Incident versus prevalent progressive supranuclear palsy: analyses of U.S. Alzheimer's Disease Research Centers data indicate clinical and neuropathological features of a relatively common and underrecognized tauopathy in the aging brain.

Progressive supranuclear palsy (PSP) is an adult-onset neurodegenerative disease with a heterogeneous spectrum of clinical and pathological features. The epidemiology of PSP-as defined by the gold standard of autopsy diagnosis-remains unresolved, but prevalence estimates are typically ~1:10,000. Here, we analyzed the National Alzheimer's Coordinating Center (NACC) Neuropathology Data Set, aggregated from 38 U.S. Alzheimer's Disease Research Centers (ADRCs). We compared clinical and neuropathologic features of autopsied participants, stratifying on cognitive status at recruitment into the cohor

Imbalance in excitation and inhibition in sgACC is associated with depression in dementia with Lewy bodies.

Depression in dementia with Lewy bodies (DLB) is a common neuropsychiatric symptom associated with reduced quality in life. Structural, functional and neurochemical abnormalities in glutamatergic and GABAergic neurotransmission are implicated in the pathophysiology of depression, showing changes in regions involved in emotional processing, including the subgenual cingulate cortex (sgACC), which shows pathological changes in DLB. Using post-mortem tissue from DLB patients and controls, we assessed synaptic and neurochemical changes within the sgACC in relation to depression in DLB. We identifie

Plant-based models of neurodegenerative CAG repeat expansion diseases.

Expansion of cytosine-adenine-guanine (CAG) trinucleotide repeats causes Huntington's disease and other neurodegenerative disorders, yet no disease-modifying treatments exist. Plant-based CAG models expressing mutant huntingtin protein provide novel insights into expanded CAG-induced toxicity, complementing traditional systems for studying neurodegeneration and therapeutic discovery, and may offer unique opportunities for advancing plant biology research.

Normative Modeling of Amygdala Subfields and Disease-Specific Vulnerabilities Across Neurological Disorders.

As an integrative center of the limbic system, the amygdala and its subfields participate in diverse neural functions and demonstrate heterogeneous structural changes in various neurological disorders. In this study, three-dimensional T1-weighted MRI data from 22,258 healthy participants were analyzed using normative modeling to establish normative references for the volumes of the amygdala and its subfields. These models elucidated the developmental and aging curves of the amygdala and identified distinct ages at peak volume across subfields, ranging from 24.6 to 43.7 years. Based on these re

Multiplexed impedimetric zinc oxide nanorods-modified aptasensor on carbon ink-inscribed paper electrodes for the specific and sensitive co-detection of Alzheimer's and Parkinson's disease.

Neurodegenerative diseases, including Alzheimer's disease (AD) and Parkinson's disease (PD), represent a growing health burden, while early diagnosis remains challenging because established diagnostic approaches, including cerebrospinal fluid analysis and neuroimaging, are invasive, expensive, and unsuitable for decentralized point-of-care testing. Recent progress in electrochemical and paper-based biosensors has enabled low-cost, portable, and multiplexed biomarker analysis. However, their clinical translation remains limited by low biomarker concentrations, matrix interference, nonspecific a

Striatal astrocyte μ-crystallin (Crym) loss contributes to early-stage Huntington's disease pathophysiology.

A striatal astrocyte subpopulation expressing μ-crystallin (gene: Crym) regulates perseverative behaviors, but its role in disease is unclear. We found that astrocytic Crym and μ-crystallin protein expression levels are reduced in human Huntington's disease (HD) tissue, cerebrospinal fluid, and HD mouse models. Selective astrocytic μ-crystallin restoration in a HD mouse model improved astrocyte and neuronal morphology, electrophysiology, gene expression, and early-stage compulsive and repetitive behaviors. Late-stage motor deficits remained mostly unchanged. Approximately 32% of the μ-crystall

Synaptic Proteostasis in Health and Disease.

Neurons rely on tightly coordinated mechanisms of protein synthesis and degradation to maintain cellular homeostasis, a process known as proteostasis. Given their highly polarised and compartmentalised nature, regulation of proteostasis is especially important at neuronal synapses, which are spatially distant from the soma yet require rapid on-demand adaptation of the synaptic proteome. Neurons meet their demand for synaptic proteostasis through localised mRNA transport and translation, alongside specialised pathways for protein degradation. Recent advances reveal that thousands of transcripts

Dual Mobility Total Hip Arthroplasty for Recurrent Dislocation After Femoral Neck Fracture in an Elderly Patient: A Case Report.

