Disseminated Nocardia otitidiscaviarum Presenting as Lung-Brain-Skin Syndrome in a Renal Transplant Recipient: A Case Report.

We present a case of disseminated Nocardia otitidiscaviarum infection in a 72-year-old male with a history of renal transplantation on chronic immunosuppressive therapy with prednisone, cyclosporine, and mycophenolate mofetil. He presented with pneumonia and was initially treated with ceftriaxone and azithromycin. Bronchoalveolar lavage (BAL) and cultures from a biopsied thigh lesion both grew N. otitidiscaviarum. His hospital course was complicated by visual hallucinations, and brain magnetic resonance imaging (MRI) revealed multiple intracranial abscesses. He initially received meropenem, tr

Clinical characteristics of pulmonary nocardiosis in patients with and without bronchiectasis: a single-center retrospective comparative study.

BACKGROUND: Bronchiectasis is a common structural lung disease associated with pulmonary nocardiosis (PN), but the differences between PN patients with and without bronchiectasis remain insufficiently characterized. METHODS: We conducted a single-center retrospective study of hospitalized patients with PN at Beijing Chaoyang Hospital from January 2012 to December 2024. Patients were grouped by bronchiectasis status, and their clinical, radiological, and microbiological features were compared. Multivariable logistic regression was used to identify factors independently associated with bronchie

Pulmonary nocardiosis in patients with underlying pulmonary alveolar proteinosis: a case series and review of the literature.

OBJECTIVES: Pulmonary alveolar proteinosis (PAP) and pulmonary nocardiosis are relatively rare, their co-occurrence constitutes a clinically important comorbidity. Nocardia infection exhibits variable clinical manifestations, and carries a substantial risk of mortality in the absence of timely, pathogen-directed antimicrobial therapy. This study aims to summarize the clinical features of Nocardia infection in PAP patients. CASE PRESENTATION: We present four confirmed cases of PAP complicated by Nocardia infection. These cases illustrate the distinct microbial species, disease courses and clin

Therapeutic drug monitoring of linezolid in a case of pulmonary nocardiosis: a case report.

This clinical case discusses a man between 60 and 70 years old with lung adenocarcinoma who developed a Nocardia cyriacigeorgica infection. Because it is an intracellular bacterium, we need to start antibiotic treatment and maintain for a year. For initial therapy, sulfamethoxazole/trimethoprim (SMX/TMP) is the preferred antibiotic, but it can cause side effects, such as liver damage. Linezolid is another valid alternative.Pharmacokinetic monitoring of linezolid was essential in managing the patient because the drug can cause significant side effects like myelosuppression. The pharmacist track

Pulmonary nocardiosis masquerading as refractory haemoptysis in a bronchiectasis patient: the critical role of multidisciplinary management.

An immunocompetent male in his 30s with a history of bronchiectasis presented with recurrent haemoptysis (5-10 mL every 2 months) over 4 years. High-resolution CT revealed cystic bronchiectasis and ground-glass opacities. Bronchoscopy identified active bleeding from a ruptured bronchial artery, which was successfully managed with emergent embolisation. Bronchoalveolar lavage demonstrated weakly acid-fast branching filaments, later confirmed as Nocardia asteroides by matrix-assisted laser desorption-ionisation time of flight mass spectrometry. The patient responded well to trimethoprim-sulfamet

Pulmonary Nocardia otitidiscaviarum in immunocompetent individuals: a case series and literature review.

Nocardia are filamentous, Gram-positive, aerobic bacteria widely distributed in diverse environments. Nocardia otitidiscaviarum, first isolated from the middle ear of guinea pigs, is an opportunistic pathogen. Nocardia primarily infects immunocompromised hosts, However, recently there is an increasing incidence in immunocompetent individuals. Pulmonary nocardiosis is commonly caused by N. asteroides, Nocardia otitidiscaviarum pulmonary infections remain exceptionally rare. Here, we report four cases of Nocardia otitidiscaviarum infection of lung in immunocompetent individuals, including one wi

Nocardia Infection Complicated by Severe Hyponatremia in a Renal Transplant Patient.

