Muscle quality index combined with plasma metabolomics: enhancing risk stratification for post-stroke dementia.

OBJECTIVE: Post-stroke dementia is a severe long-term stroke complication with heavy medical burdens, and early risk identification is critical. This study explored the association of muscle quality index (MQI) and plasma metabolomics with dementia among stroke survivors. METHODS: This study included 7,047 UK Biobank participants with a history of stroke who were free of dementia at the baseline assessment. MQI was defined as dominant handgrip strength (HGS) divided by corresponding arm fat-free mass. Plasma metabolomics was analyzed via high-throughput nuclear magnetic resonance (NMR). Metab

Medical Decision-Making Capacity in Early-Stage Alzheimer Patients: Comparison With Healthy Controls and Benefit of Assistive Information.

BACKGROUND: Early-stage Alzheimer Disease (AD) may lead to restricted decision-making capacity and without standardized instruments, there is great variability in determining patients' ability to give consent. This study aimed to assess decision-making capacity of AD patients in comparison to a healthy control group and investigated the usefulness of visual aids. METHODS: A group of 30 ad patients and 30 healthy controls were compared using the MacArthur Competence Assessment Tool for Treatment (MacCAT-T). Additionally, we implemented a modified version of the MacCAT-T with visual aids. RESU

Microglial dynamics associate with the spatial pattern of tau deposition in early Alzheimer's disease.

BACKGROUND: Neuroinflammation is implicated in Alzheimer's disease (AD) pathogenesis, yet how changes in TSPO PET signal are associated with tau deposition, and how brain network architecture shapes this relationship remain poorly understood. METHODS: We conducted a prospective, multimodal neuroimaging study in 56 participants: 35 amyloid-positive individuals across the AD spectrum (ADS) and 21 amyloid-negative healthy controls (HC). Amyloid status was classified primarily by CSF Aβ42/Aβ40 and, when eligible CSF was unavailable, by baseline amyloid PET. Participants underwent TSPO PET using [

Multimodal neuroimaging and electrophysiological signatures in amnestic mild cognitive impairment with different amyloid-β deposition status: correlations with cognitive function and diagnostic value.

BACKGROUND: Amnestic mild cognitive impairment (aMCI) is a heterogeneous transitional cognitive state between normal aging and dementia. Aβ positivity elevates dementia conversion risk but does not equate to prodromal Alzheimer's disease (AD), and not all Aβ-positive aMCI individuals progress to clinical AD dementia. Distinguishing Aβ-positive from Aβ-negative aMCI facilitates pathological subtyping rather than definitive early AD diagnosis. This cross-sectional study aimed to characterize scalp-level resting-state EEG and structural MRI signatures specific to Aβ-positive aMCI, explore their c

Global brain activity links subcortical degeneration to cortical tau progressively across Braak regions over early Alzheimer's disease stages.

Alzheimer's disease (AD) is characterized by early tau pathology in subcortical neuromodulatory nuclei, followed by progressive cortical tau accumulation; however, the mechanisms linking subcortical dysfunction to cortical tau pathology remain unclear. Using multimodal neuroimaging data from the ADNI cohort, we examined how infra-slow ( < 0.1 Hz) global brain (i.e., gBOLD) activity is related to the volume of the nucleus basalis of Meynert (NbM) and cortical tau accumulation in the early stages of AD. NbM degeneration was associated with reduced gBOLD activity and spatially co-localized tau ac

RaiLED-AD: Rationale-Guided Knowledge Transfer for Alzheimer's Disease Prediction from Electronic Health Records.

In Alzheimer's Disease and Related Dementia (ADRD) prediction, Electronic Health Records (EHRs) provide rich but fragmented information. Without a coherent clinical narrative, models tend to rely on a few dominant signals (e.g., age-related patterns) rather than capturing the underlying clinical mechanisms. Such reliance becomes problematic in younger-onset cases, where these signals are less informative. To address this challenge, we propose RaiLED-AD, a dual-encoder teacher-student framework where the student learns from serialized EHR data and the teacher leverages LLM-generated narratives

"Morphological, Functional, and Molecular Progression of Blood-Brain Barrier Dysfunction in Alzheimer's Disease".

