Explanatory limitations of CSF, plasma, and MRI biomarkers for inflammation and blood-brain barrier disruption in dementia.

Inflammation and blood-brain barrier (BBB) disruption are increasingly implicated in cognitive decline, but it remains unclear which fluid and imaging measures best capture these processes. Participants were drawn from two overlapping research programs, MarkVCID and UNM ADRC. We studied 149 participants with Alzheimer's disease, leukoaraiosis, mixed dementia, subcortical ischemic vascular dementia, or memory impairment. BBB permeability was assessed using the albumin index (Qalb), a global blood-to-CSF leakage ratio, and dynamic contrast-enhanced MRI permeability (Ktrans), a regional gadoliniu

Socioeconomic and lifestyle factors influencing non-communicable disease prevalence among rural Jamaican adults: a population-based study.

BACKGROUND: Few studies in the scientific literature have analyzed frailty status as an ordinal variable (non-frail, pre-frail and frail) rather than as a binary variable (frail vs non-frail). These studies have found that pre-frailty behaves differently from frailty (no ordinality in the variable). However, although the comparison between pre-frail and frail individuals is clinically relevant to understanding how to treat pre-frailty, this comparison was not performed in previous studies. MATERIALS AND METHODS: A cross-sectional observational study was designed with 621 older individuals age

Biological classification of memory clinic patients.

Neurodegenerative diseases have traditionally been defined in vivo based on clinical symptoms. However, the development of biomarkers has enabled a shift towards in vivo biological definitions. There is now a need to characterize memory clinic populations using multi-dimensional biomarker information. Here, we employed a data-driven approach to develop a biological framework for categorizing individuals in a heterogeneous memory clinic cohort based on the presence, extent and sequence of several common pathologies. We studied 1677 individuals, including subjective cognitive decline (SCD, n = 2

Simultaneous multiple post-labelling delay ASL MRI and [18F]FDG PET in a mixed memory clinic population and healthy controls.

PURPOSE: To assess the quantitative and visual concordance of multiple post-labelling delay (multi-PLD) arterial spin labelling (ASL) MRI cerebral blood flow (CBF) measurements and [18F]-fluoro-deoxyglucose (FDG) PET in a mixed memory clinic population. METHODS: Hybrid [18F]FDG PET/MRI including multi-PLD pseudo continuous ASL from 96 memory clinic patients and 38 elderly controls were analysed along with ASL data from 12 healthy young volunteers. ASL image interpretability, concordance with [18F]FDG PET, and value of Z-score maps were rated visually. Regional associations of CBF with [18F]FD

The enigma of vascular dementia: current state and emerging perspectives.

Vascular dementia (VaD) is a neurocognitive disorder attributed to different types of cerebrovascular disease (CVD) and characterized by complex and multifold pathophysiological mechanisms. The most common forms in the elderly brain are multi-infarct, subcortical and strategic infarct dementias caused by atherosclerosis, small vessel disease, and cerebral amyloid angiopathy (CAA), hereditary forms being rare. VaD prevalence, accounting for 5% to 20% of all dementias, in comparison to Alzheimer disease and mixed dementias declines due to recent improvement of cardiovascular risk factors. Its di

Frontal white matter hyperintensity burden predicts cognitive response to N-acetylcysteine and exercise in vascular mild cognitive impairment.

Vascular mild cognitive impairment (vaMCI) is a prodromal stage of dementia defined by cognitive deficits due to cerebrovascular disease. Increased white matter hyperintensity (WMH) volume has been associated with reduced executive function (EF). We explored whether lower baseline frontal and global WMH volume predicted an improvement in EF in vaMCI participants treated with N-acetylcysteine (NAC) and exercise as compared to placebo and exercise. Fifty-eight individuals with vaMCI received exercise therapy and were randomized to NAC or placebo. EF was assessed using the Trail Making Test Part

Patterns and Trajectories of Behavioral and Neuropsychiatric Symptoms in Frontotemporal Dementia and Primary Progressive Aphasia.

