Huntington's disease human lateral ganglionic eminence precursors differentiate into functionally mature medium spiny neurons exhibiting pathology.

Huntington's disease (HD) is an autosomal dominant neurodegenerative disease characterized by the loss of GABAergic medium spiny neurons (MSNs). Cellular models of HD are mainly derived from human embryonic stem cells or induced pluripotent stem cells. These models are limited by their DNA embryonic age, low neuronal yields, and limited disease pathology. We propose direct reprogramming, which maintains the aging signature of the cells, to human induced lateral ganglionic eminence precursors (hiLGEP) results in the generation of high yields of functionally mature MSNs exhibiting pathological h

Double strand breaks drive toxicity in a Huntington's disease mouse model with or without somatic expansion.

Genome-wide association studies (GWAS) have provided strong evidence that modifiers of CAG tract length have a crucial influence on Huntington disease onset, but somatic expansion alone may not be sufficient to drive neuronal death. Here, we report that DSBs drive neuropathology in male HdhQ(150/150) mice, regardless of somatic expansion of the inherited disease allele. DSBs and somatic expansion occur simultaneously in the HD brain, but the two types of DNA damage drive disease by distinct mechanisms. The site-specific increases in CAG tract length are driven by active mismatch repair (MMR),

TRACE: Open-source software for quantifying somatic variation of tandem repeats by capillary electrophoresis.

Expanded short tandem DNA repeats are implicated in over 60 human disorders. In many, somatic instability (SI) of the repeat plays a critical role in disease pathogenesis. For example, SI in vulnerable neurons is a key driver of clinical symptoms in Huntington's disease. Quantifying SI has traditionally relied on PCR followed by capillary electrophoresis, with metrics describing the shape of repeat size distributions, such as the expansion index. However, current tools often require costly proprietary software, are time-consuming, and rely on custom pipelines that vary between labs. To address

Induced pluripotent stem cells from a transgenic minipig model of Huntington's disease reveal early metabolic changes.

Huntington's disease (HD) is a neurodegenerative autosomal dominant hereditary disease caused by a CAG triplet repeat expansion mutation in the gene encoding the huntingtin (HTT) protein. The main feature of HD is the loss of striatal neurons, accompanied by metabolic and transcriptional alterations in both neural and peripheral tissues. Induced pluripotent stem cells (iPSCs) derived from a transgenic HD (TgHD) minipig model expressing a mutant HTT construct were generated to investigate early metabolic, antioxidant and DNA integrity changes associated with HD development. Gene expression anal

Developing prognostic models that predict onset and progression rates in genetic neurodegenerative diseases: perspectives of healthcare professionals in genetic counselling.

BACKGROUND: Research on the development of prognostic biomarker-based models for genetic neurodegenerative diseases lacking disease-modifying treatments is advancing rapidly. Currently, prognostic models are being developed to predict individual disease onset and progression rates for Huntington's disease, spinocerebellar ataxia type 1, and spinocerebellar ataxia type 3. These models have potential applications in research, such as evaluating onset-delaying treatments and their timing, and in clinical practice, where they may offer personal utility to mutation carriers. However, the perspectiv

Changing care for patients at risk for Huntington's disease in the United States due to the 21st Century Cures Act.

The Huntington's Disease Society of America (HDSA) protocol requires in-person result disclosure for predictive testing due to the increased suicidality risk in the Huntington's disease (HD) population. The 21st Century Cures Act (Cures), a United States law requiring immediate release of all healthcare test results, contradicts this protocol. There is no published literature exploring how Cures has impacted HDSA centers of excellence (CoE). This study used semi-structured interviews with 15 HDSA CoE healthcare professionals (HCPs) to explore the impact of Cures on the HDSA CoE's disclosure of

Sleep in Huntington's disease gene expansion carriers: A systematic review.