Blood-brain barrier (BBB) dysfunction is increasingly recognized as an early pathological event in sporadic Alzheimer's disease (AD), which represents over 95% of cases. Vascular risk factors and cerebral insulin resistance are considered potential contributors to its initiation. This review synthesizes evidence available through March 2026 on the temporal progression of morphological, functional, and molecular BBB alterations in AD patients and experimental models. This review emphasizes the proposed role of BBB disruption in perpetuating neurodegeneration through feedback loops involving per

A scoping review on the time to diagnosis, early symptoms, and the barriers and facilitators of diagnosis in young-onset dementia.

BackgroundYoung-onset dementia (YOD) occurs when symptoms start before age 65 and affects about 3.9 million people globally. Variability in first symptoms, the young age, and prevalent non-cognitive symptoms pose challenges in recognizing YOD, leading to diagnostic delays. Better awareness and recognition of warning signs in primary care is essential for early YOD diagnosis, facilitating access to appropriate services and informal care arrangements.ObjectiveTo gain insights into the prediagnostic phase of YOD by exploring literature on early symptoms and factors influencing a timely diagnosis.

Charting the future: A multi-setting medical chart study of advance care planning in Parkinson's disease and atypical Parkinsonism.

BackgroundAdvance Care Planning (ACP) enables people with progressive diseases to document and communicate future treatment preferences. For Parkinson's disease (PD) and atypical parkinsonism (AP), ACP is particularly important because of disease complexity, symptom burden, the progressive nature of the disease, and cognitive decline, yet its implementation across care settings remains unclear.MethodsWe conducted a medical chart review of 189 deceased patients with PD or AP who died in nursing homes (n = 91), hospitals (n = 58), or at home (n = 40) in the Netherlands. For patients who died at

Mitochondrial carbonic anhydrase-VB inhibition rescues brain endothelial stress and memory in Alzheimer's disease models.

Alzheimer's disease (AD) is a devastating neurodegenerative disorder with no effective cure, characterized by the cerebral parenchymal and vascular accumulation of aggregated amyloid-β (Aβ) and hyperphosphorylated tau. Cerebrovascular and mitochondria dysfunction are early causal events in the progression of AD. Previous studies support that inhibiting carbonic anhydrases (CAs) may prevent mitochondrial and cerebrovascular dysfunction in AD models. Here, we selectively target the mitochondrial CA isoform CA-VB by pharmacological and genetic manipulation in human cerebral microvascular endothel

CNPY3 variants associated with a recessive developmental and epileptic encephalopathy affect inflammasome and TLR function.

BACKGROUND: Canopy FGF signalling regulator 3 (CNPY3) is an endoplasmic reticulum (ER)-resident chaperone essential for toll-like receptor (TLR) folding, trafficking and inflammasome activation. In rare paediatric cases, biallelic CNPY3 variants have been associated with developmental and epileptic encephalopathy (DEE), but available data remain sparse. METHODS: 50 individuals from 38 unrelated families with biallelic CNPY3 variants and DEE were evaluated for exome and genome findings, standardised clinical phenotype descriptions, EEG results, and neuroimaging data. A selection of variants un

Single-cell, spatial, and machine learning analyses identify ANKIB1 as an oligodendrocyte-anchored palmitoylation-associated candidate biomarker of Alzheimer's disease.

Alzheimer's disease (AD) is characterized by progressive neurodegeneration and cognitive decline, yet most candidate therapies targeting amyloid-β have failed to robustly modify disease progression, underscoring an incomplete mechanistic understanding. Oligodendrocytes (ODCs) have recently emerged as active contributors to AD pathology, but the cell-type-specific role of protein S-palmitoylation - a reversible lipid post-translational modification regulating membrane targeting and trafficking - in AD remains uncharacterized. Here we performed an integrated multi-omics analysis combining single

Key circuits through which sleep affects memory and their role in Alzheimer's disease.

Sleep is a critical physiological process for maintaining the cognitive function of brain, particularly about memory consolidation. This review systematically elaborates on the multiscale regulatory mechanisms of the sleep-wake cycle, with a focus on the neural circuits involved in sleep-dependent memory consolidation, such as the hippocampus-prefrontal-thalamus network, characteristic neural oscillations, and microglia-mediated synaptic changes. Building upon these foundations, the article delves into Alzheimer's disease (AD), providing an in-depth analysis of the complex bidirectional relati

Sex-specific brain reserve and progression from mild cognitive impairment to Alzheimer's disease dementia.