Patients with solid organ transplants are at high risk for opportunistic infections due to the use of potent immunosuppressive drugs. Nocardiosis is a Gram-positive, aerobic actinomycete infection that is uncommon but can be serious in solid organ transplant recipients if not treated promptly. It often presents pulmonary or disseminated disease. Nocardia can infect immunocompromised patients mainly by airborne transmission. We recently encountered a challenging case of pulmonary nocardiosis with severe hyponatremia in a renal transplant recipient. We chose to write this case to highlight the a

Redefining the diagnostic pathway for pulmonary nocardiosis: The imperative for early metagenomic sequencing.

In this article, we comment on the pivotal article by Wang et al. We focus on the critical intersection of advanced imaging and molecular diagnostics highlighted by their findings. The study delineates specific high-risk computed tomography patterns, notably consolidation with nodules/cavities, particularly in immunocompromised hosts or patients with bronchiectasis, that should serve as immediate red flags for pulmonary nocardiosis. Traditionally, diagnosis has relied on slow-growing cultures, leading to dangerous therapeutic delays. This editorial argues that the presence of these defined rad

Pulmonary infection by Nocardia saintgeorgesii mimicking lung cancer with concurrent pulmonary embolism in an immunocompetent host: a case highlighting the diagnostic role of mNGS.

BACKGROUND: Pulmonary nocardiosis presents a diagnostic challenge due to its frequent mimicry of lung cancer on imaging and the low sensitivity of conventional cultures. We report a case initially misdiagnosed as malignancy in an immunocompetent host, where metagenomic next-generation sequencing (mNGS) provided a definitive diagnosis and revealed a concurrent pulmonary embolism, suggesting a potential underrecognized association. CASE PRESENTATION: This report describes a case of PN in an immunocompetent patient who was initially misdiagnosed with lung cancer based on imaging findings but lat

Delayed Microbiological Diagnosis and Fluoroquinolone-Associated Tendon Rupture in Pulmonary Nocardiosis Complicating Autoimmune Pulmonary Alveolar Proteinosis: A Case Report.

This report describes pulmonary nocardiosis in autoimmune pulmonary alveolar proteinosis (PAP), characterized by delayed microbiological diagnosis and a severe fluoroquinolone-associated adverse reaction. The autoimmune form, mediated by anti-granulocyte-macrophage colony-stimulating factor (GM-CSF) antibodies, predisposes patients to opportunistic infections due to impaired alveolar macrophage function. We describe a 48-year-old male smoker with a history of recurrent respiratory infections who presented with fever and hypoxemic respiratory failure. High-resolution CT demonstrated bilateral g

Case Report: Pulmonary nocardiosis: three case reports and literature review.

Pulmonary nocardiosis is a relatively uncommon opportunistic infection characterized by nonspecific clinical features, often leading to delays in diagnosis and treatment. We describe the clinical data of three confirmed cases of pulmonary Nocardia infection. We also systematically reviewed relevant literature from the PubMed database between January 2015 and September 2025, and performed a pooled analysis of 119 patients. Case 1 involved an elderly patient with bronchiectasis, diagnosed via targeted next-generation sequencing (tNGS) of sputum, who showed significant radiological improvement af

Letter to the Editor: Evidence for a two-step species-level pulmonary nocardiosis diagnostic approach.

Pulmonary nocardiosis, a rare and diagnostically challenging infection, usually presents with heterogeneous radiographic findings, compounded by the low sensitivity of traditional confirmatory cultures. In their most recent work, Wang et al analyzed 102 patients with pulmonary nocardiosis to address these concerns, investigating species-characteristic imaging patterns, pathological associations, and the role of metagenomic next-generation sequencing (mNGS) in the diagnostic approach. High-resolution computed tomography (CT) in adult patients with pulmonary infections caused by Nocardia wallace

Pulmonary Nocardiosis Diagnosed by Plasma Metagenomic Next-Generation Sequencing in a Patient With Recurrent Febrile Neutropenia.

BACKGROUND: Nocardiosis disproportionately affects immunocompromised hosts. Early identification of Nocardia infections is critical as delays can lead to worse outcomes. Species such as N. farcinica are associated with increased risk of dissemination and resistance. Diagnosis of opportunistic infections in immunocompromised populations relies on culture, antigen, or serologic methods that often have limited sensitivity or specificity. Plasma microbial cell-free DNA metagenomic next-generation sequencing (mNGS) offers a noninvasive approach for early diagnosis of opportunistic infections. CASE
