serine and threonine residues on nuclear and cytoplasmic proteins. The identity of O-GlcNAcylated proteins during ageing and neurodegenerative disease remains incompletely defined. Thus, animal models play a crucial role for the systematic characterization and cataloguing of O-GlcNAc-modified proteins. In this study, proteomic analysis was performed to identify O-GlcNAc-modified proteins in both L1 larval and adult stages of wild-type N2 and of aex-3p::Tau(V337M) transgenic Caenorhabditis elegans, a nematode model

Path integration is navigation in the absence of environmental landmarks and is a primary cognitive mechanism underlying spatial memory. Path integration performance is primarily assessed in humans using the Triangle Completion Task (TCT). In humans, TCT has shown promise for the early diagnosis of Alzheimer's disease. In rodents, however, path integration is assessed using a wide variety of tasks, but none of which currently provide a homologue for the TCT. As rodents are routinely used as preclinical models, homologous path integration tasks that result in comparable performance metrics betw

INTRODUCTION: We investigated whether a multi-day learning curve (MDLC), derived from remote digital cognitive assessments over 1 week, adds prognostic value to plasma phosphorylated tau at threonine 217 (p-tau217) for predicting cognitive decline in cognitively unimpaired (CU) older adults.
METHODS: A total of 183 CU participants (mean age 73.3; 70% female) completed multi-day digital cognitive tests and provided plasma p-tau217. Linear mixed-effects models evaluated the independent and combined effects of baseline p-tau217 and MDLC on longitudinal cognitive decline over 2.5 years.

BACKGROUND: The introduction of disease-modifying therapies (DMTs) for Alzheimer’s disease has prompted major changes in diagnostic pathways, referral processes, and service coordination in dementia care. Japan, as an early adopter of DMTs within a nationally coordinated dementia-care framework, offers an opportunity to examine how health service structures respond to these changes. This study explored health service–related support needs emerging across the DMT pathway, focusing on patient and informal caregiver experiences, with complementary perspectives from service providers. METHODS: A q

Alzheimer’s disease (AD) is characterized by progressive cognitive impairment accompanied by behavioral disturbances and neuropsychiatric manifestations. Conventional clinic-based assessments and biomarkers provide essential diagnostic information, but these episodic measurements are limited in capturing longitudinal AD-related symptoms. Digital phenotyping has emerged as a complementary approach that addresses this limitation by enabling continuous monitoring of cognitive and functional changes in everyday life. This narrative review defines digital phenotyping as a longitudinal monitoring ap

Alzheimer's disease (AD) is a major neurodegenerative disorder that severely impacts the global elderly population. It is characterized by progressive memory loss, cognitive impairment, and neuropsychiatric disturbances. To date, AD lacks definitive curative treatments, making it a persistent clinical challenge. Consequently, there is an urgent need to develop cost-effective and highly specific biomarkers for the early detection of AD. MicroRNAs (miRNAs) are evolutionarily conserved, small non-coding RNAs. They are highly enriched in the central nervous system (CNS), where they orchestrate ess

Alzheimer disease is characterized by progressive cognitive decline driven by amyloid-β (Aβ) accumulation, tau hyperphosphorylation, synaptic loss and brain atrophy. Emerging evidence positions metabolic reprogramming encompassing impaired glucose uptake/glycolysis, mitochondrial dysfunction, redox imbalance, and altered lipid metabolism as an early and central contributor to AD pathogenesis. This review summarizes recent biochemical and translational findings linking glucose hypometabolism including reduced GLUT1/GLUT3 expression, disrupted glycolytic enzyme activity to neuronal energy failur

Alzheimer's disease (AD) is pressing global health concerns, for which early diagnosis is critical to effective intervention. However, conventional approaches, including neuropsychological assessments and neuroimaging techniques, are resource-intensive and impractical for community-level screening. In contrast, artificial intelligence-driven behavioral analyses, including speech pattern and facial expression recognition, have demonstrated considerable potential for scalable and non invasive cognitive assessment. This work presents a community-oriented intelligent screening system for cognitive

Lecanemab is an IgG1 monoclonal antibody that has emerged as the first FDAapproved drug to slow the progression of Alzheimer's disease by targeting amyloid plaques, with the potential to serve as a disease-modifying therapy. Ongoing clinical studies are evaluating the efficacy and safety of the medication; however, much of the current literature remains mixed regarding the clinical effectiveness of lecanemab. The results from the Clarity AD study, the largest clinical study regarding the effectiveness of lecanemab to date, revealed a statistically significant 27% reduction in the progression o

This study aims at implementing a 3D cell culture model of Alzheimer's disease (AD). To that end we engineered human induced pluripotent stem cell (iPSC)-derived neural stem cells to conditionally overexpress FAD mutant APP and PSEN1 variants. After differentiation in 3D basement membrane matrices, cultures exhibited increased Aβ42 and Aβ40 levels and a highly pathogenic shift of the Aβ42/40 ratio. Typical AD phenotypes such as amyloid deposition and tau pathology were observed alongside impaired mitochondrial integrity and neuronal damage. Pathophenotypes were ameliorated by γ-secretase inhib
