Comparison of tetrabenazine, tiapride and olanzapine in Huntington's disease: a one-year French randomized multicenter study (Neuro-HD).

INTRODUCTION: Chorea and behavioral symptoms in Huntington's disease (HD) have been treated with neuroleptics and related drugs for over 50 years, yet direct randomized comparisons are scarce. This study assessed the efficacy and safety of olanzapine, tetrabenazine, and tiapride in HD, with the Independence Scale as the primary outcome and components of the Unified Huntington's Disease Rating Scale as secondary outcomes. METHODS: We conducted a pragmatic, randomized, open-label trial across 11 centers of the French-Speaking Huntington's Network, enrolling 179 patients with Huntington's diseas

Targeted genome editing of ZKSCAN3 mitigates the neurotoxicity caused by mutant HTT (huntingtin) in a Huntington disease animal model and three-dimensional cell culture of Huntington disease.

Huntington disease (HD) is a neurodegenerative disease caused by the expression of a mutant form of HTT (huntingtin; mHTT), caused by an abnormal expansion of polyglutamine in HTT. In HD, macroautophagy/autophagy dysfunction can cause mHTT accumulation. Moreover, the promotion of autophagy is considered a therapeutic strategy for the treatment of HD. ZKSCAN3 (zinc finger with KRAB And SCAN domains 3) has been identified as a transcriptional repressor of TFEB (transcription factor EB), a master regulator of autophagy and lysosomal functions. In this study, we conducted CRISPR-Cas9-based gene ab

Development of cognitive, motor, metabolic, and mutant huntingtin aggregation in the zQ175 mouse model of Huntington's disease.

Huntington's disease (HD) is an inherited neurodegenerative disease. In humans, the clinical diagnosis is often dependent on the emergence of motor symptoms. However, cognitive impairments and metabolic changes can be early indicators. HD mouse models are a useful tool to understand disease progression, however, relatively few studies have monitored the timeline for the emergence of cognitive indices with motor and metabolic phenotypes in parallel. In this study, cognitive, motor, and metabolic phenotypes were investigated at different ages in the zQ175 knock-in mouse alongside immunohistochem

Systematic Review with Meta-Analysis of Biofluid Markers for Huntington's Disease.

Primary therapeutic objectives for Huntington's disease (HD) necessitate continued therapy for a long period before clinical motor diagnosis and its concurrent functional incapacities. Therefore, the need is paramount for alternative biomarkers that are not only highly sensitive but also clearly reflect the disease progression. Current trials increasingly rely upon biological definitions of disease to initiate intervention before significant decline. The primary biological measure of early-stage HD is volumetric evidence of structural decline on magnetic resonance imaging. This comprehensive r

The association between smoking and Huntington's disease symptom onset and progression - results from Enroll-HD: The observational cohort study of people with Huntington's disease.

AIM OF THE STUDY: To assess the impact of tobacco smoking on the onset of Huntington's disease (HD) and the progression of its motor and cognitive symptoms. CLINICAL RATIONALE OF THE STUDY: HD is an incurable neurodegenerative disorder. Therefore, identifying lifestyle factors with an impact of its progression is of particular interest. Tobacco smoking influences the course of neurodegenerative disorders. It has been shown to be a risk factor for dementia, but, on the contrary, seems to have a protective role in Parkinson's disease. 3D cultured human medium spiny neurons functionally integrate and rescue motor deficits in Huntington's disease mice.

Dysfunction of striatal medium spiny neurons (MSNs) is implicated in several neurological disorders, including Huntington's disease (HD). Despite progress in characterizing MSN pathology in HD, mechanisms underlying MSN susceptibility remain unknown, driving the need for MSNs derived from human pluripotent stem cells (hPSCs), especially subtypes in research and therapy. Here, we established a scalable 3D-default culture system to produce striatal MSNs efficiently from hPSCs by activation of the endogenous sonic hedgehog (SHH) pathway. These cells expressed canonical markers of striatal progeni

Genetic dissection of Huntington's disease modification by variation at RRM2B.

