Alzheimer's disease: choline acetyltransferase activity in brain tissue from clinical and pathological subgroups.

Choline acetyltransferase activity was measured postmortem in five brain regions to determine if such activity provided biochemical support for clinical and pathological subgrouping of Alzheimer's disease. Seven patients with Alzheimer's disease were divided into groups based on age at onset, severity of neuropathological changes, history of myoclonus, family history of dementia, cerebellar amyloid plaques, and congophilic angiopathy. Thirty-two age-matched normal control subjects and 17 neurological control patients with Huntington's disease were also studied. Patients with early-onset and la

High levels of brain dolichols in neuronal ceroid-lipofuscinosis and senescence.

Dolichols as unesterified alcohols were identified as significant components of lipid extracts from storage cytosomes isolated post-mortem from the brains of patients with the infantile, late infantile, and juvenile types of neuronal ceroid-lipofuscinosis (NCL). Very small amounts of dolichols were present in the corresponding subcellular fractions of non-NCL brains. The nuclear fraction from NCL cerebral cortex contained the highest dolichol content expressed per milligram protein or lipid, whereas the crude mitochondrial fraction was the richest in normal brain. Highly significant elevations

Basal nucleus of Meynert. A neuropathological study in Alzheimer's disease, simple senile dementia, Pick's disease and Huntington's chorea.

The basal nucleus of Meynert (bnM) was examined in 9 patients with Alzheimer's disease--4 presenile and 5 senile cases--in 3 patients with simple senile dementia, 5 with Pick's disease, 5 with Huntington's chorea and 5 non-demented controls. The histopathological study was followed by a quantitative analysis of the magnocellular population of neurons and by the determination of their nucleolar volume. In Alzheimer's disease there was a neuronal loss ranging from 44 to 76%, which was negatively correlated with both the age at onset and age at death of the patients. Numerous surviving cells show

Why assess the aged? A clinical psychologist's view.

Four major areas of research: the early detection of Alzheimer's disease; the psychological side-effects of anticholinergic drugs; the significance of late onset depression; and the relationship between physical activity, ageing and cognitive status, are discussed with reference to the problem of improving the quality of life in the aged. The four areas of research are unified by discussing two aspects of information processing: speed of performance and memory. The significance of and need for the assessment of cognitive status in the elderly are also discussed, as well as the notion that test

Longevity and mortality in Down's syndrome.

The life span of individuals with DS has gradually increased since the 1920s. The DS individual now has an average life expectancy of 35 years. Despite advances in the health care of the retarded and improvements in the quality of institutional care, the overall mortality rate remains elevated by five-fold. Specific mortality rates from respiratory diseases (particularly pneumonia), infectious diseases, congenital heart disease, leukaemia and neurological disorders are still substantially increased. Disorders of immunological functioning, particularly T-cell mediated, appear related to this in

Twins with Alzheimer's disease.

Twins of identical appearance were both affected by Alzheimer's senile dementia. In one, dementia began in her late 60s. She died at age 74. In the other, onset was at age 83. She died of ovarian carcinoma at age 85. The histopathologic findings of Alzheimer's disease were confirmed in both twin sisters.

Regional pattern of degeneration in Alzheimer's disease: neuronal loss and histopathological grading.

The various structural components of the cortical degeneration of Alzheimer's disease were defined and graded. The severity of the degenerative process was thus mapped in different cortical areas where neuronal numbers and cortical width were also measured and compared with controls. Contrary to the general opinion that the degenerative process is rather uniformly diffuse, though accentuated in the medial temporal and frontal cortex, we found a consistent and more elaborate pattern with clearcut regional differences. Thus the degeneration involved, besides basal medial temporal limbic areas, t

[Anatomo-clinical study of 4 cases of Jakob-Creutzfeldt disease (spongiform encephalopathy)].

Four anatomo-clinical cases of Creutzfeldt-Jakob disease (spongiform encephalopathy) are described. In 3 cases the correct diagnosis was made while the patients were living. The most important diagnostic elements are the following: rapid evolution of a presenile or senile dementia, with neurological symptoms unusual in the Alzheimer's disease. The importance of EEG for the diagnosis of Creutzfeldt-Jakob disease is confirmed: however the onset of the typical EEG abnormalities was very late in its manifestation, the CT alterations are in general absent or very limited. The pathological alteratio

Immunoglobulins, cognitive status and duration of illness in alzheimer's disease.

Immunologic changes have been reported in the dementing illnesses of mid and late life. The results of two studies of drug-free males meeting research diagnostic criteria for primary neuronal degeneration of the Alzheimer's type suggest that serum IgG levels decrease with the progression of dementia. Serum IgG levels were inversely correlated with duration of illness and the levels of psychiatric symptomatology. Performance on the mini-mental status examination was positively associated with serum IgG concentrations.

Lecithin in Parkinson's disease.

Pathological and biochemical evidence reviewed favours the hypothesis that the dementia seen in Parkinson's disease, particularly after long-term levodopa therapy, is akin to Alzheimer's disease. We postulate, in late Parkinson's disease, the development of a relative cholinergic deficiency due to the accelerated process of aging and the presence of neurofibrillary tangles (with choline acetyl transferase deficiency.) This process would be enhanced by the imbalance in favour of dopaminergic predominance caused by chronic levodopa therapy, and would partially explain the increase in dementia. A

Brain aluminum in aging and Alzheimer disease.

Aluminum was assayed by atomic absorption spectroscopy in 274 brain samples, and assayed in neurons isolated in bulk from the frontal cortex of patients with Alzheimer dementia and from age-matched patients with no neurologic disease. Brain aluminum concentration increased with age, from late middle age to old age. There were no statistically significant differences in brain aluminum concentration between the 10 patients with Alzheimer disease (mean, 2.7 microgram per gram dry weight of tissue; mean age, 81 years), and the 9 nonneurologic controls (mean, 2.5 microgram per gram; mean age, 73 ye

[Several diagnostic errors in cerebral atherosclerosis (clinico-anatomic study)].

A clinicoanatomical study was conducted in 218 cases of atherosclerotic dementia. 12 cases (5,5% of the total amount) showed diagnostic errors. Atherosclerotic dementia with Alzhemier-like symptomatology during life was considered to be Alzheimers disease, while Alzheimers disease, complicated by cerebral atherosclerosis as atherosclerotic dementia. Some objective and subjective factors of diagnostic errors were established. Late detection of such patients is considered as one of the risk factors of diagnostic errors during life.

EEG in presenile dementia related to cerebral blood flow and autopsy findings.

EEG, regional cerebral blood flow (rCBF) and neuropathological autopsy data were studied in 17 patients with presenile dementia. RCBF was measured in 14 cases using the intra-arterial-133-Xenon clearance technique. The neuropathological study included semi-serial whole brain microscopical sections in which the severity and regional distribution of the neuronal degeneration was studied, particularly in the cortex and the brain stem. There were two main diagnostic groups, seven cases of Alzheimer's disease and five cases with a cortical atrophy of the Pick (DFT) type. Both groups showed fronto-t

Alzheimer degeneration in Down syndrome. Electrophysiologic alterations and histopathologic findings.

Progressive electroencephalographic disorganization and decreased voltage amplitude in the late components of the averaged visual evoked potentials were recorded in the last two years of life of a patient with Down disease and Alzheimer degeneration. Taken together with quantitative histopathologic findings, the electrophysiologic alterations are interpreted in terms of recent evidence from an experimental animal model of dementia. Neurons with neurofibrillary degeneration become electrically inactive and contribute to the loss of voltage generators associated with neuron death in Alzheimer di