Huntington's disease (HD) is a progressive neurodegenerative disorder characterized by an abnormal CAG repeat expansion in the huntingtin gene, resulting in motor, cognitive, and psychiatric impairments. Sleep disturbances are a prevalent yet underexplored feature in both premanifest gene carriers and individuals with manifest disease. Given their potential as early disease markers and their significant impact on quality of life, understanding these sleep alterations is clinically critical. This systematic review synthesized findings from 78 studies, revealing a wide prevalence of sleep distur

Management of catatonia in Huntington disease: A scoping review.

INTRODUCTION: Huntington disease (HD) is a progressive neurodegenerative disorder caused by CAG repeat expansion in the HTT gene. It typically presents with choreiform movements, cognitive decline, and executive dysfunction. Moreover, psychiatric symptoms such as depression, psychosis, and disinhibition are common. Catatonia, a rare and mostly unstudied comorbidity of HD, presents with mutism, rigidity, and agitation. Overlapping psychomotor and behavioral features make the diagnosis of HD with concomitant catatonia challenging. We conducted a scoping review to examine reported cases of catato

Distress, not symptoms: Reframing psychological difficulties in neurodegenerative diseases of the motor system.

Psychological distress is common among people living with neurodegenerative diseases of the motor system (NDMS) such as Parkinson's disease, motor neurone disease/amyotrophic lateral sclerosis, and Huntington's disease. Yet the way psychological difficulties are conceptualised in these populations is heavily shaped by medicalised language. Terms such as 'non-motor symptoms' and 'neuropsychiatric manifestations' were originally introduced to draw attention to difficulties beyond movement changes but they now risk positioning mood, anxiety, apathy, and related experiences solely as direct manife

40+ years of the Huntington disease predictive genetic testing protocol: Recommendations for an update.

Guidance for predictive genetic testing for Huntington disease (HD) has been available for more than four decades and has been applied to other adult-onset hereditary conditions, especially neurogenetic conditions. Yet this protocol has not been updated since 2016 even though there have been significant changes in healthcare delivery and policy/legal issues, including widespread use of telemedicine and new legislation impacting patients' ready access to medical records. There also have been technological advances where HD status can be revealed with genomic sequencing and societal paradigm shi

Exploring stigma in Huntington's disease: A scoping review of methods and conceptualizations for understanding experiences of gene expansion carriers and at-risk individuals.

Many individuals affected by the hereditary neurological condition Huntington's disease (HD) have reported experiences of stigmatization, yet the extant literature is currently theoretically and methodologically underdeveloped. Therefore, this scoping review aimed to examine the methodological approaches and theoretical conceptualizations of stigma used and to identify areas of underrepresentation given the emerging evidence base. A systematic search of five databases and hand-searches of included papers resulted in 3273 articles. Thirty-two met predefined inclusion criteria, which included pr

Beyond motor symptoms: efficacy of pharmacotherapies on motor, functional, and cognitive domains in Huntington's disease - a systematic review.

INTRODUCTION: Huntington's disease (HD) is a progressive neurodegenerative disorder characterized by motor, neuropsychiatric, cognitive, and peripheral alterations impairing functional capacity and quality of life (QoL). Pharmacotherapies, including dopaminergic and glutamatergic drugs, primarily target motor symptoms, while cholinergic modulators address cognitive decline. However, the literature lacks comprehensive reviews evaluating the combination of these three drug classes across a broad spectrum of clinical outcomes, extending beyond the total motor score (TMS). OBJECTIVE: This systema

The holistic burden of Huntington's disease: A qualitative study of multigenerational impact on individuals and family caregivers across Europe.

Huntington's disease (HD) is a progressive, inherited neurodegenerative condition that imposes a profound, multidimensional burden on individuals with HD and their families. This qualitative study explored the physical, emotional, socioeconomic, and intergenerational impact of HD across six European countries, drawing on interviews with individuals diagnosed with HD and family caregivers. Findings reveal that HD disrupts daily functioning, employment, relationships, and long-term planning. Cognitive and psychiatric symptoms were particularly burdensome, driving declines in quality of life (QoL

Inhibition of the mitochondrial pyruvate carrier attenuates the integrated stress response activation in a cellular model of Huntington's disease.

