Neuropathology-specific language features in primary progressive aphasia.

INTRODUCTION: Primary Progressive Aphasia (PPA) clinical syndromes do not align consistently with underlying pathology. This study aimed to identify language markers for specific neuropathologies using both standard clinical tests and narrative speech analysis. METHODS: We analyzed data from 82 autopsy-confirmed PPA cases, including Alzheimer's disease (AD), transactive DNA-binding protein 43 (TDP-43) type C (TDP-C), Pick's disease, and 4R-tauopathies (progressive supranuclear palsy/ cortico-basal degeneration (PSP/CBD). Linear mixed-effects regression was used to analyze performance on stand

The Pick fold in tau filaments from human MAPT mutants.

Mutations in MAPT, the tau gene, give rise to forms of frontotemporal dementia and parkinsonism linked to chromosome 17 (FTDP-17 T), with abundant filamentous tau inclusions in brain cells. Some mutations that encode missense and deletion variants can give rise to a clinical picture of Pick's disease and filaments made of three-repeat tau in nerve cells. Here we report the electron cryo-microscopy (cryo-EM) structures of tau filaments from the brains of individuals with MAPT mutations D252V, G272V, S320F and ΔG389-I392. The two-layered Pick fold was present in the brains of individuals with mu

Prevalent versus incident progressive supranuclear palsy: An analysis of the frequencies of neuropathological and clinical features at U.S. Alzheimer's Disease Research Centers indicate a relatively common tauopathy of aging.

Progressive supranuclear palsy (PSP) is a neurodegenerative disease diagnosed according to its histopathologic pattern of tau proteinopathy ("tauopathy"). It is increasingly appreciated that PSP is heterogeneous in both clinical and pathological presentations. However, the prevalence of PSP subtypes, in comparison to other tauopathies, remain incompletely characterized. Here we analyzed NACC Neuropathology Data Set data aggregated from 37 U.S. Alzheimer's Disease Research Centers (ADRCs). Clinical and gold-standard neuropathologic features of autopsied participants were compared, stratifying o

Symptomatic conversion in a case of sporadic frontotemporal dementia: Longitudinal neuroimaging of the presymptomatic stage.

BACKGROUND: Identifying preclinical stages of behavioral variant frontotemporal dementia (bvFTD) is exclusively possible in familial bvFTD. In non-genetic (sporadic) cases this is prevented by the lack of biomarkers. In particular, serial neuroimaging might preview early abnormalities in asymptomatic genetic bvFTD, whereas such data are lacking in sporadic bvFTD. CASE: Here, we present a pathology-confirmed case of sporadic bvFTD in a 61-year-old male with the unique availability of multiple presymptomatic magnetic resonance imaging (MRI) scans following a resection of meningioma 20 years pri

Evaluating MAPT p.A152T as a risk factor for the 3R tauopathy Pick's disease.

Genetic studies have significantly advanced our understanding of tauopathies, yet the genetic aetiology of Pick's disease, a rare 3-Repeat tauopathy, remains unclear. The MAPT p.A152T variant has been identified as a risk factor for Alzheimer's disease and progressive supranuclear palsy, but its role in Pick's disease is unknown. In this study, we examined the prevalence of MAPT p.A152T in the largest series of neuropathologically confirmed Pick's disease cases to date (n = 401). Through genotyping, we identified a single mutation carrier in the Pick's disease cohort (minor allele frequency =

Clinical tools to differentiate behavioral variant frontotemporal dementia from phenocopies.

INTRODUCTION: Behavioral variant frontotemporal dementia (bvFTD) is often misdiagnosed, particularly in psychiatric phenocopies of the disease (phFTD). We compared the top-performing bvFTD-specific behavioral tools, identified in a prior systematic review, for differentiating bvFTD from phFTD. METHODS: In this prospective diagnostic accuracy study, consecutively recruited participants with suspected bvFTD by a specialist physician completed nine bvFTD-specific tools at baseline with a blinded assessor. Final diagnoses were assigned by multidisciplinary consensus after follow-up. Diagnostic pe

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Artificial intelligence for early detection of diabetes mellitus complications via retinal imaging.

OBJECTIVE: Recent studies have demonstrated that C-peptide exerts beneficial effects on the diabetic state, including improvements in kidney and nerve function. Thus, we investigated the effect of residual pancreatic C-peptide secretion on the cardiac autonomic nervous system in well- and poorly controlled type II diabetic patients. DESIGN: Randomised cross-sectional study. PATIENTS: Forty type II diabetic patients free from diabetic neuropathy, with similar anthropometric parameters, volunteered for our study. MEASUREMENTS: Insulin action, residual pancreatic C-peptide secretion and the ca

Dysregulation of muscle cholesterol transport in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder affecting motor neurons, with a typical lifespan of 3-5 years. Altered metabolism is a key feature of ALS that strongly influences prognosis, with an increase in whole body energy expenditure and changes in skeletal muscle metabolism, including greater reliance on fat oxidation. Dyslipidaemia has been described in ALS as part of the metabolic dysregulation, but its role in the pathophysiology of the disease remains controversial. Among the lipids, cholesterol is of particular interest as a vital component of cell m

Unraveling the clinical-pathological correlations of subjects with isolated and mixed neurodegenerative processes in the National Alzheimer's Coordinating Center dataset.

Although Alzheimer disease neuropathologic change (ADNC) is the most common pathology underlying clinical dementia, the presence of multiple comorbid neuropathologies is increasingly being recognized as a major contributor to the worldwide dementia burden. We analyzed 1051 subjects with specific combinations of isolated and mixed pathologies and conducted multivariate logistic regression analysis on a cohort of 4624 cases with mixed pathologies to systematically explore the independent cognitive contributions of each pathology. Alzheimer disease neuropathologic change and limbic-predominant ag

Current Progress and Future Directions in Non-Alzheimer's Disease Tau PET Tracers.

