BACKGROUND: Nocardiosis disproportionately affects immunocompromised hosts. Early identification of Nocardia infections is critical as delays can lead to worse outcomes. Species such as N. farcinica are associated with increased risk of dissemination and resistance. Diagnosis of opportunistic infections in immunocompromised populations relies on culture, antigen, or serologic methods that often have limited sensitivity or specificity. Plasma microbial cell-free DNA metagenomic next-generation sequencing (mNGS) offers a noninvasive approach for early diagnosis of opportunistic infections.

Pulmonary nocardiosis is an uncommon but potentially serious infection caused by aerobic actinomycetes of the genus Nocardia. It predominantly affects individuals with impaired immunity or underlying structural lung disease, but can also occur in apparently immunocompetent hosts. The clinical and radiological features often overlap with tuberculosis and fungal infections, particularly in endemic regions, leading to frequent misdiagnosis and delay in appropriate therapy. We present a series of three patients with microbiologically confirmed pulmonary nocardiosis, each representing different cli

A woman in her 80s, who had a history of treatment with pembrolizumab for biliary tract cancer and was taking low-dose corticosteroids for immune-related adverse events (irAEs) involving nephritis and hepatitis, was admitted to the hospital complaining of a cough and increased sputum production that had begun two weeks prior. Computed tomography (CT) revealed patchy infiltrates predominantly in the lower lobes of both lungs. Since one of the Nocardia species was detected in the sputum collected at admission, we diagnosed her with pulmonary nocardiosis. In addition to this case, other cases of

Lung infections with opportunistic pathogens, such as Pneumocystis or Nocardia, are infrequent in patients with severe asthma despite the use of high doses of inhaled or oral corticosteroids. We report two patients with severe eosinophilic asthma complicated by Nocardia infection in the respiratory tract. Both patients had exposure to chronic corticosteroid therapy and developed recurrent respiratory infections. The first patient had nodular pulmonary nocardiosis associated with hypocomplementemia and heterozygous variants in C1QB and NOD2, while the second patient had Nocardia farcinica trach

An 80-year-old man with diffuse large B-cell lymphoma developed pulmonary nocardiosis caused by Nocardia gipuzkoensis, with a concomitant central nervous system lesion. The isolate was identified as N. gipuzkoensis by secA1 gene sequencing. To our knowledge, this is the first reported human infection caused by N. gipuzkoensis in Japan.

Pulmonary actinomycosis and pulmonary nocardiosis are rare chronic respiratory infections that may closely resemble lung cancer due to their overlapping clinical, radiological, and bronchoscopic characteristics. This hybrid case series and narrative review aims to elucidate the presenting patterns, diagnostic pathways, management strategies, and practical implications of these infections in the differential diagnosis of lung cancer. We analyzed four consecutive cases managed at the Sotiria Thoracic Diseases General Hospital in Athens from 2021 to 2025, supplemented by eight additional cases id

INTRODUCTION: Pulmonary nocardiosis lacks specific symptoms and characteristic test findings, making diagnosis and treatment challenging. Therefore, to clarify its clinical features, we analyzed 21 cases diagnosed as pulmonary nocardiosis at our institution. METHODS: We conducted a retrospective review of cases in which Nocardia species were isolated from respiratory specimens at Saitama Red Cross Hospital between January 2017 and May 2024. RESULTS: The cohort included 14 females and 7 males, with a mean age of 71.8 years. Underlying lung diseases were present in 14 cases (66.7%), with pulmona

BACKGROUND: Nocardiosis is a rare opportunistic infection typically affecting immunocompromised individuals and those with chronic lung disease, prolonged corticosteroid therapy, malignancy, or other conditions impairing cell-mediated immunity, while tuberculosis (TB) remains a leading global health concern. Dual infections with Nocardia and TB are rare but clinically important, as overlapping clinical and radiologic features make diagnosis difficult. Reports have also documented co-infections with Nocardia and other pathogens, including Aspergillus and nontuberculous mycobacteria, underscorin