INTRODUCTION: Recurrent dislocation after bipolar hemiarthroplasty for femoral neck fracture is a challenging complication, particularly in elderly patients with risk factors for instability such as cognitive impairment, soft-tissue insufficiency, and muscle weakness. Dual mobility total hip arthroplasty has been increasingly used to improve hip stability in patients at high risk of dislocation. CASE REPORT: An 83-year-old woman with dementia sustained a Garden type III femoral neck fracture and underwent bipolar hemiarthroplasty through a posterior approach. Role of subcortical iron in Parkinson's disease, prodromal-depression, and dementia: Comparative links to anemias and osteoporosis.

Three reports employing Mendelian randomization and quantitative susceptibility mapping from 41,581-patients in the UK biobank provide evidence to support that excess subcortical iron accumulation causally accelerates Parkinson's disease (PD). While subcortical iron was not a significant driver of Alzheimer's disease (AD), neural shrinkage and ferroptosis in grey matter featured in vascular dementia. Subcortical iron was changed in depression, anemia and osteoporosis patients. Since iron chelators were not beneficial to either AD or PD patients in clinical trials, we propose that translational

Clinical and biomarker predictors of α-synuclein seeding in cognitive impairment and motor parkinsonism.

Dopamine transporter (DAT) imaging and α-synuclein seed amplification assay (SAA) are biomarkers for Lewy body disease (LBD). We aimed to identify clinical and biomarker factors associated with SAA positivity in patients with cognitive impairment and frequent motor parkinsonism. Dual-phase DAT positron emission tomography (PET) scans, cerebrospinal fluid (CSF) phosphorylated-tau/β-amyloid ratio (PTAR) measurement, and SAA were performed in 108 patients, including 99 with motor parkinsonism. Diagnoses, which were not mutually exclusive, included normal pressure hydrocephalus (NPH, N = 60), Alzh

Joint Analysis for Multivariate Longitudinal and Event Time Data With a Change Point Anchored at Interval-Censored Event Time.

Huntington's disease (HD) is an autosomal dominant neurodegenerative disorder characterized by motor dysfunction, psychiatric disturbances, and cognitive decline. The onset of HD is marked by severe motor impairment, which may be predicted by prior cognitive decline and, in turn, exacerbate cognitive deficits. Clinical data, however, are often collected at discrete time points, so the timing of disease onset is subject to interval censoring. To address the challenges posed by such data, we develop a joint model for multivariate longitudinal biomarkers with a change point anchored at an interva

Caregiver Burden and Its Determinants After Subthalamic Nucleus Deep Brain Stimulation for Parkinson's Disease: A Cross-Sectional Long-Term Follow-Up Study.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder, leading to increasing functional dependence even after deep brain stimulation (DBS). Although DBS improves motor outcomes, its long-term impact on caregiver burden remains poorly understood. OBJECTIVES: Our aims were to evaluate caregiver burden after subthalamic nucleus (STN)-DBS and identify patient- and caregiver-related factors associated with burden across different postoperative time points. METHODS: A cross-sectional study of caregivers of PD patients who underwent STN-DBS at a tertiary center, stratifie

Dementia with Lewy bodies and amyloid pathology: Eligible for anti-amyloid treatment or not?

In the future, treatment of dementia will be personalized, targeted to contributing pathologies, rather than the clinical syndrome. Anti-amyloid drugs for Alzheimer's disease are the first approved drugs; however, the impact of co‑pathology on the treatment effects remains incomplete. Currently, only patients with a clinical diagnosis of Alzheimer's disease are eligible for these new drugs, but could they also be beneficial when amyloid is present in other neurodegenerative diseases? For example, roughly 50% of patients with dementia with Lewy bodies are amyloid-positive. In this perspective,

Target engagement and efficacy in disease-modifying trials of synuclein-related disorders.

Disease-modifying trials in Parkinson's disease (PD), multiple system atrophy (MSA), and dementia with Lewy bodies (DLB) increasingly incorporate fluid and imaging biomarkers to demonstrate target engagement, de-risk subsequent phases of drug development and provide mechanistic evidence of biological activity. Whether biomarker modulation predicts clinical benefit remains unclear. We conducted a PRISMA-compliant review of phase II and III disease-modifying trials (2020-2025) that included at least one biomarker intended to assess target engagement. Twenty-six trials met inclusion criteria (PD=

One year after spaced retrieval training: enduring memory enhancement for older adults with and without mild cognitive impairment.

OBJECTIVE: Spaced retrieval training (SRT) can enhance memory in older adults with and without cognitive impairment, but the durability of these memories remains unclear. This study examined whether memory benefits from an app-based SRT program persisted one year after training in older adults with mild cognitive impairment (MCI) due to Alzheimer's disease (AD) and healthy controls. METHOD: We recruited participants from a prior study in which they used a mobile app to engage in SRT for half of two sets of stimuli, verbal facts and face-name associations, across a four-week intervention.
