OBJECTIVE: Quaking-induced conversion (QuIC) tests, which detect prion-seeding activity in cerebrospinal fluid (CSF), have markedly advanced the antemortem diagnosis of prion diseases such as Creutzfeldt-Jakob disease (CJD). These tests provide high diagnostic accuracy and enable timely differentiation from other rapidly progressive neurodegenerative disorders. However, a key limitation of current QuIC tests are the reduced sensitivity in detecting inherited prion diseases and rare sporadic subtypes, including variably protease-sensitive prionopathy (VPSPr). To address this gap, we evaluated a

We present a case of sporadic Creutzfeldt-Jakob disease (sCJD) in a 73-year-old patient, in which initial suspicion arose from unusual magnetic resonance imaging (MRI) findings and was later confirmed by cerebrospinal fluid analysis. This case report highlights the role of MRI in the early diagnosis of sCJD, facilitating timely palliative care, and highlights the key clinical and imaging characteristics of this rare and fatal prion disease.

BACKGROUND: Creutzfeldt-Jakob disease (CJD) is a fatal neurodegenerative disorder caused by prion proteins, with microglial activation being a key immunopathological feature. This case report demonstrates the application of 18F-DPA-714 positron emission tomography (PET)/MRI, a second-generation translocator protein ligand, to visualise microglial activation in vivo in a patient with sporadic CJD (sCJD).
CASE PRESENTATION: A 57-year-old woman presented with progressive rapid cognitive decline and behavioural change. MRI revealed restricted diffusion within the patchy cortical ribbon of bilater

Creutzfeldt–Jakob disease (CJD) is a rare and fatal neurodegenerative disorder caused by misfolded prion proteins. It progresses inexorably to death once clinical symptoms emerge. Early manifestations are often nonspecific and may mimic other neurological conditions, making diagnosis particularly challenging. We report the case of a 54-year-old Palestinian male who initially presented with dysarthria, vertical gaze nystagmus, dizziness, and gait ataxia while maintaining preserved cognition. Over the following month, he developed short-term memory loss, hallucinations, and disorientation. Exten

Genetic prion diseases are caused by mutant prion protein (PrP) misfolding, eventually leading to the formation of PrPSc, the infectious prion isoform that propagates by inducing misfolding of native PrP. Different mutations are thought to generate distinct prion strains with unique self-replicating and neurotoxic properties, contributing to the phenotypic diversity of genetic prion diseases. We previously showed that transgenic mice expressing the mouse PrP homologs of the D178N-M129 and D178N-V129 mutations linked to fatal familial insomnia (FFI) and genetic Creutzfeldt-Jakob disease (CJD178

Recent evidence suggests glial dysfunction, particularly involving oligodendrocytes and myelin, as an important part of the primary disease mechanism of sporadic Creutzfeldt–Jakob disease. To the best of our knowledge, cerebrospinal fluid (CSF) biomarkers reflecting oligodendrocyte or myelin damage have not yet been systematically investigated in this context. CSF samples from patients with sporadic Creutzfeldt–Jakob disease (CJD), Alzheimer's disease (AD) and non-neurodegenerative controls (n = 18, respectively) were included in the study, and levels of myelin basic protein (MBP), neural/glia

Prion diseases are a family of transmissible, neurodegenerative conditions caused by misfolded proteins called prions. Human cerebral organoids can be infected with prions from sporadic Creutzfeldt-Jakob Disease (sCJD) brain tissue. Initial experiments indicated that the cerebral organoids may be able to differentiate biological properties of different sCJD subtypes. If so, it would be possible to investigate the pathogenic similarities and differences. Herein, we investigated multiple infections of cerebral organoids with 2 sCJD subtypes, comparing hallmark features of disease as well as neur

INTRODUCTION: Prion diseases (PrD) are a group of rapidly progressive dementias. Among its subtypes, Creutzfeldt-Jakob Disease (CJD) is the most common, affecting around 1-2 individuals per million inhabitants yearly. In 2005, Brazil's Ministry of Health (MH) initiated a surveillance program for CJD, creating a protocol to report the cases. Despite advances, the MH still struggles to make a reliable database to determine PrD profile in Brazil. Therefore, the aim of the present study was to understand the Brazilian PrD surveillance system.
METHODS: This is a retrospective and descriptive study

Growth-associated protein-43 (GAP43) is a neuronal protein essential for synaptic function and plasticity, and its reduction has been observed in brains of prion diseases (PrDs) and rodent models. However, its status in the cerebrospinal fluid (CSF) of patients with PrDs remains unclear. CSF samples from 140 PrD cases, including 48 sCJD, 35 T188K-gCJD, 22 E200K-gCJD, 35 D178N-FFI, and 36 non-PrD controls, were analyzed for GAP43 by Western blot. The results were compared with 14-3-3 and calmodulin (CaM) detected by WB, and associated with clinical features. GAP43 positivity was significantly h

Background: Prion diseases represent a group of rare, progressive, and invariably fatal neurodegenerative disorders. Their hallmark is the infectious nature of the misfolded prion protein (PrP^Sc), which propagates by inducing conformational changes in the physiological form (PrP^C). Despite advances in basic science, these disorders still pose major clinical and therapeutic challenges. Methods: A narrative review of the scientific literature was conducted across major biomedical databases, including PubMed, Scopus, Web of Science, and Google Scholar, covering publications up to January 2025.

