Hyperhomocysteinemia and Vitamin B Deficiency as Potential Aggravating Factors in Huntington's Disease: A Prospective Monocentric Study.

BACKGROUND: Although not confirmed, some studies have suggested that elevated homocysteine levels are common in patients with Huntington's disease (HD). Its clinical relevance remains unclear. OBJECTIVES: We aimed to assess vitamin B and homocysteine levels in HD patients and explore the relationships among hyperhomocysteinemia, vitamin B deficiency, and motor symptom progression. METHODS: In this monocentric case-control study, 77 HD patients were prospectively included between January 2020 and June 2023 at Nancy University Hospital. As controls, biological data were obtained retrospectivel

Restoring cortical disinhibition improves Huntington's disease phenotypes.

Huntington's disease (HD) is a devastating movement disorder without a cure at present1. Although the monogenic basis of HD is well defined2, the complex downstream effects that underlie behavioural symptoms are poorly understood. These effects include cortical dysfunction3,4, yet the roles of specific cortical neuronal subtypes in HD symptoms remain largely unexplored. Here we used longitudinal in vivo two-photon calcium imaging to examine the activity of three cortical inhibitory neuron (IN) subtypes and excitatory corticostriatal (CStr) projection neurons in the motor cortex of the transgen

Berbamine improves behavioral impairments in Huntington's disease mice models through inhibiting Src/AKT1/NFκB signaling pathway and attenuating Src-mediated mitophagy suppression.

BACKGROUND: Investigate the effects of berbamine (BBM) on alleviating motor and cognitive impairment in animal models of Huntington's disease (HD). METHODS: Intraperitoneal injection of 3-nitropropionic acid (3-NP) mice and B6-hHTT130-N transgenic mice were used as the HD models. We evaluated the anti-HD effect of BBM through behavioral experiments and employed molecular biological techniques, network pharmacology analysis, to test the potential mechanisms. RESULTS: BBM improved motor and cognitive impairment in 3-NP-injected and B6-hHTT130-N mice. BBM improved pathological damage in the bra

CDKN1A protects medium spiny neurons from Huntington's disease pathology.

Huntington's disease (HD) arises from abnormal expansion of CAG trinucleotide repeats within the HTT gene, leading to mutant huntingtin (mHTT) aggregation, progressive loss of striatal medium spiny neurons (MSNs), and progressive neurodegeneration. While the genetic cause is established, the mechanisms that confer selective MSN vulnerability, particularly those linked to aging, remain unclear. We employed a combination of miR-9/9*-124-driven reprogramming and MSN-specific transcription factors to generate patient-derived MSNs from fibroblasts of symptomatic HD patients (HD-MSNs), pre-symptomat

Structural insights into the MLH1-FAN1 interaction reveal an uncharacterized binding interface on MLH1.

Huntington's disease is driven by CAG repeat expansion in the mutant huntingtin gene. Nuclease FAN1 and mismatch repair protein MLH1 regulate repeat expansion through direct interaction, but the underlying structural basis remains unclear. Here, we show that the MLH1 C-terminal domain binds to FAN1-derived peptides containing either the MIP or MIM motif with comparable affinities. Crystal structures of this domain bound to each motif provide structural insights into human MLH1-FAN1 interaction, revealing a conserved mechanism for FAN1-MIP recognition and a previously unrecognized binding site

Genistein (5,7-dihydroxy-3-(4-hydroxyphenyl)-4H-1-benzopyran-4-one) Is Effective in Reducing Symptoms of Huntington's Disease in Females of the R6/1 Mouse Model.

BACKGROUND: Huntington's disease (HD) is an inherited (autosomal dominant) disorder caused by the occurrence of a pathogenic variant of the HTT gene. The genetic defect consists of an expansion of CAG repeats in exon 1, resulting in the production of a toxic misfolded huntingtin protein that forms aggregates. Currently, there is no registered therapy for this severe, neurodegenerative disease. Recent studies have demonstrated that treatment with genistein (4',5,7-trihydroxyisoflavone or 5,7-dihydroxy-3-(4-hydroxyphenyl)-4H-1-benzopyran-4-one), which acts by stimulating autophagy through a FOXO

Targeting DNA mismatch repair in Huntington's disease.

