Dual Mobility Total Hip Arthroplasty for Recurrent Dislocation After Femoral Neck Fracture in an Elderly Patient: A Case Report.

INTRODUCTION: Recurrent dislocation after bipolar hemiarthroplasty for femoral neck fracture is a challenging complication, particularly in elderly patients with risk factors for instability such as cognitive impairment, soft-tissue insufficiency, and muscle weakness. Dual mobility total hip arthroplasty has been increasingly used to improve hip stability in patients at high risk of dislocation. CASE REPORT: An 83-year-old woman with dementia sustained a Garden type III femoral neck fracture and underwent bipolar hemiarthroplasty through a posterior approach. Three months after the initial su

Role of subcortical iron in Parkinson's disease, prodromal-depression, and dementia: Comparative links to anemias and osteoporosis.

Three reports employing Mendelian randomization and quantitative susceptibility mapping from 41,581-patients in the UK biobank provide evidence to support that excess subcortical iron accumulation causally accelerates Parkinson's disease (PD). While subcortical iron was not a significant driver of Alzheimer's disease (AD), neural shrinkage and ferroptosis in grey matter featured in vascular dementia. Subcortical iron was changed in depression, anemia and osteoporosis patients. Since iron chelators were not beneficial to either AD or PD patients in clinical trials, we propose that translational

Clinical and biomarker predictors of α-synuclein seeding in cognitive impairment and motor parkinsonism.

Dopamine transporter (DAT) imaging and α-synuclein seed amplification assay (SAA) are biomarkers for Lewy body disease (LBD). We aimed to identify clinical and biomarker factors associated with SAA positivity in patients with cognitive impairment and frequent motor parkinsonism. Dual-phase DAT positron emission tomography (PET) scans, cerebrospinal fluid (CSF) phosphorylated-tau/β-amyloid ratio (PTAR) measurement, and SAA were performed in 108 patients, including 99 with motor parkinsonism. Diagnoses, which were not mutually exclusive, included normal pressure hydrocephalus (NPH, N = 60), Alzh

Joint Analysis for Multivariate Longitudinal and Event Time Data With a Change Point Anchored at Interval-Censored Event Time.

Huntington's disease (HD) is an autosomal dominant neurodegenerative disorder characterized by motor dysfunction, psychiatric disturbances, and cognitive decline. The onset of HD is marked by severe motor impairment, which may be predicted by prior cognitive decline and, in turn, exacerbate cognitive deficits. Clinical data, however, are often collected at discrete time points, so the timing of disease onset is subject to interval censoring. To address the challenges posed by such data, we develop a joint model for multivariate longitudinal biomarkers with a change point anchored at an interva

Comparative effects of six exercise modalities on global cognitive function in Alzheimer's disease, Parkinson's disease, and mild cognitive impairment: a systematic review and network meta-analysis.

BACKGROUND: Alzheimer's disease (AD), Parkinson's disease (PD), and mild cognitive impairment (MCI) are commonly accompanied by cognitive decline. Although exercise is recommended as a non-pharmacological strategy, the comparative effects of different exercise modalities on global cognition remain uncertain. OBJECTIVE: To compare six exercise modalities-aerobic exercise (AE), mind-body exercise (MBE), resistance training (RT), multi-component exercise (MCE), flexibility training (FT), and functional training (FTr)-for global cognitive function in people with AD, PD, or MCI, and to explore dos

Caregiver Burden and Its Determinants After Subthalamic Nucleus Deep Brain Stimulation for Parkinson's Disease: A Cross-Sectional Long-Term Follow-Up Study.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder, leading to increasing functional dependence even after deep brain stimulation (DBS). Although DBS improves motor outcomes, its long-term impact on caregiver burden remains poorly understood. OBJECTIVES: Our aims were to evaluate caregiver burden after subthalamic nucleus (STN)-DBS and identify patient- and caregiver-related factors associated with burden across different postoperative time points. METHODS: A cross-sectional study of caregivers of PD patients who underwent STN-DBS at a tertiary center, stratifie

Dementia with Lewy bodies and amyloid pathology: Eligible for anti-amyloid treatment or not?

In the future, treatment of dementia will be personalized, targeted to contributing pathologies, rather than the clinical syndrome. Anti-amyloid drugs for Alzheimer's disease are the first approved drugs; however, the impact of co‑pathology on the treatment effects remains incomplete. Currently, only patients with a clinical diagnosis of Alzheimer's disease are eligible for these new drugs, but could they also be beneficial when amyloid is present in other neurodegenerative diseases? For example, roughly 50% of patients with dementia with Lewy bodies are amyloid-positive. In this perspective,

Target engagement and efficacy in disease-modifying trials of synuclein-related disorders.

