Antibiotic Tolerability in Nocardia Infection Among Kidney Transplant Recipients: A Multicenter Cohort Study.

BACKGROUND: Nocardia infection is a rare but severe complication in kidney transplant (KT) recipients, with high morbidity and mortality. Data on antibiotic tolerability in this population are scarce. METHODS: We conducted a retrospective, multicentric study across six Spanish transplant units to analyze clinical characteristics, treatment strategies, and antibiotic tolerability in KT patients diagnosed with Nocardia infection. RESULTS: Twenty-six cases of Nocardia infection were identified, predominantly in elderly male patients with significant comorbidities and a high immunosuppressive bu

Outcomes and treatment for disseminated nocardiosis caused by Nocardia otitidiscaviarum: a systematic review.

INTRODUCTION: Nocardiosis is a rare aerobic gram-positive cause of infection, often in immunocompromised individuals. Nocardiosis is usually a localized pulmonary disease, however, it can lead to disseminated disease. More than 40 species of Nocardia have been described to cause infections in humans, with many having variable susceptibility and resistance patterns to antimicrobials, such as Nocardia otitidiscaviarum. Because disseminated infection due to N. otitidiscaviarum is uncommon and has been reported to exhibit distinct and sometimes unpredictable antimicrobial susceptibility, we focuse

Case Report of a Solitary Brain Abscess due to Nocardia veterana in a Host Without Known Immunodeficiency.

Nocardiosis is a rare bacterial infection caused by a wide array of species of the genus Nocardia. These microbes are found ubiquitously in soil and cause infection mainly when immunocompromised hosts inhale these bacteria or from direct inoculation through breaks in the skin. Once infection is established, Nocardia spp. have a predilection to spread to the central nervous system, causing brain abscesses. Here, we present a rare case of a solitary Nocardia veterana brain abscess in a patient without known immunodeficiency who was successfully treated with an 8-week course of antibiotics. Overa

Transcriptomic Analysis and Machine Learning Identify Cross-Pathogen Biomarkers for Bacterial and Parasitic Infections in Silver Pomfret (Pampus argenteus).

Silver Pomfret is increasingly threatened by many diseases under intensive artificial culturing conditions, yet conserved host biomarkers across different infections remain poorly defined. In this study, we integrated transcriptomic datasets from independent infections with Cryptocaryon irritans, Nocardia seriolae, and Photobacterium damselae subsp. damselae to identify shared host-response genes. By combining differential expression analysis with weighted gene co-expression network analysis, we prioritized six candidate genes associated with cross-pathogen infection responses. Random Forest a

Clinical Utility of Bronchoalveolar Lavage PCR for Nocardia and Mucorales in Lung Transplant Recipients: A Retrospective Cohort Study.

BACKGROUND: Lung transplant recipients (LTRs) are susceptible to opportunistic infections, including invasive nocardiosis (IN) and invasive mucormycosis (IM). Polymerase chain reaction (PCR) testing of bronchoalveolar lavage (BAL) fluid offers rapid pathogen detection, but its clinical utility in LTRs is understudied. METHODS: We conducted a single-center, retrospective cohort study of adult LTRs who underwent BAL PCR testing for Nocardia spp. or Mucorales spp. (January 2020-December 2024); samples from both surveillance and "for-cause" bronchoscopies were included. The primary outcome was de

Vertebral osteomyelitis caused by Chaetomium Globosum in a young adult: A case report and literature review.

RATIONALE: Chaetomium globosum (C. globosum) is a saprophytic fungus widely distributed in nature, characterized by low clinical pathogenicity but capable of causing severe infections, particularly in immunocompromised hosts or under conducive environmental conditions. Spinal infections attributable to C. globosum are exceedingly rare and often present diagnostic dilemmas due to atypical presentations. This case underscores the importance of considering fungal etiologies in spinal pathologies. Furthermore, the strain mentioned in this study has been appropriately preserved and is available for

Species Identification And Antibiotic Susceptibility Testing Of The Nocardia Genus: Advances And Clinical Challenges.

