Retrospective studies in patients with nonmuscle invasive bladder cancer have reported a significant reduction in Alzheimer's disease incidence (12 to 78%) among Bacille Calmette-Guérin (BCG) recipients versus controls. To investigate the underlying mechanisms, we evaluated BCG in the PS19 mouse model of tauopathy. We found that BCG administration reduced hippocampal phospho-tau and microgliosis while preserving neuronal markers. In vivo volumetric T2-magnetic resonance Imaging (MRI) demonstrated attenuation of brain atrophy accompanied by increased glutamate-weighted chemical exchange saturat

BACKGROUND AND OBJECTIVES: Blood-based biomarkers offer a cost-effective, minimally invasive alternative to neuroimaging and CSF biomarkers. Newly developed fluid biomarkers, including the N-terminal tau fragment (NT1), show promise for identifying individuals at risk for Alzheimer disease (AD) and for monitoring disease progression. Here, we determine whether NT1 levels are associated with key clinical and biomarker measures in a large, well-characterized cohort.
METHODS: Participants from the Alzheimer's Disease Neuroimaging Initiative (ADNI) cohort with available plasma NT1, β-amyloid (Aβ)

Restless legs syndrome (RLS) is a sleep-related sensorimotor disorder affecting approximately 3% of adults worldwide. This review aims to synthesize current evidence on associations between RLS and major neurological conditions, provide an exploratory quantitative synthesis summarizing association patterns and heterogeneity, and identify priority mechanistic and clinical research gaps. Databases were searched from inception until August 2026. In our exploratory synthesis of 119 unique studies, RLS was significantly associated with Parkinson's disease (case-control OR 3.22, 95% CI 2.35-4.41; co

INTRODUCTION: Apolipoprotein E (APOE) ε4 is the strongest genetic risk factor for Alzheimer's disease (AD) and associated with increased risk of depression. We examined whether depression modifies the association between APOE ε4 and cognition.
METHODS: Using the Brain Health Registry (BHR), APOE ε4 status was determined by saliva-based genotyping. Depression and depression severity were self-reported as history of depression (yes/no), the Geriatric Depression Scale (GDS; range 0-14), and Patient Health Questionnaire (PHQ-9 range: 0-27). Cognitive function was measured via the Paired Associate

Brain connectivity disruption is a hallmark of neurodegenerative disorders, including Alzheimer's disease (AD), yet how network breakdown gives rise to cognitive deficits remains unclear. Here, we review the evidence bearing on three interconnected processes that have been proposed to link molecular pathology to cognitive decline. First, we summarize evidence that misfolded proteins propagate along brain connectivity pathways, making the connectome the biological substrate for disease progression. Second, we discuss the divergent trajectory reported across the AD continuum, an initial compensa

Early differentiation of mild cognitive impairment (MCI) from Alzheimer's disease (AD) remains difficult because diagnosis integrates cognitive, imaging, laboratory, and neurological evidence. EEG-based models may provide complementary information, but fixed descriptors can miss signal-specific structure. SOFEx was developed as an adaptive and interpretable EEG feature-engineering framework. The analysis used 1,905 EEG segments from 451 recordings of 168 participants (59 AD, seven MCI, and 102 controls). SOFEx combined self-organized rose pattern extraction, fold-wise feature selection, channe

BACKGROUND: Despite the approval of lecanemab for Alzheimer's disease (AD), evidence regarding its performance outside controlled trial settings remains scarce, leaving its real-world safety and clinical utility largely unknown.
METHODS: We searched PubMed, Embase, and Web of Science (through October 1, 2026) for real-world studies of lecanemab in AD. Outcomes included ARIA, discontinuation, infusion-related reactions, serious adverse events, MMSE, MoCA, CDR-SB, and amyloid PET. Random-effects models with REML and Hartung-Knapp adjustment were used. Subgroup analyses were stratified by region

OBJECTIVE: Alzheimer's disease (AD) molecular mechanisms remain unclear, and effective therapies are lacking. This study employed whole-transcriptome sequencing to investigate the expression profiles of circRNAs, lncRNAs, miRNAs, and mRNAs in the hippocampus of APP/PS1 transgenic mice, aiming to identify ceRNA regulatory networks and clarify their roles in AD pathogenesis.
METHODS: Differentially expressed RNAs were integrated into ceRNA networks, and programmed cell death-related pathways were analyzed using GO and KEGG. The computationally predicted circRNA_13083/miR-298-5p/CD14 axis was pr