Disease-modifying trials in Parkinson's disease (PD), multiple system atrophy (MSA), and dementia with Lewy bodies (DLB) increasingly incorporate fluid and imaging biomarkers to demonstrate target engagement, de-risk subsequent phases of drug development and provide mechanistic evidence of biological activity. Whether biomarker modulation predicts clinical benefit remains unclear. We conducted a PRISMA-compliant review of phase II and III disease-modifying trials (2020-2025) that included at least one biomarker intended to assess target engagement. Twenty-six trials met inclusion criteria (PD=

One year after spaced retrieval training: enduring memory enhancement for older adults with and without mild cognitive impairment.

OBJECTIVE: Spaced retrieval training (SRT) can enhance memory in older adults with and without cognitive impairment, but the durability of these memories remains unclear. This study examined whether memory benefits from an app-based SRT program persisted one year after training in older adults with mild cognitive impairment (MCI) due to Alzheimer's disease (AD) and healthy controls. METHOD: We recruited participants from a prior study in which they used a mobile app to engage in SRT for half of two sets of stimuli, verbal facts and face-name associations, across a four-week intervention. Update on somatic mutations in major neurodegenerative diseases: mutational signatures in neurons and mosaicism.

Somatic mutations in major neurodegenerative diseases are poorly understood, but research over the past 2 years provides valuable insights. Single-cell whole-genome sequencing of vulnerable neurons in Alzheimer's disease and amyotrophic lateral sclerosis/frontotemporal dementia (ALS/FTD) revealed excess accumulation of single-nucleotide variant signatures and two-bp deletions independent of pathogenic protein aggregation. Increased oxidative DNA damage and possibly impaired DNA damage repair pathways may explain more pronounced signatures in patients. Ultra-deep sequencing of disease-associate

Longitudinal Cognitive Function in Progressive Supranuclear Palsy-A Review of the Evidence for Progression.

Progressive supranuclear palsy is a rare and rapidly progressive neurodegenerative syndrome. Clinical manifestations are heterogeneous and include Parkinsonism, postural instability, oculomotor dysfunction and cognitive impairment, with 80% of patients progressing to dementia within 3 years. Cognitive impairment is consistently associated with poorer outcomes independent of motor function, including increased mortality, morbidity, caregiver burden and healthcare utilization. The prototypic cognitive phenotype of PSP demonstrates marked deficits in executive functions and slowed processing spee

Global and regional amyloid-β in explaining APOE-ε4 and sex associations with medial temporal tau: a neuropathology study.

The medial temporal lobe (MTL) is a key region in Alzheimer's disease (AD) pathogenesis, affected early by tau pathology. While female sex and APOE-ε4 are two known risk factors that affect tau levels, whether these factors drive tau in the MTL via amyloid-dependent or amyloid-independent mechanisms remains unclear. We investigated these relationships in two large neuropathological cohorts comprising 2550 participants from the Religious Orders Study/Memory and Aging Project (ROS/MAP; n=1587, main cohort) and Arizona Study of Aging and Neurodegenerative Disorders (AZSAND; n=963, replication coh

Considerations for therapeutic targets for clinical trials in biomarker-characterized prodromal Parkinson's disease populations.

Parkinson's disease (PD) is a growing public health problem and biologically targeted therapeutics (BTTs) that slow disease progression are urgently needed. However, the development of effective BTTs is challenging as PD is a clinically defined syndrome with pathobiological heterogeneity and the pathobiological process begins many years before the onset of clinically detectable motor symptoms. Testing BTTs in a biomarker-defined population at an earlier stage in the disease course, before a clinical PD diagnosis, may be the key to achieving this objective. The recent emergence of biological de

RVG29-functionalized nanocarriers for brain-targeted therapy of neurodegenerative diseases: mechanisms, therapeutic advances, and translational perspectives.

Neurodegenerative diseases (NDs) affect more than 55 million people worldwide, with this number projected to exceed 139 million by 2050 due to population aging. Alzheimer's disease (AD) accounts for 60-70% of dementia cases, while Parkinson's disease (PD) affects over 10 million individuals globally, highlighting the growing clinical and socioeconomic burden of these progressive neurological disorders. Effective treatment of AD, PD, amyotrophic lateral sclerosis (ALS), and related central nervous system disorders remains hindered by the blood-brain barrier (BBB), poor brain accumulation, rapid