Mitochondrial pyruvate carrier (MPC) inhibition was found protective in models of neurodegenerative diseases, such as Alzheimer's and Parkinson's. However, little is known about MPC as a potential therapeutic target in Huntington's disease (HD), a neurodegenerative disorder with dysregulation of the pro-survival pathway integrated stress response (ISR). Here, we investigate if MPC inhibition modulates the ISR and mitigates mutant huntingtin (mut-Htt) proteotoxicity in a cellular HD model. We treated cells expressing N-terminal fragments of wild-type- (wt-) or mut-Htt with two MPC inhibitors (m

A machine learning-derived speech index as a biomarker for Huntington's disease severity.

BACKGROUND: The development of disease-modifying therapies for Huntington's disease (HD) necessitates sensitive, scalable, and objective biomarkers for patient stratification and tracking. Current clinical scales are rater-dependent and time-consuming, while neuroimaging is costly and inaccessible. We aim to develop and validate a novel Speech Index, derived from automated acoustic analysis, to stratify HD stages and predict the severity of disease. METHODS: We recruited 141 HTT gene carriers (37 premanifest, 104 manifest HD) and 69 healthy controls. Participants read a standardized passage,

The pathological Huntingtin CAG triplet expansion differentially affects the diagnosis of systemic and organ-specific autoimmune diseases.

BACKGROUND: In Huntington's disease (HD), signs of inflammatory activation are found in the brain, cerebrospinal fluid, and blood. HD monocytes are reported to be hyperreactive in vitro. Thus, HD mutation might affect the immune system. AIM: To explore the frequency of autoimmune diseases (AIDs) in HD mutation carriers (people with the HD mutation, PwHD) compared to control participants (CPs) as markers of immune dysfunction related to CAG triplet expansion in the Huntingtin (HTT) gene. METHODS: Analysis of the Enroll-HD periodic dataset #5 (European sites) was conducted. Definite AIDs, code

DNA methylation profiling in Huntington's disease reveals disease associated changes in the striatum.

BACKGROUND: Huntington's disease is caused by a trinucleotide CAG repeat expansion in the HTT gene. Despite displaying autosomal dominance, phenotypic variation exists amongst mutation carriers, in particular relating to the age that symptoms first occur. This variation is primarily driven by an inverse relationship between CAG expansion size and age of symptom onset. However, the majority of variation in age of onset that is independent of CAG repeat length is thought to be driven by environmental influences. Since DNA methylation can be altered by environmental factors, and as methylomic var

Molecular features of a Huntington's disease knock-in minipig.

Huntington's disease is caused by a CAG expansion in the HTT gene, leading to somatic repeat instability, alternative processing of HTT pre-mRNA, and mutant huntingtin protein production. To model these features, we generated a knock-in minipig (KI-85Q-HD) carrying a (CAG)82CAA(CAG)2 repeat in the endogenous HTT locus. To evaluate this, we quantified somatic expansion in various tissues using small pool- and bulk-PCR; detected HTT1a, an aberrantly spliced HTT transcript, using 3' rapid amplification of cDNA ends and quantitative PCR; and assessed mutant huntingtin protein isoforms using homoge

ROS-HIF1α-driven glycolytic reprogramming sustains ATP production in Huntington's disease.

Huntington's disease (HD) is a progressive neurodegenerative disorder in which mitochondrial dysfunction and impaired energy metabolism contribute to disease pathogenesis. Surprisingly, we find that ATP levels are not diminished but instead elevated in the striatum of R6/2 HD mice despite impaired TCA cycle intermediates and mitochondrial deficits. Integrative metabolomics, gene expression profiling, and pharmacological perturbation reveal that increased reactive oxygen species stabilize hypoxia-inducible factor-1α (HIF1α), driving enhanced glucose uptake and glycolytic flux.