Clinicopathologically, dura mater graft-associated Creutzfeldt-Jakob disease (dCJD) presents as either a non-plaque or plaque type. Here, we report an autopsy case of plaque-type dCJD, supported by genetic and biochemical analyses. A 41-year-old man manifested right-hand paraesthesia. He had received a dural graft in the right parietal region following a traumatic acute subdural haematoma 27 years before symptom onset. The clinical course was slowly progressive. The patient became unable to walk independently 17 months after onset, showed cognitive decline at 22 months, and developed myoclonus

IntroductionSporadic Creutzfeldt-Jakob Disease (sCJD) is a rapidly progressive prion disease that can be challenging to distinguish from other dementias, especially in early stages or when classical EEG findings, such as periodic sharp wave complexes (PSWCs), are absent. This study aimed to assess the utility of quantitative EEG (QEEG) features-derived from Food and Drug Administration-approved software-in differentiating sCJD from non-sCJD dementias.MethodsEEG data from 78 participants were retrospectively analyzed: 19 sCJD patients, 49 non-sCJD dementia patients, and 10 healthy controls. sCJ

BACKGROUND: Knowledge on the prevalence and clinical effects of Lewy body (LB) pathology in patients with rapidly progressive dementia (RPD) has been limited by the lack of a pathology-specific marker. The introduction of the alpha-synuclein (α-syn) seed amplification assay (SAA) has recently provided the first accurate in vivo biomarker of LB pathology. Based on data from limited neuropathological cohorts, we hypothesised that LB pathology is frequent in non-prion RPD and often clinically unrecognised. METHODS: We studied 171 consecutively referred clinically well-characterised patients with

Prions are unprecedented infectious pathogens that cause a group of rare and inevitably fatal neurodegenerative diseases, affecting approximately 1 person per 1 million inhabitants worldwide each year. These diseases include Creutzfeldt-Jakob disease (CJD), Gerstmann-Sträussler-Scheinker syndrome (GSS), kuru, fatal insomnia (FI) and variably protease-sensitive prionopathy (VPSPr), all of which involve a conformational change of normal cellular prion protein (PrPC) into abnormal scrapie prion protein (PrPSc) through a posttranslational process. This structural change is associated with profound

Differentiating Creutzfeldt-Jakob disease (CJD) with the V180I mutation from other types of dementia is extremely difficult. Additionally, its differentiation is sometimes determined after performing neurosurgery, which is associated with a high risk of V180I prion contamination; however, the infectivity of the V180I prion has not been properly investigated. Especially in East Asia, this issue must be addressed to respond effectively to accidental contamination that leads to iatrogenic CJD. The results of our transmission experiments involving various humanized knock-in mice clearly indicate t

BACKGROUND: Variant Creutzfeldt-Jakob disease (vCJD) is a fatal transmissible prion disorder attributed primarily to the ingestion of meat from cattle infected with bovine spongiform encephalopathy (BSE), and it can also be transmitted through blood transfusions and occupational exposure in laboratories. As of July 2024, 233 deaths had been recorded in 12 countries, mostly in the United Kingdom (UK) and France.
OBJECTIVE: This study aimed to describe the results of vCJD surveillance in Spain from 1993 to 2021 and the responses to those results.
METHODS: Surveillance spanned the following thr

Creutzfeldt-Jakob disease (CJD) is a rare, fatal prion disease of the central nervous system that develops due to the conversion of the normal cellular protein PrPc to the abnormal PrPSc molecule. The first clinical cases were described in the 1920s. The aim of this paper is to present the clinical progress of the disease and the diagnostic process, including some of the most common diagnostic traps. The paper highlights a range of symptoms that should serve as a potential warning signal for clinicians-not just neurologists-indicating the need to evaluate the patient more thoroughly.

Sporadic Creutzfeldt-Jakob disease (sCJD) is a rare, rapidly progressive, and fatal neurodegenerative disorder caused by the accumulation of abnormal prion proteins in the central nervous system. The Heidenhain variant (HvCJD), a rare subtype of sCJD, is characterized by prominent visual symptoms at onset. Diagnosing sCJD remains challenging due to non-specific clinical presentations and the limited sensitivity and specificity of conventional tests such as magnetic resonance imaging (MRI), electroencephalography (EEG), and cerebrospinal fluid (CSF) 14-3-3 protein analysis. Recently, real-time

Myelin oligodendrocyte basic protein (MOBP) is an abundant oligodendrocyte gene implicated in multiple neurodegenerative diseases. Genetic variation at the MOBP locus has been associated with risk for progressive supranuclear palsy (PSP), amyotrophic lateral sclerosis (ALS), frontotemporal lobar degeneration (FTD), corticobasal degeneration (CBD), Alzheimer's disease (AD), Lewy body dementia (LBD), and Creutzfeldt-Jakob disease (CJD). Epigenetically, MOBP promoter hypermethylation and reduced expression have been reported in multiple system atrophy (MSA). Although MOBP is thought to play a rol

Prion diseases are a group of fatal neurodegenerative disorders caused by misfolded proteins. Understanding the regulatory networks of ferroptosis in prion diseases could unveil new diagnostic and therapeutic strategies. To explore this, we systematically evaluated ferroptosis-associated alterations across human sporadic Creutzfeldt-Jakob disease (sCJD) brain samples, the ME7-infected mouse model, and in vitro using PrP106-126-treated SH-SY5Y cells. In sCJD patients, we observed a significant decrease in GPX4 expression, accompanied by elevated lipid peroxidation, as confirmed by malondialdehy