BACKGROUND AND OBJECTIVES: Behavioral and neuropsychiatric symptoms are common in frontotemporal dementia (FTD) and primary progressive aphasia (PPA). However, little is known about their patterns, time course, and association with brain atrophy. We, therefore, aimed to describe behavioral and neuropsychiatric phenotypes in patients with FTD and PPA, leveraging a hypothesis-free/data-driven approach. METHODS: We included participants diagnosed with behavioral variant FTD (bvFTD) or PPA according to Rascovsky and Gorno-Tempini criteria from the German Center for Neurodegenerative Diseases Clin

Reproductive lifespan and hormonal therapy in relation to later-life neurovascular health: A population-based study of women in the Gothenburg H70-1944 Birth Cohort.

BACKGROUND: Women face greater vulnerability to dementia and Alzheimer's disease (AD), potentially due to estrogen fluctuations across the lifespan. However, its role in vascular brain health is unclear. We investigated associations between lifelong estrogen exposure-endogenous (reproductive span) and exogenous (oral contraceptives [OC], menopausal hormone therapy [MHT])-and late-life vascular brain injury, AD-related atrophy, and APOE-ε4 modification. METHODS AND FINDINGS: We included 352 cognitively unimpaired 70-years-old women from the Gothenburg H70-1944 Birth Cohort with brain MRI and 5

High vulnerability of medial prefrontal pyramidal neurons in post-stroke, vascular, Alzheimer's disease, and aging-related dementias.

INTRODUCTION: The medial prefrontal cortex (mPFC) is critical for executive function, behavioral inhibition, and memory. Its high vulnerability to dementia, compared to other prefrontal regions, remains unclear. METHODS: We analyzed post mortem brain tissue from 118 older subjects, including post-stroke survivors, Alzheimer's disease; vascular, mixed, and frontotemporal dementia (FTD); and cognitively unimpaired controls. Three-dimensional stereology was used to assess pyramidal neuron densities and volumes in mPFC layers III and V. Immunohistochemistry evaluated metabolic dysfunction via cyt

Associations Between Cerebral Microbleed Patterns and Incident Dementia: The ARIC-Neurocognitive Study.

BACKGROUND: Cerebral microbleeds (CMBs) are associated with dementia, but the impact of specific microbleed patterns (with distinct pathophysiologies) is unclear. Lobar CMBs commonly result from cerebral amyloid angiopathy (sometimes with cortical superficial siderosis [cSS]), while subcortical microbleeds indicate a hypertensive cause. This study investigates the association of CMB presence, patterns, and frequency with dementia risk. METHODS: Participants from the ARIC-NCS (Atherosclerosis Risk in Communities-Neurocognitive Study), a community-based longitudinal cohort, with a 3T research m

Hypoperfusion in early-phase amyloid PET as a predictor of cognitive decline in Alzheimer's disease.

PURPOSE: Dual-phase amyloid PET enables concurrent assessment of amyloid burden and cerebral perfusion, providing both molecular and functional information within a single visit. While amyloid PET is well validated diagnostically, the prognostic value of early-phase perfusion remains less explored. We aimed to determine whether perfusion in Alzheimer’s disease (AD)–relevant regions is associated with longitudinal cognitive trajectories. METHODS: We analyzed 218 participants from a memory-clinic cohort ranging from cognitively unimpaired (CU, n = 65) to mild cognitive impairment (MCI, n = 115)

Carotid arteries in cerebral small vessel disease and dementia.

Carotid artery disease (CAD) is a recognised cause of stroke. However, the relationships between CAD, cerebral small vessel disease (SVD) and dementia remain unclear. We hypothesised that CAD in older individuals contributes to cerebral SVD pathology by altering cerebral perfusion. We performed a clinicopathological study in patients from the Cognitive Function After Stroke (CogFAST) study and prospectively recruited patients with various dementia diagnoses and evidence of cerebral SVD. In addition to brain tissues, we collected postmortem samples of the internal carotid arteries (ICA) from th

APOE-mediated sex differences in microvascular pathology and AD-associated proteinopathies in the medial temporal lobe.

