First confirmed case of systemic granulomatosis caused by Nocardia seriolae in farmed snubnose pompano Trachinotus blochii in the Philippines.

Ulcerative granulomatous disease in farmed snubnose pompano Trachinotus blochii (Lacepede, 1801) in the Philippines was documented in 2019 by the Bureau of Fisheries and Aquatic Resources and has since become an annual problem for fish farmers, with outbreaks occurring between September and October. Three apparently healthy adults and 5 diseased adults displaying ulcerated skin were subjected to diagnosis. During postmortem examination, multiple nodules were present in all kidneys and the spleen in the 5 diseased fish. Histopathology revealed severe granulomatous formations at different stages

Actinomycetoma of the wrist: diagnostic insights from a pathology perspective (case report).

Actinomycetoma is a chronic, progressive subcutaneous infection caused by filamentous bacteria of the Actinomycetales order, characterized by tumefaction, sinuses, and grains. We report a rare case of actinomycetoma affecting the wrist of a 45-year-old male, which presented as a chronic, slowly enlarging swelling with multiple discharging sinuses. Initial clinical suspicion was broad, encompassing various chronic infections or soft tissue tumors. However, definitive diagnosis was established through meticulous histopathological examination of a biopsy specimen, revealing characteristic "sulphu

Reverse endothelial scraping technique for refractory deep infectious keratitis: Novel surgical technique and outcome evaluation.

This study describes a novel approach of corneal reverse endothelial scraping technique (REST) for the diagnosis and management of deep stromal and endothelial infectious plaques. This retrospective, interventional study included 16 patients with infectious deep stromal endothelial infiltrates that did not respond to conventional medical therapy. Briefly, under aseptic precautions, a reverse Sinskey hook was used to delicately debulk and scrape the endothelial plaque via a paracentesis incision, and the samples were sent for microbiological evaluation. Smears were prepared for Gram's stain, 10

Emerging imipenem resistance in Nocardia infections in Taiwan: a retrospective cohort study at a tertiary teaching hospital.

BACKGROUND: Nocardia species are emerging pathogens of increasing significance, particularly in immunocompromised hosts. Due to diverse clinical manifestations and resistance patterns, accurate identification is critical. This study characterized species distribution in Taiwan, evaluated identification methodologies, and identified mortality risk factors. METHODS: We analyzed 99 Nocardia isolates from 76 patients at a Taiwanese tertiary center (2011-2025). Identification via MALDI-TOF MS and concatenated gene sequencing (16S rRNA, secA1, hsp65) was compared against whole-genome sequencing (WG

Persistent CD4+ lymphopenia is associated with recurrent Nocardia farcinica infection and acquired resistance in an AIDS patient: a case report with immunological warning.

After severe depletion of CD4 T cells in AIDS patients, they are not only prone to a first-time Nocardia infection, but also, even if cured, unable to form protective immune memory, leaving them susceptible to reinfection with the same pathogen. More seriously, in the absence of immune surveillance, irregular drug use can accelerate the selection of drug-resistant strains. A 32-year-old man with AIDS and persistent CD4+ count below 100 cells/μL for over three years (nadir 2 cells/μL) developed right lower lobe pneumonia caused by Nocardia farcinica four years before the current admission, whic

Advances in the molecular pathogenesis of Nocardia keratitis.

Nocardia keratitis is an infectious ocular disease that poses substantial diagnostic and therapeutic challenges worldwide. It occurs predominantly in individuals with corneal barrier disruption, contact lens wear, or impaired host immunity. Due to its relatively insidious onset, the clinical manifestations are nonspecific, and laboratory diagnosis is technically demanding. As a result, delayed identification and suboptimal treatment are common, often resulting in poor visual outcomes. Although diagnostic and therapeutic techniques have improved in recent years, a comprehensive understanding of

Essential fatty acids disrupt the mycolic acid-rich cell envelope of clinical Nocardia isolates: a membrane-targeting approach to combat persistent infections.

BACKGROUND: Conventional antibiotics against slow-growing, mycolic acid-rich Nocardia often require prolonged therapy and are associated with high relapse rates. Membrane-targeting essential fatty acids may circumvent growth-dependent resistance. METHODS: Five essential fatty acids were screened against 82 clinical Nocardia isolates by broth microdilution. Docosahexaenoic acid (DHA) was further assessed for bactericidal kinetics, membrane integrity by propidium iodide staining and scanning electron microscopy, transcriptomic profiling, and combined activity with amikacin and tobramycin. A qPCR identification scheme to detect the most common causative agents of actinomycetoma in Africa.

Mycetoma is a neglected tropical disease characterized by mutilating tumorous lesions in the subcutaneous tissue. The causative agents are found embedded in granules called grains. Mycetoma is either caused by bacteria (actinomycetoma) or fungi (eumycetoma). To initiate the appropriate treatment, it is important to identify the causative agent rapidly and molecular identification for eumycetoma revolutionized the time to identification. For actinomycetoma this was not possible yet. Here we developed a multiplex qPCR identification scheme for the most common causative agents of actinomycetoma i

A bivalent oral yeast vaccine displaying Nocardia seriolae FHA and LMBV MCP confers protection in largemouth bass (Micropterus salmoides).

