Cardiorespiratory fitness (CRF) is a modifiable determinant of brain health and has been associated with greater brain volume, improved cognition, and reduced dementia risk. However, whether CRF influences cerebral oxygen extraction fraction (OEF), a marker reflecting the balance between cerebral oxygen delivery and metabolic demand, remains poorly understood in aging and Alzheimer's disease (AD). We performed a secondary analysis of the Acute Exercise Response on Brain Imaging and Cognition (AEROBIC) study to investigate relationships among CRF, cerebral blood flow (CBF), and OEF in cognitive

BACKGROUND: Repeated serum metabolite measurements may contribute to predicting clinical progression, but their incremental value requires evaluation beyond established clinical predictors.
METHODS: We linked Alzheimer's Disease Neuroimaging Initiative (ADNI) clinical data to Biocrates Quant 500 serum measurements. Annualized metabolite slopes were estimated from baseline through month 12, usually from two observations. At the month-12 landmark, participants without dementia were characterized by diagnosis, memory performance, and APOE ε4 status; progression was assessed at approximately mont

Brain-derived phosphorylated tau species are postulated to improve the detection of Alzheimer's disease pathology by mitigating non-specific increases. However, their true diagnostic performance, relative to conventional CSF and plasma phosphorylated tau assays, remains uncharacterized. The objective was to compare the diagnostic performance of brain-derived and conventional phosphorylated tau assays across multiple analytes (phosphorylated tau 181, phosphorylated tau 217, phosphorylated tau 231) in CSF and plasma, and to examine biomarker differences across amyloid-β status and cognitive grou

Alzheimer's disease (AD) is a progressive neurodegenerative disorder, where traditional diagnostic methods are constrained by limited sensitivity and restricted clinical utility. This review synthesizes recent applications of artificial intelligence (AI) in AD diagnostics, focusing on machine learning (ML) and deep learning (DL) across neuroimaging modalities and multi-omics research. AI algorithms can automatically extract subtle microstructural, metabolic, and molecular features, enabling high-precision classification of AD stages, patient stratification, and non-invasive early screening thr

Memantine is a noncompetitive N-methyl-D-aspartate (NMDA) receptor antagonist widely utilized in the management of moderate-to-severe neurocognitive disorders, specifically Alzheimer's disease. While its common adverse effects, such as dizziness, headache, and confusion, are well documented, parasympathetic or motor-related side effects such as drooling are not listed in the standard prescribing information and remain rarely reported in clinical practice. We present a case of a 68-year-old man with a primary diagnosis of moderate-to-severe major neurocognitive disorder due to Alzheimer's disea

BACKGROUND: This narrative review proposes a conceptual, lactate-informed framework for individualized exercise prescription in mild cognitive impairment (MCI) and Alzheimer's disease (AD). Exercise is recommended in these populations, but fixed external doses do not account for heterogeneity in disease stage, frailty, sarcopenia, cardiovascular risk, or recovery.
MAIN TEXT: We integrate clinical exercise evidence, geriatric assessment, lactate physiology, muscle-brain signaling, implementation science, and trial methodology. The framework uses peripheral blood lactate as one component of a m

BACKGROUND: In mild cognitive impairment and dementia due to Alzheimer's disease (AD), postmortem and in vivo neuroimaging studies have demonstrated significant neuronal loss in the basal forebrain cholinergic system (BFCS), which provides the primary cholinergic input to the cerebral cortex. Within the BFCS, atrophy is most prominent in the nucleus basalis of Meynert (nbM), a posteriorly located cluster of magnocellular neurons. However, less is known surrounding the relationship between amyloid deposition, BFCS atrophy, and medial temporal lobe (MTL) volume loss in preclinical AD. This study

In-hospital mortality among resuscitated out-of-hospital cardiac arrest (OHCA) patients remains high. Identifying patients who will benefit from intensive care while avoiding prolonged, potentially unnecessary treatments is essential. Blood-based biomarkers provide a novel method for evaluating brain injury. The ability of glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL), neuron-specific enolase (NSE) and total-tau (t-tau) to measure white and grey matter injury is unestablished in comatose OHCA survivors treated with targeted temperature management (TTM) or xenon combin

PURPOSE: The goal of this study was to enhance the fairness and clinical applicability of deep learning models for classifying Alzheimer's disease from structural MRI scans. To achieve this, we addressed two key challenges: age-related bias and unreliable probability estimates. Specifically, we investigated in-processing debiasing strategies to achieve age-invariant predictions while preserving diagnostic accuracy. We also systematically evaluated post hoc calibration methods to improve the reliability, interpretability, and clinical usefulness of predicted probabilities across diverse patient

This cross-sectional study examined associations of 88 serum lipid measures with clinical-amyloid group, CSF biomarkers, cognition, and hormones in 175 ADNI-1 participants. PC ae C38:1 was the only lipid associated with ordinal group score after false discovery rate correction (β = 0.231; q = 0.021). The categorical group omnibus test also met the corrected threshold (q = 0.043). Twenty lipid-CSF Aβ42 associations were identified before group adjustment, compared with none afterward in the original linear models. Rank-based models with categorical adjustment identified two Aβ42 associations. S

BACKGROUND: As Alzheimer's disease and related dementias (ADRD) progress, many people become homebound and depend on home-based medical care (HBMC), but differences in HBMC, telemedicine, and office visits between Traditional Medicare (TM) and Medicare Advantage (MA) plans are not well understood.
METHODS: We analyzed 2021-2022 Medicare claims and encounter data to categorize 2022 outpatient visits for community-living TM and MA enrollees with ADRD as HBMC, telemedicine, or office-based. We summarized HBMC recipient characteristics by plan type and used Poisson regression with inverse weights

BACKGROUND: Microglial metabolic dysfunction is increasingly recognized as a key driver of neuroinflammation in Alzheimer's disease (AD). However, the specific metabolic regulators linking lipid metabolism to pathological activation remain poorly defined.
METHODS: We performed integrative transcriptomic analysis on four human AD brain datasets to identify core metabolic hubs. Sprouty homolog 1 (SPRY1) expression was examined in human post-mortem tissues, and its functional role was validated in primary microglia, Drosophila, and APP/PS1-21 mice using genetic knockdown, behavioral assays, and

INTRODUCTION: Alzheimer's disease (AD) involves neuroinflammation that worsens cognitive decline. Thymoquinone (TQ) has potent anti-inflammatory activity, but its therapeutic application is limited by poor solubility, stability, and systemic bioavailability, which may restrict effective brain exposure. This study developed TQ-loaded hexosomal nanoparticles (TQ-HNP) to enhance TQ delivery and therapeutic efficacy in a dizocilpine (MK-801)- induced mouse model of AD.
METHODS: TQ-HNP were prepared using glyceryl monooleate, oleic acid, and poloxamer 407, optimized to ~104 nm size, -31 mV zeta po