INTRODUCTION: It remains unclear whether medial temporal lobe (MTL) hyperperfusion in cognitively unimpaired apolipoprotein E (APOE) ɛ4 carriers reflects resilience or dysfunction. We examined whether cortical MTL cerebral blood flow (CBF) relates to cognition differently in APOE ɛ4 carriers and noncarriers with and without AD pathophysiological changes.
METHODS: One hundred forty-eight cognitively unimpaired older adults underwent pseudo-continuous arterial spin labeling MRI to quantify MTL CBF and assessment of memory and executive function. In a subset (n = 126), AD pathology was indexed u

In Alzheimer's disease (AD), activation of the mechanistic target of rapamycin (mTOR) pathway promotes microglial neuroprotective functions by engaging multiple cell receptors. However, the relevant downstream effectors are not fully understood. mTOR complex 1 (mTORC1) promotes mRNA translation by phosphorylating and inactivating eukaryotic translation initiation factor 4E-binding protein 1 and 2 (4E-BP1/2), which are expressed in microglia. 4E-BP1 levels in cerebrospinal fluid (CSF) are elevated in AD and correlate with markers of microglial reactivity and disease progression. Using linear re

BACKGROUND: Alzheimer's disease and related dementias (ADRDs) are increasingly common conditions that are highly disruptive to individuals and families. ADRDs impact cognition and can undermine a person's ability to participate in cooking, preparing meals, and engaging in conversations. Mealtime participation implicates key determinants of adequate nutritional intake and social connectedness. However, engaging in mealtime routines amid challenges can also exacerbate the stress and burden felt by family caregivers. Behavioral interventions can mitigate these downstream impacts, though they are

BACKGROUND: Alzheimer's disease (AD) is a neurodegenerative disorder and oxidative stress (OS) is believed to play a role in disease pathogenesis. However, reliable OS markers are lacking. In this study we aim to assess OS in AD by evaluating free thiols, which serve as key indicators of the systemic redox status.
METHODS: SCD (subjective cognitive decline), MCI (mild cognitive impairment), AD, and other dementias (Non-AD) were recruited. Comorbidity data were collected and free thiols were measured in plasma. Additionally, phosphorylated-Tau (pTau217), amyloid-beta ratio (Aβ42/40), Glial Fib

INTRODUCTION: Epigenetic dysregulation is increasingly linked to aging and neurodegeneration, yet its contribution to retinal and brain pathology in Alzheimer's disease (AD) remains unclear.
METHODS: We quantified the repressive epigenetic mark histone H3 lysine 9 trimethylation (H3K9me3) in post mortem retinas and brains from donors spanning normal cognition, mild cognitive impairment, and AD dementia. Using a multi-enzyme methyltransferase panel, we characterized the in vitro inhibitory activity of ETP69, an epidithiodiketopiperazine, against suppressor of variegation 3-9 homologue 1 (SUV39

Alzheimer's disease (AD) remains a major cause of dementia, yet effective disease-modifying therapies are still limited. Although amyloid-β (Aβ) and tau pathology remain core features of AD pathogenesis, they do not fully explain how these pathological alterations drive progressive cellular dysfunction. Increasing evidence suggests that mitochondria are involved at this critical interface. In AD, mitochondrial abnormalities impair energy production, compromise organelle quality control, disrupt calcium (Ca2+) homeostasis, and alter communication with the endoplasmic reticulum and glial cells.

We investigated the effects of an orexin receptor antagonist on sleep profiles assessed by actigraphy, and cognitive function in patients with dementia with Lewy bodies (DLB). Thirteen probable DLB patients who were taking an orexin receptor antagonist for insomnia were recruited and underwent actigraphy before and after taking lemborexant. Cognitive function and severity of sleep disturbance were compared before and after taking lemborexant. No significant differences in sleep quality and cognitive function were observed in DLB patients between before and after taking lemborexant. These resul

Alzheimer's disease (AD) is a neurodegenerative disease that leads to cognitive impairment and dementia, and retinal imaging is being explored as a biomarker. We examined the differences in retinal thickness by optical coherence tomography (OCT) between 47 participants with mild cognitive impairment due to Alzheimer's disease (Aβ+MCI) and 23 age-matched controls, as well as associations between cognition and OCT. There was no significant difference in retinal thickness between groups. However, retinal measures were robustly associated with global cognition and executive function in those with

Brain health is a state of physiological functioning of the organ that allows individuals to fully achieve their age-related potential. Throughout life, a wide range of endogenous and exogenous factors can influence positively and negatively brain health. Among them, age is undoubtedly one of the most important, particularly for its effect on brain's cognitive functions. Pathological cognitive decline is characterized by anatomical changes in areas involved in memory, execution, and reasoning, which may result from various mechanisms such as mitochondrial dysfunction, altered proteostasis, and
