Sigma-2 Receptor Antagonism Enhances the Neuroprotective Effects of Pridopidine, a Sigma-1 Receptor Agonist, in Huntington's Disease.

Pridopidine is a selective sigma-1 receptor (S1R) agonist in clinical development for Huntington's Disease (HD) and Amyotrophic Lateral Sclerosis (ALS). Activation of the S1R by pridopidine is neuroprotective in multiple preclinical models of neurodegenerative disease. The sigma-2 receptor (S2R) is evolutionarily and structurally unique from the S1R. Nevertheless, the S1R and S2R share an overlapping yet distinct ligand binding profile. Inhibition of the S2R is neuroprotective and S2R antagonists are in clinical development for Alzheimer's Disease (AD), ⍺-synucleinopathies, and dry age-related

Autophagy and mitophagy at the synapse and beyond: implications for learning, memory and neurological disorders.

The human brain is one of the most metabolically active tissues in the body, due in large part to the activity of trillions of synaptic connections. Under normal conditions, macroautophagy/autophagy at the synapse plays a crucial role in synaptic pruning and plasticity, which occurs physiologically in the absence of disease- or aging-related stressors. Disruption of autophagy has profound effects on neuron development, structure, function, and survival. Neurons are dependent upon maintaining high-quality mitochondria, and alterations in selective mitochondrial autophagy (mitophagy) are heavily

Quantitative relationship between tominersen concentrations in cerebrospinal fluid and biomarker changes in Huntington's disease patients.

AIM: Intrathecally administered antisense oligonucleotide tominersen aims to slow Huntington's disease progression by lowering mutant huntingtin (mHTT) protein levels. This study used non-linear mixed effects population pharmacokinetic and pharmacodynamic (PKPD) modelling to characterize the relationship between tominersen concentration in cerebrospinal fluid (CSF) and CSF mHTT reduction. Additionally, the relationship between tominersen CSF exposure and changes in other CSF biomarkers was investigated to understand tominersen's pharmacodynamic profile. Finally, PKPD model simulations were con

Post-Translational Modifications of Huntingtin: Mechanistic Insights and Therapeutic Opportunities in Huntington's Disease.

Huntingtin (HTT) is a large, ubiquitously expressed scaffold protein that participates in multiple cellular processes, including vesicular transport, transcriptional regulation, and energy metabolism. The mutant form of HTT (mHTT), characterized by an abnormal polyglutamine (polyQ) expansion in its N-terminal region, is the causative agent of Huntington's disease (HD), a progressive neurodegenerative disorder. Current therapeutic efforts for HD have primarily focused on lowering HTT levels through gene silencing or promoting mHTT degradation. However, accumulating evidence suggests that post-t

Epidemiology and economic burden of selected rare genetic diseases in Germany - a claims database study.

BACKGROUND: Research on rare genetic diseases is challenging due to their low prevalence and complex clinical manifestations. Despite their individual rarity, the cumulative burden of these diseases is substantial. Larger claims databases could enable a quantification and characterization of patients with rare diseases and contribute to determining their specific healthcare needs. This study examines the prevalence, comorbidities, and annual direct costs of Huntington’s disease (HD), Beta-Thalassemia (BT), and Spinal Muscular Atrophy type 1 (SMA type 1) in Germany. METHODS: Utilizing anonymize

The impact of tau deletion on Huntington's disease: An in vivo perspective.

Although an emerging body of evidence suggests that abnormal forms of tau are present and contribute to Huntington's disease (HD)-a genetic neurodegenerative disorder primarily characterized by the aggregation of the mutant huntingtin (mHtt) protein affecting cognitive, motor, and psychiatric function-it is not clear to what extent this is relevant to the disease phenotype, and hence future treatments. We therefore generated a novel murine model by crossing heterozygous zQ175 knockin HD mice with homozygous tau knockout mice (mTKO). Tau deletion exacerbated both motor and cognitive deficits in

Handgrip strength and the Index of pulmonary dysfunction: Practical screening tools for cough dysfunction in huntington's disease.