Huntington's disease (HD) is driven by somatic expansion of the HTT CAG repeat, with onset modified by genetic factors. One such modifier, 8AM1, maps to chromosome 8 near RRM2B, a gene not directly involved in the machinery that lengthens the repeat. To investigate this locus, we performed capture sequencing and identified variants at both the 5' and 3' ends of RRM2B with expected minor allele frequencies. A polymorphic frameshift variant (rs1037699) in an alternate exon 1 disrupts expression of a previously uncharacterized RRM2B isoform 2, but not isoform 1. Functional analyses in RRM2B knock

Challenging or less challenging oral diadochokinetic tasks-what works best in Huntington disease? A cross-sectional study.

BACKGROUND: In Huntington disease (HD), speech alterations are common and may emerge before onset of chorea. Slow and irregular motion rates, i.e. altered oral diadochokinesis (oDDK), are a distinctive feature. This study investigated oDDK using alternating (AMR) and sequential motion rate (SMR) tasks in manifest HD and explored the impact of antidopaminergic medications (ADM). METHODS: Speech samples were acquired from 30 healthy controls (14 men; 27-78 years¸ age- and gender-matched to HD subjects) and 35 individuals with early-to-moderate HD (18 men; 22-76 years) phenotyped using standardi

Challenges in Polyglutamine Diseases: From Dysfunctional Neuronal Circuitries to Neuron-Specific CAG Repeat Instability.

Several genetic diseases affecting the human nervous system are incurable and insufficiently understood. Among them, nine rare diseases form the polyglutamine (polyQ) family: Huntington's disease (HD), spinocerebellar ataxia types 1, 2, 3, 6, 7, and 17, dentatorubral pallidoluysian atrophy, and spinal and bulbar muscular atrophy. In most patients, these diseases progress over decades to cause severe movement incoordination and neurodegeneration. Although their inherited genes with tandem-repeat elongations and the encoded polyQ-containing proteins have been extensively studied, the neuronal-ty

Update on Genetic Chorea.

Hereditary choreas are a clinically and genetically heterogeneous group of monogenic disorders in which chorea constitutes the core or an early-dominant feature. These conditions result from various genetic mutations affecting the structures and pathways involved in movement control, primarily the caudate and putamen, ultimately impairing the basal ganglia circuits involved in the regulation of movement, cognition, and behavior. This review focuses on the main forms of hereditary choreas, including Huntington's disease, neuroacanthocytosis syndromes, Huntington's disease phenocopies, benign he

Ticagrelor as a dual RAS modulator and mitochondrial stabilizer: A multifaceted neurotherapeutic strategy in Huntington's disease.

The pathobiology of Huntington's disease (HD) is far from complete. Although limited, patients have been reported to exhibit altered platelet function, enhanced ENT1, and changes in renin-angiotensin system (RAS), all of which may impact cognitive function. Ticagrelor has demonstrated neuroprotective effects in stroke and parkinsonism models through mechanisms that extend beyond its therapeutic benefits as a P2Y12 receptor antagonist and ENT1 inhibitor. In this study, we investigated the potential efficiency Ticagrelor post-treatment on motor defects in a 3-nitropropionic acid (3-NP)-induced H

Bariatric surgery and semaglutide in a youth with juvenile Huntington disease and severe obesity: a case report.

BACKGROUND: Managing severe obesity in youth with chronic, progressive conditions, such as juvenile Huntington disease, presents unique challenges. Juvenile Huntington disease, is characterized by rapid neurodegeneration, with most patients experiencing a significant decline in motor and cognitive functions within 10-20 years of symptom onset, leading to reduced life expectancy. Obesity further complicates these conditions, reducing quality of life and exacerbating physical, cognitive, and metabolic dysfunctions. A multidisciplinary approach is essential to address these complex issues. CASE