INTRODUCTION: Cerebral small vessel disease (CSVD) contributes to the development of Alzheimer's disease (AD) dementia and co-occurs with AD-associated proteinopathies. However, how sex modulates the interaction between CSVD and AD-associated proteinopathies in the medial temporal lobe (MTL) remains unclear. METHODS: One hundred fifty-two autopsy cases from the Massachusetts Alzheimer's Disease Research Center were included. Deep-learning and semiquantitative scores were applied to MTL histological sections to obtain quantitative measures of proteinopathies and CSVD (cerebral amyloid angiopat

Neurovascular large artery dilatation increases the risk for Alzheimer's disease pathology.

Alzheimer's disease and related dementias are increasing globally, highlighting the urgent need to clarify disease mechanisms to improve diagnosis and treatment. Vascular alterations are a major pathological feature of Alzheimer's disease. Beyond the established contributions of small vessel disease and large artery atherosclerosis, our group has shown that dilatation of large cerebral arteries is associated with an increased risk of dementia and Alzheimer's pathology. The most severe manifestation of this non-atherosclerotic vascular phenotype is dolichoectasia, characterized by abnormal enla

Effects of Sarcopenia and Frailty on Cognitive Function, Brain Volume, and Dementia Risk: A Prospective Cohort Study Based on UK Biobank.

BACKGROUND AND PURPOSE: Despite the rising prevalence of sarcopenia, frailty, and dementia, their interrelationships remain unclear. This study investigated the associations of sarcopenia and frailty with cognitive function, brain structure, incident dementia, and their longitudinal changes. METHODS: This study included 390,903 participants aged 40-70 years from the UK Biobank. Sarcopenia was assessed using hand grip strength, muscle mass index, and gait speed; frailty was measured based on weight loss, exhaustion, physical activity, gait speed, and grip strength. Outcomes included cognitive

Transcranial direct current stimulation to improve cognitive function in patients with vascular cognitive impairment: a literature review.

Vascular pathology represents the second most common cause of cognitive impairment after Alzheimer's disease and contributes substantially to mixed dementia involving both vascular and neurodegenerative mechanisms. Vascular cognitive impairment (VCI) emerges as a term encompassing cognitive impairments resulting from stroke, cerebral small vessel disease (CSVD), or other forms of vascular pathology. While numerous studies have demonstrated the beneficial effects of transcranial direct current stimulation (tDCS) on mild cognitive impairment and dementia, evidence specifically addressing its eff

Perfusion as a biomarker of brain dysfunction in dementia (AD, DLB, FTD/PPA, PDD): comparison of CT, MRI (ASL/DSC/DCE), SPECT, and PET with interpretive pitfalls - a narrative review.

The term 'brain perfusion' is applied in clinical practice to a family of neuroimaging techniques that measure, in reality, quite different physiological quantities in the brain circulation. CT perfusion (CTP) and DSC-MRI track a contrast bolus to derive semi-quantitative haemodynamic parameters. ASL-MRI estimates cerebral blood flow (CBF) without contrast, but the result is sensitive to arterial transit time, haematocrit, and the patient's haemodynamic state on the day of the scan. DCE-MRI quantifies blood-brain barrier (BBB) permeability, a property of the neurovascular unit rather than a fl

Retinal Optical Coherence Tomography-Angiography and Longitudinal Changes in Cognition and Cerebral Small Vessel Disease.

BACKGROUND AND OBJECTIVES: Cross-sectional studies suggest that retinal microvascular abnormalities measured by optical coherence tomography angiography (OCTA) are associated with cerebral small vessel disease (cSVD) and cognitive impairment, but longitudinal relationships remain unclear. We investigated whether OCTA measures are associated with brain cSVD markers and cognition over time and whether baseline OCTA was associated with brain and cognitive outcomes after mild ischemic stroke. METHODS: We conducted a prospective longitudinal cohort study recruiting patients with lacunar or mild co