Largemouth bass ranavirus (LMBV) and Nocardia seriolae are major pathogens affecting largemouth bass (Micropterus salmoides), causing significant economic losses. In this study, recombinant yeasts expressing the fibronectin-binding protein A (FHA) of N. seriolae or the major capsid protein (MCP) of LMBV were constructed using the yeast surface display (YSD) system. Based on these recombinant strains, a bivalent oral yeast-based vaccine was developed and its protective efficacy was systematically evaluated. Oral administration of the vaccine induced specific antibodies against FHA and MCP in se

Blockade of the PD-1 / PD-L1 Axis Exacerbates Chronic Nocardia Brasiliensis Infection by Disrupting Bacterial Immune Containment in BALB/c Mice.

BACKGROUND: Therapeutic blockade of the PD-1/PD-L1 axis is a key strategy in cancer immunotherapy to reverse T cell exhaustion. However, this approach carries the risk of reactivating latent pathogens in individuals with chronic infections. The role of this immune checkpoint in granulomatous infectious diseases, particularly chronic actinomycetoma, remains largely unexplored. METHODS: To investigate this, we treated BALB/c mice with established chronic Nocardia brasiliensis actinomycetoma using the PD-L1 inhibitor atezolizumab. We assessed survival, clinical progression, and histopathology, a

Septic arthritis caused by Nocardia asiatica: a case report and narrative review.

Nocardia asiatica is a rare opportunistic pathogen typically associated with pulmonary and central nervous system infections; however, musculoskeletal infections are uncommon. We report a case of septic arthritis caused by N. asiatica in a 72-year-old man with a history of chronic obstructive pulmonary disease (COPD) and pneumoconiosis. The patient presented with subacute knee swelling following a recent episode of pneumonia. Gram staining of intraoperative synovial samples revealed branching filamentous organisms, and the isolate was identified as N. asiatica by mass spectrometry. Although th

Potential clinical implications due to essential differences in antimicrobial susceptibility profiles of actinomycetoma causative agents of the Actinomadura, Nocardia and Streptomyces genera towards commonly used anti-actinomycetoma drugs.

BACKGROUND: Actinomycetoma is a neglected tropical disease characterized by large tumorous lesions. It is caused by actinomycetes belonging to the genera Actinomadura, Nocardia and Streptomyces. Despite the differences between these genera and species, all actinomycetoma cases are treated with the same antibiotics. It is currently not known if the actinomycetoma causative agents have similar susceptibility patterns against these antibiotics. We therefore set out to determine the MICs for the most common causative agents against commonly used antibiotics. MATERIALS AND METHODS: We used the sta

Concurrent Presentation of Nocardia abscessus Infection and Pyoderma Gangrenosum Following Trauma.

Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis characterized by painful skin ulcers with undermined borders and surrounding erythema. Cutaneous nocardiosis can present similarly, as ulcerative lesions, often following mechanical trauma, which is a shared risk factor for both conditions. In this case report, we describe an elderly patient on low-dose corticosteroids for polymyalgia rheumatica who developed rapidly progressive skin ulcers after sustaining a mechanical injury while gardening. The patient did not respond to broad-spectrum antibiotics. Histopathological examination of

Primary cutaneous nocardiosis caused by Nocardia brasiliensis: isolation, susceptibility, and molecular identification in eight consecutive cases from a tertiary hospital.

Nocardiosis is caused by Gram-positive aerobic bacteria of the genus Nocardia, which are soil-dwelling saprophytes. Infection may also occur via direct inoculation, leading to primary infections of the skin and subcutaneous tissues that often manifest as localized nodular lesions. Diagnostic identification can be challenging or even unfeasible using conventional laboratory techniques, particularly when Matrix-Assisted Laser Desorption/Ionization Time-of-Flight Mass Spectrometry (MALDI-TOF MS) is not available. We conducted a retrospective analysis of eight cases of primary cutaneous nocardiosi

A case of primary cutaneous nocardiosis presenting as unilateral papulopustular eruption in an immunocompetent host.

Primary cutaneous nocardiosis is often challenging to diagnose in a timely manner due to its diverse and nonspecific clinical manifestations, compounded by the difficulty in culturing and identifying Nocardia species. This case report describes a patient with primary cutaneous nocardiosis caused by Nocardia brasiliensis, who presented with normal immune function and no significant history of trauma. The patient initially developed blood blisters on the ankle, which then spread to papules and pustules throughout the entire left lower limb. Through repeated secretion of bacterial cultures and ma

Diagnostic Challenges in Disseminated Mycobacterium szulgai Infection: A Case Report in the Setting of Advanced HIV Infection.

Mycobacterium szulgai is a rare, slow-growing non-tuberculous mycobacterium (NTM), accounting for less than 0.2% of NTM isolates worldwide. It most often presents with pulmonary disease resembling tuberculosis, while disseminated infection is exceptionally uncommon and typically occurs in immunocompromised hosts. We report a 34-year-old male newly diagnosed with advanced HIV infection who presented with painful cutaneous nodular lesions, intermittent fever, anorexia, and significant weight loss. Initial investigations suggested cutaneous nocardiosis or fungal infection, with cultures yielding

Nocardia brasiliensis infection causes skin abscesses around the eyes: A case-report and review of literature.

Nocardia species are widely distributed in environments such as soil. Nocardia brasiliensis is the primary pathogen responsible for primary cutaneous nocardiosis. The clinical manifestations of this disease are often non-specific, making it prone to misdiagnosis. This article reports a case involving a 42-year-old immunocompetent female patient who presented with a right periorbital skin abscess accompanied by cervical lymphadenitis, without a clear history of trauma or insect bite. Empirical antibacterial therapy proved ineffective. The infection was definitively diagnosed as Nocardia brasili