Pulmonary nocardiosis may occur in immunocompetent patients with structural lung disease and may resemble the progression of nontuberculous mycobacterial pulmonary disease. We report an 81-year-old immunocompetent woman with chronic bronchiectasis and prior Mycobacterium kansasii, M. intracellulare, and M. abscessus infections. Despite persistently negative sputum mycobacterial cultures, chest computed tomography (CT) revealed progressive nodular bronchiectatic changes. Bronchial washing from the right B5 bronchus yielded Nocardia cyriacigeorgica; mycobacterial studies were negative. Trimethop

We report a case of pulmonary nocardiosis and pulmonary tuberculosis following steroid pulse therapy for presumed myelitis. A 74-year-old man with transthyretin amyloid cardiomyopathy, chronic heart failure, and type 2 diabetes mellitus received two courses of steroid pulse therapy for suspected inflammatory myelitis. One month later, he developed progressive dyspnea and was admitted to our hospital. Chest computed tomography revealed multifocal right-sided infiltrates and pleural effusion. Although empirical piperacillin/tazobactam was initiated for presumed bacterial pneumonia, sputum Gram a

Recovery of multiple microorganisms from respiratory specimens complicates pathogen attribution in patients with chronic lung disease. A 71-year-old woman with COPD and bronchiectasis presented with severe pneumonia and respiratory failure. She had T-cell lymphopenia (CD3+, CD4+, and CD8+ counts: 189, 147, and 41 cells/μL) and negative HIV testing. BALF Gram staining showed branching Gram-positive rods suspicious for Nocardia spp., and cultures yielded a Nocardia isolate, Aspergillus fumigatus, and Candida albicans. The isolate was presumptively identified as N. terpenica by MALDI-TOF MS (scor

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

We describe a rare case of spontaneous remission of Pneumocystis jirovecii pneumonia (PCP) in a 42-year-old patient with human immunodeficiency virus (HIV) infection, followed by severe pulmonary nocardiosis. To our knowledge, this is the first report of spontaneous remission of PCP in a completely untreated patient with HIV infection. The patient, a bisexual Japanese man, presented with fever and anorexia and had a history of non-compliance with antiretroviral therapy (ART) for 13 years. PCP was initially suspected on the basis of imaging and laboratory findings, but the patient refused furth

Invasive pulmonary infections are a significant cause of morbidity and mortality in patients with hematological malignancies and hematopoietic stem cell transplantation (HCT) recipients. A delay in identifying a causative agent may result in late initiation of appropriate treatment and adverse clinical outcomes. We examine the diagnostic utility of PCR-based assays in evaluating invasive pulmonary infections from bronchoalveolar lavage (BAL). Patients with hematological malignancies and HCT recipients who underwent bronchoscopy with BAL from January 2020 to January 2024 for unexplained pulmona

PURPOSE: Despite the increasing recognition of nocardiosis, there are still few reported cases of infections caused by Nocardia otitidiscaviarum (N. otitidiscaviarum). This particular strain is categorized as an opportunistic pathogen that predominantly affects individuals with compromised immune systems while seldom infecting immunocompetent individuals. The conventional diagnostic techniques for nocardiosis present difficulties due to their restricted effectiveness.
CASE DESCRIPTION: We report on a case study of a 67-year-old male patient who was admitted to the hospital with a respiratory

RATIONALE: Pulmonary nocardiosis (PN) is an opportunistic infectious disease caused by Nocardia of the lungs, which lacks specificity in clinical symptoms and imaging, is rare in individuals with normal immune function, and is highly prone to clinical misdiagnosis and missed diagnosis.
PATIENT CONCERNS: A 55-year-old woman with normal immune function was admitted to the hospital due to a cough and expectoration for more than 20 days.
DIAGNOSES: Chest computed tomography revealed a mass in the left hilum and obstructive atelectasis in the lingular segment of the left upper lobe. Initially sus

The patient, a 56-year-old female with a history of childhood bronchiectasis and controlled hypertension, presented with fever, cough, and hemoptysis. Despite initial treatment for exacerbation of bronchiectasis, her condition worsened, leading to sepsis. Blood tests revealed leukocytosis and elevated CRP. Lung HRCT showed necrotic consolidation and nodules in the left and right lung, suggestive of necrotizing pneumonia. Bronchoscopy revealed Nocardia species, leading to intravenous co-trimoxazole treatment. After a week in ICU, the patient improved and was discharged with oral co-trimoxazole.