Somatic expansion of the HTT CAG repeat is a key feature of Huntington's disease (HD) pathogenesis. Mismatch repair (MMR) enzymes drive this process through erroneous DNA repair, with variants in MMR genes modifying the onset and progression of disease features. Cell-type-specific CAG repeat sizing recently confirmed that elevated somatic expansion underlies the selective vulnerability of HD medium spiny neurons, with expansion beyond certain CAG thresholds associated with distinct stages of cellular pathogenesis. In this review, we synthesise insights from post-mortem brain tissue, cell syste

The nano-intervention in Huntington's disease: A theragnostic approach.

Huntington's disease is a genetic autosomally dominant neurological disorder defined by chorea, uncontrollable movements, emotional instability, and cognitive decline. No disease-reversing therapy is available at present to treat disease. The blood brain barrier acts as a selectively permeable barrier eliminating the entry of therapeutic agents administered to provide relief rendering them ineffective in the management of the disease. Existing medicines which include tetrabenazine, deutetrabenazine, and off-label drugs offer only symptomatic treatment to improve the patient's quality of life a

Astrocytic lipid dysregulation as an early driver of neurodegeneration.

Astrocytes have traditionally been cast as supportive glia, but they are increasingly recognized as metabolic hubs that regulate cholesterol synthesis, fatty acid detoxification, lipid droplet dynamics and redox homeostasis in the CNS. Neurons have a limited intrinsic capacity for lipid storage and detoxification and rely heavily on astrocytes to maintain a safe lipid environment. Emerging evidence indicates that dysregulation of astrocytic lipid homeostasis precedes overt neuronal degeneration in a range of neurodegenerative diseases, including Alzheimer disease, Parkinson disease, amyotrophi

Therapeutic potential of flavonoids and rutin against inflammation-induced on a huntington animal model.

This study tested the hypothesis that flavonoids (diosmine, hesperidin and rutin®) play a protective role in the brain and duodenum against 3-Nitropropionic acid (3-NPA) free radical induced inflammation. This was achieved by measuring the levels of GABA, 5-HIAA, dopamine and some inflammation and oxidative stress markers. Male young Wistar rats (weight 60g) received Salmonella thyphimurium ATCC14028 1x106 CFU/g every week, for two consecutive weeks, plus the following treatments: group A, 0.9% NaCl (Control); group B, diosmine (300 mg) + hesperidin (33.3g) + rutin (150 mg/kg); group C, 3-NPA;

Emotion regulation across disease stages in Huntington's disease gene expansion carriers.

OBJECTIVE: Emotion regulation difficulties are increasingly recognized in Huntington's disease (HD) yet remain understudied with inconsistent findings. While motor symptoms are prominent, patients and caregivers often describe emotional and behavioral changes as distressing. This study assessed perceived emotion regulation abilities and strategies across disease stages, with a focus on anger and irritability. METHODS: We conducted a cross-sectional study including individuals with premanifest HD (n = 49), manifest HD (n = 41), and gene-negative family members previously at risk (n = 34). Part

The Recommendation Paradox: Perspectives on Genetic Testing in Huntington's Disease Families.

Huntington's disease (HD) families face complex decisions about predictive genetic testing and reproductive options, including preimplantation genetic testing (PGT), and particularly PGT for Monogenic Disorders (PGT-M). We examined attitudes toward genetic testing and reproductive options across affected groups within HD families (e.g., people with HD, spouse/caregivers, and at-risk family members), in a healthcare system with full funding for genetic services. We conducted semi-structured interviews with 51 participants (17 people with HD, 20 at-risk relatives, 14 spouses/partners), recruited

From glycemic control to neuroprotection: alogliptin as a repurposed candidate for Huntington's disease.