BackgroundAs Huntington's disease (HD) progresses, it impairs airway protection, increasing the risk of aspiration pneumonia-the leading cause of death in HD. Although voluntary peak cough flow (vPCF) assesses cough effectiveness, its clinical use is limited by time and equipment constraints.ObjectiveThis study evaluates handgrip strength (HGS) and the Index of Pulmonary Dysfunction (IPD) as simpler screening tools and aims to define gender-specific thresholds for identifying cough dysfunction (dystussia) in HD patients.MethodsIn this prospective cross-sectional study, 71 patients with HD unde

Inhba, Homer1 and Bdnf are major targets of transcriptomic dysregulation by neurodegenerative disease-associated excitotoxic NMDA receptor signaling.

Synaptic activity-regulated gene expression supports neuroprotection, plasticity, and memory. The transcription factor CREB is central to these processes. It is activated by synaptic NMDA receptors but inactivated by excitotoxic extrasynaptic NMDAR (esNMDAR) signaling. Using primary hippocampal neurons, we modeled neurodegeneration and found that esNMDAR activation, which causes CREB shut-off and inactivation of the ERK/MAPK-ELK1/SRF pathway, extensively distorted control of synaptic activity over transcription. This resulted in the suppression of key neuroprotective genes, in particular Inhba

Huntingtin protein in health and Huntington's disease: Molecular mechanisms, pathology and therapeutic strategies.

Huntington's Disease (HD) is a neurodegenerative, genetic disorder that affects the brain and is caused by the expansion of cytosine-adenine-guanine (CAG) trinucleotide in the huntingtin (HTT) gene exceeding 35 units. Further, the mutation occurs, which leads to the generation of mutant huntingtin (mHTT) protein, which is a toxic protein that damages the neurons and their functions, leading to disease progression. Phosphorylation, SUMOylation, O-GlcNAcylation, and ubiquitination are some of the post-translational modifications (PTMs) that affect the toxicity, location, and aggregation of this

Differential effects of overexpression of mutant huntingtin and TDP-43 in agouti-related protein neurons in the arcuate nucleus of the hypothalamus in mice.

The spectrum of frontotemporal dementia/amyotrophic lateral sclerosis (FTD/ALS) and Huntington disease (HD) are fatal neurodegenerative disorders with no major disease-modifying therapies. Recent work has shown that the hallmark pathological proteins TAR DNA binding protein of 43 kDa (TDP-43) in FTD/ALS and mutant huntingtin (mHTT) in HD may be interlinked. Furthermore, these disorders share early features of altered metabolism and psychiatric symptoms that have been suggested to arise from pathology in the hypothalamus, an important brain region involved in the regulation of metabolism and em

The profiling of extracellular vesicle subtypes in Huntington's disease brains identifies Alix as a novel marker of neuropathology.

BACKGROUND: Huntington's disease (HD) is the most frequent autosomal dominant neurodegenerative disorder, which is caused by a CAG repeat expansion in the HTT gene. Despite its well-defined genetic origin, there is currently no cure, and reliable biomarkers for disease progression and pathophysiology remain limited. Mutant huntingtin protein accumulates in endosomal compartments, disrupting endosomal trafficking and potentially affecting the biogenesis, release, and cargo of exosomes-extracellular vesicles (EVs) derived from the endosomal pathway. However, the role of exosomes in HD pathogenes

Discovery and Targeted Proteomic Studies Reveal Striatal Markers Validated for Huntington's Disease.

OBJECTIVE: Clinical trials for Huntington's disease (HD) enrolling persons before clinical motor diagnosis (CMD) lack validated biomarkers. This study aimed to conduct an unbiased discovery analysis and a targeted examination of proteomic biomarkers scrutinized by clinical validation. METHODS: Cerebrospinal fluid was obtained from PREDICT-HD and ancillary studies. Cohorts included HD family members who were gene-tested and considered prodromal following neuroexam. An initial unbiased mass spectrometry proteomics analysis identified candidate disease biomarkers that were then added to a target

Do cognitive tests capture symptoms that matter to people with Huntington's disease?

