Cas9 nickase-mediated contraction of CAG/CTG repeats is associated with phenotypic improvements in a Huntington's disease mouse model.

Expanded CAG/CTG repeats cause more than 15 different diseases that all remain without a disease-modifying treatment. Because repeat length accounts for most of the variation in disease severity, contracting CAG/CTG repeats presents an attractive therapeutic avenue. Here, we show that the CRISPR-Cas9 nickase targeted to CAG/CTG repeats leads to efficient repeat contractions in Huntington's disease (HD) patient-derived neurons and astrocytes and in myotonic dystrophy type 1 patient-derived neurons. The approach was allele selective and free of detectable off-target mutations. Injection of the C

Neuroprotective Role of Withaferin A in Huntington's Disease: Current Evidence and Future Perspectives.

Huntington's disease (HD) is an inherited neurodegenerative disorder caused by an expanded CAG repeat in the huntingtin (HTT) gene. Its pathogenesis involves mutant huntingtin aggregation, impaired proteostasis and autophagy, transcriptional dysregulation, mitochondrial dysfunction, oxidative stress, neuroinflammation and progressive neuronal loss. Withaferin A (WA), a steroidal lactone derived from Withania somnifera, has demonstrated antioxidant, anti-inflammatory, proteostasis-modulating and stress-response effects in experimental studies. In a transgenic HD mouse model, WA-induced heat-sho

Mechanistic Evaluation of Psilocybin in Neurodegenerative Disorders: Relevance of 5-HT2A Receptors and Downstream Pathways.

More than fifty million individuals worldwide are experiencing neurodegenerative disorders, which include Parkinson's disease, Alzheimer's disease, amyotrophic lateral sclerosis, Huntington's disease, multiple sclerosis, and frontotemporal dementia. These disorders have a significant role in the gradual degradation of cognition and motor skills. Most current treatment approaches have little effect on changing the course of the disease and are largely symptomatic. Psilocybin, a naturally occurring tryptamine alkaloid from psilocybin mushrooms, has become a promising medicinal agent because of i

Huntington's disease vasculopathy is barrier-type-selective and arteriolar-dominant with spatially-coupled astrogliosis and reversible by S1pr1 stimulation.

Brain vascular barriers are dysfunctional in many neurological disorders, including Huntington's disease (HD), but which vessels are affected and how remains unclear. Using in vivo two-photon microscopy in R6/2 HD-model mice, we reveal a striking divergence in barrier dysfunction. Surface vessels of the blood-cerebrospinal fluid barrier (BCSFB) drive pathology in HD by paracellular leakage, while parenchymal vessels forming the blood-brain barrier (BBB) exhibit increased vesicular transport and adsorptive-mediated transcytosis (AMT), with arterioles representing the most critically impacted va

Lipid dysregulation in polyglutamine diseases.

Polyglutamine (polyQ) diseases are fatal neurodegenerative disorders caused by pathological CAG repeat expansions in disease-associated genes. They are characterized by progressive motor dysfunction and typically late onset. Although multiple mechanisms have been proposed, accumulating evidence indicates that lipid dysregulation contributes to neuronal dysfunction and degeneration. Lipids are essential for brain development and homeostasis, and disrupted lipid metabolism is implicated across neurodegenerative conditions. Within the polyQ spectrum, Huntington's disease is the best studied and s

Interrogating DNA methylation associated with Lewy body pathology in a cross brain-region and multi-cohort study.

Here, we conduct a large meta-analysis of DNA methylation across multiple cortical brain regions, in relation to increasing burden of Lewy body pathology. Utilizing a dataset of 1205 samples (832 unique donors), we identify 24 false discovery rate significant differentially methylated loci associated with Lewy body pathology, with loci located in UBASH3B and PTAFR, and an intergenic locus passing the genome-wide significance threshold. Ontological enrichment analysis highlights several neurologically relevant traits, including synaptic alterations. We compare methylation signatures between neu

Human iPSC-Derived Brain Cells, Organoids, and Organoids-on-Chips: Advancing Neurological Disease Modeling to Improve Clinical Outcome.

Neurological disorders are the leading cause of overall disease and disability worldwide, affecting more than 3.4 billion people globally and over 180 million Americans. Yet there are no curative treatments available for any of the cardinal neurological diseases, such as Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis-frontotemporal dementia (ALS-FTD) and Huntington's disease (HD). Many compounds that demonstrate excellent activity in preclinical models fail in clinical trials. The most compelling reason for this failure is attributed to inherent limitations o

The small giant actor in the pathogenesis of neurodegenerative diseases: Microtubules.

Neurodegenerative diseases are a clinically and pathologically diverse group of disorders characterized by progressive neuronal dysfunction, synaptic impairment, and neuronal loss. Although research has traditionally focused on disease-associated protein misfolding and aggregation, increasing evidence implicates abnormalities of cytoskeletal organization and intracellular transport in several neurodegenerative conditions. Microtubules contribute to neuronal polarity, axonal and dendritic transport, organelle distribution, and synaptic maintenance. These functions are regulated in part by tubul

HMGB1 functional plasticity in neurodegenerative diseases and its translational implications.

High mobility group box 1 (HMGB1) is a chromatin-associated protein that can be redistributed and released during cellular stress, when it acts as an extracellular alarmin within the broader family of damage-associated molecular patterns (DAMPs). Its functions depend on subcellular localization, redox and post-translational modification state, cellular source, mode of release, and molecular partners. This critical narrative review examines how these variables shape HMGB1 biology in Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS), Huntington's disease (HD

The experiences of being a young carer to a parent with Huntington's disease: a thematic synthesis.