Huntington's disease (HD) is a progressive, autosomal dominant neurodegenerative disorder characterized by motor dysfunction, cognitive decline, and psychiatric disturbances, for which no disease-modifying therapies are currently available. Emerging evidence implicates metabolic impairment, mitochondrial dysfunction, oxidative stress, and neuroinflammation as central contributors to HD pathogenesis, thereby highlighting novel avenues for therapeutic intervention beyond conventional strategies. In this context, drug repurposing has gained considerable attention as an efficient approach to accel

USP28 Deficiency is Linked to Impaired Ubiquitin-dependent Proteostasis in Huntington's Disease.

Huntington's disease (HD) is characterized by mutant huntingtin (mHTT) aggregation and impaired proteostasis; however, upstream regulators of ubiquitin system imbalance remain incompletely understood. This study identified the deubiquitinase USP28 as a potential modulator of ubiquitin-dependent proteostasis in HD. Bulk RNA sequencing of R6/2 mouse brain tissues showed reduced USP28 expression compared with wild-type controls. Consistently, USP28 expression decreased in STHdh striatal cells expressing expanded polyQ huntingtin (Q111). HD cells (STHdh-Q111) exhibited increased accumulation of ub

A deep learning model for speech-based prediction of clinical scores in people with Huntington's disease: a longitudinal study with cross-sectional replication.

BACKGROUND: Sensitive monitoring tools are needed to track progression in neurodegenerative diseases and assess interventions before overt brain damage occurs. We propose speech as a non-invasive, easily collected biomarker to capture disease-related variation over time. We developed and validated Neurodegenerative Disease Speech Network (NDSNet), an automated deep learning model that generates individual speech-derived estimates of contemporaneous clinical scores at each visit in people with Huntington's disease, from presymptomatic stages (Huntington's Disease Integrated Staging System [HD-I

A novel machine learning approach for the classification of Huntington disease manifestation using motor features.

BACKGROUND: Currently, diagnosis of manifest Huntington disease (HD) is made when there is ≥ 99% confidence that observed motor abnormalities, rated on the Unified Huntington Disease Rating Scale (UHDRS), are due to HD (Diagnostic Confidence Level, DCL, = 4). However, there are no explicit rules relating UHDRS motor score items to an appropriate DCL, leaving diagnosis potentially subjective and variable. OBJECTIVE: We sought to develop algorithms to classify manifest HD, based on UHDRS motor scores. METHODS: We applied supervised machine learning techniques including elastic net logistic reg

Grape Seed Extract as an Adjuvant Therapy for Huntington Disease; A Narrative Review of Biological Plausibility and Potential Clinical Outcomes.

Huntington disease (HD) is a debilitating, genetic disorder with a prevalence of 2.7 per 100,000 people. It is neurodegenerative, leading to cognitive, behavioral, and motor symptoms from neuronal loss within the striatum of the basal ganglia and cortex. Currently, the treatments involve symptomatic management, instead of treating the pathophysiology of the disease. Grape seed extract (GSE) is a complex mixture of polyphenols, proteins, and lipids with antioxidant and anti-inflammatory properties. This literature review examines the possibility of using GSE as a potential adjunctive therapy fo

Cholesterol nose-to-brain delivery as a possible therapeutic strategy in Huntington's disease.

BACKGROUND: Huntington's disease (HD) is a genetically dominant neurodegenerative disorder characterized by several pathological mechanisms, including the disruption of brain cholesterol homeostasis. In several HD animal models, brain cholesterol biosynthesis and levels are reduced. Since circulating cholesterol cannot reach the brain, providing exogenous cholesterol has been shown to improve HD phenotypes. However, the methods used for cholesterol delivery were invasive and not easily transferable to clinical practice. METHODS: Cholesterol-enriched liposomes were developed by using freeze-an