Alzheimer's disease (AD) and non-AD tauopathies are dominant public health issues driven by several factors, especially in the aging population. The discovery of first-generation radiotracers, including [18F]FDDNP, [11C]PBB3, [18F]flortaucipir, and the [18F]THK series, for the in vivo detection of tauopathies has marked a significant breakthrough in the fields of neuroscience and radiopharmaceuticals, creating a robust new category of labeled compounds: tau positron emission tomography (PET) tracers. Subsequently, other tau PET tracers with improved binding properties have been developed using

Respiratory failure as main presentation sign of MAPT-related disorder.

INTRODUCTION: The MAPT gene encodes Tau, a protein mainly expressed by neurons. Tau protein plays an important role in cerebral microtubule polymerization and stabilization, in axonal transport and synaptic plasticity. Heterozygous pathogenic variation in MAPT are involved in a spectrum of autosomal dominant neurodegenerative diseases known as taupathies, including Alzheimer's disease, Pick's disease, fronto-temporal dementia, cortico-basal degeneration and progressive supranuclear palsy. Taupathies are characterized by the constant presence of neuronal and/or glial aberrant Tau inclusions lea

Sensitive detection and propagation of brain-derived tau assemblies in HEK293-based wild-type tau seeding assays.

The assembly of tau into filaments defines tauopathies, a group of neurodegenerative diseases including Alzheimer's disease (AD), Pick's disease (PiD), corticobasal degeneration (CBD), and progressive supranuclear palsy (PSP). The seeded aggregation of tau has been modeled in cell culture using pro-aggregate modifications such as truncation of N- and C-termini and point mutations within the microtubule-binding repeat domain. This limits the applicability of research findings to sporadic disease, where aggregates contain wild-type, full-length tau. We describe a sensitive and specific biosensor

The underappreciated contribution of Oskar Fischer to the knowledge of Frontotemporal Lobar Degeneration.

Frontotemporal Lobar Degeneration (FTLD) encompasses progressive neurodegeneration primarily affecting the frontal and/or temporal lobes, presenting in varied pathological entities. These pathologies lead to diverse clinical syndromes collectively known as Frontotemporal Dementia (FTD), which includes a behavioral variant, and primary progressive aphasia, with its non-fluent and semantic variants, and related conditions with parkinsonism or motoneuron disease features. The historical development of FTD concept is rooted in the pioneering work of Arnold Pick, who first described clinical cases

Genetic therapies for movement disorders - current status.

Movement disorders are a group of heterogeneous neurological conditions associated with alterations of tone, posture and voluntary movement. They may either occur in isolation or as part of a multisystemic condition. More recently, the advent of next generation sequencing technologies has facilitated better understanding of the underlying causative genes and molecular pathways, thereby identifying targets for genetic therapy. In this review, we summarize the advances in genetic therapy approaches for both hyperkinetic and hypokinetic movement disorders, including Parkinson's Disease, Huntingto

Probe-dependent Proximity Profiling (ProPPr) Uncovers Similarities and Differences in Phospho-Tau-Associated Proteomes Between Tauopathies.

BACKGROUND: Tauopathies represent a diverse group of neurodegenerative disorders characterized by the abnormal aggregation of the microtubule-associated protein tau. Despite extensive research, the mechanisms underlying the diversity of neuronal and glial tau pathology in different tauopathies are poorly understood. While there is a growing understanding of tauopathy-specific differences in tau isoforms and fibrillar structures, the specific composition of heterogenous tau lesions remains unknown. Here we study the protein composition of tau aggregates in four major tauopathies: Alzheimer's di

New insight into the role of altered brain cholesterol metabolism in the pathogenesis of AD: A unifying cholesterol hypothesis and new therapeutic approach for AD.

The dysregulation of cholesterol metabolism homeostasis has been universally suggested in the aeotiology of Alzheimer's disease (AD). Initially, studies indicate that alteration of serum cholesterol level might contribute to AD. However, because blood-brain barrier impedes entry of plasma cholesterol, brain cells are not directly influenced by plasma cholesterol. Furthermore, mounting evidences suggest a link between alteration of brain cholesterol metabolism and AD. Interestingly, Amyloid-β proteins (Aβ) can markedly inhibit cellular cholesterol biosynthesis and lower cellular cholesterol con

Adult-onset Niemann-Pick disease type C presenting as progressive chorea mimicking Huntington's disease.

The etiologies of progressive generalized chorea in adults are typically acquired or genetic causes, such as Huntington's disease (HD). We present the case of a 43-year-old woman who experienced progressive generalized chorea and cognitive decline over the course of 8 years. Abnormal eye movements, such as the "round-the-houses" and "zig-zag" signs, were observed. Brain MRI showed caudate atrophy, with a phenotype resembling HD. A rare genetic etiology of Niemann-Pick disease type C (NPC) was ultimately identified as the final diagnosis for this index patient. In this movement disorder round,

ApoE4 requires lipidation enhancement to resolve cellular lipid and protein abnormalities following NPC1 inhibition.

Apolipoprotein E (ApoE) variants are central to Alzheimer's disease (AD), Lewy body dementia (LBD) and Niemann-Pick disease type C (NPC). The ApoE4 variant elevates AD risk by 3-15-fold. ApoE's normal function in lipid transport is known. The question remains how different ApoE isoforms cause cellular pathogenesis. We determined the effects of ApoE isoforms on lipid accumulation induced by inhibiting the endo-lysosomal cholesterol transporter NPC1. In human fibroblasts and astrocytes, NPC1 inhibition caused a 4-fold cholesterol accumulation and mis-localization with altered cholesterol sensing