PURPOSE: This thematic synthesis explored the experiences of being a young carer to a parent with Huntington's disease. METHODS: A systematic search strategy was utilised across eight databases, resulting in 13 papers that met the inclusion criteria. Thomas and Harden's thematic synthesis approach was used to guide the methodological decisions and analyse the data. RESULTS: The analysis resulted in three themes (1) Lack of awareness, undervalued and unsupported: barriers to young carers accessing support (2) Adult responsibilities: impact on developmental stages and worries about the future,

Remote smartphone-based spoken language screening predicts clinical markers in Huntington's disease.

Assessment and monitoring of Huntington's disease (HD) symptoms remain limited to infrequent, clinic-based evaluations. We evaluated whether fully automated linguistic analysis of speech tasks recorded via smartphone can remotely capture core clinical markers of HD severity. In this cross-sectional multicenter study across Czech and German sites, 53 participants, including 30 HD (9 pre-symptomatic, 3 prodromal, and 18 manifest) and 23 healthy controls, completed a smartphone-based speech assessment, including spontaneous monologue and fairy tale retelling for 7 consecutive days. Mesenchymal stromal cells in age-related neurodegenerative disorders: mechanisms, translational barriers, and precision strategies.

Ageing remodels the neuroimmune, metabolic, and vascular environment in ways that accelerate neuronal vulnerability and constrain tissue repair. Mesenchymal stromal cells (MSCs) and MSC-derived extracellular vesicles (MSC-EVs) are therefore attractive candidates for neurodegenerative disorders because their effects extend beyond cell replacement to coordinated paracrine, immunomodulatory, trophic, and vascular regulation. This critical review evaluates the biological rationale, disease-specific evidence, translational limitations, and precision-development strategies for MSC-based intervention

Plant-based models of neurodegenerative CAG repeat expansion diseases.

Expansion of cytosine-adenine-guanine (CAG) trinucleotide repeats causes Huntington's disease and other neurodegenerative disorders, yet no disease-modifying treatments exist. Plant-based CAG models expressing mutant huntingtin protein provide novel insights into expanded CAG-induced toxicity, complementing traditional systems for studying neurodegeneration and therapeutic discovery, and may offer unique opportunities for advancing plant biology research.

Striatal astrocyte μ-crystallin (Crym) loss contributes to early-stage Huntington's disease pathophysiology.

A striatal astrocyte subpopulation expressing μ-crystallin (gene: Crym) regulates perseverative behaviors, but its role in disease is unclear. We found that astrocytic Crym and μ-crystallin protein expression levels are reduced in human Huntington's disease (HD) tissue, cerebrospinal fluid, and HD mouse models. Selective astrocytic μ-crystallin restoration in a HD mouse model improved astrocyte and neuronal morphology, electrophysiology, gene expression, and early-stage compulsive and repetitive behaviors. Late-stage motor deficits remained mostly unchanged. Approximately 32% of the μ-crystall

Joint Analysis for Multivariate Longitudinal and Event Time Data With a Change Point Anchored at Interval-Censored Event Time.

Huntington's disease (HD) is an autosomal dominant neurodegenerative disorder characterized by motor dysfunction, psychiatric disturbances, and cognitive decline. The onset of HD is marked by severe motor impairment, which may be predicted by prior cognitive decline and, in turn, exacerbate cognitive deficits. Clinical data, however, are often collected at discrete time points, so the timing of disease onset is subject to interval censoring. To address the challenges posed by such data, we develop a joint model for multivariate longitudinal biomarkers with a change point anchored at an interva

One year after spaced retrieval training: enduring memory enhancement for older adults with and without mild cognitive impairment.

OBJECTIVE: Spaced retrieval training (SRT) can enhance memory in older adults with and without cognitive impairment, but the durability of these memories remains unclear. This study examined whether memory benefits from an app-based SRT program persisted one year after training in older adults with mild cognitive impairment (MCI) due to Alzheimer's disease (AD) and healthy controls. METHOD: We recruited participants from a prior study in which they used a mobile app to engage in SRT for half of two sets of stimuli, verbal facts and face-name associations, across a four-week intervention. Trends and Disparities in Dementia- and Sepsis-Related Mortality in Older US Adults (1999-2020).

INTRODUCTION: This observational study evaluated demographic and regional disparities in mortality due to dementia with comorbid sepsis among older adults (≥65 years) in the USA from 1999 to 2020. METHODS: We analyzed death certificate data from the CDC WONDER Multiple Cause of Death database (1999-2020). Age-adjusted mortality rates (AAMRs) per 100,000 population were calculated and standardized to the 2000 US population. Temporal trends were assessed using Joinpoint regression to estimate annual percentage change (APC) and average annual percent change (AAPC). Analyses were stratified by ag

A New Frontier in CWD Detection: Antemortem Plasma Biomarkers and Behavioral Profiling in Transgenic Mouse Models.

Chronic Wasting Disease (CWD) is a fatal transmissible spongiform encephalopathy (TSE) that is confined to cervids (deer, moose, elk, and reindeer) but shares key properties with human neurodegenerative conditions such as Alzheimer's, Parkinson's, Huntington's disease and frontal-temporal dementia. CWD and other TSEs are caused by the misfolded prion protein (PrP). Although the identification of diagnostic and prognostic biomarkers at all stages of disease progression is becoming exceedingly critical as CWD continues to increase in prevalence, accurate antemortem testing techniques are extreme