The genus Nocardia comprises bacteria widely distributed in nature that can cause infections in both humans and animals. Due to their diverse clinical manifestations and prolonged culture time, infections are frequently misdiagnosed or overlooked. In recent years, advances in biological techniques have markedly improved molecular diagnostic methods, enabling more precise species identification. However, the increasing issue of antimicrobial resistance poses significant challenges for clinical management, particularly among immunocompromised patients, for whom treatment is more complex. Microbiome analysis of bronchoalveolar lavage (BAL) specimens from immunocompromised patients with pneumonia compared to those from healthy volunteers.

BACKGROUND: Metagenomic sequencing of bronchoalveolar lavage (BAL) specimens is increasingly being applied for the diagnosis of lower respiratory tract infections, offering agnostic pathogen detection and a faster turnaround time. While metagenomic sequencing of BAL specimens can reveal a wide range of organisms, their clinical relevance is often unclear because of the challenge of distinguishing true pathogens from background taxa. This study compared the BAL microbiomes of immunocompromised patients with pneumonia to those of healthy volunteers, with the aim of assisting clinical interpretat

Operationalizing One Health Surveillance for Tropical Fungal Diseases: Practical Implications of Bioinformatics for Clinical and Epidemiological Practice.

Fungal infections are an increasing global public health concern yet remain underprioritized because of persistent challenges in diagnosis, treatment, and surveillance. The WHO Fungal Priority Pathogens List underscores the growing burden of antifungal resistance and includes agents responsible for neglected tropical mycoses, such as Paracoccidioides spp. and eumycetoma. Despite recent advances, fungal diagnostics remain limited by poor standardization, high costs, and restricted applicability in endemic, resource-limited settings. In addition, mounting evidence shows that fungal pathogens evo

Genome-wide identification of the cystatin gene family and functional characterization of cystatin B in flathead mullet (Mugil cephalus).

The cystatin superfamily is a class of endogenous cysteine protease inhibitors that play a key role in immune regulation and host defense. However, there remains a lack of genome-wide identification and functional studies of this gene family in the flathead mullet. Therefore, this study conducted a systematic identification of the cystatin gene family in flathead mullet at the genome-wide level and further characterized the function of McCysB (Mugil cephalus Cystatin B). The results revealed that seven cystatin genes were identified in the flathead mullet genome, which were classified into thr

Madura foot in a non-endemic setting: surgical management and reconstruction with skin graft.

Mycetoma is an uncommon, chronic, granulomatous infectious condition that affects the skin, dermis and subcutaneous tissues. It is prevalent in tropical and subtropical areas. The patient presented to our hospital with a left foot lesion that he first noticed 2 years ago. The lesion consisted of multiple pits discharging black granules and pus. Culture revealed that the grains comprise a mass of fungal hyphae organised within a deeply eosinophilic amorphous matrix (Splendore-Hoeppli material). The patient received multidrug therapy, radical excision of mycetoma and a skin graft. He responded w

Clinical characteristics and therapeutic dilemmas in nocardiosis: Insights from a case series.

OBJECTIVES: Nocardiosis often requires prolonged antimicrobial therapy, and trimethoprim-sulfamethoxazole (TMP-SMX) is considered the cornerstone of treatment. However, TMP-SMX-related adverse events frequently complicate long-term therapy, and real-world data regarding treatment continuity and outcomes remain limited. METHODS: We conducted a retrospective observational study of patients with Nocardia species isolated from clinical specimens at a tertiary-care academic hospital in Japan between January 2015 and April 2025. Clinical characteristics, antimicrobial therapy, adverse events, and o

Oral amoxicillin-clavulanate: an alternative treatment to refractory Nocardia keratitis-a case report.

BACKGROUND: Bacterial keratitis (bacterial corneal infection) is a serious, vision-threatening disease caused by a wide range of bacterial species. The gram-positive bacterium Nocardia spp. is increasingly recognized in keratitis, yet is particularly challenging to treat due to delayed diagnosis and limited efficacy of current ophthalmic therapeutics. Antimicrobial resistance among clinical Nocardia isolates is also rising, further complicating patient outcomes. Herein, we report a case of Nocardia amikacinitolerans keratitis, resistant to amikacin, ciprofloxacin, moxifloxacin, clarithromycin,