INTRODUCTION: Sex differences exist in Alzheimer's disease (AD) treatment response, but the role of sex in disease development and progression remains unclear. METHODS: We performed multimodal analyses of AD biomarker-positive individuals with mild cognitive impairment (MCI) and AD from the Alzheimer's Disease Neuroimaging Initiative (ADNI; MCI n = 593, AD n = 270) and National Alzheimer's Coordinating Center (MCI n = 436, AD n = 174). Baseline gray matter volume (GMV) was compared by sex. In ADNI, tau positron emission tomography burden and cerebrospinal fluid growth-associated protein 43 (G

Regional oxygen extraction fraction is higher in older adults with type 2 diabetes and obesity: associations with cognition and metabolic markers.

Type 2 diabetes (T2D) and obesity are highly prevalent among older adults and associated with increased Alzheimer's disease and related dementia (ADRD) risk, but underlying mechanisms linking T2D and obesity to cognitive impairment remain partially understood. Quantitative susceptibility mapping (QSM)-derived oxygen extraction fraction (OEF) provides a noninvasive MRI-based marker of cerebral metabolism and may capture early alterations in brain physiology. In this single-center, cross-sectional pilot study, we examined QSM-derived OEF in older adults with T2D + obesity (N = 14) and age- and s

Inhibition of soluble epoxide hydrolase rescues cognitive deficits by preserving neurovascular integrity and attenuating glial and neuropathology in diabetic-related dementia.

Diabetes mellitus (DM) is a major risk factor contributing to the development of Alzheimer's disease-related dementias (ADRD). While one of the early symptoms of both Alzheimer's disease (AD) and DM-related ADRD is a reduction in cerebral blood flow, the underlying biological mechanisms driving this decline remain to be fully elucidated. Genome-wide association studies have linked AD/ADRD to single-nucleotide polymorphisms in the gene encoding soluble epoxide hydrolase (sEH), an enzyme we previously reported to be upregulated in the brains of an AD rat model. Our previous studies also demonstr

Intermittent hypoxia training alleviates cerebral hypoperfusion and amyloid-β deposition in a mixed dementia model established by bilateral common carotid artery stenosis in early-stage 5xFAD mice.

BackgroundAlzheimer's disease (AD) is the most common form of dementia and often coexists with vascular pathology in clinical settings, resulting in mixed dementia with more severe symptoms and more rapid progression.ObjectiveTo investigate whether chronic cerebral hypoperfusion aggravates early AD-related pathology, we established a mixed dementia-like model by inducing bilateral common carotid artery stenosis (BCAS) in early-stage 5xFAD mice and evaluated the effects of intermittent hypoxia treatment (IHT).MethodsEarly-stage 5xFAD mice were subjected to BCAS to induce chronic cerebral hypope

Capturing silent oxidative stress in early Alzheimer's disease: prediction of CSF biomarkers from sleep qEEG data.

Oxidative stress is a central pathogenic process in the earliest stages of Alzheimer's disease (AD), promoting non-enzymatic protein modifications that accumulate in cerebrospinal fluid (CSF) before measurable neurodegeneration. These alterations impair proteostasis and disrupt sleep-regulating neural circuits, producing characteristic changes in sleep electroencephalographic patterns. Because CSF sampling is invasive, quantitative electroencephalography (qEEG) has emerged as a promising non-invasive proxy for early oxidative processes. Here, we investigated whether nonlinear and time-domain s

Sleeve Gastrectomy Improves Metabolic Health, Cognition, and Alzheimer's Disease Pathology in 3xTG Mice.

Obesity and diabetes are well-established risk factors for Alzheimer's disease (AD), implicating metabolic dysfunction in AD pathogenesis. Sleeve gastrectomy (SG) is among the most effective metabolic interventions available, yet its impact on AD progression remains poorly understood. We hypothesized that SG performed early in life would improve metabolic health, attenuate AD pathology, and preserve cognition in a transgenic AD mouse model. Five-week-old female 3xTg-AD mice were preconditioned on a Western diet (WD) to induce obesity and glucose intolerance, then randomized to SG or sham surge