The Huntington's Disease Cognitive Assessment Battery (HD-CAB) is frequently used in clinical trials; however, it is unclear whether the HD-CAB captures meaningful symptoms from patients' perspectives. We aimed to explore whether HD-CAB tests are considered meaningful to people with Huntington's disease (HD) by mapping them to relevant symptoms and impacts. Eighteen people with HD before and after clinical motor diagnosis completed a semi-structured interview with symptom mapping to hierarchically rank the importance and bothersomeness of cognitive symptoms and discussed the relevance of each

Insights into neurodevelopmental features of Huntington's disease from stem cell-derived models including organoids.

Two- and three-dimensional (2D and 3D) cell models derived from human stem cells have shed light on a wide range of molecular and cellular features of Huntington's disease (HD). Here we review the use of human stem cell-derived models to explore neurodevelopmental contributions to HD. We provide a timeline of key advances made in 2D and 3D model systems, ranging from differentiated monocultures to brain-like organoids and assembloids. Models along this spectrum have advanced our understanding of various disease-associated characteristics including disease protein (huntingtin) aggregation, soma

A human CAGinSTEM platform for decoding HTT repeats' somatic instability links CAG interruption to HD pathology in neurons.

Somatic CAG instability in the mutant Huntingtin (HTT) gene is increasingly recognized as a key hallmark of Huntington's disease (HD). Using our novel human CAGinSTEM platform, we manipulated cis genetic elements influencing instability in human HD neurons, monitoring repeat length. Quality-controlled CRISPR-engineered stem cells with increasing CAG lengths and clinical haplotypes were analyzed using third-generation sequencing. Our findings link interruptions in the CAG repeat, especially the loss or duplication of the penultimate CAA of canonical alleles, to significant instability modulatio

Epitope-specific antibodies can distinguish between soluble huntingtin exon-1 and its diverse cellular aggregates.

Misfolding and aggregation of huntingtin exon-1 (Httex1) with an expanded polyglutamine region is a key pathological hallmark of Huntington's disease, making conformationally specific Httex1 binders potentially valuable diagnostic or therapeutic tools. To define epitopes, which might confer conformationally specific Httex1 binding, we characterized five newly developed huntingtin antibodies (PHP5-PHP9). Binding to recombinant proteins as well as staining of human embryonic kidney 293 cells and R6/1 mice shows that PHP5 and PHP6 preferentially bind monomers over fibrils. Using electron paramagn

β-Blocker effects in huntington's disease: A caution on clinical interpretation.

A recent retrospective analysis of Enroll HD data suggesting β-blockers slow Huntington's disease progression has triggered patient demand but requires caution. The findings rely solely on small observational subsets and are vulnerable to bias and confounding. A prior Mendelian-randomization study found no causal link between β-blockers and HD onset; instead, genetically higher blood pressure was associated with later onset, raising concern that β-blockers' BP-lowering effects could be harmful. HD patients also have lower hypertension rates, and β-blockers carry risks such as depression and br

Axial Spinal Traction as a Potential Modulator of Cerebrospinal and Glymphatic Circulation in Neurodegenerative Diseases: A Technical Report and Biomechanical Hypothesis.

Impairment of glymphatic function contributes to the accumulation of metabolic and proteinaceous waste products implicated in neurodegenerative diseases such as Alzheimer's disease, frontotemporal dementia, Parkinson's disease, amyotrophic lateral sclerosis, Huntington's disease, and certain spinocerebellar ataxias. Pelvis-stabilized axial spinal traction (PSAST) is a biomechanical technique designed to produce brief, controlled cranio-caudal elongation of the vertebral column and spinal dural sac, potentially generating transient pressure gradients capable of influencing cerebrospinal fluid (
